Influence of Job Crafting on Evidence-Based Practical Skills of Dental Hygienists

  • Background: As the medical knowledge base grows at an accelerating rate, evidence-based clinical performance becomes increasingly important for providing quality care. Previous studies have highlighted the need to promote job crafting to actualize evidence-based practical skills in the medical field. This study aimed to investigate the degree of evidence-based practice among dental hygienists and assess the impact of job crafting on the evidence-based practical skills of dental hygienists. Methods: Dental hygienists working at dental hospitals and clinics in Seoul and Gyeonggi Province were surveyed between February 28 and April 6, 2023. The sample was comprised of 267 participants. The hypotheses were tested independent t-tests, one-way analysis of variance, Pearson's correlation coefficients, and multiple regression analyses using SPSS 29.0. Results: The degree of job crafting by dental hygienists demonstrated significant differences based on educational attainment, workplace size, and workplace type. Evidence-based practical skills exhibited significant variations based on educational attainment and job position. All job crafting subfactors demonstrated positive correlations with evidence-based practical skills. The job crafting subfactors affecting the evidence-based practical skills of dental hygienists were 'increasing structural job resources' and 'increasing challenging job demands,' which together explained 38.7% of the variance in evidence-based practical skills. Conclusion: This study demonstrates that job crafting was positively and significantly correlated with evidence-based practical skills. To strengthen the job crafting ability of dental hygienists, improving environmental conditions and fostering an organizational culture that motivates continued participation in education is necessary. The development and promotion of programs that enable learning of the latest evidence should be actively pursued. Additionally, regular attendance at workshops and participation in organizational evidence-based practice education programs are necessary.

치위생 과정 기반의 임상 치위생 교육과 실무 (Clinical Dental Hygiene Education and Practice based on Dental Hygiene Process)

  • Dental hygiene was originated from dentistry and dental hygiene knowledge was a component of dental knowledge body. Since the late 1980s dental hygiene theory was began to develop. Nursing theories such as metaparadigm, nursing process and human need theory affected theory development as dental hygiene process. Dental hygiene process provides a framework for high quality dental hygiene care. Dental hygiene process include five phases; assessment, dental hygiene diagnosis, dental hygiene planning, implementation, evaluation. Dental hygiene process of care is recognized as standard for dental hygiene education and clinical dental hygiene practice. Dental hygiene practice has moved from auxiliary model to professional model. Critical thinking skill and disposition are necessary to provide evidence-based dental hygiene care using dental hygiene process as clinical process and critical thinking process. Critical thinking, problem solving and evidence-based practice must be integrated into dental hygiene process for quality dental hygiene care.

  • Dental caries is biofilm induced disease throughout life and is recognized significant oral health problem. This article reviewed new trends in dental caries management by risk assessment, including history, protocol/guideline, and collaborated model. Dental caries prevention and treatment according to caries management by risk assessment (CAMBRA) model is patient-centered, risk-based, evidence-based practice. Team approach is necessary and clinician need to integrate science, practice and product. Dental hygienist take a important role in implementing CAMBRA. CAMBRA model could be incorporated into clinical dental hygiene education based on dental hygiene process of care as standard of dental hygiene practice and education. Dentist and dental hygienist able to provide scientific and ethical care managing dental caries by risk assessment.

포괄치위생관리 과정을 적용한 스케일링 수행 경험에 관한 질적 연구 : 근거이론적 접근 (Qualitative study on the scaling experience through the application of comprehensive dental hygiene care : A grounded theory approach)

  • Objectives: This study was performed in order to provide evidence-based data for the expected professional impact of dental hygienists, and to apply and disclose the comprehensive dental hygiene care process through an in-depth analysis of their scaling experience and investigation of the importance of an evidence-based scaling work performance. Methods: The data were collected from June 3, 2019 to October 3, 2019 by conducting in-depth individual interviews on 10 dental hygienists who are working in dental clinics and hospitals by region. The data were analyzed by using the grounded theory methodology, which is a field of qualitative research method. Results: Study results showed that the core category derived from the paradigm model and change process in this study was 'a process of becoming a mature professional outside practical work'. Conclusions: In this study, the participants were able to gain a sense of occupational accomplishment as dental hygienists by performing scaling based on the comprehensive dental hygiene care (CDHC) process, and to advance into professionals through continuous efforts and research in order to enhance their job competencies.

  • 본 연구는 미국과 우리나라의 치위생 교육이념을 비교 분석하였다. 미국의 치위생 프로그램은 우리나라와 구별되는 이념적 특징으로서 치위생과정에 근거한 포괄적 치위생관리 능력, 표준적 수행능력, 비판적 사고와 문제해결 능력, 자율적 의사 결정 능력의 배양을 지향하고 있다. 또한 평생교육의 참여와 자기주도 학습을 수행할 수 있는 기초능력의 배양, 대인관계 및 의사소통 기술을 증진하고자 하는 교육이념을 표방하고 있다.

