Purpose : The purpose of this study is to investigate the effects of falls prevention exercise program(center of gravity control training, multiple sensory training, strategic posture training, ambulation training, muscle strengthening training) to balance and muscular strength in the elderly females. Methods : A total of 30 elderly womens participated in this study. All subjects have participated in exercise program on three times a week for eight weeks. Before and after of exercise program, They have measured about Berg Balance Scale (BBS), Performance Oriented Mobility Assessment (POMA), Time Up & Go (TUG), Sit to stand, Fall Efficacy Scale (FES), Quality of Life (QOL). Results : The results of this study were as follows ; 1) There were statistically significant difference in the BBS, POMA, FES, QOL test on within-subject. 2) There were not significant difference in the TUG, Sit to stand test on within-subject. 3) The BBS was correlated with POMA and QOL. The POMA was also correlated with QOL. Conclusion : The result of this study shows that falls prevention exercise program was meaningful increasing of balance ability and quality of life on elderly women
Objectives: The purpose of this study was to report the efficacy of using Korean medicine treatment, for anxiety induced by chemotherapy and traffic accident. Methods: We treated a patient's anxiety with Korean medicine treatments (Yangshimtang-Gamibang, ear acupuncture, acupuncture, cupping, interferential current therapy, and herbal acupuncture therapy). To evaluate the results of this treatment, we measured the Beck Anxiety Inventory, EuroQol-Five Dimensions, Numeric rating scale, Neck Disability Index, Oswestry Disability Index, total sleep duration, time duration to fall asleep, and the number of wake-up times during sleep. Results: Improvements of the Beck Anxiety Inventory, EuroQol-Five Dimensions, Numeric rating scale, Neck Disability Index, Oswestry Disability Index, and total sleep duration were observed after the treatments. Conclusions: This study showed one case of a patient who received Korean medicine treatment for the anxiety induced by anticancer therapy and traffic accident, and improvement was observed.
본 연구는 노화로 발생하는 근골격계 변화를 예방하기 위해서 적용한 운동 프로그램이 노인 여성의 통증, 근력, 균형능력 및 낙상효능감에 미치는 영향을 알아보기 위해 실시하였다. 65세 이상 노인 여성을 대상으로 대퇴사두근 셋팅 운동, 브릿지 운동, 저항밴드 운동, 폼 롤러를 이용한 운동 등을 포함한 운동 프로그램을 주 1회, 8주간 적용하였다. 통증을 평가하기 위해 시각적 아날로그 척도를 사용하였으며, 디지털 근력측정기를 이용하여 근력을 평가하였고, Berg 균형척도를 이용하여 균형능력을 측정하였으며, 낙상효능척도를 이용하여 낙상에 대한 두려움 정도를 평가하였다. 연구결과에서 실험 전에 비해 허리와 무릎 부위의 통증이 유의하게 감소하였고 대퇴사두근과 대둔근의 근력이 유의하게 증가하였으며, 균형능력과 낙상효능감이 유의하게 개선되었다. 결론적으로, 다양한 운동방법이 포함된 프로그램의 적용은 근골격계를 강화하여 노인 여성의 신체적 활동에 긍정적인 영향을 미친다. 또한, 노화에 따른 신체적 변화를 예방하기 위해서는 근골격계를 강화할 수 있는 다양한 운동방법들이 포함된 운동 프로그램이 적용되어야 한다.
PURPOSE: This study aimed to investigate the effect of a progressive balance training program with whole-body vibration stimulation on knee joint pain, dysfunction, psychosocial status, and balance ability in individuals aged ≥ 65 years with knee osteoarthritis. METHODS: A total of 40 individuals aged ≥ 65 years with osteoarthritis of the knees participated in the study. Using a randomization program, participants were assigned to an experimental group (n = 20) or a control group (n = 20). Both groups were assigned a knee strength training program, and a progressive balance training program with whole-body vibration stimulation was assigned to the experimental group. All interventions were conducted three times a week for four weeks. Participants were evaluated for the following: pain (numeric rating scale, NRS), knee dysfunction (Korean version of the Western Ontario and McMaster Universities Arthritis Index, K-WOMAC), fall efficacy (Korean Version Falls Efficacy Scale, K-FES), quality of life (Euro Quality of life 5 Dimension, EQ-5D), and advanced balance scale score (Fullerton advanced balance scale, FAB) before and after the intervention, and the effects of the intervention were compared accordingly between groups. RESULTS: Both groups showed significant differences in the results of the NRS, K-WOMAC, K-FES, and EQ-5D assessments before and after the intervention, and there was a significant difference in the amount of change between the two groups (p < .05). There was a significant improvement in FAB in all but items FAB 8 and FAB 9 after the intervention in the experimental group. In the control group, there was a significant improvement in FAB 1, FAB 2, FAB 7, and FAB total after the intervention (p < .05). In addition, there was a significant difference in the amount of change between the two groups in all items except FAB 8 and FAB 9 (p < .05). CONCLUSION: The progressive balance training program with whole body vibration stimulation is an effective intervention method with clinical significance in improving knee joint pain, knee disability index, psychosocial level, and balance ability in adults aged ≥ 65 with osteoarthritis of the knees.
