Comparison of the Level and Side Effects of Spinal Anesthesia with Hyperbaric Bupivacaine in the Supine, Lateral, and Prone Positions (고비중 Bupivacaine 척추마취 후 체위에 따른 마취수준과 부작용의 비교)

  • Purpose: This study attempted to test whether there are differences in the level and hemodynamic side effects (blood pressure, heart rate, $O_2$ saturation), and nausea & vomiting of spinal anesthesia using hyperbaric bupivacaine according to position (supine, lateral, and prone positions) in orthopedic surgery patients who received podiatric surgery under spinal anesthesia. Methods: This study was conducted with 53 patients who had received orthopedic surgery under spinal anesthesia at I General Hospital. Data were analyzed using SPSS 20.0 through repeated-measures ANOVA, post-hoc test, Chi-test, and Fisher's exact test. Results: The change of position after spinal anesthesia with hyperbaric bupivacaine caused a change in the level of spinal anesthesia (F=12.768, p<.001). However, no difference of blood pressure, heart rate, $O_2$ saturation and nausea and vomiting caused by the change in anesthesia level was observed, and in prone position, drug was administered for the correction of side effects. Conclusion: As expected, recognizing that there can be a change in the level of spinal anesthesia after the change of position in surgical patients, nurse anesthetists should monitor their conditions carefully and continuously.

Effects of a Training Program on Infection Prevention for Staff of Child Daycare Centers (보육시설 방문교육프로그램이 보육교사의 영유아 감염에 대한지식, 태도 및 감염예방행위에 미치는 효과)

  • Purpose: To identify the effects of a training program on the knowledge of, attitudes towards child infection, and infection prevention behaviors in staff of daycare centers. Methods: A nonequivalent control group pretest-posttest design study was conducted with 34 staff of 6 daycare centers over 3 months. The staff in the 3 centers designated as the experimental centers received the training program weekly for 8 weeks. The program included on-site education for one and half hours and monitoring of infection prevention behaviors. Knowledge and attitudes of the staff were measured, and their infection prevention behaviors were observed and recorded by research assistants before and after the program. $X^2$-test, t-test, Fisher's exact test, and Mann-Whitney U-test with SPSS Win program were used to analyze the data. Results: The group of staff who received the training program recorded higher scores in knowledge, attitudes, and their infection prevention behaviors compared with staff in the control group. Conclusion: This finding suggests that the training program had a significant impact on knowledge, attitude, and infection prevention behaviors of staff which could prevent child infection in daycare centers. Nurses need to be involved in daycare centers for the health of the children, and they could intervene effectively in child infections by using this program.

Effect of Koryo Hand Therapy on Menopausal Symptoms and Attitude in Perimenopausal Women (수지요법이 폐경증상과 태도에 미치는 효과)

  • Purpose: This study was to examine the effects of Koryo Hand Therapy on menopausal symptoms and attitude in permenopausal women. Methods: The research design was a nonequivalent controlgroup pretest-posttes. The subjects consisted of 45 women between the ages of 40 and 60 years in the Gyeonggi-do, Korea. There were 23 women in the experimental group and 22 in the control group. In this study, the experimental group received KHT of three times a week, for a total of 8weeks. The instruments for this study were to measure the effects of KHT were the menopausal symptoms scale and attitudes to the menopause(Maoz, Downty, Antonovsky & Wijsenbeek, 1970). The data were analyzed by $X^2$-test and Fisher's Exact Test, Mann-Whitney U-test, Wilcoxon signed rank test procedures, using SPSS/Win PC 13.0. Results: For menopausal symptoms, there were statistically significant differences between experimental group and control group. For menopausal attitudes, there were no statistically significant differences between groups. Conclusions: This result showed that KHT was effective in decreasing menopausal symptoms in perimenopausal women.

