Acupuncture in Patients with a Vertebral Compression Fracture: A Protocol for a Randomized, Controlled, Pilot Clinical Trial

  • Objectives: A vertebral compression fracture (VCF) is characterized by back pain and fracture of a vertebral body on spinal radiography. VCFs of the thoraco lumbar spine are common in the elderly. In general, appropriate analgesics should be prescribed to reduce pain and, thus, promote early mobilization. The ideal treatment approach for VCFs has not been determined. In Korea, acupuncture and herbal medication have been used to treat VCFs for many years. There is empirical evidence that acupuncture might benefit patients with a VCF. However, no randomized, controlled, clinical trials evaluating the efficacy and the safety of acupuncture for treating a VCF have been published. Therefore, we designed a randomized, controlled, pilot, clinical trial to obtain information for the design of a further full scale trial. Methods: A five week protocol for a randomized, controlled, pilot, clinical trial is presented. Fourteen patients will be recruited and randomly allocated to two groups: a control group receiving interlaminar epidural steroid injections once a week for three weeks, and an experimental group receiving interlaminar epidural steroid injections plus acupuncture treatment (three acupuncture sessions per week for three weeks, nine sessions in total). The primary outcomes will be the pain intensity (visual analogue scale and PainVision$^{TM}$ system). The secondary outcome measurements will be the answers on the short form McGill pain questionnaire and the oswestry disability index. Assessments will be made at baseline and at one, three, and five weeks. The last assessment (week five) will take place two weeks after treatment cessation. This study will provide both an indication of feasibility and a clinical foundation for a future large scale trial. The outcomes will provide additional resources for incorporating acupuncture into existing treatments, such as nonsteroidal anti-inflammatory medications, narcotics and vertebral augmentation. This article describes the protocol.

  • Nowadays, recognizing human activities is an important subject; it is exploited widely and applied to many fields in real-life, especially in health care and context aware application. Research achievements are mainly focused on activities of daily living which are useful for suggesting advises to health care applications. Falling event is one of the biggest risks to the health and well-being of the elderly especially in independent living because falling accidents may be caused from heart attack. Recognizing this activity still remains in difficult research area. Many systems equipped wearable sensors have been proposed but they are not useful if users forget to wear the clothes or lack ability to adapt themselves to mobile systems without specific wearable sensors. In this paper, we develop a novel method based on analyzing the change of acceleration, orientation when the fall occurs and measure their similarity to featured fall patterns. In this study, we recruit five volunteers in our experiment including various fall categories. The results are effective for recognizing fall activity. Our system is implemented on G1 smart phone which are already plugged accelerometer and orientation sensors. The popular phone is used to get data from accelerometer and results showthe feasibility of our method and significant contribution to fall detection.

Correlation Analysis Between O/D Trips and Call Detail Record: A Case Study of Daegu Metropolitan Area (모바일 통신 자료와 O/D 통행량의 상관성 분석 - 대구광역시 사례를 중심으로)

  • Traditionally, travel demand forecasts have been conducted based on the data collected by a survey of individual travel behavior, and their limitations such as the accuracy of travel demand forecasts have been also raised. In recent, advancements in information and communication technologies are enabling new datasets in travel demand forecasting research. Such datasets include data from global positioning system (GPS) devices, data from mobile phone signalling, and data from call detail record (CDR), and they are used for reducing the errors in travel demand forecasts. Based on these background, the objective of this study is to assess the feasibility of CDR as a base data for travel demand forecasts. To perform this objective, CDR data collected for Daegu Metropolitan area for four days in April including weekdays and weekend days, 2017, were used. Based on these data, we analyzed the correlation between CDR and travel demand by travel survey data. The result showed that there exists the correlation and the correlation tends to be higher in discretionary trips such as non-home based business, non-home based shopping, and non-home based other trips.

