The Effects of a Shift Improvement Pilot Project on Nurses' Shift Adjustment, Work-Life Balance, and Turnover Intention: A Mixed-Methods Study (교대제 개선 시범사업이 간호사의 교대근무 적응, 일과 삶의 균형 및 이직의도에 미치는 효과: 혼합연구방법)

  • Purpose: This study aimed to investigate the impact of a nurse shift improvement pilot project on nurses' shift adjustment, work-life balance, and turnover intention, as well as nurses' experiences of participating in the pilot project. Methods: An explanatory sequential mixed-methods study was performed. Survey data was collected from a total of 1,051 nurses both in participant and non-participant groups. For the qualitative study, focus group interviews were conducted with 22 nurses who participated in the shift improvement pilot, including 12 staff nurses, 7 nurse managers, and 3 substitute nurses. Results: There were significant positive effects on the key variables of nurses' shift adjustment, work-life balance, and turnover intention in the group that participated in the nurse shift improvement pilot project. Based on the qualitative study, participants in the shift improvement pilot project indicated that it enabled nurses to recover their circadian rhythm, and achieve a better work-life balance, and they wanted to continue working as a nurse, and hoped expanded application of the pilot project. Conclusion: In order for expanded implementation of the pilot project and continued improvement in the shift system, we recommend establishing compensation standard, systematizing the use of substitute nurses, developing guidelines for shift schedules, and expanding the shift improvement pilot project to other departments such as the emergency room.

Awareness of good death, perception of life-sustaining treatment decision, and changes in nursing activities after decision to discontinue life-sustaining treatment among nurses in intensive care units at tertiary general hospitals (상급종합병원 중환자실 간호사의 좋은 죽음인식, 연명의료결정 인식 및 연명의료중단 결정 후 간호활동의 중요도 변화)

  • Purpose : This study aims to explore nursing activities after the decision to discontinue life-sustaining treatment, awareness of a good death, and perception of life-sustaining treatment decisions among nurses in intensive care units (ICUs) at tertiary general hospitals. Methods : Participants were 173 nurses working in two tertiary general hospitals. The data were collected using structured questionnaires and analyzed using an independent t-test, paired t-test, one-way ANOVA, Scheffé's test, and Pearson's correlation coefficient. Results : Participants were 173 nurses working in two tertiary hospitals. The nursing activity increase was the greatest in the spiritual domain, and the physical domain was where the activities decreased the most. There were significant associations between Awareness of good death (Clinical) and Perception of life-sustaining treatment decision(r=.26, p <.001), Awareness of good death (Closure) and Perception of life-sustaining treatment decision(r=.36, p <.001), and Awareness of good death (Personal control) and Perception of life-sustaining treatment decision(r=.49, p <.001). Conclusion : Based on the results, systematic education programs and job training are required to improve the awareness regarding good death and perception of life-sustaining treatment decision for nurses in ICUs where discontinuing life-sustaining treatment decisions are made.

Development of Nursing Clinical Judgment Scale (간호사의 임상판단 측정도구 개발)

  • Purpose: This study aimed to develop a nursing clinical judgment scale (NCJS) and verify its validity and reliability in assessing the clinical judgment of nurses. Methods: A preliminary instrument of the NCJS comprising 38 items was first developed from attributes and indicators derived from a literature review and an in-depth/focus interview with 12 clinical nurses. The preliminary tool was finalized after 7 experts conducted a content validity test based on a data from a preliminary survey of 30 hospital nurses in Korea. Data were collected from 443 ward, intensive care unit, emergency room nurses who voluntarily participated in the survey through offline and online for the verification of the construct validity and reliability of the scale. Results: The final scale comprised 23 items scored on a 5-point Likert scale. Six factors - integrated data analysis, evaluation and reflection on interventions, evidence on interventions, collaboration among health professionals, patient-centered nursing, and collaboration among nurse colleagues - accounted for 64.9% of the total variance. Confirmatory factor analysis supported the fit of the measurement model, comprising six factors (root mean square error of approximation = .07, standardized root mean square residual = .04, comparative fit index = .90). Cronbach's α for all the items was .92. Conclusion: The NCJS is a valid and reliable tool that fully reflects the characteristics of clinical practice, and it can be used effectively to evaluate the clinical judgment of Korean nurses. Future research should reflect the variables influencing clinical judgment and develop an action plan to improve it.

