  • The purpose of this thesis is to lay groundwork for the development of emergency care system in metropolitan area. It compares the performance and outcome of the current system with foreign counterparts and investigates the changing aspects of future medical environment. Emergency medical system can be divided into two parts of both pre-hospital care, which refers to the emergency measures taken before arriving at a hospital, and hospital care that is given within a hospital. Pre-hospital care includes on-the-spot expedients, information system and delivery system, whereas hospital care is related to the classification and specialization of medical care facilities. This research focuses on the evaluation of the performance of a rescue party, which is part of pre-historical care system. As a result, it provides valuable material for the development of the emergency medical system in Seoul.

  • This study was analyze that they knows how to think improvement about the emergency relief squad 119. The result is following: 1. Convenience when to use an ambulance is not difference of the voluntariness by regional groups, and then result get that ambulance is necessary with going out as quick as possible. 2. Improvement of the ambulance os not difference of the voluntariness by regianal groups, and then result get that ambulance is necessary with improvement for the ambulance and equipment. 3. First remembrance when you call the ambulance is not difference of the voluntariness by sex distinction, and then result get that they will have the public relations.

A Study on Lawsuit Cases and Measures of Emergency Medical Service (응급의료서비스 중 발생되는 소송사례와 대책 연구)

  • Civil complaints and lawsuits filed in the process of providing emergency medical service include fall accident on the way of carrying the patient, transfer consent, refusal and rejection of rescue request, range and behavior restriction of emergency medical technicians, false registry of logbook, neglect of duty and emergency patient, and violation of traffic laws on the way of dispatch to the scene of accident. This study suggested the measures by cases as follows. 1. The accidents on the way of carrying a patient could be divided into fall of patient and fall by paramedic's mistake. In the former case, damages caused by the ambulance's shaking must be notified to the patient and guardian and recommended to fasten seat belt, in the latter case, the plan of patient's posture, route of transport, rescue and equipments should be comfirmed before fixing the patient. 2. Transfer consent must be made as implied when the patient is unconscious under delusion and was not able to consent physically, and paramedic must take an action by his judgment and record details of services on logbook. 3. When a patient refused to transfer, get 'confirmation of transfer refusal' and inform him of refusal. Paramedic should receive the signature. In addition, in case of refusal, transfer request should be made after hearing doctor's opinion and it should be notified to transfer request and superintendent of fire station after making 'confirmation of transfer refusal'. 4. Emergency medical technicians should perform their duties within the range of services prescribed by Article 41 of Law of Emergency Medical Service and Article 33 of Its Enforcement Regulations and shall not make announcement of death. In case of reporting the death to guardian, it is desirable to use record data like ECG results. 5. The best way to have protection from legal problems is making and keeping the exact records of accident and patient. Paramedic should not mention his subjective opinion about the accident-related matter. He must record correctly and keep the original medical records. 6. As emergency medical technicians are responsible for taking care of emergency patients, they must contact a briefing room when they meet a difficult situation suddenly due to vehicle stop or treatment of other patients and then must have support from neighboring hospital and other safety centers. 7. Since the ambulance operator is responsible for safety and careful driving of ambulance, he must be careful when he violates traffic regulations unavoidably. The operator should drive slowly below 10km/h at an intersection and pass it after getting way from general vehicles driving from all directions.

Efficacy of Nefopam Analgesia for Trauma Patients in the Emergency Department

  • Purpose: Nefopam is a centrally acting non-narcotic analgesic that has mostly been used for postoperative pain. We examined the efficacy of nefopam analgesia (alone and in combination with ketorolac) for trauma patients in the emergency department. Methods: We performed a retrospective chart review to select trauma patients who received nefopam at the emergency department of Korea University Medical Center Guro Hospital between January 2012 and December 2012. Patients younger than 15 years were excluded. The primary outcome measure was change of pain score (numeric rating scale) from baseline (before medication) to 30 min after medication. The secondary outcome measure was requirement for additional analgesia (pethidine). Results: Records of 1465 trauma patients who received analgesics in the emergency department from January 2012 to December 2012 were examined. Patients were classified into five groups according to initial analgesic: nefopam (n=112), ketorolac (n=867), pethidine (n=365), nefopam+ketorolac (92), and nefopam+pethidine (22). There were no significant differences in pain score reductions among the five groups. Twenty-two patients in the nefopam group, 141 in the ketorolac group, and 29 in the nefopam+ketorolac group required rescue analgesia with pethidine; these rates were not significantly different. Conclusion: The efficacy of nefopam analgesia for trauma patients in the emergency department is comparable to that of more commonly used agents, including ketorolac and pethidine.

The Relationship between Hopelessness and Suicidal Lethality of Suicide Attempters (자살기도자의 절망감과 자살치명도간의 관계)

  • The objective of this study is to evaluate the relationship of hopelessness and suicidal lethality of suicide attempters. The subjects were 72 persons who attempted suicide and came to the emergency room of Chungbuk National University Hospital from April 1st, 1995 to March 31st, 1996. The hopelessness of suicide attempter was assessed by Beck Hopelessness Scale(BHS), and the suicidal lethality was assessed by Risk-Rescue score. The Risk-Rescue Scores exhibited no significant differences in demographic characteristics (sex, age, education, religion, marital status, socioeconomic class, occupation) and psychiatric diagnoses. But the Risk-Rescue scores were significantly higher in high BHS group(BHS score ${\geq}$ 9) than in low BHS group(BHS score <9) and positive correlation was observed between the BHS and the Risk-Rescue score(Rs=0.77 : p<0.05). These results suggest that level of hopelessness be a major risk factor for suicidal lethality.

  • Currently police and fire departments use a Network/Wifi/GPS based emergency location positioning system established by mobile carriers to directly link with the device of the people who request the rescue to accurately position the expected location in the call area. However in the case of mountain rescue it is difficult to rescue the victim in golden time because the location of the search area cannot be limited when the victim is located in a radio shadow area of the mountain or the device power is off and this situation become worse if victim fail to report 911 by himself due to the injury. In this paper, we are expected to solve the previous problem by propose the mobile telecommunication forensic simulator consist of time series of cell information, human mobility model which include some general and specific features (age, gender, behavioral characteristics of victim, etc.) and intelligent infer system. The results of analysis appear in heatmap of polygons on the map based on the probability of the expected location information of the victim. With this technology we are expected to contribute to rapid and accurate lifesaving by reducing the search area of rescue team.

Route Exploration Algorithm for Emergency Rescue Support on Urgent Disaster (긴급 재해 발생 시 피난 지원을 위한 탈출 경로 탐색 알고리즘)

  • The emergency evacuation support system supports evacuation assistance when an urgent disaster occurs. We have implemented evacuation route search algorithm to assist people's escape when a disaster occurs such as fires or terrorism in the building. The algorithm will guide the escape route at the fastest emergency exit of each region at the emergency state. The algorithm calculates the escape route by applying the weighting factor of age groups and population density around the emergency exit and of other regions. So the system helps escape to bypass the crowded emergency exit and the disaster area, and reduces the congestion of emergency exit and overloading of evacuation route.
