요부 안정화운동과 하지 근력강화운동이 만성 요통환자의 통증과 하지근력에 미치는 영향 (The Effects of Lumbar Stabilization Exercise and Strengthening Exercise of Lower Extremity on Pain and Muscle Strength of Leg in Patients with Chronic Low Back Pain)

  • Purpose: This study was to investigate the effects of lumbar stabilization exercise and lower extremity strengthening program on pain, lower extremity muscle in patients with chronic low back pain. Method: The subject were randomly divided two groups. 15 people who were conducted lower extremity exercises and lumbar stabilization exercises called the combined exercise group and other(15 people) who were only conducted lower extremity exercise group. The assessment tools were the pain level and the led muscle power. Exercise was conducted for eight weeks. Result: Pain of the combined exercise group showed significant differences in the change in pain level(p<.05). The leg muscle power showed significant differences within group which hip flexor, extensor muscles and abductor muscles. there were significant differences within combined exercise group (p<.05). But the knee joint in each group showed a significant difference within group (p<.05). Conclusion: This study suggest that the lumbar stabilization exercises and lower extremity exercises showed more efficient results in the pain levels and leg muscles power than only the lower extremity exercise for patients with lumbar instability.

Effects of Manual Therapy on Upper Extremity Pain after Breast Cancer Surgery: A Systematic Review and Meta-analysis

  • Objective: The most common type of cancer in women is breast cancer, and pain in the upper extremity and trunk is a discomfort experienced by more than half. Based on the evidence that manual therapy is effective for pain control in postoperative rehabilitation, this study aims to analyze the effects of manual therapy on upper extremity pain and function in patients after breast cancer surgery. Design: A systematic review and meta-analysis. Methods: We searched MEDLINE, Embase, PEDro, and CINAHL databases until August 2021. We included randomized controlled trial evaluating pain and function in patients after breast cancer surgery. Qualitative analysis was performed using Cochrane's risk of bias tool, and quantitative analysis was performed using RevMan 5.4 to analyze post-intervention outcomes. Results: Four randomized controlled trials were selected to evaluate the effects of upper extremity pain and function in 133 patients who underwent manual therapy after breast cancer surgery. In the results of qualitative and quantitative analysis, the experimental group treated with manual therapy showed a significant improvement in pain compared to the control group (-0.62; 95% confidence interval (CI) -0.97 to -0.27). However, there was no significant improvement in upper extremity function (-0.09; 95% CI -0.43 to 0.25). Conclusions: Current evidence shows that manual therapy is effective for pain control in patients who complain of upper extremity pain after breast cancer surgery.

유방암 생존자의 상지기능, 통증 및 불안과의 관계 (Relationships among Pain, Upper Extremity Function, and Anxiety in the Breast Cancer Survivors)

  • Purpose: This study was to compare pain, upper extremity function, and anxiety among disease characteristics in the breast cancer survivors and to clarify the relationship among these variables. Methods: One hundred twenty two participants with breast cancer survivors over the age of 30 were recruited from a general hospital. Data were collected from November 1 to December 25, 2006 using a structured questionnaire. Results: The mean age was 51.17 and their mean survival period was 38.08 months. The breast cancer survivors who had received radiation therapy reported lower levels of pain and upper extremity function, and higher levels of anxiety than those who had other treatments. Pain and anxiety were positively related, and upper extremity function was negatively related to pain and anxiety. Conclusion: The breast cancer survivors experienced pain, upper extremity function disorder and anxiety. This study indicates that nursing interventions for the breast cancer survivors may be needed to improve upper extremity function, and to reduce pain and anxiety.

