Case Report: Miller Fisher Syndrome (한방치료로 호전된 Miller Fisher 증후군 환자 증례보고)

  • Objectives: To describe the effect of traditional Korean medicine and treatment on Miller Fisher syndrome (MFS).Methods: A 54-year-old woman diagnosed with MFS presented with eyeball dysfunction, diplopia, vertigo, right facial palsy, and back dysesthesia. The patient had been treated with immunoglobulin for 21 d, but her symptoms failed to improve. Thus, herbal medicine, acupuncture, electropuncture, pharmacopuncture, and moxibustion were added. Length of eyeball movement, distance that the patient recognize double images in the eyes and Visual Analogue Scale (VAS) are measures for the syndrome.Results: The symptoms of the patient considerably improved, with the return of eyeball movement to normal and disappearance of diplopia.Conclusions: The results suggest that Korean medicine may be an effective therapy for MFS.

Accelerated life test plan under modified ramp-stress loading with two stress factors

  • Accelerated life tests (ALTs) are frequently used in manufacturing industries to evaluate the reliability of products within a reasonable amount of time and cost. Test units are subjected to elevated stresses which yield quick failures. Most of the previous works on designing ALT plans are focused on tests that involve a single stress. Many times more than one stress factor influence the product's functioning. This paper deals with the design of optimum modified ramp-stress ALT plan for Burr type XII distribution with Type-I censoring under two stress factors, viz., voltage and switching rate each at two levels- low and high. It is assumed that usage time to failure is power law function of switching rate, and voltage increases linearly with time according to modified ramp-stress scheme. The cumulative exposure model is used to incorporate the effect of changing stresses. The optimum plan is devised using D-optimality criterion wherein the ${\log}_{10}$ of the determinant of Fisher information matrix is maximized. The method developed has been explained using a numerical example and sensitivity carried out.

The effect of the tongue in groove technique on the nasolabial angle and nasal tip projection

  • Background: The tongue in groove technique (TIG) is a useful technique for the correction of the nasal tip projection and the nasolabial angle. The purpose of this study was to determine the utility of this technique for nasal tip rotation and projection correction in the Iranian society. Methods: This is a retrospective clinical trial study of 20 patients undergoing open septo-rhinoplasty using TIG technique from January 2017 to August 2019 at the oral and maxillofacial unit of Bu Ali Hospital and private sector. Preoperative and postoperative profile view photographs were compared to assess the changes in tip projection and rotation. Results: Fifteen patients (75%) had normal angular size, and 5 of them (25%) were not within the normal range after the surgery. The Fisher exact test showed that this success was statistically significant (P = 0.006). Ten patients (50%) had normal projection size, postoperatively. The Fisher exact test showed that this effect was statistically significant (P < 0.01) Conclusion: The study demonstrated the benefit of TIG on the correction of nasal tip projection and rotation.

The Effect of Abdominal Massage on Sleep Colonoscopy Subjects (수면 대장내시경 검진자에게 시행한 복부마사지의 효과)

  • The purpose of this study is to investigate the effect of abdominal massage, on abdominal bloating and the recovery of bowel movement after sleep colonoscopy. The study design is the time difference before and after the non-equality control. The subjects were 22 patients each in experimental group and control group who recieved sleep colonoscopy at the G hospital in C city, The data collection period was from June 1, 2018 to August 25, 2018, and the control group's data were first collected with time difference from the collection of experimental group's data to prevent the spread of the experiment. Data were analyzed using IBM SPSS 24.0 and tested by independent sample t-test, $x^2-test$ and Fisher's exact probability test to verify the homogeneity of general characteristics of the subjects. Shapiro-Wilk was used to verify the normality of the abdominal circumference changes in the experimental and control groups. Mann-Whitney U test was used to verify the pre-homogeneity of the abdominal circumference of the subjects. The experimental results were verified by Fisher's exact probability test, Friedman test and Mann-Whitney U test. These results suggest that the abdominal massage can be used as a nursing intervention after sleep colonoscopy by reducing the gas discharge time($x^2=19.75$, p<.001) and abdominal bloating($x^2=29.93$, p<.001).

A study on the minimal acupuncture using a modified guide tube for Korean female participants familiar with acupuncture therapy (침치료에 익숙한 한국의 여자 임상시험 피험자를 대상으로 한 침관 변형을 통한 Minimal acupuncture 경험)

  • Purpose: To find objective validity of minimal acupuncture using a modified guide tube as a method to verify the effect of acupuncture. Methods: Subjects of this study were participants of a randomized clinical trial which was practiced to verify the effect of acupuncture on postmenopausal hot flash. There were 2 arms of treatment, one's for active acupuncture group(AG) with manipulation and De qi, the other's for minimal acupuncture group(MG) using a modified guide tube which was designed to give minimal stimulation to the patient. After 8 weeks' treatment followed by 4 weeks' observation, participants were asked to fill in self-report forms on their education, previous experiences of acupuncture, original idea on the efficacy of acupuncture and recognition of blinding. Results: 19 participants of 26 in AG and 19 of 26 in MG completed the self-report form. There was no difference between AG and MG in their education(p=0.5976, Fisher exact test) and previous experiences of acupuncture(p=0.9999, Fisher exact test). In their original ideas on the efficacy of acupuncture, most of AG and MG respondents have been thought that acupuncture is very effective or quite effective, and there was no difference between 2 groups(p=0.5065). 16 of 19 in AG and 14 of 19 in MG believed that they underwent more effective way, there was no statistical difference between 2 groups(p=0.6928, Fisher exact test). Participants believed in undergoing more effective way not because they recognized treatment method, but they were satisfied with the effectiveness of treatment retrospectively. Conclusion: As the result of this study, these authors suggest that minimal acupuncture using a modified guide tube is acceptable as a method to verify the effect of acupuncture in acupuncture-familiar Korean culture. Also further studies on the stimulation-specific effect of minimal acupuncture are demanded.

