연령증가에 따른 남녀노인들의 영양상태 및 이에 영향을 미치는 요인 분석 (A Study on Nutritional Status and Its Related Factors in Elderly Koreans with Ages)

  • Elderly Koreans(<60 year old) living in Seoul were surveyed with questionnaire to Investigate their nutritional status and those factors affected. The result could be summarized as follows : Mean energy intakes of elderly men and women were 1,528 and 1,292 kcal, and 79.1 and 65.5% of RDA, respectively. Below 215 of RDA were Ca and vile. A in men and Ca, vile. A and riboflavin in women. Compared with elderly men, elderly women showed lower quality of diet. In men intakes of nutrients were markedly decreased after 80 years old but in women these tendency showed more early stages of ages. The factors that had effect on nutrient intakes were age, height in men and SES, body weight and BMI in women.

한국노인의 신체활동 및 식생활과 정신건강 관련성에 대한 연구 (A study on relationship between Physical activity, Diet pattern and Mental health of Korean elderly)

  • 본 연구의 목적은 우리나라 만 65세 이상 노인의 연령별 신체활동 및 식생활과 정신건강의 관련정도와 영향요인을 파악하여 노인의 정신건강에 긍정적으로 대처할 수 있는 요인을 확인하고, 노인건강프로그램 개발의 기초자료로 활용하기 위함이다. 본 연구는 2015년도 국민건강영양조사결과를 기초자료로 한 이차자료 분석연구이며, 우리나라 만 65세 이상의 노인 1,484명을 대상으로 하였다. 자료분석은 IBM 23 ver. SPSS를 이용하였다. 본 연구 결과 전기노인은 정신건강변수의 세부항목 중에서 우울감과 스트레스 인지가 높았으며. 후기노인은 자살생각이 높았다. 식생활형편은 전기노인보다 후기노인에서 '음식섭취 부족' 이 높았다. 회귀분석 결과 노인의 식생활 형편, 중강도 신체활동이 노인의 정신건강과 관련성이 있는 것으로 보고되었다. 연구결과를 토대로 세분화된 연령대에 적합한 차별화된 간호중재 프로그램개발이 필요하다.

서울시내 거주 노인의 영양섭취실태 및 식생활태도 조사연구 -은퇴한 노인을 중심으로- (A Nutrition Survey of Urban Elderly in Seoul with the Analysis of Deitary Attitude after Retirement)

  • This study deals with the conditions of nutritional intake of the urban elderly (age sixty years and older). This study analysis is based on factors that are influenced on the prevalent conditions of dietary-intake of the urban elderly either it be for better ment of health or desire. The method of this research was based on the interview-survey with questinaire in the Seoul area. In comparison to the surveys taken of the rural society and the urban slum elderly the urban elderly standard nutritional requirements was higher. Of the standard nutritional requrieemtns the twomen's nutrition intake was higher than of the men. But still the problem of malnutrition existed in the urban elderly both men and women at the probability nutritional deficienty rate higher than 30%. The food intake frequency factors which might influence the condition of nutritional intake was significant more than condition of eating. Another important factors of the conditions of nutritioal intake of the urban elderly are one's self-consciousness of health and degree of one's will change . In healthy case of the aged, their will was almost maintained and hand -grip strength was high when they had regular meal with their family . For elderl with the lower academic career and the one with more satisfactory to his meal, their wills didn't change before or after retirement. The objectiveness of this survey is to convince the elderly that the problems of nutritional deficiency can only be solved by reeducation and to improve their nutritional diet to have the enjoyments of a healtheir elderly life.

한국 여성의 연령별 식품 및 영양소 섭취량 조사 (A Survey of Food and Nutrient Intakes of Korean Women by Age Groups)

  • To study the level of change in food and nutrient intake among different age groups, a dietary survey using the 24-hour recall method was conducted among 80 female elementary school children, 84 high school students, 100 adults from 25 to 35 years old and 120 elderly people over 60 years of age. The results of the study showed that plant food and total food intake were higher in adults and animal food intake was significantly higher in children. The percentage of plant food to total food intake was highest in the elderly and that of animal food to total food intake was highest in the children. The elderly consumed significantly less meat and poultry and more fish than the other groups. However, the children consumed significantly higher amounts of milk and dairy products. The elderly had the lowest Ca intake. Carbohydrate energy intake in the elderly was 70.3%, significantly higher than that for the other age groups. All of the age groups consumed a quantity of Ca below the Korean RDA and 77.5% of the elderly consumed an amount below 75% of the RDA. Nutrient adequacy ratio (NAR) of most nutrients was lowest in the elderly. Mean adequacy ratio (MAR), an index of overall dietary quality, was 0.88 for the children, 0.84 for the adolescents, 0.80 for the adults, and 0.70 for the elderly. Therefore, the results show that food and nutrient intake changes with advancing age, and the overall quality of the diet of elderly people is lower than that for the other age groups.

