The purpose of this research is to find the reasons of overcrowding in the emergency department of an hospital, then to shorten the total stay time of patients. The fact that main causes of the overcrowding exist in the process of the emergency department was discovered by analysis of the data. In order to improve these process, simulation model was developed by ARENA 7.0. Staff's service time, staff's organization, process ratio, and patient's waiting time were estimated in the simulation model in consideration of the decision of the patient's course of action. Several scenarios such as the simplification of the process, the setup of dedicated pathology lab, and mixed method were suggested and evaluated. Total stay time of the patients would be reduced up to 28.45%.
Purpose: The purpose of this study was to identify the effects of an educational intervention by evaluating neonatal emergency airway management knowledge, critical thinking disposition, problem-solving ability, and confidence in clinical performance after developing and operating a simulation-based neonatal emergency airway management education program for nurses in a neonatal intensive care unit. Methods: The participants were 30 nurses in a neonatal intensive care unit. Data were collected from June 6 to 15, 2018 and analyzed using IBM SPSS version 22.0. Results: The results of the pretest and posttest for each educational group showed statistically significant improvements in neonatal emergency airway management knowledge, critical thinking, problem-solving ability, and confidence in clinical performance. Conclusion: The simulation-based neonatal emergency airway management training program was an effective educational program that enhanced neonatal emergency airway management knowledge, critical thinking disposition, problem-solving ability, and confidence in clinical performance among nurses in a neonatal intensive care unit. Therefore, it is suggested that the program described in this study can contribute to improving nursing quality by enhancing the ability of nurses to cope with emergencies in practice. It can also be used for education for new nurses and contribute to the development of nurses' practices.
This study performed a survey on the need for an automobile accident-simulation training program for 60 emergency workers and nine emergency medical professors. The results showed that the need for cervical spine immobilization use was highest (76.8%) in first-aid requiring simulation training, and the need for head injury was highest (75.4%) in the scenario requiring simulation training. Based on the 43-month working experience of emergency workers, the uses of pneumatic anti-shock garment, KED, and the need for rapid extrication were significantly different between the two groups (p=0.01, p=0.05). In addition, the scenario showed statistically significant differences in the need for face, abdomen, and other injuries in both group (p=0.05, p=0.04, p=0.03). The needs of emergency workers and emergency medical professors for an automobile accident simulation-training program were high, and the development of a simulation program is needed.

This study examined the factors affecting the emergency nursing performance of nursing students during a simulation. The variables related to the simulation were the objective and subjective emergency nursing performance, critical thinking, program outcome, simulation satisfaction, simulation confidence, and simulation immersion. The participants were 92 senior students who provided informed consent with an explanation about attending the study. The general characteristics and simulation-related data were obtained from the students before and after the simulation. An assessor evaluated the objective emergency nursing performance during the simulation and program outcome through a report reviewed after the simulation. Data analysis was performed using the number, percentage, mean, standard deviation, X2-test, one-way ANOVA, Pearson correlation, and progressive regression through the SPSS/WIN 22.0 program. Emergency nursing performance by the assessor was negatively associated with the satisfaction of clinical practice (r=-.31, p=.005) and positively related to the subjective clinical performance (r=.23, p=.036). The affecting rate was just 12%, which was low for explaining the objective emergency nursing performance during the simulation. Further studies will be needed to explore other factors affecting the emergency nursing performance.
Purpose: The purpose of the study is to confirm the effect of ACLS program using simulations and understanding self-efficacy, practice satisfaction, learning attitude, and interest in ACLS after theory classes and simulation-based practice. Methods: A non-equivalent simulation-based practice post test design was used. The participants were 28 paramedic students. The students participated in simulation-based practice for 3 weeks and conventional instruction class for 12 weeks. Results: The students showed higher level of self-efficacy(p=.043), practice satisfaction(p<.001) and learning attitude(p=.003) compared to the conventional lecture students. Conclusion: Level of self-efficacy after simulation-based practice for ACLS was higher than that of self-efficacy after conventional instruction classes. Level of practice satisfaction was also higher. Academic achievement after simulation-based practice was higher than that in conventional instruction classes.
Purpose: The purpose of this study was to test the effect of simulation-based learning on knowledge, confidence, and critical thinking of paramedical students enrolled in the Korean Advanced Life Support (KALS) program. This study used a one group, pre-post test design. Methods: The subjects of this study were 79 paramedical students in D city. Data were collected before and after the simulation-based training using a structured questionnaire. The data were analyzed using SPSS version 22.0. Results: After the simulation-based KALS education, knowledge (t=-6.88, p<.001) and confidence (t=-10.12, p<.001) increased among paramedical students. There was a positive correlation between confidence and critical thinking disposition (r=0.37, p=.001). Conclusion: A practical module for simulation-based education that can improve knowledge, confidence, and critical thinking disposition related to professional resuscitation is needed for use with paramedical students. Additionally, follow-up studies should be conducted to verify the educational effects of such a program.
Purpose: The purpose of this study was to explore EMT-paramedic students' experience of simulation education and analyze the confidence before and after education, learning attitude and course evaluation. Method: Research survey was conducted on 38 EMT-paramedic students during November, 2011 and EMT-paramedic students' experience of simulation education was analyzed after applying head, spinal, and chest injury scenario. The confidence before and after education, learning attitude and course evaluation in gender were analyzed by Mann-Whitny U test and the difference of confidence before and after education was analyzed by Wilcoxon signed rank test and learning attitude & course evaluation were analyzed by evaluating frequency, percentage, mean, standard deviation by using SPSS WIN 17.0 program. Results: 1. Students experienced various advantages such as increasing interest and self-reflection on learning, critical thinking ability, and EMT-paramedic-role experience and recognition of importance of teamwork. Students also pointed out disadvantages such as gap between real situation and simulation, limit of time and equipments, and burden of demonstration. 2. The confidence between before and after education, learning attitude and course evaluation in gender were not significant different statistically. 3. Confidence mean score elevated from 5.53(before education) to 5.87(after education), but the difference in their confidence did not show significant difference statistically. 4. Total mean score in learning attitude after simulation education was 3.70 out of 5.00, which is considerably very high. 5. Total mean score in course evaluation was 3.89 with score of 3.83 in evaluation in learning environment and 3.99 in evaluation of debriefing. Conclusion: The finding of this study demonstrate that the simulation education can provide a safe and repetitive practice environment, improve problem-solving ability and critical thinking, and increase the confidence in prehospital emergency care; therefore, simulation may be the new effective EMT-paramedic education strategy.