High-resolution Urban Flood Modeling using Cellular Automata-based WCA2D in the Oncheon-cheon Catchment in Busan, South Korea (셀룰러 오토마타 기반 WCA2D 모형을 이용한 부산 온천천 유역 고해상도 도시 침수 해석)

  • As climate change increasesthe frequency and risk of flooding in major cities around theworld, the importance ofsimulation technology that can quickly and accurately analyze high-resolution 2D flooding information in large-scale areasis emerging. The physically-based approaches based on the Shallow Water Equations (SWE) often requires huge computer resources hindering high-resolution flood prediction. This study investigated the theoretical background of Weighted Cellular Automata 2D (WCA2D), which simulates spatio-temporal changes offlooding using transition rules and weight-based system, and assessed feasibility to simulate pluvial flooding in the urbancatchment, theOncheon-cheon catchmentinBusan, SouthKorea.Inaddition,the computation performancewas compared by applying versions using OpenComputing Language (OpenCL) andOpenMulti-Processing (OpenMP) parallel computing techniques. Simulationresultsshowed that the maximuminundation depthmap by theWCA2Dmodel cansimilarly reproduce historical inundation maps. Also, it can precisely simulate spatio-temporal changes of flooding extent in the urban catchment with complex topographic characteristics. For computation efficiency, parallel computing schemes, theOpenCLandOpenMP, improved the computation by about 8~14 and 5~6 folds respectively, compared to the sequential computation.

Durability Evaluation of Cement Concrete Using Ferrosilicon Industrial Byproduct (페로실리콘 산업부산물 활용 시멘트 콘크리트의 내구성능 평가)

  • In this paper, a ferrosilicon by-product was evaluated to confirm the feasibility of recycling it as supplementary cementitious material of ordinary Portland cement in concrete. Three different levels of replacement ratio (10 %, 20 % and 30 % of total binder) were applied to find which is the most beneficial to be used as a binder. Ferrosilicon concrete was initially assessed at setting time and compressive strength. Durability was evaluated by the resistance to chloride penetration test(RCPT) and alkali-silica reaction(ASR) with a comparison to silica fume concrete due to their similarity in chemical composition. The porosimetry and X-ray diffraction analysis along with energy dispersive X-ray spectroscopy give information on the microstructural characteristics of the ferrosilicon concrete. It was found that 10 % ferrosilicon concrete has higher strength while 20 %, 30 % have lower strength than OPC concrete. However, chemical resistance to chloride attack is higher when replacement is increased. Compared to silica fume, the durability of ferrosilicon might be less efficient however, it is obviously beneficial than OPC. High SiO2 content in ferrosilicon results in producing more C-S-H gel which could make denser pore structure. Most of the risk of alkali silica reaction to silicate binders through length change tests was less than 0.2 %, and both mortar using ferrosilicon and silica fume showed better resistance to alkali silica reaction as the substitution rate increased.Reuse of industrial waste rather than producing highly refined additives might reduce environmental load during manufacture and save costs.

Flood Runoff Simulation Using GIS-Grid Based K-DRUM for Yongdam-Dam Watershed (GIS격자기반 K-DRUM을 활용한 용담댐유역 홍수유출모의)

  • Recently, the rapid development of GIS technology has made it possible to handle a various data associated with spatially hydrological parameters with their attribute information. Therefore, there has been a shift in focus from lumped runoff models to distributed runoff models, as the latter can consider temporal and spatial variations of discharge. This research is to evaluate the feasibility of GIS based distributed model using radar rainfall which can express temporal and spatial distribution in actual dam watershed during flood runoff period. K-DRUM (K-water hydrologic & hydaulic Distributed flood RUnoff Model) which was developed to calculate flood discharge connected to radar rainfall based on long-term runoff model developed by Kyoto- University DPRI (Disaster Prevention Research Institute), and Yondam-Dam watershed ($930km^2$) was applied as study site. Distributed rainfall according to grid resolution was generated by using preprocess program of radar rainfall, from JIN radar. Also, GIS hydrological parameters were extracted from basic GIS data such as DEM, land cover and soil map, and used as input data of distributed model (K-DRUM). Results of this research can provide a base for building of real-time short-term rainfall runoff forecast system according to flash flood in near future.

