Major maxillofacial bone injury itself can be life threatening from both cardiovascular point of view, as well as airway obstruction. Significant hemorrhage from facial fracture is an uncommon occurrence, and there is little in the literature to guide the management of these patients. We report a 73-year-old male driver who was transported to our hospital after a motor vehicle collision. The patient was hypotensive and tachycardic at presentation and required active fluid resuscitation and transfusion. The patient was intubated to protect the airway. All external attempts to control the bleeding, from packing to fracture reduction, were unsuccessful. Emergency angiogram revealed the bleeding to originate from terminal branches of the sphenopalatine artery, which were embolized. This was associated with cessation of bleeding and stabilization of vital signs. Despite the age and severity of injury, the patient recovered well and was discharged home at 3 months with full employment. In facial trauma patients with intractable bleeding, transcatheter arterial embolization should be considered early in the course of management to decrease mortality rate.
In biometric and biomedical applications, a special transporting mechanism must be designed for the ${\mu}$TAS (micro total analysis system) to move samples and reagents through the microchannels that connect the unit procedure components in the system. An important issue for this miniaturization and integration is microfluid management technique, i.e., microfluid transportation, metering, and mixing. In view of this, this study presents an optimal fuzzy sliding-mode control (OFSMC) design based on the 8051 microprocessor and implementation of a complete microfluidic manipulated system implementation of biochip system with a pneumatic pumping actuator, a feedback-signal photodiodes and flowmeter. The new microfluid management technique successfully improved the efficiency of molecular biology reaction by increasing the velocity of the target nucleic acid molecules, which increases the effective collision into the probe molecules as the target molecules flow back and forth. Therefore, this hybridization chip was able to increase hybridization signal 6-fold and reduce non-specific target-probe binding and background noises within 30 minutes, as compared to conventional hybridization methods, which may take from 4 hours to overnight. In addition, the new technique was also used in DNA extraction. When serum existed in the fluid, the extraction efficiency of immobilized beads with solution flowing back and forth was 88-fold higher than that of free-beads.
Purpose: This study was performed to validate the linkage between nursing diagnoses and nursing interventions by identifying performance and importance of nursing interventions linked to five NANDA nursing diagnoses. Method: Data was collected from 153 staff and head nurses working in 4 hospitals in K city. The results were analyzed using mean, SD and spearman correlation for ranking correlation. Result: The most importantly considered interventions were Medication Administration (IV) for pain, Pain Management for Constipation, Intravenous (IV) Insertion for Diarrhea, treatment, Vital Sign Monitoring for Hyperthermia, and Vital Sign Monitoring for Infection risk. The most frequently performed interventions was Medication Administration (IV) for Pain, Fluid Management for Constipation, Intravenous (IV) Insertion for Diarrhea, Vital Sign Monitoring for Hyperthermia, and Vital Sign Monitoring for Infection: Risk for. The rank correlations between importance and performance were highest in Diarrhea and lowest in Constipation. Conclusion: The above findings can be used to develop a nursing information system which can be used to facilitate documenting the nursing process, and a nursing information system developed by this research process will ultimately contribute to identifying nurses contribution to patient health.

In this report, we present a scalp defect reconstruction with lateral arm free flap. We highlight the difficulty in obtaining a recipient vein and the venous drainage managed through an open end of the donor vein. A 52-year-old woman presented with a pressure sore on the left scalp. A lateral arm free flap was transferred to cover this $8{\times}6cm$ defect. The arterial anastomosis was successful, but no recipient vein could be identified within the wound bed. Instead, we used a donor venous end for the direct open venous drainage. In order to keep this exposed venous end patent, we applied heparin-soaked gauze dressing to the wound. Also, the vein end was mechanically dilated and irrigated with heparin solution at two hour intervals. Along with fluid management and blood transfusion, this management was continued for the five days after the operation. The flap survived well without any complication. Through this case, we were able to demonstrate that venous congestion can be avoided by drainage of the venous blood through an open vessel without the use of leeches.
We are report on three cases of typical clinical characterstics and treatment response in neuroleptic maligant syndrome(NMS), and reviewed the literatures of NMS. NMS was first recognized as a life-threatening complication of dopamine receptor antagonists, and defined as a catatonic-like states associated with fever, obtundation, muscle rigidity, and unstable vital sign in patients taking neuroleptic agents. Concepts of NMS have changed because medications other than classic neuroleptic drugs have been implicated as triggering agents and syndromes identical to NMS have been observed in other conditions. The important neurochemical features are probably functional dopamine deficiency and ensuing hyperactivity of excitatory amino acid neurotransmission in the basal ganglia and hypothalamus. Recognition of NMS and early discontinuation of neuroleptics are the most important step in its management. Supportive care includes management of hyperthermia and fluid replacement. Controversial therapeutic measures include the application of dopamine receptor agonists, excitatory amino acid antagonists, or dantrolene. Psychiatric patients with a history on NMS and psychotic relapse necessitating antipsycotics do not commonly redevelop NMS.
