Purpose: This study intended to evaluate the intermediate effects of using Electronic Nursing Record System which was introduced 4 years and 4 months ago. Methods: Participants were 65 nurses engaged in 3 shifts in a general hospital. The specific methods were identification of nursing activity times by means of nurses' self-recording in structured forms, survey of user satisfaction using questionnaires, and examination of nurses' responses to open-ended questions about using the system. Results: The direct nursing activities took more than 50% of the time during day and evening shifts. In night shift, direct nursing activities took more than 41% of the time. Comparing to the initial evaluation, measured at 10 months after the introduction, the time spent in indirect nursing activities have decreased about 10% in each shift. The user satisfaction was 3.54 points which was higher than 3.33 points measured at 10 months after the introduction. The nurses recommended some changes needed in the current system to make it better, such as speeding up the system. Conclusion: The intermediate evaluation shows reduced indirect nursing time, increased direct nursing time, and improved user satisfaction.

In this paper we explain the definition and component of electronic records, and reestablish the principle of the preservation. Also we discuss the conceptual approaches and requirements for long-term preservation in an electronic environment. The preservation in digital environment is to protect itself of record identity with the reliability and authenticity, and to ensure the accessibility of records. It needs to be periodical migration along with digital environment change for the long-term preservation of the electronic records. and to select software independent standard format. to ensure the integrity of electronic records along with this preservation strategy, process and relative information required for digital preservation are to record in the form of metadata. The preservation system is separated into four functional entities ; registration process, preservation process, access process, main record management system. Also, this article is conceptually to introduce the selection criteria of storage media, the security provision for the archive collections and the disaster recovery which apply to the digital archives.

Most of electronic medical record systems which have been built in Korean hospitals are based on source oriented medical record approach. These systems hardly satisfy diverse objectives owing to the innate imperfections in system architecture and development methodology. Thus, the hybrid of source oriented and problem oriented approach is highly desirable. The purpose of this study is to present an architecture and methodology required to construct hybrid electronic medical record system and to develop a prototype based on them. Analyzing the clinical processes and data requirements of problem oriented medical record approach we developed a software process model as weel as an architecture model which consists of legacy system, clinical data repository, problem list database, prospective plan database, user interface, and synchronization procedures.

문제: 전자의무기록(EMR) 시행 후 의무기록 정리율의 저하와 질적인 측면에서의 충실성과 정확성에 대한 문제점이 제기되었다. 목적: 전자의무기록의 정리율과 충실성 검토를 통하여 문제점을 파악하고 개선점 찾아 의무기록 정리율을 향상시키고 충실성을 높이고자 하였다. 의료기관: 서울시에 소재한 대학병원 의무기록과 질 향상 활동: 전자의무기록의 문제점을 개선하기 위하여 사용자 편의를 위한 EMR 프로그램 수정 및 보완, 진단 수술 관련 작업, 업무개선, 교육, 홍보 등의 활동을 실시하였다. 개선효과: 의무기록 정리율, 전자인증미비, 경과기록 기재일수, 퇴원요약 주진단 적합률, 기록지별 필수항목 기재율, 충실성에서 향상이 이루어졌다.

As records managers are assigned in accordance with act on public records management, the record management has settled down a bit, and also much effort is put to perform the record management in accordance with law. The record management includes all the works like production, classification, organization, transfer, collection, evaluation, disuse, preservation, opening to the public and application. Among them, the record transfer is an important work that performs the initial stage in which the main agent of management is changed from administration department to record center. Thus this study suggested the improvement measures for non-electronic record transfer after examining the current transfer state of 16 local governments in Busan region and also problems occurring in the process of transfer through interviews with institutional records managers.