치위생학교육평가·인증체계 정립을 위한 학습성과기반 치위생 핵심역량에 관한 조사 (A study on core competence of dental hygiene based on learning outcomes for establishing dental hygiene education evaluation and certification system)

  • 치위생학교육평가·인증체계 정립을 위한 학습성과기반 치위생 핵심역량을 파악하고자 치위생(학)과 전임교수 207명을 대상으로 설문 조사를 실시하였다. 핵심역량별 세부영역에서 전문가적 행동 및 윤리적 의사결정 능력(1영역)과 과학적이고 전문적인 임상치위생과 임상치과업무의 지식과 기술을 적용할 수 있는 능력(2영역)의 전체 점수가 가장 높게 나타났고, 의사소통 능력 정도(3영역)는 4.48점, 근거중심의 통합적 사고와 문제해결 능력 정도(5영역)는 4.35점으로 나타났다. 핵심역량 세부영역들은 강한 상관관계가 있는 것을 확인하여 보건의료서비스 직종간의 핵심역량 파악과 임상 실무현장에 근무하고 있는 치과위생사들의 핵심역량에 대한 질적 연구가 필요하다고 사료된다.

  • This article reviewed theological background and historicak development in dental caries management by risk assessment. Dental caries prevention and treatment according to CAMBRA model is patient-centered, risk-based, evidence-based practice.   • 세계 유수의 의학관련 연구 저널의 편집자들은 제출된 원고의 질을 관리하기 위하여 연구진행의 가이드라인 역할을 할 수 있는 보고 지침을 개발하여 평가에 적용하고 있다. 따라서 본 연구는 RCTs를 위한 보고 지침인 CONSORT의 2010년 개정판 내용과 특징을 소개함으로써 치위생 연구의 계획 및 수행에 도움을 주고 더 나아가 EBP의 활성화에 기여하고자 하였다. CONSORT는 시험 보고의 질을 향상시킬 수 있도록 논문 저자들에게 보고 지침을 제공한다. 이 보고 지침은 RCTs 보고서에 반드시 포함되어야 할 필수 항목인 25개의 체크 리스트와 연구 과정의 도식화를 위한 순서도를 제시한다. 저자들은 연구 설계 시점부터 CONSORT의 각 항목을 참고함으로써 연구의 질을 향상시킬 수 있다. 따라서 우리 치위생계에서도 이러한 보고지침을 준수함으로써 치위생 연구의 타당성을 높이고, 이러한 연구 결과를 질 높은 근거 기반적 치위생 실무로 접목시켜 발전시킬 수 있을 것으로 생각된다.

건강신념모형을 적용한 치과의원급 치과위생사의 감염관리 수행도 관련요인 (The association factors of infection control practice based on health belief model in the dental hygienists)

  • Objectives : The purpose of this study was to investigate the association factors of infection control practice based on health belief model in the dental hygienists in dental clinics. Methods : A self-reported questionnaire was filled out by 278 dental hygienists in 160 dental clinics in Gwangju by a proportional stratified sampling method from September 13 to October 7, 2013. Data were analyzed by t-test, ANOVA, correlation analysis, and multiple regression analysis using SPSS version 12.0. Results : In multiple regression analysis, practice scores were significantly higher in aged dental hygienists and those who took infectious disease history from the patients before treatment. With regard to health belief model, perceived barrier was negatively associated with the practice(${\beta}$=-.16, p<.001), importance of infection control in hand hygiene(${\beta}$=.14, p=.026), and use of personal protective equipment(${\beta}$=.17, p=.043). The intention of action was positively associated with the practice(${\beta}$=.13, p=.002). Conclusions : This study will provide the basic evidence for the quality improvement of infection control and prevention. So the dental hygienists will be able to put into practice in infection control management.

Clear Aligner Therapy: Evidence, Eloquence and Reality

  • The demand for orthodontic clear aligner therapy (CAT) has increased significantly over the last decade, offering advantages over the fixed appliances (FA) including enhanced aesthetics, better hygiene and comfort, along with minimal restrictions on the patient's diet. Moreover, a marked improvement in the efficacy of tooth movement using aligners has been documented. On the contrary, there have been known limitations of CAT including the compliances issues and the apparent lack of efficacy for certain types of tooth movement such as closure of extraction space compared to FA. Thus, evidence-based evaluation of the accuracy of prediction of tooth movement with clear aligners and their ability to effectively perform major tooth and root movements compared to FA are crucial. Although several systematic reviews have investigated various aspects of the effectiveness of CAT, we are yet to obtain a rather conclusive answer to this question. The current review attempted to summarize the evidence-based findings of most systematic reviews about CAT available to date. Major issues regarding the predictability of tooth movement, the role of attachments and auxiliaries in improving the effectiveness of CAT, and the treatment outcomes in comparison to FA were investigated. Clinical recommendation have been also elaborated based on the interpretation of the findings of all systematic reviews included in this study.