본 연구의 목적은 뇌졸중 환자의 운동공포증과 낙상공포 사이에 융합적 관계를 조사하는 것이다. 총 113명의 뇌졸중 환자가 본 연구에 참여하였다. 참여자는 재활병원 입원환자로 낙상 효능감 척도(FES), 운동공포증을 위한 번역된 탐파 척도(TSK-13), 및 활동-특이성 균형 자신감 척도(ABC)를 포함한 3가지 다른 설문지를 작성하도록 요청하였다. TSK-13과 FES는 약한 음의 상관관계를 보였고(r=-0.226), TSK-13과 ABC는 중등도 음의 상관관계가 나타났다(r=0.838). FES는 ABC와 비교했을 때 매우 강한 양의 상관관계가 나타났다(r=0.838). 낙상을 경험한 환자는 낙상을 경험하지 않은 환자보다 유의하게 낮은 FES와 ABC 점수를 보였다(p<0.05). 이 결과는 뇌졸중 환자가 경증 운동공포증을 가지고 있으며, 운동공포증은 낙상공포와 관련이 있음을 제시하고 있습니다. 앞으로 뇌졸중 재활에 있어 운동공포증의 평가가 필요할 것으로 사료된다.
낙상은 노인의 신체적 건강은 물론 총체적 삶의 질을 위협하는 요인이다. 이 연구의 목적은 어떤 운동유형이 노인의 균형 능력 증진에 효과적인가를 확인하고, 이에 기초하여 노인 낙상 예방 운동 프로그램 개발에 필요한 기초 자료를 얻는 것이었다. 65세 이상 80세 미만의 여성 노인들을 스텝운동집단(21명), 코어근육운동집단(20명), 통제집단(21명) 중 한 집단에 무선 할당하였고, 8주 동안, 주 2회, 회당 20~30분의 운동을 처치하였다. 운동 효과는 기능적 균형검사(한발 정적균형검사)와 자기보고식 균형검사(낙상효능감, 균형자신감)로 평가하였다. 연구 결과를 요약하면 다음과 같았다. 첫째, 리드미컬한 스텝운동이 체간의 코어근육 강화운동보다 기능적 균형 증진에 효과적이었다. 특히, 눈을 뜨고 측정하는 한발 정적 균형검사에서 스텝운동의 효과가 뚜렷하게 나타났다. 둘째, 자기보고식 균형검사에서도 스텝운동이 코어근육운동보다 우수한 결과를 보였다. 특히, 리드미컬한 스텝운동이 체간의 코어근육운동보다 낙상 효능감 증진에 효과적이었다. 이상의 연구 결과를 종합해 볼 때, 리드미컬한 스텝운동이 체간의 코어근육운동보다 노인의 균형 능력 증진에 효과적이었다. 이는 리드미컬한 스텝운동이 하지 근육의 관여가 상대적으로 높고, 다양한 지면 변화 속에서 스텝운동을 수행하므로 낙상 발생 환경과 유사하며, 발목 고유수용성 강화에 적합한 운동유형이었기 때문이라고 사료된다. 따라서 향후 연구를 위해서 노인 낙상 사고가 발생하는 구체적인 동작 상황을 조사하고, 이에 기초하여 과제지향의 발목 고유수용성 감각강화 운동 중재 프로그램 및 운동 장비의 개발을 제안하였다.
Objectives: Cryptosporidium, a protozoan parasite, has been recognized as a frequent cause of waterborne disease due to its extremely strong resistance against chlorine disinfection. Although there has as yet been no report of a Cryptosporidium outbreak through drinking water in Korea, it is important to estimate the health risk of Cryptosporidium in water supply systems because of the various infection cases in human and domestic animals and frequent detection reports on their oocysts in water environments. Methods: This study evaluated the annual infection risk of Cryptosporidium in tap water using the quantitative microbial risk assessment technique. Exposure assessment was performed upon the results of a national survey on Cryptosporidium on the water sources of 97 large-scale water purification plants in Korea, water treatment efficacy, and daily unboiled tap water consumption. The estimates of the US Environmental Protection Agency on the mean likelihood of infection from ingesting one oocyst were applied for effect assessment. Results: Using probabilistic methods, mean annual infection risk of Cryptosporidiosis by the intake of tap water was estimated to fall within the range of $2.3{\times}10^{-4}$ to $1.0{\times}10^{-3}$ (median $5.7{\times}10^{-4}$). The risk in using river sources was predicted to be four times higher than with lake sources. With 0.5-log higher removal efficacy, the risk was estimated to be $1.8{\times}10^{-4}$, and could then be lowered by one-third. Conclusions: These estimations can be compared with acceptable risk and then used to determine the adequacy and priority of various drinking water quality strategies such as the establishment of new treatment technology.
