볼루스를 이용한 방사선영상검사 피폭선량저감 연구 (A Study of Radiation Dose Reduction using Bolus in Medical Radiation Exam)

  • 방사선영상검사에서 발생하는 피폭선량에 대한 선량제한이 없으므로 방사선방호를 위한 정당화와 최적화가 반드시 필요하다. 조직등가물질 볼루스를 이용하여 방사선영상검사로 인한 피폭선량을 저감하면서 동시에 영상화질을 유지할 수 있는 범위를 확인하였다. 손 후전방향 프로젝션을 기준으로 50 kVp, 5 mAs, SID 100 cm, 8×10 inch 조건으로 볼루스를 0, 3, 5, 8, 10 mm 적용하여 손 부위 입사선량과 영상의 SNR을 확인하였다. 볼루스의 두께별 손 부위 입사선량(μGy)은 각각 125.41±0.288, 106.85±0.255, 104.97±0.221, 91.68±0.299, 90.94±0.106으로 나타나 볼루스의 사용여부와 그 두께에 따라 유의한 피폭선량 감소를 보였다. 영상의 SNR은 볼루스 각 두께에서 13.997, 13.906, 12.240, 12.538, 12.548로 나타나 큰 차이를 보이지 않았다. 방사선영상검사의 종류와 검사 부위에 따라 적절한 두께와 크기의 볼루스를 사용한다면 영상화질의 저하없이 유의한 피폭선량 저감효과를 볼 수 있음을 확인하였다.

Fluoroscopic Radiation Exposure during Percutaneous Kyphoplasty

  • Objective: The author measured levels of fluoroscopic radiation exposure to the surgeon's body based on the different beam directions during kyphoplasty. Methods: This is an observational study. A series of 84 patients (96 vertebral bodies) were treated with kyphoplasty over one year. The patients were divided into four groups based on the horizontal and vertical directions of the X-Ray beams. We measured radiation exposure with the seven dosimetry badges which were worn by the surgeon in each group (total of 28 badges). Twenty-four procedures were measured in each group. Cumulative dose and dose rates were compared between groups. Results: Fluoroscopic radiation is received by the operator in real-time for approximately 50% (half) of the operation time. Thyroid protectors and lead aprons can block radiation almost completely. The largest dose was received in the chest irrespective of beam directions. The lowest level of radiation were received when X-ray tube was away from the surgeon and beneath the bed (dose rate of head, neck, chest, abdomen and knee: 0.2986, 0.2828, 0.9711, 0.8977, 0.8168 mSv, respectively). The radiation differences between each group were approximately 2.7-10 folds. Conclusion: When fluoroscopic guided-KP is performed, the X-Ray tube should be positioned on the opposite side of the operator and below the table, otherwise the received radiation to the surgeon's body would be 2.7-10 times higher than such condition.

디지털 유방확대촬영술에서 노출방식에 따른 피폭선량 평가 (The Evaluation of Radiation Dose by Exposure Method in Digital Magnification Mammography)

  • 디지털 유방촬영술은 유방의 두께 및 유선조직의 밀도에 따라 노출조건이 자동으로 이루어지는 자동노출제어장치를 사용하고 있는데 이는 피폭선량을 증가시키는 원인이 되기도 한다. 이 연구에서는 디지털 유방확대촬영술에서 영상의 질을 유지하며 불필요한 방사선 피폭을 줄이기 위한 최적의 방사선량 촬영조건을 알아보았다. 자동노출방식을 사용할 경우 가장 좋은 영상화질을 얻을 수 있었으나 평균유선선량 역시 가장 높았다. 수동노출방식의 경우 자동노출방식의 55 % 정도의 mAs 값만으로도 기준 팬텀영상 점수와 신호대잡음비를 얻을 수 있으며 평균유선선량은 55 % 이하로 줄일 수 있었다. 위와 같은 결과를 참고로 하면 디지털 유방확대촬영 시 자동노출방식보다는 수동노출방식을 이용함으로써 불필요한 환자의 피폭선량을 줄일 수 있을 것이라 생각된다.