  • Purpose: This study was conducted to examine the effects of case-based learning on the coping ability of daycare center teachers in emergency situations. In the study knowledge of emergency treatment methods, attitude, intention and ability to perform CPR between were examined in an experimental group (daycare center teachers who participated in the small groups discussing case studies and comparing them with their own experience) and a control group. Methods: The study was designed using a nonequivalent control group pretest-posttest design. Sixty-four teachers were recruited from 40 child daycare centers in S city located in J province. Thirty-three teachers were assigned to experimental group and participated in the case-based small group learning. The data were analyzed using the SPSS 18.0 program to perform ${\chi}^2$-test, Pearson correlation coefficients and t-test. Results: The experimental group showed significantly higher posttest scores in knowledge, attitude, intention, and performance ability than those of the control group. Conclusion: The study results indicate that case-based small group learning on coping ability in emergency situation is effective in increasing knowledge, attitude, intention, and performance ability for daycare center teachers.

A Study on the Nursing Profession as Stipulated by Health & Medical Laws of Korea (우리나라 보건의료법령에 명시된 간호에 관한 연구)

  • The purpose of the study is to find out how laws related to the nursing profession can be improved by analyzing the rules and regulations concerning nursing. Furthermore, to help settle legal matters in the process of doing nursing work. The data used for the study are the Health and Medical Act, the Maternal and Child Health Act, the School Health Act, the Special Act for Health and Medical Service in Rural Areas, the Industrial Health & Safety Act and the Notice on Nursing Professional Courses analyzed by age and content. The results of the study are as follows : First, basic nursing practice includes 'nursing care for recuperation and assistance in medical treatment and in special areas including the pre-vention of disease, maintenance of health, control of environment, and other therapeutic activities. It is suggested that the phrase 'assistance in medical treatment' should be eliminated as it limits the basic nursing practice to the assistance of the medical treatment. Second, Article 56 of the Health & Medical Act prescribes a special nurse but it does not prescribe a specific job. Accordingly, the new provison concerning the specific jobs of a special nurse should be added or a job guide should be inseated. Third, it is prescribed that those who have completed the training course after obtaining a license are qualified to be a midwife, a special nurse and a nurse practitioner working in special areas. However, school nurses, occupational health nurses and maternal and health workers are required to obtain a nurse license, but not to take an additional training course. Nurses working in special areas should be legally recognized as nurse specialists. The regulations to control various qualification standards consistently should be established. Fourth, the qualifications and types of nurses by area prescribed by Article 54 of the Health and Medical Act are not consistent with those of special nurses as recognized by affiliated organizations of the Korean Nurse Association and some hospitals. Accordingly, the qualifications and types of special nurses should be adjusted in consideration of special nurses. Fifth, as Article 16, Paragraph 2 of the Health and Medical Act does not prescribe the type and scope of first - aid treatment that nurses can provide, the first-aid treatment of nurses might be considered as an unlicensed practice. The specific regulations regarding these matters should be established. Sixth, the contents of the nursing record, which are prescribed by Article 21 of the Health and Medical Act as a duty, include 1) matters concerning body temperature, pulse, breath and blood pressure 2) matters concerning drug prescription 3) matters concerning input and output 4) matters concerning the treatment and nursing care (Article 17 of the Enforcement Regulations, Health and Medical Act). However, these matters are limited to basic nursing care and assistance in medical treatment. The new recording methods on nursing process are suggested to be adopted legally. Seventh, the prescription right entrusted to nurses which are prescribed by the School Health Act, the Special Act on Health and Medical Service for Rural Areas, and the Industrial Health and Safety Act are not consistent with the rights of nurses as prescribed by the Health and Medical Act. New regulations prescribing the partial right for medical treatment entrusted to nurses in consideration of the restraint of time and place in emergency situations should be established.