Verbal Aggression Against Teacher and Upper Extremity Musculoskeletal Pain

  • Background: This study investigated the relationship between verbal aggression against school teachers and upper extremity (neck, shoulder, upper limb, and/or upper back) musculoskeletal pain. Methods: This was a cross-sectional study of 525 elementary school teachers from Jaboatão dos Guararapes, Northeast Brazil. Results: The prevalence of upper extremity musculoskeletal pain among teachers who reported verbal aggression in the past six months (67.7%) was higher than that among those who did not report verbal aggression (51.7%): (prevalence ratio = 1.21; 95% confidence interval = 1.04-1.40). The prevalence of upper extremity musculoskeletal pain was associated with verbal aggression, sex, and common mental disorders, controlled by skin color, age, monthly income, teachers' education, years working as a teacher, workload, and obesity. Furthermore, the measure of the association between verbal aggression and upper extremity musculoskeletal pain was modified by sex and common mental disorders, considered altogether. Teachers who suffered verbal aggression, of the feminine sex, and also having common mental disorders reported high prevalence (85.4%) of upper extremity musculoskeletal pain. Conclusion: The association between verbal violence in the school and complaints of upper extremity musculoskeletal pain was strong and modified by teachers' sex and common mental disorders.

좌측 하지의 관절구축을 동반한 작열통의 치험 (A Case of Causalgia with Contracture Deformity in Lower Extremity)

  • Causalgia is a symptom complex usually consisting of burning pain, hyperesthesia and atrophy of the involved extremity. The pain may be aggravated by contact, motion of extremity or emotional excitement. It occurs following incomplete nerve injury. The patient was a 58-year-old male with a 3-year history of causalgic pain of left lower extremity. He had multiple fractures with degloving injury of left lower extremity in an automobile accident. The pain was exacerbated by movement, and he experienced knee joint and ankle joint contracture. The patient's pain decreased after continuous epidural block with 1% lidocaine and 0.25% bupivacaine. He also received lumbar sympathetic ganglion alcohol block resulting in much improvement of level of pain and walking disturbance.

간호사의 근무로 인한 하지부종과 통증 발생 및 자가다리마사지 효과 (Lower Extremity Edema and Pain of Nurses and the Effect of Self Leg Massage)

  • Purpose: This study was done to compare edema and pain after completing a nurse's daily shift and to examine the effects of self leg massage which was usually used for reducing nurses' lower extremity edema and pain after their shift. Methods: The research design was a nonequivalent control group pretest-posttest design. Study subjects were 81 nurses who took a self leg massage program created by the researcher. Self leg massage was done 15 times during 3 weeks. Data were collected from September 21 to October 31, 2007. The level of lower extremity edema was measured by ankle and calf circumference by a tapeline in cm and the pain score was measured by using a subjective numbering rating scale. Data were analyzed with the SPSS 12.0 program using statistics of repeated measures ANOVA. Results: There was a statistically significant difference in pain according to the department. There was a statistically significant difference in lower extremity edema and pain in nurses after their shift and self leg massage. Conclusion: Self leg massage was effective for relieving nurses' lower extremity edema and pain. Therefore, it is proposed that standardized self leg massage should be applied as a method for nurses' lower extremity edema and pain.

체외충격파치료가 팔꿉관절 가쪽위관절염 환자의 통증과 악력 및 팔 기능에 미치는 영향 (Effect of Extracorporeal Shock-wave Therapy on Pain, Grip Strength, and Upper-extremity Function in Patients with Lateral Epicondylitis)

  • Purpose: The purpose of this study was to investigate the effects of extracorporeal shock-wave therapy (ESWT) on pain, grip strength, and upper-extremity function in patients diagnosed with lateral epicondylitis and to provide an effective intervention method for lateral epicondylitis. Methods: Twenty patients with lateral epicondylitis were randomly assigned to the ESWT group (n = 10) and the stretching exercise group (n = 10). Interventions in both groups were performed six times twice a week for three weeks. The visible analog scale (VAS) was used to measure pain change. A dynamometer was used to measure grip strength (GS). Patient-rated tennis elbow evaluation (PRTEE) was used to measure the upper-extremity function. Results: There were significant differences in pain, grip strength, and upper-extremity function in both groups before and after intervention (p < 0.05). There were also significant differences in pain, grip strength, and upper-extremity function between the groups after intervention (p < 0.05). Conclusion: This study showed very positive improvement in pain, grip strength, and upper-extremity function after ESWT in patients with lateral epicondylitis. Therefore, ESWT can be recommended for patients with lateral epicondylitis.