항공승무원의 우주방사선 피폭 저감에 관한 연구 (A Study on the Reduction of Cosmic Radiation Exposure by Flight Crew)

  • The purpose of this study is to analyze the radiation dose data of the space crew of the flight crew and to present a plan for the health management of the flight crew on the basis of the analysis. The analysis show that the average exposure dose of the flight attendants continued to rise, and the exposure dose of the flight attendants was five(5) times higher than that of the radiation workers. As a way to reduce the effects of cosmic radiation, this paper suggests appropriate personnel allocation by model, balanced allocation of high and low latitude routes by crew according to the aircraft type, and a low altitude flight plan for high latitude flight. This study will help aviation crew members understand cosmic radiation and trust in the company's policies. In the future, it will be necessary to enhance the flight safety of the crew by deriving meaningful results by analyzing data related to cosmic radiation of various routes.

중재적 시술 시 팬텀을 이용한 환자의 피폭선량 분석 (TA Study on Patient Exposure Dose Used the Phantom for Interventional Procedure)

  • Because interventional procedure operates looking at premier as real time when perate intervention enemy, by patient is revealed during suitableness time in radiation, side effect such as radiation injury of skin is apt to happen. It established by purpose of study that measure exposure dose that patient receives about these problem, and find solution for radiation injury and repletion method. In this study, we used Rando phantom of identical structure with the human body which becomes accomplished with 4 branch ingredient of the attempt and system equivalent material them and absorbed dose were measured by TLD. According to the laboratory, it shows that operations such as TFCA procedure or uterine myoma embolization are more dangerous than TACE procedure. If both operations are inspected during a short time, it is not affected in being bombed. However, it can lead to palliative agenesis or depilate, definitive agenesis only if operations are repeated more than three times. Dose distibution based on experiment, to reduce radiation exposure to patients result from reduction of scatter ray as we control field size of radiation and protection of side organs except for tumor. also we knew that we can protect patients form radiation exposure, if we increas SOD and decrease SID.

Estimation of long-term effective doses for residents in the regions of Japan following Fukushima accident

  • A large amount of radioactive material was released from the Fukushima Daiichi Nuclear Power Plant (FDNPP) in 2011 and dispersed into the environment. Though seven years have passed since the Fukushima Daiichi Nuclear Power Plant accident, some parts of Japan are still under the influence of radionuclide contamination, especially Fukushima Prefecture and prefectures neighboring Fukushima Prefecture. The long-term effective doses and the contributions of each exposure pathway (5 exposure pathways) and radionuclide ($^{131}I$, $^{134}Cs$, and $^{137}Cs$) were evaluated for people living in the regions of Fukushima and neighboring prefectures in Japan using a developed dose assessment code system with Japanese specific input data. The results estimated in this study were compared with data from previously published reports. Groundshine and ingestion were predicted to contribute most significantly to the total long-term dose for all regions. The contributions of each exposure pathway and radionuclide show different patterns for certain regions of Japan.

A study on pressurizer cutting scenario for radiation dose reduction for workers using VISIPLAN

  • The operations in the design lifecycle of a nuclear power plant targeted to be decommissioned lead to neutron activation. Operations in the decommissioning process include cutting, decontamination, disposal, and processing. Among these, cutting is done close to the target material, and thus workers are exposed to radiation. As there are only a few studies on pressurizers, there arises the need for further research to assess the radiation exposure dose. This study obtained the specifications of the AP1000 pressurizer of Westinghouse and the distribution of radionuclide inventory of a pressurizer in a pressurised water reactor for evaluation based on literature studies. A cutting scenario was created to develop an optimal method so that the cut pieces fill a radioactive solid waste drum with dimensions 0.571 m × 0.834 m. The estimated exposure dose, estimated using the tool VISIPLAN SW, in terms of the decontamination factor (DF) ranged from DF-0 to DF-100, indicating that DF-90 and DF-100 meet the ICRP recommendation on exposure dose 0.0057 mSv/h. At the end of the study, although flame cutting was considered the most efficient method in terms of cutting speed, laser cutting was the most reasonable one in terms of the financial aspects and secondary waste.