There have recently been an increasing number of reports claiming that the periodontal disease, the most typical oral disease, is closely associated with various systemic diseases such as cardiovascular disease. Therefore, it is imperative to explore the methods that can ensure effective and cost-effective monitoring of periodontal disease in specific population groups, as well as individuals in the electronic medical record environment. However, traditional periodontal examination methods, such as periodontal probe, are considerably demanding and time-consuming for dental specialists, and furthermore, cause the risk of bacteremia as they are invasive procedures. Thus, the objective of this study was to validate the capability of new method for screening the periodontal disease using the self-reported questionnaire for periodontitis which is the non-invasive procedure to replace radiography.

Purposes: It is very important to establish a clinical data warehouse based on a common data model to offset the different data characteristics of each medical institution and for drug surveillance. This study attempted to establish a clinical data warehouse for Dankook university hospital for drug surveillance, and to derive the main items necessary for development. Methodology/Approach: This study extracted the electronic medical record data of Dankook university hospital tracked for 9 years from 2013 (2013.01.01. to 2021.12.31) to build a clinical data warehouse. The extracted data was converted into the Observational Medical Outcomes Partnership Common Data Model (Version 5.4). Data term mapping was performed using the electronic medical record data of Dankook university hospital and the standard term mapping guide. To verify the clinical data warehouse, the use of angiotensin receptor blockers and the incidence of liver toxicity were analyzed, and the results were compared with the analysis of hospital raw data. Findings: This study used a total of 670,933 data from electronic medical records for the Dankook university clinical data warehouse. Excluding the number of overlapping cases among the total number of cases, the target data was mapped into standard terms. Diagnosis (100% of total cases), drug (92.1%), and measurement (94.5%) were standardized. For treatment and surgery, the insurance EDI (electronic data interchange) code was used as it is. Extraction, conversion and loading were completed. R language-based conversion and loading software for the process was developed, and clinical data warehouse construction was completed through data verification. Practical Implications: In this study, a clinical data warehouse for Dankook university hospitals based on a common data model supporting drug surveillance research was established and verified. The results of this study provide guidelines for institutions that want to build a clinical data warehouse in the future by deriving key points necessary for building a clinical data warehouse.

This is a convergence study to provide an effective way of managing nurses' job stress by improving nurse satisfaction through an integrated identification of the effect satisfaction level on job stress for nurses using an electronic medical record(EMR) system. The data were collected from August 1 to September 1, 2014. The participants were 377 nurses from C university hospital of G metropolitan city were analyzed using SPSS 21.0. The result shows that job stress decreases as nurses' satisfaction level of using the EMR system increases. Thus, a practical job stress mediation program should be developed to continuously enhance nurses' satisfaction with the EMR system, which is expected to shorten the nursing record writing time and improve the quality of nursing service for patients. Furthermore, to improve the satisfaction with the EMR, appropriate management, such as continuous nursing education on the EMR system, will be needed.

The system for reporting record production has been operated to collect and transfer the public records adequately since the enactment of the Public Records Management Act in 1999. The Act, which was revised in 2007, regulates automated reporting methods in which the production reporting files are generated in the records creation systems and transferred to the records management systems. However, only one type of record is being notified electronically among seven types, which should be notified under the Act. The remaining six types of records are laboriously reported using complicated templates. Furthermore, the current working electronic notification mechanism is also causing various errors mainly because of inadequate specifications. This study analyzes the causes of failure of the electronic notification through the electronic records systems and suggests some policies for its improvement.

Currently, different medical institutions have been carrying out the e-healthcare system project. The system includes electronic medical record and prescription delivery system, and, the Medical Treatment law permits electronic signature for medical record management, which reduced the relevant costs and enabled sharing medical record. And medical solution using online certificates is expanding its application. In that light, the role of certificates became more important than ever. However, in contrast to active effort made to manage personal certificates, certificates related to medical solutions and other types of work are not being managed properly. Most work-related certificates are saved in office computers, which makes them vulnerable to various security threats. Although certificate servers can be used as a solution to this problem, hospitals must build the server separately and, therefore, small and medium-size hospitals can be reluctant to bear the burden. This study proposed a way to design and implement an effective and secure certificate management system by save the certificate file as a BLOB, using existing resources without needing to build a separate certificate server, at minimized costs.