Objective: The purpose of this study was to investigate the effects rhythmic and non-rhythmic aerobic exercises on depression and balance of healthy elderly people. Design: Randomized controled trial. Methods: Nineteen older subjects were randomly divided into 2 groups for rhythmic and non-rhythmic aerobic exercises. Both aerobic exercises consisted of functional movements such as turning in opposite directions, and running in place, the exercise consisted of movements that could activate balance. The rhythmic training group initially used music with 8 beats, and then later progressed to 16 beats. Additionally, we adjusted the pace of the music using songs from 125 beats per minute (bpm) to 160 bpm. Both groups were exercised for 50 minutes a day, twice a week, for a total of 8 weeks. We measured the condition of the patients before the intervention, and after 8 weeks of intervention. The Beck depression inventory (BDI) was used to measure the degree of depression. The Berg balance scale (BBS) was used to measure static and dynamic balance ability. We measured the subject's subjective balance confidence using the fall efficacy scale (FES). Results: Both groups showed significant improvement in BDI, BBS, and FES (p<0.05). The rhythmic aerobic exercise group showed a significant improvement only in the BBS change values compared to the non-rhythmic group (p<0.05). Conclusions: According to this study, both rhythmic and non-rhythmic aerobic exercises resulted in significant improvement in the degree of depression and balance ability of the elderly. The rhythmic aerobic exercise was more effective for dynamic balance ability.
본 연구는 지역사회에 거주하는 여성 노인을 대상으로 인지기능에 따른 낙상발생빈도, 낙상 두려움, 지각된 건강상태의 차이를 비교하고 관계를 규명하는 것을 목적으로 하였다. 대상자는 S시 S구 보건소에 방문하는 대상자로 65세 이상 여성 노인으로 선정하였다. 2009년 9월부터 2010년 3월까지 사전에 교육을 받은 간호학과 대학원생 조사원들이 일대일 면접을 통해 자료를 수집하였다. 연구에 참여한 311명의 평균연령은 71.68±5.13세였고, 대상자 평균 인지기능은 22.14±4.32점으로 정상 노인 경우 25.25±1.80점, MCI 노인의 인지기능은 18.02±3.57점으로 나타났다. 지난 1년간 낙상을 경험한 대상자는 109명으로 전체 35%였고 대상자의 평균 낙상 횟수는 1회 이하가 78%에 해당되었다. 일반적 특성에 따른 인지기능 점수 차이를 비교한 결과 나이가 많을수록 인지기능 점수가 유의하게 낮게 나타났고, 배우자가 있는 경우와 교육정도가 높을수록 인지기능의 점수가 유의하게 높게 나타났다. 인지기능 정도에 따른 두 그룹(MCI그룹, 정상 인지그룹)간 낙상횟수 및 낙상두려움, 지각된 건강상태를 분석한 결과 지각된 건강상태에서 유의한 차이를 보여 인지기능이 좋은 그룹에서 스스로 지각하는 건강상태가 높은 것으로 나타났다. 또한 인지기능과 낙상횟수, 낙상두려움 및 지각된 건강상태와의 상관관계를 분석하였다. 낙상 두려움은 인지기능과 유의한 부적 상관관계를 나타내었으며, 지각된 건강상태는 인지기능과 유의한 정적인 상관성을 나타내었다. 즉 인지기능이 떨어질수록 낙상 두려움의 증가와 지각된 건강상태가 낮은 것으로 나타났다. 본 논문은 향후 인지기능 정도에 따른 노인 낙상예방 프로그램 개발에 두 가지의 정책적 함의를 제공한다고 할 수 있다. 첫째, 인지기능 상태를 고려하여 낙상 두려움과 지각된 건강상태를 통합적으로 관리할 수 있는 낙상 예방 프로그램 개발이 필요하다. 둘째, 본 연구에서 분석되지 않은 일반적으로 알려진 다른 요인들을 포함하여 병원, 노인 요양시설 등 다양한 곳에 거주하고 있는 남·여 노인을 대상으로 한 지속적인 연구의 필요성을 제시한다. 이 연구를 토대로 인지기능 향상과 낙상예방을 위한 포괄적인 교육을 통해 낙상두려움을 감소시키고 낙상 발생률을 낮추면, 궁극적으로 노인의 삶의 질이 향상될 것이다.
Purpose: The purpose of this study was to investigate the relationships between a experience of falling, fear of falling, depression, and perceived health status in urban areas. Methods: After obtaining Institutional Review Board (IRB) approval, a one-time, face-to-face, and private interview was conducted with each participant who was eligible and agreed to participate in this study from May 2007 to August 2007 by trained graduate-level nursing students. The questionnaires consisted of Fall Efficacy Scale, K-GDS, and SF-36. The collected data were analyzed with SPSS/PC 12.0 program, which was used for frequency, percentage, mean, standard deviation, t-test, and hierarchical regression. Results: The major findings of this study were as follows; 1) approximately 9% of participants had fallen within one year. Study participants reported moderate fear of falling(M=43.80); moderate physical health (M=42.31) and mental health(M=46.05); and low depression status(M=10.38). 2) there were significant differences in fear of falling according to gender, experience of falling, exercise, and depression. 3) significant factors influencing on fear of falling were experience of falling and physical health status. Conclusion: The results of this study indicate that nurses working closely with older adults should be taught the impact of previous falls and physical health status on fear of falling and need to reinforce exercise behavior for older adults who are vulnerable to fear of falling.
