Background: Migraine patients can be sensitive to external or internal stimuli, such as light, noise, or hormonal changes. Using transcranial Doppler ultrasonography (TCD) with breath-holding method, we evaluated the changes of cerebrovascular reactivity (CVR) to hypercapnia in women with migraine without aura between fasting and postprandial period. Methods: Twelve women with migraine without aura and the same number of age and sex-matched healthy controls with no significant history of headache participated in this study. Using TCD examinations, we studied mean flow velocity in middle cerebral artery with better temporal window. Each subject was examined consecutively before and after a standard meal, together with serum glucose level and blood pressure. CVR was evaluated with breath-holding index (BHI). Results: Postprandial-BHI (mean+SD) was significantly higher than fasting-BHI (mean+SD) in patients group but not in controls (in patient group; postprandial-BHI=1.38, fasting-BHI=1.08, in control group; postprandial-BHI=1.25, fasting-BHI=1.18, P=0.021 and 0.239, respectively). After meal, serum glucose level was significantly enhanced but blood pressure was not in both groups. Serum glucose level of patients showed a tendency of mild positive correlation with BHIs (${\gamma}$=0.448, P=0.032). Conclusions: Although exact mechanisms are unclear, cerebrovascular reactivity of some women with migraine without aura may be influenced by prandial state.
The aim of this study was to examine the hypoglycemic effect of chlorella in 6 week-old type 2 diabetic Goto-Kakizaki (GK, n=30) rats and 6 week-old normal Wistar (n=30) rats. Animals were randomly assigned to 3 groups respectively, and were fed three different experimental diets containing 0%, 3% or 5% (w/w) chlorella for 8 weeks. In diabetic GK rats, the insulinogenic-indices were not significantly different among the groups. The concentrations of fasting plasma glucagon and hepatic triglyceride, and the insulin/glucagon ratios of the GK-3% chlorella and GK-5% chlorella groups were significantly lower than those of the GK-control group. The HOMA-index and the concentrations of fasting blood glucose and plasma insulin of the GK-3% chlorella and GK-5% chlorella groups were slightly lower than those of the GK-control group. In normal Wistar rats, the insulinogenic-indices were not significantly different among the normal groups, but that of the Wistar-5% chlorella group was slightly higher than the other groups. The concentrations of fasting blood glucose and plasma insulin, and the HOMA-index of the Wistar-5% chlorella group were a little higher, and the fasting plasma glucagon concentration and the insulin/glucagon ratio of the Wistar-5% chlorella group were significantly higher than those of the Wistar-control and Wistar-3% chlorella groups. In conclusion, this study shows that the glucose-stimulated insulin secretion was not affected by the intake of chlorella, which could be beneficial, however, in improving insulin sensitivity in type 2 diabetic GK and normal Wistar rats.
We found previously that Undaria pinnatifida extract has an effect of lowering blood glucose levels in diabetic rats. Therefore, an effect of Undaria pinnatifida extract on the insulin secretion directly from the pancreas was examined in this study. Neonatal diabetes were induced by intraperitoneal injection of Streptozotocin (100 mg/kg body weight) at age of day 1. Rats were fed a rodent pellet diet until they were grown to adults (age of 7 weeks). Rats having a fasting serum glucose level over 250 mg/dL were used in this feeding study and they were divided into two diet groups as follows; a diet with Undaria pinnatifida extract (5%) and a diet without this extract (control group). Fasting (12 hr) blood glucose and serum insulin levels were measured before and after feeding a diet with Undaria pinnatifida extract for 4 weeks. At the last day of feeding, in vitro pancreas perfusion was performed. Pancreas was stimulated with a perfusate without glucose during a period of 0~10 minutes and with a perfusate containing 200 mg/dL glucose during a period of 11~40 minutes. Insulin amount was measured using a radioimmuno assay. In results, amount of the insulin secreted from the pancreas in the diabetic rats fed Undaria pinnatifida extract was significantly greater than that in the diabetic control group during the periods of the equilibration period (0~10 min) and the first phase (11~20 min) of the insulin secretion (P<0.05). It is concluded that Undaria pinnatifida extract increases insulin secretion from the pancreas in the neonatal diabetic rats. Therefore, the blood glucose lowering effect of the Undaria pinnatifida extract may be elucidated by mechanisms with promoted insulin secretion from the pancreas in diabetic rats.