Geological Factor Analysis for Evaluating the Long-term Safety Performance of Natural Barriers in Deep Geological Repository System of High-level Radioactive Waste (지질학적 심지층 처분지 내 천연방벽의 고준위 방사성 폐기물 장기 처분 안전성 평가를 위한 지질학적 인자 분석)

  • In this study, an investigation was conducted on the features, events, and processes (FEP) that could impact the long-term safety of the natural barriers constituting high-level radioactive waste geological repositories. The FEP list was developed utilizing the IFEP list 3.0 provided by the Nuclear Energy Agency (NEA) as foundational data, supplemented by geological investigations and research findings from leading countries in this field. A total of 49 FEPs related to the performance of the natural barrier were identified. For each FEP, detailed definitions, classifications, impacts on long-term safety, significance in domestic conditions, and feasibility of quantification were provided. Moreover, based on the compiled FEP list, three scenarios that could affect the long-term safety of the disposal facility were developed. Geological factors affecting the performance of the natural barrier in each scenario were selected and their relationships were visualized. The constructed FEP list and the visualization of interrelated factors in various scenarios are anticipated to provide essential information for selecting and organizing factors that must be considered in the development of mathematical models for quantitatively evaluating the long-term safety of deep geological repositories. In addition, these findings could be effectively utilized in establishing criteria related to the key performance of natural barriers for the confirmation of repository sites.

A Study on the Evaluation Indicators for the Establishment of Marine Fisheries Safety Education Facilities (해양수산안전 교육시설 설립을 위한 입지평가요인 도출에 관한 연구)

  • In this study, an expert survey was conducted using the Delphi technique to select items and indicators for evaluation before installing educational facilities in the marine fisheries safety field, in which the educational infrastructure gap between regions is wide. Seven indicators were selected as geographic, social, and administrative factors. In order to objectively evaluate each indicator, evaluation indicators that could be evaluated using public data such as the "Comprehensive National Balanced Development Information System" and "National Statistical Portal" were developed. The Analytic Hierarchy Process (AHP) method was applied to select the weight for each indicator, resulting in 10 most important influencing factors on the selection of the location of educational facilities of the Marine Fisheries Safety Education Facilities: the distribution of marine officers, access to high-speed railways, the number of small ships less than 5 tons, access to highways interchange, the distribution of fishing boats, the close relationship of related industries, the planned new port, the distribution of commercial ports, the number of marine leisure riders, and the availability of long-term land leases in local government councils. The location evaluation index of marine and fishery safety education facilities developed in this study can be used to evaluate each region using national public data, and has the advantage of enabling objective evaluation. Therefore, it is judged that this evaluation index can be used to verify the feasibility of installing marine fisheries safety education facilities as well as other marine-related facilities.

Draft List and Relative Importance of Principal Processes in the Geosphere to be Considered for the Radiological Safety Assessment of the Domestic Geological Disposal Facility through Analyzing FEPs for KBS-3 Type Disposal Repository of High-level Radioactive Waste(HLW) (KBS-3 방식 고준위방폐물 심층처분장 FEP 분석을 통한 국내 사용후핵연료 심층처분시설 방사선학적 안전성 평가용 지권영역 주요 프로세스 항목 및 상대적 중요도 도출)