Ileus is blockage of the intestine that prevents the contents of the intestine from passing to the lower bowel. In western medical treatment, surgery is mandatory for severe cases such as strangulation ileus. However more generally, conservative management is used for treatment. Examples of conservative management are decompression with tube, fluid treatment, enema and fasting cure. Until now, many oriental medical studies have been conducted on chronic ileus, but almost none on acute ileus. The results for the case in this report suggest that oriental medical treatment such as acupuncture, moxibustion, herbal medicine is effective in treating acute ileus. We think that further studies should be conducted on a combination therapy with other oriental medical treatment such as retention enema.
Because of their longer operating times and larger size relative to conventional fans, the cooling fans mounted in buses consume larger amounts of energy. Most of the cooling fans mounted in a bus are connected to the engine by a viscous clutch. A viscous cooling fan's speed is determined by its fluid temperature, which is affected by the air flow through the radiator. The fan does not react immediately to the coolant temperature and in doing so causes unnecessary energy consumption. Therefore, the fuel economy of buses using viscous fans can be improved by changing to an electric cooling fan design, which can be actively controlled. In addition, electric power consumption is increased by using electric cooling fans. Thus, when electric fans are applied in conjunction with the alternator management system (AMS), the fuel economy is further enhanced. In this study, simulations were performed to predict coolant temperature and cooling fan speeds. Simulations were performed for both viscous and electric cooling fans, and power consumption was calculated. Additionally, fuel economy was calculated applying both the alternator management system and the electric cooling fan.
Acute viral meningitis and myositis are rare complications of varicella-zoster virus (VZV) reactivation. A 71-years-old immunocompetent man, who presented with lower back pain radiating to the left lower extremities, developed vesicles on the L5 dermatomal area. The next day, he had complained of aberrant vesicles on the trunk, face and scalp, with generalized myalgia, headache and dizziness. He was confirmed with VZV meningitis and myositis, as demonstrated by the presence of VZV DNA in the blood and cerebral spinal fluid using a polymerase chain reaction (PCR) amplification. PCR has been used in patients with a VZV infection associated neurological symptoms, and provides a useful tool for the early diagnosis of VZV-associated neurological disease. The patient was treated with bed rest, with intravenous acyclovir for the VZV infection, and intravenous Patient-controlled Analgesia for pain management and the prevention of postherpetic neuralgia. When he visited the outpatient department 3 months later, the skin lesion, leg pain, headache and myalgia had all improved, without sequelae. Here, this case is reported, with a discussion of the relevant literature on its diagnosis and management.
Recently, various engineering approaches have been widely used in the accident investigation field to identify the cause of the accident and to predict damage by accident. Computational analysis is the most commonly used method of accident investigation technique. This technique is mainly used to identify the mechanism of the accident generation and to determine the cause when it is difficult to reproduce the situation at the time of the accident or when it is impossible to perform a reproduction experiment. In this study, The computational fluid dynamics analysis for nitrogen asphyxiation accident generated by defect of building structural between diffusion outlet and cooling tower was performed to determine the inflow path of the suffocation gas, death possibility by concentration of suffocation gas and predicted the time of death due to the accident using 3D modeling and FLACS program. We can quantify diffusion concentration of asphyxiation gas and predict mechanism of death occurrence by accident and evaluate the consequence Analysis through this study. In the future, This method can be widely used in the field of gas safety by improving the reliability and validity of the analysis.
An extensive range of pumping facilities are employed in the regional water supply system in metropolitan areas, and optimization and the systematic combination of the Pump facilities have direct bearing on the stability and economy of the water supply system concerned. These systems must be able to guarantee stability, efficiency and offer high reliability. Preparation of metropolitan area regional water supply system construction project must include a basic plan which takes into account the suitability of pumping facilities to be used, the environment in which facilities will be installed, man-power requirements and basic operational and management policies. This paper contains over-all analysis of the management of metropolitan area regional water supply systems as like Jayang, Paldang 1st pump station. In the study, it aims to prepare counterplan which will be operated and managed the pump upon the operational conditions and to suggest the proposal for water facilities codes to decides total head of pump in Korea.