Relations Between the Types of Communication and, Job Satisfaction and Nursing Performance in Staff Nurses (일반간호사의 조직 내 의사소통 유형과 직무만족 및 간호업무성과의 관계)

  • This study attempts to investigate the relations between the types of communication, the job satisfaction. and the nursing performance of the staff nurses. The subjects of this study were the 114 nurses working in general wards, intensive care units, and the emergency rooms of the general hospital in Kyongbuk province. They were asked to answer the questionnaires as regards the types of communication and job satisfaction, and their performance of nursing, and these were evaluated by the head nurses, and their direct seniors. The data were collected for 10 days from the September 17 to 26, 2002. According to the purpose of the study, descriptive analysis, ANOVA. Pearson correlation coefficient, and stepwise multiple regression were applied for the data analysis using SPSS 10.0. The results are as follows : 1. The types of communication of staff nurses showed that the upward communication was $3.12\pm0.56$. the downward communication was $3.56\pm0.42$. the horizontal communication was $2.98\pm0.44$. and the informal communication was $2.59\pm0.49$. It showed that the most frequent communication was the downward communication. 2. In terms of the job satisfaction, there was significant difference according to the length of the period working for one ward (F=5.95, p=0.003), 3. The performance of nursing was significantly different according to the age (F=23.52, p=0.000), the working duration as a nurses (F=21.9, p=0.000), the types of nursing unit (F=5.33, p=0.002), and the working duration in present nursing unit (F=15.87, p=0.000). 4. In terms of communication type, the job satisfaction was associated with quality of the upward communication (F=6.23, p=0.003), the downward communication (F=5.78, p=0.004), and the horizontal communication (F=11.73, p=0.000) There were no association between the types of communication and the performance of nursing. 5. There was a positive correlation between the communication and the job satisfaction. And the downward communication (r=0.398) showed the greatest correlation with the job satisfaction. There was no statistically significant correlation between the communication and the performance of nursing. And the upward communication (r=-0.058) showed a negative, though not significant correlation with performance of nursing. 5. There was a positive correlation between the communication and the job satisfaction. And the downward communication (r=0.398) showed the greatest correlation with the job satisfaction. Comparison between Self-esteem and Health Promotion Behavior of Health Department and Non-Health Department College Students - Focused on comparison between Emergency Medical Technology Department Students and Engineering College Students - (보건계열과 비보건계열 대학생의 자아존중감과 건강증진행위에 관한 비교 연구 - 응급구조과와 공과계열 학생 간 비교 -)

  • Purpose: This study examines and compares the differences between self-esteem and health promotion behavior of health department and non-health department college students. Method: The research instruments were as follows : The first one is RSES developed by Rosenberg and translated into Korean by Jeon. The second one is HPLP developed by Walker et al, and including six subareas by Lee and modified to be suitable to Korean culture by Suh. Data were statistically analyzed with SPSS PC/12.0. Results : 1. Comparison of self-esteem had no significant difference(p=.190) and health promotion behaviors had significant differences(p=.000) between the two groups. 2. According to general characteristics and health-related characteristics, the differences of self-esteem in health department showed significant differences in grade(p=.020), sex(p=.000), age(p=.000), parents' economic conditions(p=.048), and smoking(p=.030) while non-health departments revealed significant differences in grade(p=.003), sex(p=.000), age(p=.000) and smoking(p=.001). 3. According to general characteristics and health-related characteristics, the differences of health promotion behaviors in health department showed significant differences in grade(p=.008), sex(p=.021), age(p=.000) and parents' economic conditions(p=.017) while non-health department revealed significant differences in sex(p=.000) and age(p=.000). 4. There were positive correlations between self-esteem, health promotion behaviors and subareas in health department (r=.422) and non-health department (r=.383). Conclusion: There were significant positive correlations between self-esteem and health promotion behaviors in health and non-health department college students. Therefore, health related programs should be developed to enhance self-esteem and health management by college students can influence themselves on self-esteem and health promotion behaviors.

