Background: Isolated infraspinatus tear is very rare and clinical features are not as well known, therefore the purpose of this study was to evaluate clinical characteristics and outcomes of isolated infraspinatus tear that authors experienced. Methods: Authors reviewed 288 cases of full-thickness rotator cuff tear involving infraspinatus between 2010 and 2015, and retrospectively analyzed six cases of isolated infraspinatus tear. Perioperative clinical characteristics, postoperative functional outcomes of 6 months were investigated. Functional evaluation included visual analogue scale (VAS), range of motions, American Shoulder and Elbow Surgeons (ASES) score, and Constant score. Results: Calcific tendinitis was accompanied in 4 cases (66.7%). Three of them received steroid injection or aspiration or extracorporeal shockwave therapy. Mean preoperative pain VAS was 7.1 (range, 5-9), and mean postoperative pain VAS at 6 months later was 1.6 (range, 0-5). Preoperative muscle strength by isokinetic muscle performance test showed 52% deficit of abduction and 37.6% deficit of external rotation. All 6 patients had arthroscopic repair of the infraspinatus tendon. All the patients at the 6 months follow-up exhibited clinical improvement in the Constant score (67.8 [range, 45-77] to 89.3 [range, 81-100], p=0.029), and ASES score (52.3 [range, 30-77] to 90.0 [range, 80-100], p=0.002). There was no healing failure on imaging. Conclusions: Isolated infraspinatus tendon tear was frequently accompanied by calcific tendinitis, but pathophysiologic relationship should need more study. To rule out neurogenic etiology, magnetic resonance imaging and electromyography would be helpful. Arthroscopic infraspinatus tendon repair and supraspinatus debridement showed relatively good result in painful shoulder.
Early in 21st century, researches about intelligent service robot that provide various services for a human out of the industrial robot only has simple pattern repetition. It concentrates in the research regarding the URC(Ubiquitous Robotic Companion) robot which connects the network in the intelligent service. This paper proposes the robot system based on network to guide a way. The robot has made by lego brick and used ultrasonic sensor, rotation sensor and RFID tag to recognize external environment. Also, it includes a PDA to process the data between robot and server. The network server transmits information to robot controller by bluetooth and it controls the course movement and evasion of the robot. In this research, the robot system based on network to guide a way is easy to expand service and is able to process a data in real time due to data processing in the server as a part of intelligent robot. And it can reduce the cost to build a robot thank to use cheaper sensor equipment.

Background: The purpose of this study was to evaluate the efficacy of suction drain use following arthroscopic rotator cuff repair by comparing early pain score and range of motion (ROM) between groups with and without suction drains. Methods: The study included 153 patients with rotator cuff tears who underwent arthroscopic repairs at our clinic from April 2014 to March 2015. Following surgery, a suction drain was used in 85 patients (group D) and not used in 68 patients (group ND). There was no statistical difference between the groups in terms of age, gender, or total operation time. The clinical outcome with regard to pain (assessed by pain scores and analgesic requests) and passive ROM was assessed preoperatively and postoperatively. Results: Immediate postoperative analgesic requirement was significantly higher in group D (p=0.001), although there was no difference in pain outcomes between the groups during the 3-month follow-up period. A statistically significant difference in passive ROM was observed at the postoperative 2- and 6-week follow-ups (p=0.036, 0.035, and 0.034 in forward elevation (FE), external rotation at the side (ER) and 90 ER at weeks 2, respectively; 0.045 and 0.009 in FE and ER at weeks 6, respectively); however no significant difference was observed at the end of 3 months. During the study period, no complication was reported in either group. Conclusions: Use of suction drains after arthroscopic rotator cuff repair provided little benefit in terms of ROM or pain in the early postoperative period (up to 3 months).