  • X-선과 감마선의 에너지에 따른 선질특성과 선량 및 필름의 사진농도와의 관계를 이용한 필름배지선량계의 선량환산식을 실험적으로 체계화 하였고, 시간경과에 따른 잠상퇴행 특성과 방사선의 입사방향에 따른 방향특성을 실험적으로 조사하여 보정계수를 산출하였다. 본 연구에서 구한 선량환산식은 필름배지선량계의 기술기준인 성능판정 기준을 잘 만족시키는 것으로 나타났다.

병원 규모별 진단용 X선 발생장치의 성능 평가 (A Performance Evaluation of Diagnostic X-ray Unit Depends on the Hospitals Size)

  • 일부지역의 병원에 설치, 운영되고 있는 진단용 X선 발생장치틀 의원, 병원, 종합병원별로 관 전압, 관 전류/량, 조사기간, 조사선량 성능을 평가하여 측정방법과 기술을 배우고 중요성을 알리고자 한다. 진단용 X선발생장치를 그룹별로 10대씩 총 30대에 대하여 관전류/량, 조사시간측정기를 사용하여, 백분율평균오차로 평가하였고 그에 따른 조사선량의 재현성은 변동계수를 계산하여 평가하였다. 진단용 X선 발생장치 30대 중 부적합한 장치는 관전압정확도 시험에서 5대(16.7%), 관전류정확도시험에서 3대(10,0%), 관전류량정확도 시험에서는 4대(13.3%), 조사시간정확도 시험에서는 5대(16.7%), 조사선량의 재현성 시험에서는 7대(23.3%)로 조사되었다. 진단용 X선 발생장치를 관리규정에 의거하여 성능검사 결과에 의해 성능관리의 심각성을 알 수 있었고, 이에 주기적으로 실시하여야 함은 물론 안전하게 관리함으로써 환자 및 방사선관계종사자에 대하여 방사선으로 인한 방사선 피폭경감, 화질영상관리, 재촬영 감소 둥에 의해 환자에게 양질의 의료서비스를 제공하게 될 것이다. 따라서 정기적인 성능검사가 필요하다고 사료된다.

마우스 소뇌과립층의 apoptosis를 지표로 한 진단용 초음파의 안전성 검증 (The evaluation on the biological safety of diagnostic ultrasound using radiation-induced apoptosis in the external granular layer of mouse cerebellum)

  • We have studied, by a nonisotopic in situ end-labeling(ISEL) technique, frequency of apoptosis in the external granular layer(EGL) of the cerebellum of immature mice by ${\gamma}$-rays irradiation from $^{60}Co$ or diagnostic ultrasound exposure. The total number of normal cells and cells showing morphological features of apoptosis were counted. The frequency of apoptotic cells was expressed as a percentage of the total number of cells in EGL. The extent of changes following 200 cGy(1090 cGy/min) was studied at 2, 4, 6, 8, 12, or 24 hours after exposure. The maximal frequency was found 6~8 hours after exposure. The immature mice that received 18, 36, 54, 108, 198, 396 cGy of ${\gamma}$-rays or diagnostic ultrasound(7.5MHz, 4.2mW, $I_{SPTA}=7.9mW/cm^2$, $I_{SPTA}=114.3W/cm^2$) for 10 or 30 minutes were examined 6 hours after irradiation. Measurements performed after ${\gamma}$-ray irradiation showed a dose-related increase in apoptotic cells in each of the mice studied. The dose-response curves were analyzed by a linear-quadratic model ; frequency of apoptotic cell in the EGL was y = $(0.1349{\pm}0.01175)D$+$(-0.0001522{\pm}0.0000334)D^2$+0.048($r^2$ = 0.981, D = dose in cGy). In the experiment of ultrasound exposure, the frequency of apoptotic cell was $0.106{\pm}0.130$(10 minutes exposure) and $0.167{\pm}0.220$(30 minutes exposure). We estimated the relative dose of the yield from the experiment with ultrasound by substituting the yield from ultrasound exposure into the curve from the ${\gamma}$-irradiation. The relative dose of ultrasound exposure compared with ${\gamma}$-irradiation were 0.432 cGy(10 minutes exposure) and 0.885 cGy(30 minutes exposure). We have found that there is no evidence to indicate that diagnostic ultrasound involves a significant risk.
