Comparative Analysis of Fall-Related Physicopsychological according to Virtual Exercise and Lumbar Stabilization Exercise in the Patient with Stroke (가상현실 체험형 운동과 요부안정화운동에 따른 뇌졸중환자의 낙상관련 신체·심리적 비교 분석)

  • This study conducted the following experiment to examine change of physicopsychological function on lumbar stabilization exercise(LSE) and virtual reality game training(Nintendo Wii Sport-NWS) to stroke patients subject for fall prevention. Psychological function was measured by falls efficacy with stroke patients and physical function was measured by static and dynamic balance on comparative analysis of pre, post exercise and each groups in 30 stroke patient subject. Static balance was measured by BBS, FRT, dynamic were measured by TUG, 10m walking test and falls efficacy with stroke patients was measured index of falls efficacy. These result lead us to the conclusion that each group were statistically improved at all physicopsychological test, but BBS, FRT, 10m walking test were more statistically improved at LSE group and falls efficacy with stroke patients were more improved at virtual reality game training group. Consequently, virtual reality game training would be lead to positive increment of physicopsychological function on stroke patient.

  • It is very important for the elderly to maintain their independence and to live safe and comfortable daily lives in order to lead an independent life. The bathroom in the house has the greatest number of fall accidents and has a great influence on the independence of the elderly. Therefore, the purpose of this study is to develop a bathroom design method that improves the independence of the elderly whose physical function is deteriorated through preventing falls. In this study, first of all, we investigated the causes of falling in bathrooms based on the previous studies, and identified the intelligent services that can prevent falls and selected them according to individual needs. Secondly, we investigated the bathroom size of the apartment among the various housing type, analyzed the type of the bathroom, and developed a standard type of the bathroom. Thirdly, we analyzed the design guidelines for the elderly residents in Korea and proposed improvement directions. We also divided the intelligent sanitary appliances and fall-prevention products and proposed the installation method and directions. Therefore, the comprehensive bathroom design standards and proposals proposed in this study will be the basis of bathroom design to prevent falls and improve independence of the elderly, and it will be easy to understand and help the designer in designing. Finally, this study is meaningful in that it provides independence of the elderly through fall prevention and increases the easiness of hygiene action and suggests direction to maintain self-sustaining life of the elderly.

Effects of virtual reality combined with balance training on upper limb function, balance, and activities of daily living in persons with acute stroke: a preliminary study

  • Objective: The aim of this preliminary study was to investigate the effects of virtual reality combined with balance training on upper limb function, balance, and activities of daily living (ADL) in persons with acute stroke. Design: Randomized controlled trial. Methods: Fourteen acute stroke survivors were recruited and randomly assigned into two groups: the experimental group (n=7) and the control group (n=7). Both groups performed the conventional rehabilitation therapy for 30 minutes a day, 5 times a week, for 4 weeks. Additionally, the experimental group conducted the virtual reality training for 30 minutes on an unstable surface during each session, whereas the control group performed balance training for 30 minutes on a stable surface. All measurements were performed before and one day after intervention. Upper limb function, balance, and ADL were assessed using the Manual Function Test (MFT), the Berg Balance Scale (BBS), and the Korean version of the Modified Barthel Index (K-MBI), respectively. Results: Both groups showed significant improvements in MFT, BBS, K-MBI after intervention (p<0.05). There were no significant differences between the experimental and control groups with respect to MFT, BBS, and K-MBI after intervention. The experimental group showed a greater decrease in fall risk (BBS<45) after intervention than the control group (p<0.05). Conclusions: These findings suggest that virtual reality combined with balance training has a better effect on balance improvement than virtual reality training alone in persons with acute stroke.

Correlation Analysis between MBI and MMSE after Exercise Program for Dementia Elderly (치매노인을 위한 운동프로그램 적용후 MBI와 MMSE 사이의 상관관계분석)

  • 29 dementia patients over 60 years of no in the nursing home were assigned to execute rehabilitation program a month for 3 months. 1 evaluated MBI scores and MMSE scores and analyzed correlation between both scores. Analyses of results were as follows : 1. In the experimental group with rehabilitation program. ADL and cognitive function were improved significantly(p<0.05). 2. In the control group with no rehabilitation program. ADL was decreased significantly(p<0.05), cognition was not changed (pgt;0.05). 3. There were significant rises in MBI scores and MMSE scores in the experimental group(p<0.05). 4. There was a significant fall in MBI scores in the control group(p<0.05). a no change in MMSE scores in the control group(p>0.05). 5. There was strong correlation between ADL and cognition in the experimental group and the control group. 6. Inverse correlation revealed between the experimental group and the control group in ADL. 7. Inverse correlation revealed between the experimental group and the control group in cognition.

