농촌 고령자의 일상활동능력 및 우울감이 생활만족도에 미치는 영향 (The Effects of the Instrumental Activities of Daily Living and Depression on the Life Satisfaction of the Rural Elderly)

  • The purpose of this study was to investigate the effects of the instrumental activities of daily living (IADL) and depression on the life satisfaction of the rural elderly. The research subjects of this study were 1,000 people aged 65 years and over living in the rural areas of Korea. The data were collected using a questionnaire with direct interviews and analyzed using SPSS ver. 18.0 for Windows. The results of this study were as follows: First, the rural elderly's depression was affected by the IADL, cost of living, education level, and farming variables. The higher the IADL, the more sufficient the cost of living, the higher the education level and the more the farming time that the rural elderly had, the lower was the level of depression that they had. Second, the life satisfaction of the rural elderly was affected by the cost of living, depression, IADL, need for life services, and gender. The more sufficient the cost of living, the lower the level of depression, the higher the IADL, the less the need for life services, the higher was the life satisfaction that they experienced. Third, the results of the path analysis revealed that life satisfaction was influenced both directly and indirectly. The factors that affected life satisfaction directly were the cost of living, IADL, need for life services, and gender. The factors that affect life satisfaction indirectly were the IADL, cost of living, education level, and farming. Among them, farming and education level influenced life satisfaction indirectly through the parameter variables of depression.

우리나라 노인의 우울과 주관적 구강건강 관련성: 국민건강영양조사 제6기 2차(2014년) 자료 이용 (Association between depression and poor oral health in Korean elderly: the six Korean national health and nutrition examination survey (KNHANES VI-2))

  • Objectives: The purpose of this study was to examine the association between depression and poor oral health in Korean elderly using Korean version of the Patient Health Questionnaire-9 (PHQ-9) for assessment of depressive symptoms. Methods: This study used the data from Korean National Health and Nutrition Examination Survey (KNHANES VI-2). The study included 1,454 elderly Korean aged over 65. Variables included demographic characteristics (gender, age), socioeconomic factor (income, education), systemic diseases, oral health related factor (tooth brush, dental products), health related factor (alcohol drinking, smoking), and depression. Logistic regression analysis was used as sequential models. Effects were quantified as odds ratios (OR) and 95% confidence intervals (CI). Results: From frequency analysis, being female, primary school or less, non-alcohol drinking, poor oral health were significantly related to depression. In the multiple logistic regression model, depression was significantly associated with poor oral health (OR=1.96, CI=1.15-3.53) after adjustment for other covariates including demographic characteristics, socioeconomic factor, systemic diseases, oral health related factor, and health related factor (OR=1.91, CI=1.13-3.27). Conclusions: Depression had an influence on the poor oral health after adjustment as confounding variable in the elderly. It should be focused on the health promotion for the elderly vulnerable to depression and poor oral health. The development of the mental health and oral health should be established.

  • Purpose: This study was conducted to examine the effects of foot reflexology on arthralgia, ankylosis, depression and sleep in community-dwelling elderly women with osteoarthritis. Methods: The research adopted a non-equivalent control group pretest-posttest design. Participants were 47 elderly women: 22 in the experimental group and 25 in the control group. The experimental group received foot reflexology twice a week for 4 weeks. The results were analyzed using $x^2$-test, t-test, and ANCOVA with the SPSS/WIN 21.0 program. Results: The results showed that foot reflexology was statistically significantly effective in reducing arthralgia, ankylosis, and depression and in improving the quality of sleep as evidenced by differences between two groups. Conclusion: The results of this study indicated that foot reflexology is an effective nursing intervention in reducing arthralgia, ankylosis, and depression, and improving the quality of sleep. Therefore, it is necessary to develop foot reflexology as an independent nursing intervention for elderly women with osteoarthritis in community.

노인의 스트레스, 우울 및 건강위험 행위에 관한 연구 (Stress, Depression and Health Risk Behaviors in Korean Elders)

  • Purpose: The purpose of this study was to investigate stress, depression and health risk behaviors in elderly people. Methods: The subjects were 238 Korean elders who had visited the convalescent center. The level of stress was measured by the Korean version of BEPSI, which was developed by Bae, Jeung & Yoo (1992) and modified by Yim et al.(1996). CES-D was used to measure the level of depression. For health risk behaviors, we measured cigarette consumption and alcohol drinking during the last one month. Results: Of the subjects, 17.4% were excessive drinkers and 18.9% were currently smokers. The level of stress was between moderate and high, and 62.7% of the subjects were depressed. There were significant differences in stress and depression according to economic status and self-reported health. In addition, depression had positive correlations with stress and QF score. Conclusion: Stress and depression management programs for the elderly are needed. These findings indicate that nurses must consider stress and alcohol drinking when they take care of depressed elderly people.

