The Relationship between Sensory Processing Abilities and Gross and Fine Motor Capabilities of Children with Cerebral Palsy

  • PURPOSE: The purpose of this study was to investigate the difference and relationship between sensory processing abilities, gross motor and fine motor capabilities in children with cerebral palsy. METHODS: 104 children with cerebral palsy participated in the study. Sensory processing abilities of the subjects were measured by Short Sensory Profile (SSP). Gross and fine motor abilities were each measured using the Gross Motor Function Classification System (GMFCS) and Manual Ability Classification System (MACS), respectively. RESULTS: There were significant correlations between SSP level and GMFCS (R=.72, p<.00) or MACS (R=.77, p<.00) levels. Significant differences were showed each gross motor (p=.01) and fine motor level (p=.00) among sensory processing level of children. In addition, sub-items of sensory processing as Tactile sensitivity, Movement sensitivity, Auditory filtering and Low energy/Weak were significantly were showed significant correlations gross motor and fine motor level (p=.01). Also, multiple regression result was showed that as MACS level and GMFCS level were higher, the SSP total score was higher all of participants (adjusted $R^2=.62$). CONCLUSION: Sensory processing abilities of children with cerebral palsy were related with gross motor and fine motor capabilities. Also gross motor and fine motor capabilities are as higher, the sensory processing skill was well of cerebral palsy.

고유-전정감각 중심의 감각통합 중재가 초등학교 저학년 아동의 글씨쓰기와 소운동에 미치는 영향 (The Effect of Sensory Integrative Intervention Focused on Proprioceptive-Vestibular Stimuli on the Handwriting and Fine Motor Function in Lower Grade Elementary School Children)

  • 목적 : 본 연구는 고유-전정감각 중심의 감각통합 중재가 초등학교 저학년 아동의 글씨쓰기와 소근육 능력 향상에 영향을 미치는지 알아보고자 하였다. 연구방법 : 본 연구는 부산 소재 초등학교에 재학 중인 저학년 아동 8명을 대상으로 단일집단 사전-사후 설계(one group pretest-posttest design)를 사용하였다. 2016년 5월에서 10월까지 주 2회, 총 14회기의 고유-전정감각 중심의 감각통합중재를 실시하였으며 글씨쓰기와 소근육 능력에 대한 측정은 한글자모쓰기검사와 Bruininks-Oseretsky Test of Motor Proficiency (BOTMP)의 Fine motor영역 검사를 실시하였다. 결과 : 중재 후 한글자모쓰기검사 결과 자음쓰기와 전체 총점은 통계적으로 유의한 차이를 나타냈으며 모음쓰기는 통계적으로 유의한 차이는 없었으나 점수의 향상을 나타내 글씨쓰기 능력의 향상을 나타냈다. BOTMP-Fine motor결과 Response Speed항목은 통계적으로 유의한 차이를 나타냈으며 Visual-Motor control항목은 중재 전후로 점수의 증가가 있었으나 통계적으로 유의한 차이는 없었다. 결론 : 본 연구는 고유-전정감각 중심의 감각통합 중재가 초등학교 저학년 아동의 글씨쓰기와 소근육 능력 향상에 긍정적 영향을 미치는 것을 확인하였다.

한국형 베일리 영유아 발달검사 3판 소근육 운동척도 예비 연구 (A Preliminary Study for Standardizing the Fine Motor Scale of the Korean-Bayley Scales of Infant and Toddler Development, Third Edition)

  • The purpose of the present study was to conduct a preliminary study for developing a Korean version of the Fine Motor Scale of the Bayley Scales of Infant and Toddler Development, $3^{rd}$ Edition(Bayley-III). The subjects for this study included 313 Korean infants and toddlers aged from 16 days to 42 months and 15 days. The results of this study were as follows. Firstly, the analysis of the degree of item difficulty indicated that the arrangement of the items corresponded with child development by age. Secondly, the analysis of the inter-item consistency, the test-retest reliability and the inter-rater reliability revealed a high degree of reliability of the scale. Thirdly, the Fine Motor Scale of K-Bayley-III score showed a significant high correlation with the Motor composite score but a significant moderate correlation with the Language composite score, providing evidence of its convergent and discriminant validity. This study demonstrated that the preliminary version of the K-Bayley-III Fine Motor Scales can be applied to assess the development of fine motor skills of Korean infants and toddlers.