The Prediction of the Expected Current Selection Coefficient of Single Nucleotide Polymorphism Associated with Holstein Milk Yield, Fat and Protein Contents

  • Milk-related traits (milk yield, fat and protein) have been crucial to selection of Holstein. It is essential to find the current selection trends of Holstein. Despite this, uncovering the current trends of selection have been ignored in previous studies. We suggest a new formula to detect the current selection trends based on single nucleotide polymorphisms (SNP). This suggestion is based on the best linear unbiased prediction (BLUP) and the Fisher's fundamental theorem of natural selection both of which are trait-dependent. Fisher's theorem links the additive genetic variance to the selection coefficient. For Holstein milk production traits, we estimated the additive genetic variance using SNP effect from BLUP and selection coefficients based on genetic variance to search highly selective SNPs. Through these processes, we identified significantly selective SNPs. The number of genes containing highly selective SNPs with p-value <0.01 (nearly top 1% SNPs) in all traits and p-value <0.001 (nearly top 0.1%) in any traits was 14. They are phosphodiesterase 4B (PDE4B), serine/threonine kinase 40 (STK40), collagen, type XI, alpha 1 (COL11A1), ephrin-A1 (EFNA1), netrin 4 (NTN4), neuron specific gene family member 1 (NSG1), estrogen receptor 1 (ESR1), neurexin 3 (NRXN3), spectrin, beta, non-erythrocytic 1 (SPTBN1), ADP-ribosylation factor interacting protein 1 (ARFIP1), mutL homolog 1 (MLH1), transmembrane channel-like 7 (TMC7), carboxypeptidase X, member 2 (CPXM2) and ADAM metallopeptidase domain 12 (ADAM12). These genes may be important for future artificial selection trends. Also, we found that the SNP effect predicted from BLUP was the key factor to determine the expected current selection coefficient of SNP. Under Hardy-Weinberg equilibrium of SNP markers in current generation, the selection coefficient is equivalent to $2^*SNP$ effect.

Convergence Effect of Horticulture Activity Program on Self-esteem and School Life Adaptation of Multicultural Adolescent. (원예활동프로그램이 다문화청소년의 자아존중감과 학교생활적응에 미치는 융합효과)

  • The purpose of this study was to investigate the effect of horticulture activity program on self-esteem and school life adaptation of multicultural adolescent. A non-equivalent control group pre-post test design with experimental group (n=39) and control group (n=36) was used. The program was conducted from Jan. 2 to Feb. 28, 2017, and the questionnaire was conducted before and after the program. Collected data was analyzed by SPSS(Ver, 24.0), conducting ${\chi}^2-test$, Fisher's exact test, t-test, paired t-test. There were statistically significant increase in self-esteem(t=2.41, p=.021) and school life adaptation(t=3.85, p=.000) by difference between a pretest and a posttest in the experimental group compared to that in the control group. Therefore, the horticulture activity program for multicultural adolescent have a significant effect for better performance of self-esteem and school life adaptation. Also, It was confirmed that the program used for improving adaptability of multicultural adolescent.

Effect of Music Therapy on Vital Signs, Anxiety, Cortisol and Pain of Cataract Surgery Patients in Elderly (음악요법이 노인백내장 수술환자의 활력징후, 불안, 코티졸 및 통증에 미치는 효과)

  • Purpose of this study was to investigate the effect of music therapy on vital signs, anxiety, cortisol, and pain of Cataract Surgery Patients in elderly. Non equivalent control group pre-post test design was adopted. The number of subjects consists of 41 elderly having cataract surgery, 21 in the experimental group and 20 in the control group. Data were analyzed by $x^2$ test, Fisher's exact test, independent sample t-test, and Mann-Whitney U test. Results were: 1) there was no difference between two groups in their vital signs changes 2) anxiety, cortisol in the saliva, and pain of the experimental group decreased more significantly than those of the control group. This study confirmed the ability of music therapy to relieve pain and anxiety in cataract surgery, and suggested that music therapy could be used effectively in various interventions for the elderly.

The Effect of Aroma Inhalation Method on the Preoperative Anxiety among Patients with Upper and Lower Limbs Surgery (향기요법이 상하지 수술 환자의 수술 전 불안에 미치는 효과)

  • The study was done to analyze the effects of aroma inhalation method on preoperative anxiety of upper and lower surgical patients. The research design was a nonequivalent control group non-synchronized design. The subjects were a total 60 patients of each group 30 patients that were operated on under general anesthesia for upper and lower limbs surgery. The tool of the Amsterdam preoperative anxiety information scale(APAIS), systolic and diastolic blood pressure, and pulse rate levels was measured the day before surgery. The data were analyzed by the $x^2$ test Fisher's exact test, paird t-test, and the independent t-test using SPSS 20.0. Study result indicated that Lavender aroma therapy had the effect on reduction of anxiety before surgery and reduction of blood pressure, and pulse rate levels. Therefore, the study result could be used as a scientifical data that can be applied to the nursing interventions that use the aroma inhalation method and contributing to development of the holistic nursing care.
