Management of an Intra-abdominal Fluid Collection after Gastric Cancer Surgery (위암 수술 후 발생한 복강 내 체액 저류의 치료)

  • Purpose: Intra-abdominal fluid collection is one of the risk factors associated with postoperative morbidity or mortality for patients who undergo gastric cancer surgery. The aim of this study was to analyze the clinicopathological characteristics of the patients with postoperative intra-abdominal fluid collection and to identify the indications for inserting a percutaneous drain (PCD) in patients with intra-abdominal fluid collection. Materials and Methods: Among the 1,277 patients who underwent operations for gastric cancer at Seoul National University Hospital between April 2005 and July 2006, the data of 117 patients with an intra-abdominal fluid collection were reviewed. Results: The number of patients' with pathologic stage I, II, III and IV disease was 42 (36.8%), 23 (20.2%), 16 (14%) and 33 (28.9%), respectively. Forty-three patients (36.3%) underwent PCD insertion and the other 43 patients received conservative management. A univariate analysis of multiple clinical variables revealed that age, gender, diabetes, liver disease, lymph node dissection, the pathologic stage and the body mass index (BMI, $kg/m^2$) were not significantly associated with PCD insertion (P>0.05). However, the univariate analysis showed that two characteristics were associated with a significantly high incidence of PCD insertion: a diameter of an intra-abdominal fluid collection greater than 4 cm and infectious signs such as leukocytosis, fever and bacteremia. Conclusion: About two thirds of the intra-abdominal fluid collections after surgery for gastric cancer were managed with only conservative method without other morbidities of mortality. Surgeons should consider performing PCD insertion if the largest diameter of an intra-abdominal fluid collection is over 4 cm or if infectious signs are seen.

Performance Comparison of Two Wind Turbine Generator Systems Having Two Types of Control Methods

  • The purpose of this paper is to gain a greater understanding of the performance of practical wind turbine generating systems with differing output power controllers and controlling means for wind turbine speed. Subjected wind turbines, both equipped with an asynchronous power generator, are located at two sites and are defined as wind turbine A and wind turbine B in this study, respectively. Their performance differences are examined by measuring wind speed and electric parameters. The study suggests that both wind turbines have a clear linkage between current and output power fluctuations. Comparison of the fluctuations to wind speed fluctuation, although they are triggered primarily by wind speed fluctuation, clearly indicates the specific behaviors inherent to the respective turbine control mechanisms.

  • Association is an important contribution to the overall hydrogen bonding in surfactant systems, especially in systems of colloidal and biological interest. Amphiphile systems, especially micelle and microemulsion systems, showed highly non-ideal behavior due to the intermolecular association and intramolecular association. The objective of this research is to present a lattice fluid equation of state that combines the quasi-chemical nonrandom lattice fluid model with modified Veytsman statistics for intra + inter molecular association to calculate phase behavior for mixture containing surfactant systems. The present EOS could correlate the literature data well for mixtures containing nonionic surfactant systems.

  • Several papers on respiratory and critical care published from March 2012 to February 2013 were reviewed. From these, this study selected and summarized ten articles, in which the findings were notable, new, and interesting: effects of high-frequency oscillation ventilation on acute respiratory distress syndrome (ARDS); safety and efficacy of hydroxyethyl starch as a resuscitation fluid; long-term psychological impairments after ARDS; safety and efficacy of dexmedetomidine for sedation; B-type natriuretic peptide-guided fluid management during weaning from mechanical ventilation; adding of daily sedation interruptions to protocolized sedations for mechanical ventilation; unassisted tracheostomy collar of weaning from prolonged mechanical ventilations; and effects of nighttime intensivist staffing on the hospital mortality rates.