  • This study was carried out by practical method in a subjectivity study accessible in-depth, in sloughing off old habit of functional quantity analysis about a subjectivity of fire-public servant in nation. The perception pattern come out in this study were divided into four types in Q-methodology. The result is as follows ; it is divided into 1[(N=26) : Occupational Type], 2[(N=11) : Experience Type], 3[(N=1) : Human Type], 4[(N=2) : Justice Type]. There are four types of fire fighter images. In conclusion, this study is to ascertain acceptance behavior about Reception Type on a subjectivity of fire-public servant in nation ; to offer a developmental suggestion about it.

Convergence awareness of basic emergency treatment by private ambulance drivers (민간구급차 운전자의 기본 응급처치 지식에 관한 융복합적 의식조사)

  • The purpose of the study was to investigate the awareness of basic emergency care by private ambulance drivers. A self-reported questionnaire was completed by 300 private ambulance drivers in Seoul, Gyeonggi, and Incheon from February 20 to April 1, 2013. The questionnaire consisted of basic life support related airway management(3 items), respiratory management(9 items), circulation(4 items), consciousness(4 items), and infection control(3 items). The awareness of infection control accounted for 87% and emergency care including airway, breathing, consciousness, and respiration was 60%. The knowledge of emergency care was 15.20(maximum 23.0 points). Consciousness was 2.55(maximum 4.0), airway management was 2.08(maximum 3.0), respiration was 5.35(maximum 9.0), circulation was 2.56(maximum 4.0), and infection control was 2.64(maximum 4.0). In order to improve the quality of private ambulance drivers, it is important to develop the emergency care program and expansion.

A Study of Trends in Continuing Education Published in the Korean Nurse (간호협회지를 통해 본 보수교육의 역사적 경향 1962년 ~ 1995년)

  • This historical research was conducted to analyze and categorize the titles which were presented in the journal, The Korean Nurse, from August 1962 to October 1995. Titles which were published with the purpose of educating graduate nurses and to update 0 their nursing knowledge to improve professional practice were included. There were 348 articles published from the beginning of publication in August, 1962 to October, 1995. All of the journals were reviewed except nine missing journals which were not available in any library. According to the characteristics of the articles in the periodical, the articles were divided into three periods. In each of the three peroids there were five categories: Subject, Clinical Practice, Fundamentals of Nursing Science, Nursing Administration, Others. These categories were adopted from Kim's(1994) division system which was developed to analyze nurse's insurance education program. The special feature peroid was from August, 1962 to December, 1974. In this period the articles were presented in an unorganized manner in the area of special feature or main issue. The largest area was the subject category(44%). The second largest area wes the fundamental of nursing science category(31%). From May, 1975 to December, 1977, the articles with the educational purposes were published in a designated area called continuing education. This period was labelled as the continuing education period. Among the published articles in this period, 45% focused on the subject category and 45% on the fundamentals of nursing science category. In this period the articles were focusing on nurses 'work in specific areas such as industry, nurses' aid schools, and nursing administration, articles on physical assessment first started to appear. The written continuing education period was from January 1978 to October, 1995. All the educational articles published in this area were analyzed and categorized into five areas as for the other periods. 48% of the articles focused on the subject category. In the mid-eighties, the term nurse specialist first.appeared and ten years later in 1990, the titles were subdivided into more specific titles, such as, home nursing, industrial nursing, emergency nursing, 23% were in the fundamental nursing science category and they dealt with nursing process, nursing theories, theory development. For the content analysis, three articles, one from each period, dealing with cardiovscular diseases were selected for comparsion. First, the special feature period, the title of the article was on diet therapy for cardiovascular disease patients, but instead the content was about rest, hygiene, medication, observation. They recommended rings to prevent bed sores, which is now considered as a cause for bed sores. In the continuing education period, the content was more focused on rehabilitation rather than general nursing .care. It became more specific, systematic, and organized compared with the special feature period. In the written continuing education period, the focus was on rehabilitation, not broadly, but very specifically on exercise. The further research on the content analysis is recommended along with a comparison of the trends in the Journal of Nurses Academic Society.
