Objectives: The purpose of this study was to investigate the present conditions and problems of oral health care in senior citizens with dementia using a qualitative research method, through focused group interviews. Methods: Data was collected for approximately one month from May 2019. The subjects were divided into two groups: care workers and family caregivers. Fifteen participants were included in the study. Results: In-depth interviews with the care workers revealed the following three categories: characteristics of senile dementia patients, oral health care in senile dementia patients, and oral health care education. In-depth interviews with the family caregivers revealed the following four categories: characteristics of senile dementia patients, oral health care in senile dementia patients, oral health care education, and burden of care. The central themes common to both the care workers and family caregivers were the challenges owing to the characteristics of senile dementia patients, poor health condition of the senile dementia patients, difficulty in oral health care of the senile dementia patients, the desire to receive oral health care education and related information, and to access the information more easily. Additional central themes specific to the care workers were, the applicability of the intervention programs, variability between the facilities, and the problems of oral health care education. An additional central theme specific to the family caregivers was the burden of care. Conclusions: It is necessary to provide oral health care education and information to care workers and family caregivers of senile dementia patients, and to manage and support the dental health professionals ready to care for senile dementia patients. Accordingly, this study aims to investigate the oral health knowledge and attitude of caregivers on oral health management of elderly inpatients, which would affect the general health and quality of life. Methods: A survey questionnaire was distributed to the caregiver(n=165), at a neurosurgery or orthopedic hospital inpatient ward in Seoul, Gyeonggi-do from April 1 to 14, 2017. We analyzed the data with descriptive statistics analysis, chi-squared analysis and logistic regression analysis by using SPSS 23.0. Results: The response rate to the questionnaire was 92%. Oral health management provided by caregivers to inpatients in the elderly was prevention of oral diseases(92.8%), tooth brushing (89.5%), use of oral care products(68.4%), denture cleaning(90.8%) and oral cleaning(90.8%). Frequency analysis of oral health management in accordance with the educational needs of caregivers was all confirmed statistically significant(p<0.05). In the logistics regression model, oral health knowledge was significantly associated with low oral cleaning(OR=1.58) and oral health attitude was significantly associated with high denture cleaning(OR=1.29) and oral cleaning(OR=1.28) after adjustment for other covariates including gender, age, certification, education level, ward, working years. Conclusions: It is necessary to expand the scope and improve the quality of education that can change the attitudes and behaviors of the caregivers on the oral health management of the elderly on the basis of the current oral health education.
Objectives: This study aims to contribute to devising systems for family caregivers of dementia patients by examining the state of oral care of dementia patients, and depression and social support among family caregivers of dementia patients. Methods: Family caregivers of dementia patients in the metropolitan area were selected in this study. The inclusion criteria were individuals who have provided care for a dementia patient at home for at least six months and those who come in contact (including phone calls) with the patient at least twice a week. Results: Oral health knowledge of the elderly, caregiving burden, depression, and social support were examined. The mean scores for oral health knowledge of the elderly and caregiving burden were $57.11{\pm}16.94$ out of 100 and $17.33{\pm}8.61$ out of 48, respectively. Further, the mean caregiving behavior score, depression score, and social support score were $8.49{\pm}13.71$ out of 100, $5.11{\pm}3.05$ out of 10, and $72.75{\pm}17.03$ out of 100, respectively. Factors affecting oral health knowledge of the elderly were examined. The results showed that the level of oral health knowledge of the elderly increased with an increasing perception of a need for oral health education (p<0.05), caregiving burden (p<0.01), and social support (p<0.01). Conclusions: These findings suggest that developing and popularizing oral care intervention programs for family caregivers of dementia patients are necessary to ensure systematic oral care for dementia patients.