Effects of Auricular Acupressure Therapy on Back Pain and Lower Abdominal Pain in Female Undergraduate Students during Menstruation (이압요법이 여대생의 월경시 요통과 하복부 통증에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of auricular acupressure therapy on back pain and lower abdominal pain in female undergraduate students during menstruation. Methods: The study used a quasi-experimental design. Data was collected from March 15th to June 31st, 2011. The participants were 20 female university students for the experimental group, and 22 female university students for the control group. visual analogue scale (VAS) with the opening records was used to assess back pain and lower abdominal pain. Data was analyzed using the ${\chi}^2$-test, Fisher's exact test, Mann-Whitney U test, and Wilcoxon signed rank test using the SPSS/WIN 11.0 program. Results: There were statistically significant differences in back pain and lower abdominal pain between the two groups. Auricular acupressure therapy decreased back pain and lower abdominal pain in the experimental group compared to the control group. Conclusion: The study results show that auricular acupressure therapy is effective in improving back pain and lower abdominal pain in female university students during menstruation. Therefore, auricular acupressure therapy can be considered a useful nursing intervention to promote improvements in back pain and lower abdominal pain related to menstruation.

The Effects of Multimedia Diet Education on Nutritional Status in Stomach Neoplasms Patient with Gastrectomy (멀티미디어를 이용한 식이교육이 위절제술을 받은 위암환자의 영양상태 개선에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effect of diet education on nutritional status in patients with gastrectomy. Method: This study was designed with a nonequivalent control group pre-post test. Ten patients were assigned to the experimental group and fifteen were assigned to the control group. The experiemental group was given the diet education by multimedia including complications after gastrectomy, the way of how to prevent these complications, precautions that patients need to be follow, and food that patients should and shouldn't eat. Data were analysed with a $x^2$-test(Fisher' exact test) and Independent sample t-test, Repeated measures ANOVA, using SPSS WIN 15.0 program. Results: There were significant differences in weight, daily caloric intake, knowledge score between experimental and control group. However, there was a no significant difference in blood profile(albumin, total protein, hemoglobin) between two groups. Conclusion: This study shows that the diet education with multimedia could improve nutritional status for the patients who had gastrectomy.

Effects of Empowerment Education Program for Hemodialysis Patients on Self-efficacy, Self-care Agency, Self-care Activities and Physiologic Parameters (능력증강 교육프로그램이 혈액투석환자의 자기효능감, 자가간호역량, 자가간호행위 및 생리적 지표에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of empowerment education program on self-efficacy, self-care agency, self-care activities and physiologic parameters among hemodialysis patients. Method: Nonequivalent control group pretest-posttest design was used. There were 36 subjects in the experimental group and 35 subjects in the control group. The empowerment education program was administered for 4 weeks (1 time per week). Data were collected pre-treatment and post treatment. The $X^2$-test or Fisher's exact test, ANCOVA, t-test and paired t-test were performed to determine the group differences, using SPSS 14.0 version. Results: Self-efficacy, self-care agency, self-care activities and weight variation significantly improved after the empowerment education program in hemodialysis patients. Conclusion: The empowerment education program for hemodialysis patients was effective to improve the degree of self-efficacy, self-care activities and self-care agency and to decrease interdialytic weight variation.

Effects of Foot Bath Therapy on the Symptom Intensity, Distress, and Interference with Usual Activities due to Chemotherapy-Induced Peripheral Neuropathy in Patients with Metastatic and Recurrent Cancer (족욕요법이 항암화학요법을 받은 전이·재발암 환자의 말초신경병증으로 인한 증상강도, 고통정도 및 일상생활 방해정도에 미치는 효과)

  • Purpose: This study aimed to investigate the effects of foot bath therapy on the symptom intensity, distress, and interference with usual activities due to chemotherapy-induced peripheral neuropathy (CIPN) in patients with metastatic and recurrent cancer. Methods: Foot bath therapy was administered to the experimental group for >8 sessions in 2 weeks, and the chemotherapy-induced peripheral neuropathy assessment tool (CIPNAT) was used to measure its effects on the symptom intensity, distress, and interference with usual activities due to CIPN. SPSS was used to perform data analyses including descriptive statistics, chi-square test, Fisher's exact test, t-test, paired t-test, and repeated measures ANCOVA. Results: A statistically significant difference in the variation of the symptom intensity, distress, and interference with usual activities due to CIPN was observed between the two groups; however, a statistically insignificant difference was observed between the groups and time of interaction. Conclusion: Foot bath therapy can be used as a simple and effective clinical or home care nursing intervention to improve the symptom intensity, distress and interference with usual activities due to CIPN.

