휴벽에 발생한 원발성 연골육종 (A case of the primary chondrosarcoma of the chest wall)

  • A 36 years old male patient underwent en bloc excision of the primary chondrosarcoma arising from 8th, 9th and 10th ribs of the chest wall. The tumor was 11.5 x 7.5 x 6.5 cm in size and was 320 gram in weight. After en bloc excision of the tumor, the defect of the chest wall was reconstructed with pedicle muscle flap and using compression dressing for 2 weeks, there happened no respiratory problem. And so we made a case report and review of literature.

흉벽종양 21례에 대한 임상적 고찰 (Clinical Experience of Chest Wall Tumors - A Review of Twenty one Cases -)

  • A retrospective study of 21 cases of chest wall tumors, 12 benign and 9 malignant, was carried out to review their clinical radiological and pathological features. On age distribution, most cases [80.9%] were found in 4th to 6th decades. The sex ration [M:F] was represented as 2.5:1. Of the 21 lesions, there were 8 cases of soft tissue tumors, 4 cases of bone and cartilage origined tumors and 9 malignant tumors which included 3 metastatic tumors. The overall mortality was 22.2% and all of the deaths were found in the malignant tumors. All of the patients with benign tumors were treated by excision without recurrence. Distinction between benign and malignant chest wall tumors was not possible using radiographic criteria unless cortical destruction and involvement of soft tissue were visualized. On the basis of our analysis, we believe that all tumors of chest wall should be considered malignant until proven otherwise and that wide excision should be carried out. This is necessary not only to obtain as adequate diagnosis but also to provide the best chance for cure in both benign and malignant lesions.

Post-traumatic Intramuscular Hemangioma of the Chest Wall

  • Intramuscular hemangioma originated in chest wall is a rare benign tumor, with no relevant reports in Korea. In most cases, the tumor is discovered before the age of 30 years and it is reported that trauma operates as the initiation factor. It is essential to concern the clinical suspicion and conduct a CT scan for diagnosis. The principle of treatment is surgical excision with clear resection margin. The authors of this study report a case of surgical excision for post-traumatic intramuscular hemangioma of the chest wall with review of literature.

동물모델을 이용한 Hydrogel Patch의 상처치유개선 능력 평가 (Efficacy of Hydrogel Patch in Wound Rat Model)

  • Sodium polyacrylate based hydrogel patch was prepared and its wound healing efficacy in comparison with control groups was evaluated in thermal burn wound ($200^{\circ}C$, 10 sec burn) and excision wound rat model. Cytotoxicity of gamma irradiated (25 kGy) hydrogel patch was investigated in human fibroblasts and showed no significant cytotoxicity. Wound closure rates (H50%) in hydrogel patch group and BAND-$AID^{(R)}$ treated group were faster than that of control group (uncovered open wound). Hydrogel patch showed an enhancement in wound healing process.

혀에 발생한 신경초종 1예 및 고찰 (Schwannoma of Tongue : A Case Report and Review)

  • Schwannoma is a relatively uncommon, slow-growing benign tumor that is derived apparently from the Schwann cell. Tongue originated schwannomas are exceedingly rare within 1% and therefore often are not immediately included in the differential diagnosis and oral cavity tumors. In the current study, authors report a case of a schwannoma located at the tongue of a 49 years old patient treated by complete surgical excision. The patient is under clinical control, with no signs of recurrence even after 6 months.

  • We report a case of subungual solitary glomus tumor in a 28-year-old female, who has suffered from pain and tenderness of the left 4th finger tip for about 5 years. Simple surgical excision was performed for removal of the tumor mass and for the relief of the subjective symptoms. No recurrence has been observed for 5 months following excision of the tumor.

  • The giant cell tumor of tendon sheath is very rarely present inside the knee joint. The authors report a case of intraarticular giant cell tumor of tendon sheath arising from posterior cruciate ligament which was successfully excised arthroscopically using posterior trans-septal portal at the time of arthroscopic reconstruction of anterior cruciate ligament.

Staging in Vestibular Schwannoma Surgery : A Modified Technique

  • The authors herein propose the staged excision as a novel strategy to preserve facial nerve and minimize complication during microsurgery of large vestibular schwannoma (VS). At the first stage, for reducing mass effect on the brain stem and cerebellum, subtotal tumor resection was performed via a retrosigmoid craniotomy without intervention of meatal portion of tumor. With total resection of the remaining tumor, the facial nerve was decompressed and delineated during the second stage translabyrinthine approach at a later date. A 38-year-old female who underwent the staging operation for resection of her huge VS is illustrated.

  • The association of medial elbow ganglion cyst with cubital tunnel syndrome has been rarely reported. A 61-year-old man presented with progressive right hypothenar atrophy and paresthesia for 7 months. Ultrasonography and magnetic resonance imaging revealed ulnar nerve entrapment with a cystic ganglion in cubital tunnel. Decompression of ulnar nerve and excision of the ganglion were performed. Motor function of the ulnar nerve showed an improvement four months later after surgery. Because most ganglia are occult, imaging study is warranted especially in case with osteoarthritis. Excision of the ganglion performed concurrently with decompression of the ulnar nerve provide satisfactory results.

구강저에 발생한 유피낭종 1예 (A Case of Epidermal Cyst in the Floor of Mouth)

  • Epidermal cyst in the floor of mouth is relatively rare disease. Patients usually present soft and non-tender mass at the midline of mouth floor. Careful history taking and imaging study are needed for the diagnosis and surgical excision is the choice of treatment. We report a case of huge epidermal cyst located in the floor of mouth mimicking plunging ranula, which was successfully removed by transoral surgical excision without any complication. It was diagnosed as an epidermal cyst by pathologic examination.