Objective: This pilot study was performed to investigate the effect of metformin on insulin resistance, hormone levels, and lipid profiles in non-obese patients with polycystic ovary syndrome. Methods: This study included 16 non-obese patients with polycystic ovary syndrome diagnosed at our hospital from June 2006 to September 2007. Blood samples were collected before and 6 months after metformin treatment for analysis of fasting serum glucose levels, fasting serum insulin levels, a glycemic response to 75 g oral glucose tolerance test (OGTT), and hormonal blood profile including FSH, LH, estradiol, testosterone, free testosterone, serum lipid profiles. Insulin resistance was estimated by calculating fasting glucose/insulin ratio (FGIR), 2 hr glucose/insulin ratio after 75 g glucose load. And we investigated insulin resistance and pancreatic beta cell function by calculating HOMA beta cell function and HOMA IR. Results: After the treatment of metformin, there was significant increase in 2 hr glucose/insulin ratio after 75 g glucose load (p=0.04) and decrease in HOMA IR (p=0.000). But serum lipid profiles did not change significantly. Also the metformin treatment induced a significant reduction in serum free testosterone and LH levels, and LH/FSH ratio (p=0.001, p=0.000, p=0.034). Conclusion: This pilot study showed that metformin might be effective in improving insulin sensitivity, ameliorating hyperandrogenemia in non-obese patients with polycystic ovary syndrome. Further investigations with larger number of patients and long-term observations are necessary to determine the role of metformin.
Cassia tora L. seeds have previously been reported to reduce blood glucose level in human and animals with diabetes. In the present study, the effects of Cassia tora L. seed butanol fraction (CATO) were studied on postprandial glucose control and insulin secretion from the pancreas of the normal and diabetic rats. Diabetes was induced by an i.p. injection of Streptozotocin (55 mg/kg BW) into the male Sprague-Dawley rats. The postprandial glucose control was monitored during a 240 min-period using a maltose loading test. In normal rats, rats fed CATO (20 mg/l00 g BW/d) showed lower postprandial glucose levels in all the levels from 30 min up to 180 min than those in the control rats without CATO (p<0.05). In diabetic rats, those levels in the CATO group seemed to be lower during the $30{\sim}180$ min, but only glucose level at 30 min showed significant difference compared to that in the control group. Moreover, CATO delayed the peak time of the glucose rise in both normal and diabetic rats in the glucose curves. On the other hand, when CATO was administered orally to the diabetic rats for 5 days, 12 hr fasting serum glucose level was decreased in the diabetic rats (p<0.05). Degree of a decrease in 12 hr fasting serum insulin levels was significantly less in the diabetic CATO rats as compared to diabetic control rats. On the last day of feeding, P cells of the pancreas were stimulated by 200 mg/dL glucose through a 40 min-pancreas perfusion. Amounts of the insulin secreted from the pancreas during the first phase ($11{\sim}20$ min) and the second phase ($21{\sim}40$ min) in the CATO fed diabetic rats were significantly greater than those in the diabetic control group (p<0.05). These findings indicated that constituents of Cassia tora L. seeds have beneficial effect on postprandial blood glucose control which may be partially mediated by stimulated insulin secretion from the pancreas of the diabetic rats.
Purpose : The purpose of this study was to investigate the effects of the education using short messaging service(SMS) on fasting blood sugar(FBS) and diabetes adherence. Method : Forty-five diabetic patients being assessed pre and post intervention was used to assess the effectiveness of the education. Participants were requested to input the FBS and 2 hours post-prandial blood sugar everyday in http://www.biodang.com by cellular phone or wire Internet. The goal of the education was to lower FBS and keep diabetes adherence. The education was applied to the for 12 weeks. The education consisted of continuous education and reinforcement of diet, exercise, medication adjustment, as well as frequent self-monitoring of blood glucose levels. The education performed weekly. All medication adjustments were communicated to the participants' doctor. FBS and diabetes adherence were measured before and after the education. Results : The education using SMS of cellular phone had decrease FBS and increase oral medication taking, exercise, foot care, and hypoglycemia preparation adherence.
Twenty five cooking recipe with yam (Dioscorea japonica Thunb) have been developed and their hypoglycemic effects on diabetic patients were investigated. The results of the sensory evaluations revealed that the tested recipe were in good scores (overall acceptability scores 3.30-4.33) except Chojuli. The 10-panel members indicated that the yam cookings were more acceptable when they were cooked with other vegetables. Twenty seven diabetic patients were entered in a general hospital during the 2-month experimental period and they were allowed to consume the diet with yam cooking dish, 75 g of yam a day. The fasting blood glucose and 2-hour postprandial blood glucose levels were determined. The diabetic patients who consumed yam showed the lower blood glucose levels than those patients who were controlled with the hospital diet only. The hypoglycemic effects of the yam were more evident in the patients who were injected with insulin. The constant lowering effects of blood glucose levels were observed in the normal subjects who had taken the yam cookery, leading to that one subject who started with a low blood glucose level must stop the yam consumption after 10 days.
