Construction errors can have significant effects on project performance. Yet, any attempt to tackle error should be driven by the ability to understand its archetypal nature. This study aims to analyze a variety of construction errors at the execution stage, in order to develop a comprehensive set of categories that shall provide insights about the effect of different error types on the industry. We investigate a database provided by an authoritative source, which includes a number of 256 construction errors that occurred in the year 2009 in Dubai construction industry. Results from this study reveal that the most common error arose from 'poor workmanship' which accounted for 21% of the total encountered faults. The next most common types arose from the 'usage of impaired materials', followed by the 'deviation from an intended dimension'. These observations infer that the majority of construction errors are driven by workers' lack of skill or competence. Moreover, it suggests that execution-oriented errors are the major cause of faults and accidents rather than design errors.

Human error analysis has been performed to prevent accidents and reduce human error rate in diverse contexts; manufacturing, aircraft and nuclear power plants. Until now, human error in our everyday lives has not been focused. This paper addressed human error when users go up and down elevator. First of all, human error types of elevator users were categorized by a taxonomy of unsafe acts. It was also investigated which types of human error occurred in the elevator. Finally display design guidelines were suggested to reduce human error in elevator. Auditorial display and visual display can be used to reduce human errors in elevator. Future study should be performed to check if the proposed design guidelines are effective in the real situations.

본 연구에서는 중학교 2학년 106명의 학생들을 대상으로 대수 문장제 해결에서 나타난 오류의 유형을 조사하고, 학생들의 오류 유형과 Polya의 문제 해결 단계의 관련성에 대하여 알아보았다. 연구 결과 대수 문장제 해결에서 학생들의 오류는 "잘못 해석된 언어"(39.7%), "왜곡된 정리나 정의"(38.2%), "기술적 오류"(11.8%), "검증되지 않은 답"(7.4%), "오용된 자료"(2.9%), "논리적으로 타당하지 않은 추론"(0%)의 6가지 오류로 분류하였다. 대수 문장제 해결에서 나타나는 학생들의 오류가 대수 문장제 해결과 어떤 관련성을 가지고 있는지 조사한 결과 학생들의 특정 오류 유형과 문제 해결 단계 사이에 긴밀한 관련성이 발견되었다. 오용된 자료의 오류를 범하는 학생들은 대부분 계획 실행 단계를 성공적으로 수행하지 못하였고, 잘못 해석된 언어, 왜곡된 정리나 정의의 오류를 범하는 학생들은 문제의 이해, 계획의 작성, 반성 단계를 성공적으로 수행하지 못하였다. 검증되지 않은 답의 오류를 범한 학생들은 계획의 작성과 반성의 단계를 성공적으로 수행하지 못하였으며, 기술적 오류를 범한 학생들은 특히 반성 단계를 성공적으로 수행하지 못하였다.

In aviation, it is important to analyse and classify human error in detail. Because human error has been implicated in 70 or 80% of aviation accidents in literature review. But, there is little detailed classification and research of human error. In this study, Objectives are to establish human error model by classifying types of human error in detail and also to analyse human factors by using the established model. Analysis of the data uses Korea Aviation Incidents Reporting System(GYRO). The resulting from actual analysis, there is a some difference between flight steps for human error occurrence and types of human error are different according to the aviation personnel(pilot, ATC controller).

Human error is one of the major contributors to the railway accidents or incidents. In order to develop an effective countermeasure to remove or reduce human errors, a systematic analysis should be preferentially performed to identify their causes, characteristics, and types of human error induced in accidents or incidents. This paper introduces a case study for human error analysis of the railway accidents and incidents. For the case study, more than 1,000 domestic railway accidents or incidents that happened during the year of 2004 have been investigated and a detailed error analysis was performed on the selected 90 cases, which were obviously caused by human error. This paper presents a classification structure for human error analysis, and summarizes the analysis results such as causes of the events, error modes and types, related worker, and task type.

본 연구는 분수의 곱셈과 나눗셈 계산 과정에서 학생들이 보이는 오류 유형을 분석하고 진단하여, 오류를 효과적으로 교정하기 위한 오류 유형별 지도방안을 구안하는 데 그 목적이 있다. 이를 위하여 초등학교 6학년 2개의 학급을 대상으로 분수의 곱셈과 나눗셈에서 보이는 주요 오류 유형을 6가지로 분류하고 연구 대상의 오류 유형을 진단하였으며, 각 오류 유형에 맞는 교정 지도방안을 구안하여 적용하였다. 오류가 교정되었는지를 판단하기 위하여 사후평가를 2회 실시한 결과 연구 대상의 오류가 교정된 것으로 나타났다.

본 연구는 초등학생들의 문장제 해결과정에서 나타나는 오류를 분석하고 문제해결전략별 오류 유형 및 그 특징을 파악함으로써 문제해결학습의 실패 원인에 대한 정보를 제공하고 문제해결력을 향상시킬 수 있는 교수학습방안을 제안하기 위한 것이다. 문장제 해결과정에서 학생들이 선호하는 전략을 살펴보면 식 세우기와 예상과 확인, 규칙 찾기 순으로 나타났으며, 단순화하기 전략은 거의 사용하지 않고 있다. 문장제 해결과정에서 나타나는 오류 유형의 특징은 문제해결전략에 따라 차이를 보였는데, 이를테면 식 세우기의 경우, '문항 이해의 오류', '개념 원리의 오류', '풀이 과정의 오류' 순으로 나타난 반면, 그림그리기에서는 문제에서 설명하는 내용을 잘못 이해하여 그림으로 나타내는 오류를 주로 범하였고, 표 만들기의 경우 문제에서 주어진 정보를 표로 나타내는 과정에서 정보들 간의 관계를 잘못 이해하여 오류를 범하는 '문항 이해의 오류'가 많은 것으로 나타났다. 이처럼 문장제를 통한 문제해결 학습에서 학생들이 선호하는 문제해결전략을 확인함과 동시에 문제해결전략별 나타나는 오류의 특징을 확인함으로써 해결전략에 따른 오류를 예상하고 이에 대처하는 교수학습방안을 생각해볼 수 있을 것이다.

