The prevalence of temporomandibular disorders in $16\~18$ aged subjects at Yongin, Kyungkido, Korea (경기도 용인지역 $16\~18$세 연령자의 측두하악장애 유병율에 관한 연구)

  • An epidemiological investigation was carried out at Yongin, Kyungkido, South Korea to determine the prevalence of symptoms of temporomandibular disorders and parafunctional habits in adolescent aged 16, 17 and 18 years. 2,098 students(male 507, female 1,591) were randomly selected and investigated with the questionnaire. The results were obtained as follows, 1. The prevalence of symptoms of temporomandibular disorders was $80.9\%$ total, $77.3\%$ for male and $82.0\%$ for female with significance between male and female(P<0.05). 2. The prevalence of symptoms of temporomandibular disorders was gradually increased according to increasing age with significant difference among ages(P<0.001) 3. The most frequent symptom in the temporomandibular disorders is pain on chewing($65.9\%$), and then clicking($50.7\%$), Pain on mouth opening($41.8\%$), and pain on TMJ($36.8\%$) 4. There was little corelationship between clenching habits and symptoms of temporomandibular disorders(r=0.166), and also between bruxing habits and those(r=0.057).

Provisional National Nutrition Targets and Strategies for Health Promotion of Koreans (국민건강증진을 위한 영양 부문의 2000년 잠정목표와 전략)

  • This study attempts to propose a priority of national nutrition targets and strategies for health promotion by the year 2000 in Korea, as a part of the task set for national health promotion objectives and strategies. Among all of the important health issues raised, ten were chosen, nutrition was one priority area. In the first part, the current status of the nutrition-related health problems and risk factors are reviewed, in conjunction with the newly arisen health phenomena, such as changes in prevalence of lifestyle disease and causes of death, changes of food consumption patterns in our country. In the second section this study suggests six feasible national nutrition targets, eight implementing strategies and current major tasks on the basis of the assessment of present status and in consideration of the other health promotion goals and strategies, with reference to that of other developed countries. The main targets and strategies are suggested as follows ; Firstly, the national nutrition monitoring and surveillance system should be established for identifying the nutritional problems for our people, and current National Nutrition Survey is a strong need for improvement to a more comprehensive and reliable one. Secondly, effective administrative mechanism should be operation at national level for the development of nutrition policy. Ministry of Health and Welfare (MOHW) as well as local health department must be remarkably renewed and strengthened the nutrition section. And it is recommended that MOHW organize and operate “The Council of Nutrition”, in which all government authorities related with foodstuffs and nutrition would incorporated. The Council of Nutrition would act as an adjustor as well as a coordinator in nutrition related policy-making. Thirdly, healthy eating pattern will be supported by activities of introducing a nutrition labeling for providing consumers with the necessary information and skills for food selection. Fourthly, nutrition education, and nutrition intervention programs will be carried out in various settings such as health centers, schools, and clinical fields and workplace. Fifthly, the current dietary guidelines shall be continuously improved in detail, and publicly circulated to particular levels of people by age group and by health condition. And finally, researches and epidemiological studies particularly in regard to diet for development of chronic diseases are needed for more investigation and up-to-date national health and nutrition data should be collected with the support and cooperation from the various medical professional teams . Investigation of Detected by Recent Various Human Papillomavirus from General Hospital in Seoul Area (최근 서울지역 종합병원에서 다양한 인유두종바이러스의 검출에 대한 실태조사)

  • Human papillomavirus (HPV) infection is a necessary precondition of cervical cancer. A change from cytology to molecular HPV testing is, however, challenging. A new HPV DNA chip test for the infection of 22 HPV genotypes were developed in Korea. The purpose of this study was to investigate the prevalence and genotype distribution of HPV infection in the Seoul area. Over the last year, a total of 5,614 samples were tested. Using a chip test, HPV genotypes were detected in 1,596 (28.4%); of which, 679 (42.5%) were considered as high risk and low risk HPV were 152 (9.5%). 831 were single positive samples (n=1596). The most frequently found genotypes in all HPV-single positive samples (n=831) were HPV-16 (16.5%), 58 (15.2%), 52 (8.8%), 51 (7.1%) and 56 (5.9%). Mixed genotypes (n=219) were detected in 2 (n=176, 11.0%), 3 (n=37, 5.9%), and 4 (n=2, 0.1%) positive samples (n=1596). This study demonstrated that epidemical investigated HPV infection in patients of general hospital. These findings could be used to indicate a nationwide distribution of HPV and the adoption of vaccines. It is hoped that additional epidemiological research regarding the outcomes that are important to decision makers will be conducted.