하지길이 차이가 요통과 관절경직에 미치는 영향 (The Effects of leg length difference on Low Back Pain and joint stiffness)

  • Human body balances right and left leg centering around pelvis and spine. Therefore, imbalance of lower extremity means disequilibrium of the body. The difference of lower extremity length can cause a number of clinic symptoms including scoliosis, low back pain, sacroiliac pain, and sports injury. In this study, we tried to analyze low back pain and joint stiffness resulting from the difference of lower extremity length. The subjects were 80 male students who are 20-25 years old. The results of this study were as following: 1. Low back pain depending on the difference of lower extremity length One group which the difference of lower extremity length is above 12mm showed average different length as 18.0mm, the other group which one is below 12mm showed as 6.3mm. A group of above 12mm had more severe low back pain than a group of below 12mm. 2. Joint stiffness depending on the difference of lower extremity length A group of above 12mm had more severe joint stiffness than a group of below 12mm.

슬개대퇴통증 증후군 환자에게 적용한 고관절 견인과 근력 강화 운동이 무릎 통증, 관절가동범위, 하지기능에 미치는 영향-사례 연구 (A Case Study of a Female Patient with Patellofemoral Pain Syndrome for Effect of Hip Joint Traction and Hip Posterolateral Muscles Strengthening on Knee Pain, Range of Motion, and Lower Extremity Function Scale)

  • Background: The case study examined the effect of a hip joint traction and hip posterolateral muscles strengthening on knee pain, range of motion, and lower extremity function scale of patients with patellofemoral pain syndrome (PFPS). Although PFPS has previously been attributed to quadriceps dysfunction, more recent research has linked this condition to impairment of the hip musculature and kinematic. Methods: Subject is a 27-years-old female with PFPS. Performed hip joint traction with belt and posterolateral muscles(hip abductors, external rotators) strengthening for 4 weeks, 3 times a week, once a day. Before and after the therapy, measurements were made on the visual analog scale (VAS) and of the ROM, and a lower extremity functional scale (LEFS) was conducted. Results: The results showed positive changes in VAS and range of motion and lower extremity functional scale. First VAS of knee changed from 6 to 2. Second hip joint range of motion showed that internal rotation recored from $53^{\circ}$ to $58^{\circ}$ and external rotation recorded from $32^{\circ}$ to $37^{\circ}$. Third The lower extremity functional scale showed before therapy of 44; after therapy, 63. Conclusion: The hip joint traction and hip posterolateral muscles strengthening was effective in alleviating knee pain, increasing ROM and Lower extremity functional scale of the PFPS patients.

탄력압박스타킹 적용이 병동간호사의 하지통증과 하지부종에 미치는 효과 (The effects of elastic compression stockings on lower leg pain and edema in ward nurses)

  • 본 연구는 탄력압박스타킹 적용이 장시간 서서 근무하는 병동간호사의 하지통증과 하지부종에 미치는 효과를 검증하기 위해 시도되었다. 연구설계는 비동등성 대조군 전후 설계를 이용한 유사실험 연구이며, 연구 대상자는 M시에 소재한 2개 종합병원에서 병동에 근무하는 간호사로 탄력압박스타킹을 적용한 실험군 35명, 대조군 30명으로 총 65명을 선정하였다. 결과는 제 1 가설 : "탄력압박스타킹을 적용한 실험군과 탄력압박스타킹을 적용하지 않은 대조군간 하지통증에 차이가 있을 것이다" 는 지지되었다(p<.001). 제 2 가설 : "탄력압박스타킹을 적용한 실험군과 탄력압박스타킹을 적용하지 않은 대조군간 하지부종에 차이가 있을 것이다" 는 지지되었다(p<.001). 결론은 병동간호사의 하지통증과 하지부종에 대한 관리의 한 방법으로 탄력압박스타킹을 적용하여 근무 중 탄력압박스타킹 적용이 하지통증과 하지부종에 관련된 증상을 완화하고, 하지 순환계의 건강문제를 예방하여 간호사의 건강을 증진시키고, 질적인 간호를 제공할 수 있도록 하는 효과적인 방법이 될 것으로 사료된다.