Effect of a simulation-based program for post-operative care of emergency patients (응급환자의 수술 후 관리를 위한 시뮬레이션기반 교육프로그램의 효과)

  • Purpose: This study aimed to confirm the effects of a simulation-based program on knowledge and clinical performance in the post-operative management of emergency patients. Methods: This was a pre- and post-research design with a nonequivalent control group and randomly sampled 29 experimental and control groups, respectively from nursing department juniors for 4 weeks from September of 2014. The experimental group received lectures, team study, team simulation, and debriefing in post-operative management of simulation-based emergency patients and control group conducted in the traditional lecture-type setting. Educational learning effects were measured by using the knowledge and clinical performance measurement tools of 15 and 20 items, respectively. Data were analyzed by using the SPSS program, including frequency, ratio, and results from the Chi-square test, Fisher's exact test, Kolmogorov-Smirnov test, t-test. Results: Our research results indicate that, the experimental group showed significantly higher knowledge and clinical performance score compared with the control group. Conclusion: We confirmed that education on post-operative management of simulation-based emergency patients was an effective educational method to improve the knowledge and clinical performance of nursing students.

Effects of Family-Participated Cardiac Rehabilitation Program on Self-Efficacy, Health Behavior Compliance, and Family Support of Patients with Percutaneous Coronary Intervention (가족참여 심장재활 프로그램이 경피적 관상동맥중재술 환자의 자기효능감, 건강행위이행 및 가족지지에 미치는 효과)

  • Purpose: The purpose of this study was to develop a family-participated cardiac rehabilitation program and to test the effects of the program on self-efficacy, health behavior compliance, and family support for patients with percutaneous coronary intervention. Methods: The design of this study was a nonequivalent control group non-synchronize design. There were 30 participants in the experimental group and 29 in the control group. The program consisted of six consecutive cardiac rehabilitation education and counselling sessions for 5 weeks. Data were analyzed using $x^2-test$, Fisher's exact test, and t-test using the SPSS program. Results: Self-efficacy, health behavior compliance, and family support scores were significantly increased in the experimental group compared to those in the control group. Conclusion: These results suggest that this program may improve self-efficacy, health behavior compliance, and family support in patients with percutaneous coronary intervention. Therefore, this program in which family members were included in supporting patients' adherence to health behaviors is recommended for use in clinical fields for the cardiac rehabilitation.

Effects of Aroma Inhalation Therapy on Pain in Patients Following a Tonsillectomy (아로마 요법이 편도선 절제술 환자의 통증에 미치는 영향)

  • Purpose: This study was done to examine the effects of aroma inhalation therapy on pain post tonsillectomy. Method: A non-equivalent control group pre- and post-test design was used. The research instruments used in this study were a pain perception measurement and vital signs (systolic & diastolic blood pressure and pulse rate levels). Twenty-five patients in the experimental group went through aroma inhalation therapy for ten minutes after the tonsillectomy. The aroma inhalation therapy used was a blended oil, a mixture of Lavender and Loman chamomile in the ratio of 2:1. The 25 patients in the control group did not receive the therapy. Before and after the experiment, both groups were tested for pain (pain perception and vital signs). Collected data were processed with the SPSS WIN. Ver. 14.0 program and analyzed using frequencies, percentages, ${\chi}^2$-test, Fisher's exact test, t-test and ANCOVA. Results: Pain perception of patients was not significantly reduced. However systolic and diastolic blood pressure and pulse rate were significantly reduced. Conclusion: The results of the study indicate that aroma inhalation is a partially effective nursing intervention to reduce the post-operative tonsillectomy pain.