  • The deep geological repository of high-level radioactive waste shall be designed to meet the safety objective set in the form of radiation dose or corresponding risk to protect human and the environment from radiation exposure. Engineering feasibility and conformity with the safety objective of the facility conceptual design can be demonstrated by comparing the assessment result using the computational model for scenario(s) describing the radionuclide release and transport from repository to biosphere system. In this study, as the preliminary study for developing the high-level radioactive waste disposal facility in Korea, we reviewed and analyzed the entire list of FEPs and how to handle each FEP from a general point of view, which are selected for the geosphere region in the radiological safety assessment performed for the license application of the KBS-3 type deep geological repository in Finland and Sweden. In Finland, five FEPs (i.e., stress redistribution, creep, stress redistribution, erosion and sedimentation in fractures, methane hydrate formation, and salt exclusion) were excluded or ignored in the radionuclide release and transport assessment. And, in Sweden, six FEPs (i.e., creep, surface weathering and erosion, erosion/sedimentation in fractures, methane hydrate formation, radiation effects (rock and grout), and earth current) were not considered for all time frames and earthquake out of a total of 25 FEPs for the geosphere. Based on these results, an FEP list (draft) for the geosphere was derived, and the relative importance of each item was evaluated for conducting the radiological safety assessment of the domestic deep geological disposal facility. Since most of information on the disposal facility in Korea has not been determined as of now, it is judged that all FEP items presented in Table 3 should be considered for the radiological safety assessment, and the relative importance derived from this study can be used in determining whether to apply each item in the future.

Financial Fraud Detection using Text Mining Analysis against Municipal Cybercriminality (지자체 사이버 공간 안전을 위한 금융사기 탐지 텍스트 마이닝 방법)

  • Recently, SNS has become an important channel for marketing as well as personal communication. However, cybercrime has also evolved with the development of information and communication technology, and illegal advertising is distributed to SNS in large quantity. As a result, personal information is lost and even monetary damages occur more frequently. In this study, we propose a method to analyze which sentences and documents, which have been sent to the SNS, are related to financial fraud. First of all, as a conceptual framework, we developed a matrix of conceptual characteristics of cybercriminality on SNS and emergency management. We also suggested emergency management process which consists of Pre-Cybercriminality (e.g. risk identification) and Post-Cybercriminality steps. Among those we focused on risk identification in this paper. The main process consists of data collection, preprocessing and analysis. First, we selected two words 'daechul(loan)' and 'sachae(private loan)' as seed words and collected data with this word from SNS such as twitter. The collected data are given to the two researchers to decide whether they are related to the cybercriminality, particularly financial fraud, or not. Then we selected some of them as keywords if the vocabularies are related to the nominals and symbols. With the selected keywords, we searched and collected data from web materials such as twitter, news, blog, and more than 820,000 articles collected. The collected articles were refined through preprocessing and made into learning data. The preprocessing process is divided into performing morphological analysis step, removing stop words step, and selecting valid part-of-speech step. In the morphological analysis step, a complex sentence is transformed into some morpheme units to enable mechanical analysis. In the removing stop words step, non-lexical elements such as numbers, punctuation marks, and double spaces are removed from the text. In the step of selecting valid part-of-speech, only two kinds of nouns and symbols are considered. Since nouns could refer to things, the intent of message is expressed better than the other part-of-speech. Moreover, the more illegal the text is, the more frequently symbols are used. The selected data is given 'legal' or 'illegal'. To make the selected data as learning data through the preprocessing process, it is necessary to classify whether each data is legitimate or not. The processed data is then converted into Corpus type and Document-Term Matrix. Finally, the two types of 'legal' and 'illegal' files were mixed and randomly divided into learning data set and test data set. In this study, we set the learning data as 70% and the test data as 30%. SVM was used as the discrimination algorithm. Since SVM requires gamma and cost values as the main parameters, we set gamma as 0.5 and cost as 10, based on the optimal value function. The cost is set higher than general cases. To show the feasibility of the idea proposed in this paper, we compared the proposed method with MLE (Maximum Likelihood Estimation), Term Frequency, and Collective Intelligence method. Overall accuracy and was used as the metric. As a result, the overall accuracy of the proposed method was 92.41% of illegal loan advertisement and 77.75% of illegal visit sales, which is apparently superior to that of the Term Frequency, MLE, etc. Hence, the result suggests that the proposed method is valid and usable practically. In this paper, we propose a framework for crisis management caused by abnormalities of unstructured data sources such as SNS. We hope this study will contribute to the academia by identifying what to consider when applying the SVM-like discrimination algorithm to text analysis. Moreover, the study will also contribute to the practitioners in the field of brand management and opinion mining.