Treatment of Acute Renal Failure in Neonate (신생아 급성 신부전의 치료)

  • Acute renal failure (ARF) is common in the neonatal period, however, there are no uniform treatment strategies of ARF. The main treatment strategies are conservative management including medical treatment and the renal replacement therapy. Because ARF in the newborn is commonly acquired by hypoxic ischemic injury and toxic insults, removal of all the offending causes is important. Aminoglycoside, indomethacin, and amphotericin-B are the most common nephrotoxic drugs of ARF. To relieve the possible prerenal ARF, initial fluid challenge can be followed by diuretics. If there is no response, fluid restriction and correction of electrolyte imbalance should begin. Adequate nutritional support and drug dosing according to the pharmacokinetics of such drugs will be difficult problems. Renal replacement therapies may be provided by peritoneal dialysis, intermittent hemodialysis, or hemofiltration. New promising agents, bioartificial kidney, and stem cell will enable us to extend our therapeutic repertoire.

  • Management of indoor air quality of underground subway station is an important issue since the limited natural ventilation, limited sunshine incoming, and highly moistured atmosphere. The improvement in IAQ of platform is expected because most stations were installed with platform screen door currently, however, the poor air quality in tunnel might be affecting subway cabin indoor. In this study, we developed the air quality assessment model based on computational fluid dynamics. The geometry of air ventilation unit, seat, LCD monitors, and passengers were modeled using commercial software (Design Modeler) and fluid pattern and pollutants trajectories were analyzed by using CFX. We predicted the thermal comfort by predicted mean vote (PMV), distribution of CO2 and PM10 concentration. It is expected that this model can be used for the performance test of air cleaners which are under development.

Thermal Characteristics of Graphite Foam Thermosyphon for Electronics Cooling

  • Graphite foams consist of a network of interconnected graphite ligaments and are beginning to be applied to thermal management of electronics. The thermal conductivity of the bulk graphite foam is similar to aluminum, but graphite foam has one-fifth the density of aluminum. This combination of high thermal conductivity and low density results in a specific thermal conductivity about five times higher than that of aluminum, allowing heat to rapidly propagate into the foam. This heat is spread out over the very large surface area within the foam, enabling large amounts of energy to be transferred with relatively low temperature difference. For the purpose of graphite foam thermosyphon design in electronics cooling, various effects such as graphite foam geometry, sub-cooling, working fluid effect, and liquid level were investigated in this study. The best thermal performance was achieved with the large graphite foam, working fluid with the lowest boiling point, a liquid level with the exact height of the graphite foam, and at the lowest sub-cooling temperature.

  • The purpose of this article is to diagnosis all aspects of waste pneumatic transportation system, in order to provide everyone interested in the subject with a optimal design and management of the system. The results of the analyzed inlet boxes were especially designed for customers' convenience and automatically operation, side air intake, ensure good wear. And key of pipelines were showed design standard as degree of an angle. Also, the system is mostly equipped with automatic control and watch conducted inside the waste collection facilities, and it will cut down excessive manpower and be able to be managed by least trained personnel.

  • Hiccup is one of common symptoms clinically and caused by various etiologies. We have tried researching the effect of herbal medicines(Taeumjowi-tang) to treat a Taeumin patient with severe hiccup, who is presumed cerebral vascular disease, and evaluated hiccup's a time in a minute and hiccup's total time in a day. The symptoms, not only the patient's hiccup, also his general condition including the dysphagia and right side hemiparesisis, are improved considerably. The constitutional treatment with Taeumin herbal medicines(Taeumjowi-tang) may have an effect on the management of hiccup by control of Qi and Fluid Metabolism.

Design Optimization of Centrifugal Pump Impellers in a Fixed Meridional Geometry using DOE

  • This paper reports on an investigation (using RSM with commercial CFD software) of the performance characteristics of the impeller in a centrifugal pump. Geometric parameters of vane plane development were defined with the meridional shape and frontal view of the impeller. The parameters are focused on the blade-angle distributions through the impeller in a fixed meridional geometry. For screening, a $2^k$ factorial design has been used to identify the important design parameters. The objective functions are defined as the total head rise and the total efficiency at the design flow-rate. From the $2^k$ factorial design results, it is found that the incidence angles and the exit blade angle are the most important parameters influencing the performance of the pump.