우울증 노인을 위한 현실치료 프로그램의 효과 (The Effects of a Reality Therapy Program for the Elderly with Depressive Disorder)

  • Purpose: The purpose of this study is to develop a Reality Therapy program for the elderly with depressive disorder, and then examine the effects of the developed program. Methods: This study used 'non-equivalence control group pretest-posttest design'. The participants were forty-eight elderly who were on medication treatment with depressive disorder. Twenty-two participants were assigned to the experimental group and twenty-six ones were assigned to the control group. A 10-session Reality Therapy program was provided to the experimental group. The data were collected from September to December in 2012. Dependent variables were self-esteem, internal control, and depression. Results: The scores of self-esteem for the experimental group were significantly increased compared to the control group (t=-4.87, p<.001). The scores of depression for the experimental group were significantly decreased compared to the control group (t=3.61, p=.001). However, there was no significant change on the scores of internal control between the two groups. Conclusion: The results of this study indicate that the Reality Therapy program is effective in increasing self-esteem and decreasing depression for the elderly with depressive disorder.

독거 당뇨병 노인의 외로움이 자살사고에 미치는 영향에 대한 우울의 매개효과 검증: 2차 자료 분석 (The Mediating Effects of Depression on Loneliness and Suicidal Ideation in Elderly Living Alone with Diabetes Mellitus: A Secondary Data Analysis)

  • 본 연구는 독거 당뇨병 노인의 자살사고를 분석하고 자살사고와 외로움, 우울의 연관성을 규명하는 것이 목적이다. 본 연구는 O 지역의 독거노인을 대상으로 한 전수조사 자료를 활용하여 2차 자료 분석을 통해 수행되었다. 본 연구 대상자는 독거노인 중 당뇨병을 진단받은 466명이었다. 본 연구의 자료는 SPSS 26.0 프로그램을 이용하여 서술통계, t-test, Pearson's correlation coefficients, 다중선형회귀분석 및 Baron과 Kenny의 3단계 매개효과 검증 절차를 활용하여 분석하였다. 대상자의 평균 외로움 점수와 평균 우울 점수는 각각 4.52±3.30, 4.88±4.03으로 나타났으며, 자살사고가 있다고 응답한 대상자는 27.3%이었다. 대상자의 외로움(β=.20, p=.005)과 우울(β=.30, p<.001)은 자살사고를 예측하는 유의한 요인으로 나타났다. 또한 외로움과 자살사고의 관계에서 우울은 부분 매개하는 것으로 나타났다. 본 연구 결과를 통해 외로움과 우울은 독거 당뇨병 노인의 자살사고와 관련된 중요한 요인으로 나타났다. 또한 독거 당뇨병 노인의 자살사고를 개선하기 위해서는 외로움 및 우울의 수준을 낮추는 중재가 필요하다.

노인복지시설 이용자의 인구학적 특성에 따른 노화에 대한 태도와 우울에 미치는 영향 (The Effects of Attitude toward Aging and Depression According to Demographic Dharacteristics in Elderly Welfare Facility Users)

  • 본 연구는 노인복지시설 이용자의 인구학적 특성에 따른 노화에 대한 태도와 우울에 미치는 영향을 알아보고자 하였다. 이를 위해 경기도 소재의 15곳의 기관에 280명의 시설이용 노인을 대상으로 하여 설문을 실시하였으며, 최종적으로 258명의 자료를 분석에 사용하였다. 측정도구는 노화에 대한 태도 척도, 치료자의 태도 척도, 우울의 척도를 사용하였다. 본 연구의 결과는 다음과 같다. 첫째, 성별에 따른 노인복지시설 이용자의 노화에 대한 태도, 우울에 차이가 없는 것으로 나타났다. 둘째, 최종학력에 따른 노인복지시설 이용자의 노화에 대한 태도, 우울에서는 최종학력이 높을수록 긍정적인 노화에 대한 태도와 우울을 나타냈다. 셋째, 노인복지시설 이용경험에 따른 노화에 대한 태도, 우울에서는 이용 경험이 있는 많은 노인들이 노화에 대한 신체적 변화에서 긍정적인 유의미한 차이가 나타났다. 따라서 연구결과 노화에 대한 태도가 부정적이어서 꼭 우울하다고 판단할 수는 없으나 최종학력에서는 높은 관련성이 있음을 알 수 있었다. 노인복지시설에서 근무하는 치료자들은 노인들을 치료, 케어함에 있어 최종학력이 낮을수록, 이용했던 시설의 수가 적을수록 노화에 대한 태도가 부정적일 수 있고 이는 곧 우울감을 높일 수 있는 요인이 될 수도 있음을 알아야 할 것이다.