Immunological Monitoring of Urinary Aflatoxins and Estimation of Liver Cancer Incidence in Koreans

  • Polyclonal antiserum R101 against aflatoxin $B_1$ ($AFB_1$) was raised in New Zealand white rabbits after injection of bovine serum albumin-$AFB_1$ conjugate. Competitive ELISA (enzyme linked immuno-sorbent assay) demonstrated that antiserum R101 has the highest binding for $AFB_1$ (50% inhibition at 170 fmol) and aflatoxicol II (50% inhibition at 112 fmol). It also reacts with other aflatoxins such as $AFB_2$, $AFG_1$, $AFG_2$, and aflatoxin metabolites ($AFM_1$, $AFM_2$, $AFP_1$, and $AFQ_1$), but it does not cross-react with $AFG_2a$. Using this antiserum, aflatoxins were quantitated in 100 urine samples of undergraduate students at the College of Pharmacy, Sung Kyun Kwan University, Republic of Korea. By ELISA, $AFB_1$ and its metabolites were detected in human urine samples (N=100, male=89, female=11, ages=20~31 yrs) with a range of 1.4~200.6 ng/kg/day (mean$\pm$SD=$18.11{\pm}33.01\;ng\;AFB_1/kg/day$ in males, $3.82{\pm}2.65\;ng/kg/day$ in females). Assuming that urinary excretion is about 7.6% of $AFB_1$ intake (Groopman et al., 1992), we estimated that Koreans were daily exposed to a total dietary $AFB_1$ of $240.20{\pm}438.67\;ng/kg/day$ in males and $50.35{\pm}29.88\;ng/kg/day$ in females, respectively. When the human monitoring data was applied to a linear regression model of Y=21.67X-10.04 {Y=liver cancer incidence per 100,000, X=Log $AFB_1$ intake (ng/kg/day), r=0.99} developed from previously reported epidemiological data, calculated liver cancer incidences attributed to $AFB_1$ exposure were 41.56/100,000 in males and 26.84/100,000 in females. The incidences were similarly correlated with liver cancer mortality rates of 43.43/100,000 in males and 11.23/100,000 in females in Korea. These results suggest that aflatoxin exposure may be an important risk factor for the high incidence of liver cancer in Korea.

Analysis of symptom pattern through comprehensive diagnosis of Qui Xui Shui in patients with functional dyspepsia (기혈수변증(氣血水辨證)에 의한 기능성 소화불량 환자의 변증유형 분석)

  • Background & Object : It is well known that functional dyspepsia is one of the most common diseases. While many dyspepsia patients have been helped with oriental medical therapy, there has not been a study based on the concepts of oriental medicine. The aim of this study was to perform a fundamental epidemiological survey and to analyse the symptom pattern of functional dyspepsia. Methods : 86 patients(27 males, 59 females) diagnosed with functional dyspepsia in Kyunghee Oriental Medical Center from May to December 2002 were involved in this investigation. The disease characteristics of functional dyspepsia(based on Rome criteria II) and the Qui Xue Shui diagnostic procedure were investigated by questionnaire and physical examination. The total score and composition ratio of each comprehensive diagnosis were calculated from the symptom score described in the questionnaire. Results : The total score from the Qui Xue Shui diagnostic procedure was found to be influenced by the number of functional dyspepsia symptoms(p=0.026) and the patient's own cognition of their current dyspeptic situation(p=0.006), in addition to digestive ability and general congnition(p=0.006), and was not associated with sex, duration of dyspeptic symptoms in one year, or the total illness period, In the composition ratio of the Qui Xue Shui diagnostic procedure, only Qui-yu(p=0.048) diagnosis was accurate regarding the number of symptoms in one year, total illness period, or the patients' own congnition of their current dyspeptic situation, But the value of the composition ratio among the total factors involved was most similar to that of sex. Conclusions : Thus, it is shown here that the total score of using the Qui Xue Shui diagnostic procedure is mainly dependent on dyspeptic symptoms and the patinets' own cognition of their current dyspeptic situation, and that regular symptom patterns in the Qui Xue Shui diagnostic procedure exist in dyspeptic patients.