Two types of digital repetitive control systems are analyzed and designed to reduce the error spectrum including not only harmonic but also non-harmonic components. First, a novel gain scheduling algorithm is suggested for conventional and modified repetitive controller is scheduled to reduce the infinite norm of error in frequency domain. For this, the relative error transfer function is mewly defined as the ratio of the error spectrum for the system with a repetitive controller to the error spectrum for the system with a repetitive controller to the error spectrum for the system without a repetitive controller. Secondly, as an alternative of a repetitive control system with the gain scheduling, a repetitive control system with higher order repetitve function is analyzed and designed, where instead of equal weightings, weightings of the higher order repetitive function is determined in such a way that the infinite norm of relative error transfer function is minimized. To show the validities of proposed methods, computer simulation results are illustrated for a typical disk drive head positioning servo system.

The purpose of this study were to illustrate the various medication error types and causes and identified to related drugs to provide basic data for preventing nurses' medication error by analysing 73 cases of AJN 'medication Error' column(1993, Oct -2000, Nov). Nurses' types of medication error were classified into 7 types. The most frequent error types are wrong medication$(21.9\%)$ and the wrong dose$(21.9\%)$ together. The others are wrong $time(4.1\%)$, $omission(2.7\%)$, mechanical $error(2.7\%)$, incorrect IV $rate(1.4\%)$. wrong route $administration(1.4\%)$ in order. Nurses' causes of medication error were 9 kinds. The most frequent type is confusing between similar drug shape, color, size, name, injection devices and patient's $name(43.9\%)$ and the others are lack of knowledge about $drugs(26.8\%),\; slips(7.3\%),\; miscalculating\;dose(4.9\%)$, incorrect adjusts $devices(4.9\%)$, difficulty to read or illegible decimal $point(4.9\%),$ $abbreviation(2.4\%)$, fatigue with $overwork(2.4\%)$ and no communication with $patient(2.4\%)$ in order. Related drugs with medication error are as follows. - dose unit(IU. minims. mcg/min. mEq) : Heparin. insulin. synthetic calcitonin, some enzymes and hormones, vitamins, some antibiotics, tuberculin injection. MgSO4 injection. nitroglycerin - similar size, color and shape drug : $0.9\%$ N/S and acetic acid $0.25\%$ for irrigation. premixed 2mg lidocaine sol. and $0.9\%$ N/S, gentamycin 20mg/2mL for children and 80mg/2mL for adult, dextroamphetamine 5mg and 10mg capsule. sedatives chloral hydrate 250mg/5mL and 500mg/5mL - similar name :Aredia(pamidronate disodium) and Adriamycin(doxorubicin), Lamictal (lamotrigine) and Lamisil 250mg. Elderpryl and enalapril, cefotaxime and cefoxitin, carboplatin and cisplatin, sumatriptan and zolmitriptan, Celebrex and Celexa, Humulin and Humalog, Percodan and Percocet, Diabeta and Diabinese, Epivir and Retrovir, Xanax(alprazolam) and Zantac(ranitidine) - decimal point : low molecular weight warfarin, methotrexate - unfamiliar drug uses of familiar drug ; methotrexate. droperidol, imipramine, propranolol - number of drug name(misleading chemical name) : 6-thioguanine, 6-mercaptopurine, 5-fluorouracil - type of administration route : Oxycodone(OxyContin). - administration time : acarbose(Precose). - injection way (Z-track method): hydroxyzine - epidural cathether : LMWHs(enoxaparin, dalteparin), - ADD Vantage self contained delivery system : ceftriaxone(Rocephin)

4차 산업혁명 시대의 도래와 함께 스마트 팩토리의 개념이 대두되면서 설비가동률과 생산성에 악영향을 미치는 설비 오류의 발생을 데이터 분석 기법을 통해 예측하고자 하는 노력이 이루어지고 있다. 데이터 분석 기법을 활용하여 설비 오류를 예측하기 위해서는 설비 오류가 발생한 상황과 설비 오류 유형을 명시한 데이터인 설비 오류 이력이 필요하다. 하지만 많은 제조 현장에서는 설비 오류 유형이 정확하게 정의/분류가 되지 않아 설비를 운영하는 작업자가 자신의 경험적 판단에 의거하여 정형화되지 않은 텍스트의 형태로 설비 오류 유형을 작성하고, 이에 따라 데이터 분석 기법의 적용이 어렵다. 따라서 본 논문에서는 수기로 작성된 설비 오류 이력을 활용하여 설비 오류 유형을 파악하고 구조화하기 위한 구절 네트워크 구축 방법을 제안하고자 한다. 구체적으로, 단어를 쓰임새에 따라 분류한 용도 딕셔너리를 활용하여 비정형의 텍스트 데이터로부터 설비 오류 유형을 의미하는 구절을 추출하고, 추출된 구절 간의 유사도를 계산하여 네트워크를 구축한다. 제안하는 방법의 성능을 실제 제조 기업의 설비 오류 이력 데이터를 활용하여 검증하였으며, 본 연구의 결과는 텍스트 데이터에 기반한 설비 오류 유형 구조화와 나아가서는 설비 오류 발생 예측에 이용할 수 있을 것을 기대한다.