편마비 환자에서 건측 수부의 적극적 미세 운동 제공을 통한 기능적 독립성의 증가에 관한 연구 (The study for Functional Independence Measure score by fine motor exercise of unaffected hand in the hemiplegic patients)

  • This study is designed to evaluate the effects of aggressive fine motor exercise of unaffected hand in the hemiplegic patients. The 36 hemiplegic patients were classified into two groups: The experimental 18, who were treated by aggressive fine motor exercise in unaffected hand and the control, 18, who were treated by conventional exercise program. We evaluate the effects of aggressive fine motor exercise by Functional Independence Measure(FIM) and Jebsen Hand Function Test(JHFT) after 6 weeks program. After exercise program, the experimental group showed score change from $43.05{\pm}15.68$ to $58.05{\pm}17.12$ in FIM score and from $24.12{\pm}22.03$ to $55.44{\pm}21.50$ in Jebsen, and the control showed score change from $51.11{\pm}22.61$ to $57.50{\pm}23.66$ in FIM score and from $40.88{\pm}21.17$ to $52.77{\pm}19.42$ in Jebsen. The experimental group had significantly higher score in FIM and JHFT than that of the control group. The aggressive fine motor exercise is beneficial in unaffected hand in the hemiplegic patients.

학령 전기 아동의 글씨 쓰기에 영향을 미치는 요인에 관한 연구 (A Study on Factors Influencing Handwriting of Preschool Children)

  • Purpose: This study investigated the relationships of handwriting legibility and perceptual-motor skills, and handwriting speed and perceptual-motor skills. And identified the predictors that most affect the handwriting of preschool children. Methods: Twenty-three typically developing preschool aged children (mean age: 68.61 months, SD=2.04) were selected through the Korean-Denver Developmental Screening Test-2(K-DDST-2). The children were tested with regard to handwriting legibility, visual perception, visual-motor integration and fine-motor coordination. Results: First, a significant relationship was not found among handwriting legibility, visual perception, visual-motor integration and fine-motor coordination. Second, a significant relationship was found among handwriting speed, visual perception and fine-motor coordination. Third, stepwise multiple regression analyses showed that general visual perception were significant predictors for handwriting speed. Conclusion: Occupational therapists should evaluate children's visual perception levels utilizing a standardized test, and focus on general visual perception in order to improve handwriting skill(speed). Also, occupational therapists are expected to play an important role in the management and treatment of children's handwriting skills.

그룹감각통합치료가 학령전기 지적장애 아동의 소근육 기능, 상호작용 및 놀이에 미치는 영향 (Effect of Sensory Integration Group Therapy on Fine Motor, Social Interaction and Playfulness of Preschool Children With Intellectual Disabilities)

  • 목적 : 본 연구는 학령전기 지적장애아동에게 제공한 그룹감각통합치료가 소근육 기능, 상호작용 및 놀이에 미치는 영향을 알아보고자 한다. 연구방법 : 지적장애 등록을 한 만3~5세 아동 4명을 대상으로 주 1회 90분씩, 총 12회기 실시하였다. 그룹감각통합치료는 평행놀이와 또래놀이 중심의 대근육 활동과 주제에 맞는 미술활동과 감각놀이로 구성하였다. 결과측정은 Erhardt Developmental Prehension Assessment(EDPA)와 또래놀이 상호작용 평점척도(Penn Interactive Peer Play Scale; PIPPS), 놀이다움(The Test of Playfulness; ToP)를 사용하여 대상자의 사전사후 소근육 기능과 상호작용, 놀이를 평가하였다. 측정 결과는 윌콕슨 순위 검정(Wilcoxon matched-pair signed rank test)을 사용하여 소근육 기능과 상호작용, 놀이의 중재 전후로 차이를 비교하여 분석하였다. 결과 : 그룹감각통합치료 후, 소근육 기능은 통계학적으로 유의한 차이를 보이지 않았으나, 상호작용과 놀이에서는 유의한 차이가 있었다(p<.05). 결론 : 그룹감각통합치료는 상호작용과 놀이능력 향상에 유용한 치료적 접근방법이라는 것을 알 수 있었으며, 지속적인 그룹 활동을 통하여 일상생활에서의 다양한 상황에 대한 일반화가 진행되길 기대한다.