  • Purpose : We assessed about the measles elimination in Korea according to analyzing national data on measles cases for 2002-2007. Methods : We estimated the effective reproduction number according to the proportion of imported cases and outbreak size. Results : Eleven cases, thirteen cases, six cases, six cases, 25 cases (60.0% were linked to 1 outbreak of 15 cases), and 180 cases (55.6% were linked 11 outbreaks of 2, 2, 2, 3, 3, 3, 5, 9, 9, 12, 50 cases) were reported in 2002, 2003, 2004, 2005, 2006 and 2007, respectively. In 2002-2007, R is 0.95 by imported case and 0.43 by outbreak sizes. In 2007, multiple chains of transmission occurred in hospital. Most of cases were below 4 ages and unvaccinated. We didn't find most of the infection source. Conclusion : We should prevent outbreak of susceptibility pocket like nosocomial infection, conduct reliable routine surveillance and epidemiological investigation to solve a problem of the measles elimination.

An Epidemiological Investigation on Severity of Cerebro-Vascular Accident Patients in Korea (한국인의 뇌졸중 위중도에 관한 역학적 분석)

  • A study on severity of cerebro-vascular accident patients in Korea was conduced in order to determine the risk factors affecting the severity of stroke patients. This study was performed by interviewing a total of 477 hospitalized and ambulatory patient of CVA in Seoul, Taejon, and Taegu and Pusan areas from April 1, 1998 through June 30, 1998. The results are as follows; 1. Two hundred and sixty seven $(56\%)$ at Four hundred and seventy seven subjects were manes, md two hundred and ten $(44\%)$ were females. $28.3\%$ of the subjects' ages were 50's and $27.3\%$ were 60's and $18.5\%$ were 70's. $22.6\%$ of the subjects' occupations were home makers, $17.8\%$ were farming. $66.4\%$ of the subjects spent their childhood in urban areas and $33.1\%$in rural areas. $41.7\%$ of the subjects became violin of CVA in Spring, $35.0\%$ in Winter. 2. On physical and mental conditions of the subjects at the occurrence of CVA, $28.7\%$ of the subjects were engaged in physical activities. $22.6\%$ were in a rest stale $19.5\%$ were in sleeping and $18.9\%$ were mentally shocked. $79.4\%$ of the male subjects and $14.8\%$ of female subjects smoked cigarettes. $82\%$ of male subjects drank coffee. $81.1\%$ of the subjects did not exercise regularly. $45.9\%$ of the subjects had the systolic blood pressures in the range of 160 to 199 mmHg and $5.6\%$ of the subjects had hypertension before the occurrence of stroke and $11.7\%$ had diabetes. 3. Of the types of strokes, cerebral hemorrhage was the highest $(49.1\%)$. cerebral infarction was the second $(41.1\%)$. Severe strokes were found in cerebral hemorrhage cases $(52.0\%)$ and cerebral infarction cases $(40.1\%).\;50.9\%$. of the male subjects were moderate cases, $50.9\%$ were severe cases. In females, moderate cases were $72.4\%$, severe cases $15.2\%$. $37.5\%$ of the subjects who had preceding diseases were severe cases, and $15.6\%$ of the subjects without preceding diseases were severe cases. $50.7\%$ of the subjects whose family members had strokes had severe strokes. $34\%$ of the subjects sleeping less than 6 hours a day, $42.4\%$ of the subjects with irregular eating habits, $33.3\%$ of the subjects who liked meat, and $42.3\%$ of the subjects who liked salty foods had severe strokes. $35.9\%$ of the subjects with hot temper, $27.6\%$ of the subjects with moderate temper and $14.5\%$ of subjects with mild temper were severe cases. 4. The correlation coefficient between obesity and blood pressure was 0.094.