Effect of Underwater Treadmill on the Elderly's Walking and Balance Ability (수중 트레드밀 운동이 노인의 보행 및 균형능력에 미치는 영향)

  • Background: This study had been carried out with 20 elderly subjects as its object for about one month from November 3, 2008 to December 14, 2008 in order to observe the effect of Underwater Treadmill on the elderly's walking and balance ability. Methods: Subjects were assigned either experimental group (n=10) or the control group (n=10), experimental group received Underwater Treadmill program (30 min per course, 3days a week for 6weeks). Subjects were assessed for muscle power (Nicolas Manual Muscle Test), balance (Functional Reach Test), gait ability (Time Up and Go, 10M walking test) before experiments and after experiments. Results: The results of this study were as follows; 1. After underwater treadmill exercise, the change of isometric contraction indicated a beneficial increase on lower extremity muscle power of experimental group and functional test of balance function; FRT, TUG indicated beneficial difference between groups. 2. beneficial difference between groups in walking speed of hourly index change of walking function. 3. between muscle power and balance, gait ability, we could find out there's high correlation ship between lower extremity muscle power increasing and balance and gait ability of the elderly. Conclusion: Aerobic exercise using underwater treadmill effects on muscle power strengthening of the elderly, and because of this, increase of lower extremity muscle power is very helpful not only to improvement of balance ability, but also to improvement of gait ability, so it will be used as a physical therapy program on clinic and used as an exercise program for protecting the elderly from falling down very well.

  • 본 연구는 본 연구는 경두개직류자극(transcranial direct current stimulation)을 편마비 환자의 대뇌피질 운동영역에 적용하여 상지기능에 미치는 효과를 알아보고자 실시하였다. 뇌졸중으로 인해 편마비된 환자를 대상으로 두 군으로 구분하여 경두개직류자극과 일반적인 물리치료를 함께 시행하였고, 실험 전후의 두 군의 기능적 평가항목과 전기생리학적 평가를 함께 실시하였다. 통계처리 방법으로 실험 전 후 차이를 검증하였고, 모든 통계적 유의수준은 0.05로 하였다. 본 연구의 결과 실험군과 대조군에서 주관절 기능평가의 값과 손 기능 평가에서 치료 전, 후 값이 증가하였고 통계적으로 유의한 차이를 보였다(p<0.05). 결론적으로, 경두개직류자극이 뇌졸중 환자의 상지기능에 변화에 효과적인 것으로 나타났다. 따라서, 임상에서 뇌졸중 환자를 대상으로 한 경두개직류자극의 활용 가능성에 대해 재활 관점에서의 추가적인 연구가 더욱 필요할 것으로 생각된다.

Changes of Gait Variability by the Attention Demanding Task in Elderly Adults

  • Purpose: Gait variability is defined as the intrinsic fluctuations which occur during continuous gait cycles. Increased gait variability is closely associated with increased fall risk in older adults. This study investigated the influence of attention-demanding tasks on gait variability in elderly healthy adults. Methods: We recruited 15 healthy elderly adults in this study. All participants performed two cognitive tasks: a subtraction dual-task (SDT) and working memory dual-task (WMDT) during gait plus one normal gait. Using the $LEGSys^+$ system, we measured the coefficient of variation (CV %=$100{\times}$[standard deviation/mean]) for participants' stride time, stride length, and stride velocity. Results: SDT gait showed significant increment of stride time variability compared with usual gait (p<0.05), however, stride length and velocity variability did not difference between SDT gait and usual gait (p>0.05). WMDT gait showed significant increment of stride time and velocity variability compared with usual gait (p<0.05). In addition, stride time variability during WMDT gait also significantly increased compared with SDT gait (p<0.05). Conclusion: We reported that SDT and WMDT gait can induce the increment of the gait variability in elderly adults. We assume that attention demanding task based on working memory has the most influence on the interference between cognitive and gait function. Understanding the changes during dual task gait in older ages would be helpful for physical intervention strategies and improved risk assessment.