Background: Patients after rotator cuff (RC) surgery experienced pain, weakness and limited of motion of the shoulder. Physical therapists have used heat therapy, electrotherapy, range of motion (ROM) exercise and other methods to treat patients after RC surgery. In addition, functional taping is also used to support joint movement and to increase shoulder joint stability. Objects: The purpose of this study was to determine the initial effects of functional taping using non-elastic tape on pain, strength and ROM of the shoulder following RC surgery. Methods: Forty-eight patients with who underwent RC surgery volunteered for this study. The subjects were randomly divided into an experimental group (EG, $n_1=25$) and a control group (CG, $n_2=23$). First, non-allergic tape was applied to the shoulder to prevent skin irritation. The EG applied functional taping using non-elastic tape and the CG applied sham taping using elastic tape. Assessment tools included the shoulder pain and disability index for functional activity score, visual analog scale for level of pain, shoulder muscle strength, hand grip strength and ROM testing. Results: Pain score in the both group significantly decreased (p<.05), and change in pain score of in the EG increased significantly than in the CG (p<.05). Shoulder strength and ROM in the both group significantly increased (p<.05). Especially external rotation and extension of the shoulder ROM in the EG increased significantly more than in the CG (p<.05), but the rate of change in the two groups showed no significant difference. Conclusion: These results suggest that functional taping using non-elastic tape was initially effective in decreasing pain score level in patients with RC surgery.
The purpose of this study was to characterize the impact shock wave and its attenuation, and the kinematic response of the lower extremity's joints to the impact shock during downhill running in which the lower extremity's extensor acts dominantly. For this study, fifteen subjects(mean age:$27.08{\pm}4.39$; mass:$76.30{\pm}6.60$; height:$177.25{\pm}4.11$) were required to run on the 0% grade treadmill and downhill grades of 7%, and 15% in random at speed of their preference. When the participant run, acceleration at the tibia and the sacrum and kinematic data of the lower extremity were collected for 20s so as to provide at least 5 strides for analysis at each grade. Peak impact accelerations were used to calculate shock attenuation between the tibia and sacrum in time domain at each grade. Fast Fourier transformation(FFT) and power spectral density(PSD) techniques were used to analyze impact shock factors and its attenuation in the frequency domain. Joint coordinate system technique was used to compute angular displacement of the ankle and knee joint in three dimension. The conclusions were drawn as fellows: 1. Peak impact accelerations of the tibia and sacrum in downhill run were greater than that of 0% grade run, but no significant between conditions. Peak shock of PSD resembled also in pattern of peak impact acceleration. The wave of impact shock attenuation between the tibia and sacrum decreased with increasing grade, but didn't find a significant difference between grade conditions. 2. Adduction/abduction, flexion/extention, and internal/external rotation of the ankle and knee joints at support phase between grade conditions didn't make much difference. 3. At grade of 7% and 15%, there were relationship between the knee of the flexion/extension movement and peak impact acceleration during heel strike and found also it in the ankle of plantar/dorsiflexion at grade of 15%.

The purpose of this study was to determine the relationship between different soccer cleat designs and knee joint moments. Twelve physically active males (mean(SD): age: 26.4(6.2)yrs; height: 176.4(4.1)cm; mass: 74.0 (7.4)kg) were recruited Kinematic and force plate data were collected for all subjects during normal running and a $45^{\circ}$ cutting maneuver, called a v-cut. Both motions were performed at $4.0{\pm}0.2\;m/s$ on infilled artificial turf with three pairs of soccer cleats of different sole plate designs, and one pair of neutral running shoes. Inverse dynamics were used to calculate three dimensional knee joint moments, with repeated measures ANOVA and post hoc paired Student's t-test used to determine significance between shoe conditions. Significant differences were found in the extension moments of the knee for running trials, and for external rotation and adduction moments in the v-cutting trials. Knee moments were greater in v-cut than running, and the traditional soccer cleats (Copa Mondial and World Cup) tended to result in greater knee moments than the Nova runner or TRX soccer cleat. Cleat design was found to influence 3-dimensional knee moments in a v-cut maneuver. In the translational traction test, there were significant differences between all conditions. In the rotational traction test, friction with soccer shoes were greater than friction with running shoes. However, no differences were found between soccer shoes. Higher moments may lead to increased loads and stresses on knee joint structures, and thus, greater injury rates.