발달지연 아동의 시-운동 협응 및 소운동 협응 기능 향상을 위한 과제지향의 중재(task-oriented intervention) 적용: 단일대상연구 (Task-Oriented Intervention for Improvement of Visuomotor Coordination and Fine Motor Coordination Functions in Developmental Delayed Child : A Single-Subject Design)

  • 목적 : 본 연구는 발달지연의 아동을 대상으로 과제지향의 중재를 사용하여 시-운동 협응과 소운동 협응 기능의 변화를 알아보고자 하였다. 연구방법 : 연구대상은 6세의 여아 1명이었다. 아동은 진단은 받지 않았으나 전반적인 기능에 발달지연을 보였다. 연구기간은 2021년 2월 5일부터 6월 4일까지이었고, 연구 설계는 단일대상연구 A-B를 사용하였다. 기초선 A는 초기 3회기로 아동의 기능 파악을 위해 초기평가를 하였다. 중재기 B는 12회기로 과제지향의 중재(task-oriented intervention)를 사용하였다. 기초선과 중재기에 동일하게 아동의 시-운동 협응 기능을 측정하기 위해 한국판 시지각발달검사 3판의 타원그리기를 실시하였고, 소운동 협응 기능을 측정하기 위해 Clinical Observation of Motor and Postural Skills(COMPS)의 손가락-코 짚기를 실시하였다. 회기별 변화도는 그림으로 제시하였다. 결과 : 한국판 시지각발달검사 3판의 타원그리기의 기초선 평균은 23점이었고, 중재기 평균은 39.66점으로 기초선에 비해 중재기의 평균이 16점 이상 향상되었다. COMPS의 손가락-코 짚기의 기초선 평균은 0점이었고, 중재기 평균은 5.08점으로 기초선에 비해 중재기의 평균이 5점 이상 향상되었다. 결론 : 취학 전의 발달지연아동에게 과제지향의 중재는 아동의 학습에 바탕이 되는 시-운동 협응과 소운동 협응 기능에 긍정적인 영향을 미치는 것으로 보인다.

감각통합치료가 뇌성마비 아동의 감각.운동발달 및 적응행동에 미치는 영향 (Effects of Sensory Integration Therapy on Sensory. Motor Development and Adaptive Behavior of Cerebral Palsy Children)

  • The purpose of this study was to examine the effects of sensory integration therapy (SIT) on sensory' motor development and adaptive behavior of cerebral palsy children. The design of this study was quasi experiments with a non-equivalent pre- and post-test control design. Subjects of the study were arbitrarily chosen based on predetermined selection criteria among the cerebral palsy children who were treated as out-patients at two rehabilitation hospitals one in Seoul, and the other in Kyunggi-do. The study was conducted between early April and late July in 2000. Fifteen children were in the experimental group and eleven in the control group. The allocation was done based on ease of experimental treatment. A five-step SIT program was devised from a combination of SIT programs suggested by Ayres(1985) and Finks(1989), and an author-designed SIT program for cerebral palsy children. The experimental group was subjected to 20 to 30 minutes of SIT per session. two sessions a week for ten -week period. The effects of SIT were measured with respect to 9 sub-areas that can be administered to cerebral palsy children out of a total of 17 sub-areas in the Southern California Sensory Integration Test (SCSIT) developed by Ayres (1980). In addition. the scale developed by Russell (1993) for Gross Motor Function Measure (GMFM). and Perception Motor Development Test developed by 中司利一 et al.(1987) were also applied. Adaptive behavior was analyzed using guidelines in two unpublished documents - School-Age Checklist for Occupational Therapy by the Wakefield Occupational Therapy Associates, and the OTA-Watertown Clinical Assessment by the Watertown Occupational Therapy Associates-, and an author-developed Adaptive Behavior Checklist. Collected data were statistically analyzed by SPSS PC for chi square test, Mann-Whitney test, Wilcoxon signed rank test, and paired t-test. The results were as follows: 1. In sensory development, the experimental group exhibited a score increase compared to the control group, but the difference was not statistically significant, Although the experimental group showed improvements in all. 9 sub-areas compared to the control group, only right-left discrimination exhibited statistically significant change. 2. In gross motor development, the experimental group showed improvements in score compared to the control group, but it was not statistically significant. In fine motor development, the experimental group exhibited statistically significant improvements compared to the control group. In sub-area analysis, figure synthesis showed positive change. 3. In adaptive behavior development, post-experimental adaptive behavior scores were higher compared to pre-experimental scores with statistical significance. Furthermore, sub-areas emotional behavior, perception behavior, gross-fine motor function, oral-respiration function, motor behavior, motor planning, and adaptive response exhibited higher scores after SIT. In conclusion SIT was found to be partially effective in sensory and fine motor development, effective in all adaptive behavior areas, and not effective in gross motor development. Thus, this study has shown that SIT is an effective intervention for sensory development, fine motor development, and adaptive behavior for cerebral palsy children. But, for the effectiveness of SIT on gross motor development, further studies employing longer-time experiments are recommended.