Diabetes mellitus, commonly known as diabetes, comprises a group of metabolic disorders characterized by a high blood sugar level over a prolonged period of time. Diabetes is classified into type 1 diabetes and type 2 diabetes, and the incidence and prevalence of diabetes, mostly type 2, have increased remarkably in recent decades. A diagnosis of type 2 diabetes is greatly influenced by heredity, and it is important to prevent diabetes in people with a family history by improving lifestyle and environmental factors. Controlling overeating, obesity, lack of exercise, and stress is effective to prevent diabetes. The patient in this case report had impaired fasting glucose and mild hyperlipidemia. The patient experienced improvements in these sequelae after the administration of herbal medicine (Yangkyuksanwha-tang) for 12 weeks and the implementation of a plant-based diet. The complete blood count, XXXX, fasting blood sugar, HbA1c, insulin, and C-peptide levels were measured, and the Homeostatic Model Assessment for Insulin Resistance (HOMA-IR) and HOMA-β were calculated according to the FBS and fasting insulin levels. Total cholesterol, triglyceride, FBS, and HbA1c levels all decreased after 12 weeks compared with baseline measures. There was no change in the insulin secretory ability; the C-peptide level did not change as a result of β-cell function; and the HOMA-β level reflected an improved insulin secretory ability.

This study examined the prevalence of an impaired fasting glucose and related factors in normal non-diabetic adults. This study used the raw data (general characteristics, health behavior, obesity, impaired fasting glucose etc.) from the 6th Korea National Health & Nutrition Examination Survey (NHANES) in the second year (2014), and out of all 7,550 survey participants, 1,341 were selected as the final research subjects. After stratifying according to obesity, a logistic regression model was used to analyze the factors related to an impaired fasting glucose level. The older subjects were more likely to have an impaired fasting glucose in both the obesity group and normal weight group (p<.001). The obesity group showed a higher impaired fasting glucose rate when they drank more alcohol (p<.05) and consumed less fruit (p<.05). Therefore, balanced nutrients and control of the blood sugar level through proper education and public health policies of fruit intake and alcohol are needed.
This study was aimed to investigate whether abdominal obesity is associated with non-insulin dependednt diabetes. The distribution of body fat patterns was observed in 181 female patients with diabetes, aged 33 to 83 years, living in the Taegu area, Korea. The following anthropometric measuremetns were made on all participants from October 1 to November 25, 1991 : weight, height ; waist and hip circumferences in standing position. The waist hip circumference ratio was used as an index of abdominal obesity. The results were as follows; 1) The mean fasting blood glucose of diabetic subjects was 145$\pm$50.3mg/dl and the mean duration of diabetes was 4.7$\pm$7.5 years. 2) Obese subjects above the ideal body weight body weight of 120% in the investigation are presently 52%, but 63% of subjects were reported to be obese in the past. The mean BMI of the subjects is 24.57$\pm$3.15 and the past mean BMI was 27.13$\pm$3.26. One year after reaching their highest body weight, 47% of the subjects developed diabetes. Two years after reaching their peak body weight, 74% of diabetic subjects developed diabetes. 3) Using the waist-hip circumference ratio, subjects beloing to the upper body obesity(WHR>0.84) were 65.5%. 4) The average daily energy intake did not differ between the obese and non-obese diabetic subjects, whether they were assessed with BMI or with RBW. 5) The average daily energy intake was higher in the upper body obesity subjects than in the lower body obesity subjects. 6) Diabetics withing the regular exercise group had lower fasting blood glucose levels than the non-regular exercise group. Exercise did not effect the RBW, BMI, and WHR. 7) The waist-to-hip circumference ratio correlated significantly in positive with waist-circumference, but did not correlated with hip-circumference. Therefore, WHR may depended on the increased accumulation of abdominal fat in female diabetics. In conclusion, these findings suggest that caloric intake is more associated with abdominal fat accumulation in diabetic women. Blood glucose concentration is independently effected by exercise, and exercise does not affect the WHR. Therefore, control of caloric intake and development of specific exercises to change the WHR seems important for controling diabetes in female subjects.