Objectives : Hemiplegic shoulder pain is one of the common sequelae of stroke. Although many different methods of treatment are applied, none have yet been proved to be effective. In this study we assessed the effectiveness of bee venom in patients with shoulder pain after stroke. Methods : This study was a double blinded, randomized, placebo-controlled clinical trial. All subjects received either 0.6 ml bee venom injection (venom:saline = 1:10000) or 0.6ml physiological saline solution (placebo) at three acupoints. The effectiveness was assessed by visual analogue scale, pain rating score, Fugl-Meyer assessment and passive external rotation. Results : There were 24 patients in the venom group and 22 patients in the placebo group. The venom group5,hewed more effectiveness in VAS and PRS than the placebo group. Conclusions : This study suggests that bee venom injection has significant analgesic effect on hemiplegic shoulder pain. Further study based on multi-centers, larger population, and long term 1311ow-up is needed to confirm this suggestion.

Total ankle replacement (TAR) is a visible option in the surgical treatment of degenerative or inflammatory diseases of ankle joint. it is attributed to the current TAR which has improvements in surgical technique, uncemented implant fixation and minimally constrained articulation. In the clinical result, they can show promised surgical result when compared to earlier attempts in TAR. However, TAR is still not as successful as total knee replacement (TKR) or total hip replacement (THR), it needs to be note that there are limitations in concerning of long term performance of TAR, the high failure rate still associated with wear of the PE (polyethylene) component that has related with their material property and surface roughness. The aim of this study was to introduce the tribology characteristics of total ankle joint prosthesis with one of TDR model which was fabricated to try multi-axis wear test as a region of motion in ankle joint. The wear specimen of TDR was prepared with Ti-6Al-4V alloy and UHMWPE (ultra-high molecular weight polyethylene) for tibia-talus and bearing component, respectively. A wear test was carried out using a Force 5 (AMTI, Massachusetts, US) wear simulator which can be allowed to move in three axis to flexion-extension ($+3^{\circ}{\sim}-6^{\circ}$), internal-external axial rotation (${\pm}5^{\circ}$), as well as sinusoidal compressive load (1.6 kN, R=10). All tests were performed following standard ISO 14243, wear rate was calculated with weight loss of UHMWPE bearing while the specimen has tested at certain cycles. As based on the preliminary results, wear rate of UHMWPE bearing was $7.9{\times}10^{-6}mg/cycles$ ($R^2=0.86$), calculated loss weight until $10^7cycles$ was 79 mg, respectively.
The purpose of this study is to investigate the types and effects of rehabilitation by systematically examining the rehabilitation research for improving the shoulder ROM in breast cancer resection patients. For a systematic review, a total of 8 studies were finally selected through three databases including RISS. As a result of this study, the level of qualitative was 4 studies(50.0%) in level III, and the rehabilitation period varied from 4 weeks to 12 weeks. As for the type of rehabilitation, 3 studies(37.5%) were occupational therapy and physical therapy. The ROM of the shoulder joint was 7 studies(87.5%) of flexion, extension, and external rotation. A goniometer was used in 8 studies(100.0%) to evaluate the ROM of the joint as a measuring tool. This study is expected to provide basic data in the field of convergence when developing and applying interventional protocols for patients with limited ROM due to breast cancer resection.

Manual wheelchair propulsion can lead to pain and injuries of users due to mechanical inefficiency of wheelchair propulsion motion. The kinetic analysis of the upper limbs during manual wheelchair propulsion needs to be studied. A two dimensional inverse dynamic model of upper limbs was developed to compute the joint torque during manual wheelchair propulsion. The model was composed of three segments corresponding to upper arm, lower arm and hand. These segments connected in series by revolute joints constitute open chain mechanism in sagittal plane. The inverse dynamic method is based on Newton-Euler formalism. The model was applied to data collected in experiments. Kinematic data of upper limbs during wheelchair propulsion were obtained from three dimensional trajectories of markers collected by a motion capture system. Kinetic data as external forces applied on the hand were obtained from a dynamometer. The joint rotation angles and joint torques were computed using the inverse dynamic model. The developed model is for upper limbs biomechanics and can easily be extended to three dimensional dynamic model.