지역사회 거주 노인의 성별에 따른 요통 및 좌골신경통 관련 요인 (Gender Differences in Relating Factors of Low Back Pain and Sciatica in Community-dwelling Korean Elderly)

  • Purpose: The aim of this study was to investigate gender differences in risk factors and sleep, depression, and mobility of Korean elderly with and without low back pain and sciatica. Methods: Data were derived from the 2011 Korean National Survey on Older Adults. Participants included 10,674 community-dwelling elderly. The group of elderly subjects with low back pain and sciatica and the group of elderly subjects without low back pain and sciatica were compared according to gender. Result: Age, monthly income, exercise, arthritis, osteoporosis, and providing caring support were significant risk factors for low back pain and sciatica in elderly male subjects. On the other hand, monthly income, living arrangement, arthritis, osteoporosis, providing caring support, and television watching time were significant risk factors in elderly female subjects. Significant differences in depression and mobility according to low back pain and sciatica were observed in both genders. Conclusion: Risk factors for low back pain and sciatica in elderly differed according to gender. Low back pain and sciatica showed an association with depression and mobility. These findings should be considered in planning for nursing intervention for low back pain and sciatica.

건강유지 프로그램이 시설노인의 신체 건강과 정신 건강에 미치는 효과 (The Effect of a Health Maintenance Program on Physical Function and Mental Health of the Elderly in Nursing Homes)

  • Purpose: The purpose of this study was to determine the effects of a Health Maintenance Program on physical functions and mental health of the elderly in nursing homes. Method: Sixty elderly(over 65 years old) in a randomized control study participated in a 16-week group-based intervention including functional exercises and health education. The participants were divided into 3 groups(Health Maintenance Program Group, Supportive Music Exercise Group, and Control Group) of 20 elderly each. Data was collected from Dec. 1st, 2005 to Mar, 30th, 2006. Physical function of lower body strength was assessed using a 30-second chair test, flexibility was assessed using a sit-and-reach test, and static balance was assessed by the ability to balance on one leg with open and closed eyes. Depression was assessed using the Korean Form of the Geriatric Depression Scale and self esteem was assessed using Rosenberg's Self Esteem Questionnaire. Data was analyzed by Chi-square test, One-way ANOVA, and Repeated measure two factor analysis. Results: A Health Maintenance Program significantly increased muscle strength, flexibility and static balance, but depression and self-esteem scores were not significantly changed. Conclusion: Findings demonstrated that a Health Maintenance Program was more effective on physical function than mental health of the elderly in nursing homes.

장기 보호 여성노인의 수발에 관한 연구 - 수발특성과 우울을 중심으로 - (Caregiving for the Long-Term Care Elderly Women - Focusing on Caregiving Characteristics and Depression -)

  • The present study was initially designed to figure out the general condition of care giving system for the elderly women who need long term care and the level of their depression according to the conditions of care. And This research is intented to present appropriate policy that could help the establishment of supporting system for the fragile elderly women.1 used the data from <2001 National Study on the Needs for the Long-Term Care Elderly> by Korea Health and Population Institute. The results are as follows: First, Two third of all the respondents had serious problems (2-9 activities limits) in Instrumental Daily Living Ability(DAL). Most respondents reported “low” in satisfaction level related to receiving care, meaning the elderly had negative perception for the care from the family. The elderly expected their children to be as the primary care giver and mostly wanted to live with them in the future. Second, The majority of the long term care elderly women haven't used community service facilities very often and said they are not likely going to use the facilities in the future. Third, The respondents reported high in depression level as to lower satisfaction with their children's support, poorer health condition, more reluctant to use service facilities due to the cost, and fewer friends and neighbors resources around them. Therefore I could say that negative factors for the elderly women's psychological health were having unsatisfactory relationship with intimate people, developing physical illness, being in economic difficulties. That is, receiving less help from close family members, shrinking social network, and experiencing economic hardship would have negative effects on elderly women's psychological health. In the basis of these results, I suggest that in the mean time we shouldn't overlook the importance of the private support when we develop the public elderly support system.