Distribution of Rotavirus G Serotypes in ChungJu Area (충주 지역 설사 환아의 Rotavirus G Serotype 분포에 관한 연구)

  • Purpose: It is important to have the epidemiologic data of rotavirus serotypes for the application of polyvalent rotavirus vaccines. Epidemiological studies of rotavirus serotypes in Korea have been reported only in limited areas with small number of cases. Authors tried to investigate the distribution of rotavirus G serotypes in ChungJu area with RT-PCR. Method: Stool specimens were collected from 202 children with acute diarrheal symptoms, who admitted to or visited Kon-Kuk University Hospital in ChungJu from June 1998 to May 1999. Samples were screened for rotavirus with EIA method (TestPack Rotavirus, Abbott Laboratories) and rotavirus G Serotypes were determined by RT-PCR. Results: Rotavirus was positive in 46.6%. The incidence of G serotypes was as follows; G1 10%, G2 10%, G3 28%, G4 26%, and G9 20%. There were three cases of multiple serotypes; G1 with G9, G2 with G9, and G4 with G9. Serotype of G8 was not found. Conclusion: The proportion of G serotypes in ChungJu is much different from previous reports. Serotype of G9 was found which had not been reported in Korean children till now. Long term plans for the investigation of rotavirus serotypes must be needed in wide area.

Comparative Genomic Analysis of Staphylococcus aureus FORC_001 and S. aureus MRSA252 Reveals the Characteristics of Antibiotic Resistance and Virulence Factors for Human Infection

  • Staphylococcus aureus is an important foodborne pathogen that causes diverse diseases ranging from minor infections to life-threatening conditions in humans and animals. To further understand its pathogenesis, the genome of the strain S. aureus FORC_001 was isolated from a contaminated food. Its genome consists of 2,886,017 bp double-stranded DNA with a GC content of 32.8%. It is predicted to contain 2,728 open reading frames, 57 tRNAs, and 6 rRNA operons, including 1 additional 5S rRNA gene. Comparative phylogenetic tree analysis of 40 complete S. aureus genome sequences using average nucleotide identity (ANI) revealed that strain FORC_001 belonged to Group I. The closest phylogenetic match was S. aureus MRSA252, according to a whole-genome ANI (99.87%), suggesting that they might share a common ancestor. Comparative genome analysis of FORC_001 and MRSA252 revealed two non-homologous regions: Regions I and II. The presence of various antibiotic resistance genes, including the SCCmec cluster in Region I of MRSA252, suggests that this strain might have acquired the SCCmec cluster to adapt to specific environments containing methicillin. Region II of both genomes contains prophage regions but their DNA sequence identity is very low, suggesting that the prophages might differ. This is the first report of the complete genome sequence of S. aureus isolated from a real foodborne outbreak in South Korea. This report would be helpful to extend our understanding about the genome, general characteristics, and virulence factors of S. aureus for further studies of pathogenesis, rapid detection, and epidemiological investigation in foodborne outbreak.

Prevalence of Canine Coronaviral Enteritis in Korea (개 코로나바이러스성 장염의 발생 역학)

  • Canine coronavirus (CCV) is a cause of sporadic outbreaks of enteritis in dogs. This study was performed to carry out epidemiological investigation on the recent outbreaks of CCV enteritis of dogs and determined the potential prognostic factors affecting the survival of dogs. The 131 (34.4%) out of 381 fecal samples collected from dogs with enteritis were positive for CCV by RT-PCR. The fecal samples contained genotype I (30.5%), genotype II (29.0%), and both genotypes (40.5%) of CCV. The majority of dogs with CCV infection ranged 6-18 weeks of age. Age over 18 weeks was significantly associated with higher survival rate (P < 0.05). Of the clinical signs examined, dogs without anorexia were significantly higher survival rate (P < 0.01). The 90.1% of dogs with CCV infection were co-infected by CCV and CPV-2. From the results of this study, it can be concluded that CCV infection is widespread in the Korean dog population and CCV may be attributed to be one of the important agents causing enteritis in pups.