Features and implication of new physical activity guidelines (새 신체활동기준의 동향과 함의)

  • Objectives: After many national physical activity guidelines have established, recent changes are deep and rapid. So the major features and implication to reverse worsening Korean physical activity indicator is desprate but related knowledge and informations are few. So review of recent features and implications of new physical activity guidelines have made. Methods: National physical activity guidelines of advanced countries were searched through snowballing methods. Major features were described according to the nation. Implication were drew through discussion for Korean realitiy. Results: New Australian physical activity and sedentary behaviour guideline explicitly included sedentary behaviour. The age in the guideline expanded to early years. Canada also presented 24-hour movement guidelines to early years. The second generation of the physical activity guidelines reflects the extensive amount of new knowledge. New aspects include discussions of additional health benefits related to brain health, additional cancer sites, and fall-related injuries; immediate and longer term benefits for how people feel, function, and sleep; further benefits among older adults and people with additional chronic conditions; risks of sedentary behavior and their relationship with physical activity; elimination of the requirement for physical activity benefits to occur in bouts of at least 10 minutes; and tested strategies that can be used to get the population more active. Conclusions: The most important message from the new guidelines is that the greatest health benefits accrue by moving from no, to even small amounts of, physical activity. Multiple studies demonstrate that the steepest reduction in disease risk occurs at the lowest levels of physical activity. People need to understand that even small amounts of physical activity are beneficial and that reductions in the risk of disease and disability occur by simply getting moving. So various evidence based proven strstegies are needed in Korea including workforce training.

A Study of the Evaluation of the Spinal Cord Injuries (척수 손상 환자에 관한 실태 조사)

  • The incidence of spinal cord injury increase due to traffic accident, industrial accident and leisure sports. Spinal cord injury damages motor and sensory function below the injury level, also affects autonomic functions associated with voiding and defecation. Sexual dysfunction and psychosocial, vocational maladaptations are also some of the unwanted consequences of injury. The purpose of this study is look for means to prevent and to manage complications in spinal cord injury through investigation and analysis. The subjects of this study in spinal cord injured patients were admitted to the department of physical therapy, Kwangju christian Hospital, Nam Kwang Hospital, Chun Nam university Hospital and Cho Sun university Hospital, from April, 1, 1995 to March, 31, 1996. The results are as follows: 1. The subjects comprised 96 cases of spinal cord injury, ranging from 17 to 85(mean-40.8 yrs) and included 72 males and 24 females. Among these patients, 58 were cervical injury, 20 were thoracic injury and 18 were lumbar injury. 2. As for a major causative of spinal cord injury were traffic accident(59.4%), fall down (27.1 %), and motocycle(4.2%).. 3. The bladder control were taken by indwelling cathetar(41.7%), Crede maneuver(37.5%) and self voiding(16.7%). The bowel control were taken by all aid(61.5%), assitance(32.3%) and self defecation(6.2%). 4. Possible of sexual function were 35 cases (47.9%). 5. The device of transfer used wheel chair(69.8%) and bed(16.7%). 6. The patients with higher cord lesion got more serious pain than lower cord lesion. Also the patients with higher cord lesion got a serious spasticity. 7. The incidence of decubitus ulcer among 96 patients were in case 46(47.9%). The largest group of the pressure sore sites were sacral portion(82.0%), less than 1 month of onset occured a large numbers(50%). Incidence of pressure sore by spasticity occured many patients in case of mild or moderate. Incidence of pressure sore by pain occured many patients in case of severe pain.

Risk factors for functional disability among community dwelling elderly (지역사회 재가 노인의 기능장애 위험요인)

  • Objectives: The purpose of this study was to investigate risk factors for chronic ADL, IADL disability. The study explored clinical and socio-demographic risk factors of functional status decline. Methods: Data from the Survey of Living Condition of Elderly 3-year panel study were analyzed. The study subjects were 5,928 community-dwelling people aged 65 years or older who were no disability in ADL and IADL at baseline. Predisposing factors, pathology, impairment, and functional limitations were regarded as risk factors. Logistic regression analysis was used. Results: During the 3-year study period, 3.9% participants developed chronic ADL disability, 9.4% participants were IADL disabled. After controlling for predisposing factor, the best predictors for ADL disability at 36 months were fall as a pathology factor, cognitive decline, disability judgement, lower limb functional limitation. Comorbidity, fall, cognitive decline, disability judgement, lower limb and upper limb functional limitation were risk factors for IADL disability. Conclusions: Health promotion program focusing elderly is essential to prevent ADL and IADL disability. Mobilizing physical activity should be included in health promotion program for elderly.