영양교육에 의한 노인 영양증진전략연구 : 효과분석 (Strategies of Improve Elderly Nutrition through Nutrition Education : Evaluation of the Effectivenss of the Program)

  • The increased number of elderly in Korea presents great challenges for community nutrition programs. Strategies to enhance elderly nutrition are needed, as well as dietary advice that are reality-based and tailored to the need of the elderly. The purpose of this study was to evaluate the overall effectiveness of an elderly nutrition improvement program(ENIP), by assessing changes in nutrition knowledge, attitudes, and behavior after program completion. ENIP was conducted in Suwon for 4 months in 1998 at 5 centers for the elderly. The program provided individual nutrition counseling and brief biweekly group education sessions. To stimulate participation and motivation, the elderly in two centers were served free milk packs(200ml) three time a week during the program(milk+education group), and the elderly in 3 other centers were served yoghurt on the program day(education group). Altogether, 102 subjects finished the program(milk+education group 67 ; education group 35). Data about nutrition knowledge, dietary attitudes, diet records(24 hour recall) and dietary habit was collected before(baseline) and after the program(follow-up). The Drop-out rate for the milk+education group(2.9%) was significantly lower than that for the education group(30.0%)(p<.001). The Nutrition Knowledge fo females milk+education group increased significantly after the program, but no significant differences were observed in other groups. Over 60% of all the elderly showed positive responses to the healthy eating attitudes. Energy, protein, vitamin A, riboflavin, Calcium and Potassium intakes improved in the male milk+education group. The Vitamin C intake improved in the male education group. Energy and vitamin A intakes improved in the female milk+education group and energy, vitamin A, thiamin, riboflavin, niacin, Ca nad P in the female education group. A stepwise multiple regression analysis was performed to examine the effects of food intake changes on overall nutrient quality. For all elderly, the fish intake explained 12.6% of the variance, followed by candy intake and vegetable intake(model $R^2$=19.6%). Dietary habits such as meal time regularity and salty food reduction were markedly improved in both groups. Sixty-one percent of the milk+education subjects stated their participation was voluntary and active, while 51% of education subjects did. Sixty four percent of the subjects stated their interest on diet increased remarkably through program participation. Considering the results, it is conjectured that ENIP had a great impact on nutrient intake, dietary attitudes and habits. Milk supplement showed no direct effects on elderly nutrition improvement, but it encouraged the program by reducing drop-out rates and inducing voluntary participation. Therefore milk supplement could enhance the community nutrition education program. Moreover it is strongly suggested that the ENIP be expanded to other communities.

웰니스 건강 증진 프로그램 참여에 따른 고령자 건강생활습관 변화 (Changes in Life Habits of Health Associated with Participation in a Wellness Improvement Program in the Elderly)

  • 본 연구는 고령자의 건강증진에 도움을 주고자 노인종합복지시설을 이용하는 고령자들을 대상으로 신체형태 구성과 노인체력, 생활습관 및 영양수준을 파악한 후 고령자들에게 웰니스 건강증진 프로그램을 12주간 실시하여 다음과 같은 결론을 얻었다. 첫쨰, 성별에 따른 연령별 신체구성형태 변화 중 남자는 BMI 변화에서 유의한 차가 나타났고, 여자는 BMI, 체지방율 변화에서 유의한 차이가 나타났다. 둘째, 성별에 따른 연령별 노인체력 변화 중 남자는 하체근력, 상체근력, 상체유연성, 변형 전신지구력 변화에서 유의한 차가 나타났고, 여자는 하체근력, 상체근력 변화에서 유의한 차이가 나타났다. 셋쨰, 성별에 따른 연령별 생활습관 및 영양수준 변화 중 남자는 식생활습관, 운동습관에서 유의한 차가 나타났고, 여자는 운동습관, 스트레스 해소습관과 총 영양 수준 변화에서 유의한 차이가 나타났다. 이러한 결론을 종합하면, 고령자의 웰니스 증진 건강프로그램 참여는 신체구성과 일상생활의 많은 활동과 낙상에 위험요소인 하체근력을 강화시키고, 일상생활 물건의 운반과 이동에 용이한 상체근력도 증가시키는 것으로 나타났다. 또한 식생활습관을 개선시키고, 스트레스해소습관과 운동습관 변화에도 긍정적 역할을 하여 고령자의 건강에 도움을 주고 삶의 질을 향상시킬 것으로 생각된다.

에너지 섭취 조사를 위한 24시간 회상법의 정확도 평가: 여자노인을 대상으로 이중표식수법을 이용하여 (Accuracy of the 24-hour diet recall method to determine energy intake in elderly women compared with the doubly labeled water method)

  • 본 연구는 만 65세 이상의 여자 노인 23명을 대상으로 이중표식수법을 이용하여 측정한 에너지소비량을 기준으로 에너지섭취량을 조사하는 24시간 회상법의 정확도를 평가하였으며, 그 결과는 다음과 같다. 연구 대상자 평균 연령은 70.3 ±3.3세, 신장 및 체중은 각각 153.0 ± 5.9 cm와 56.0 ± 8.0 kg, 체질량지수 (BMI)는 23.9 ± 2.8 kg/㎡이었다. 24시간 회상법으로 산출된 총 에너지섭취량 (TEI)과 이중표식수법으로 측정된 총에너지소비량 (TEEDLW) 간의 피어슨 상관계수는 r = 0.482로 두 값 간에 의미 있는 양의 상관성 (p < 0.05)을 보여주었다. 그러나 24시간 회상법으로 조사된 3일간의 평균 에너지섭취량 (1,489.6 ± 211.1 kcal/day)은 이중표식수법으로 측정된 총에너지소비량 (2,023.5 ± 234.9 kcal/day)보다 -533.9 ± 228.0 kcal/day만큼 과소보고 되었으며, 두 값 간에 유의한 차이가 있었다 (p < 0.001). 총에너지섭취량과 총에너지소비량간의 과소보고율은 -25.9% ± 10.5%로 나타났다. Bland-Altman 방법으로 총에너지섭취량과 총에너지소비량간의 일치도 평가 결과로 두 값 일치 한계의 범위가 -980.8 kcal/day에서 -86.9 kcal/day로 음의 값으로 치우쳐 나타났다. 본 연구 결과에 따르면 에너지섭취량을 조사하는 24시간 회상법을 여자노인에게 적용시, 과소보고율이 -25.9%로 높았고, 에너지섭취량을 정확하게 예측한 비율 (오차범위 ± 10% 이내)도 8.7%로 낮았다. 따라서 여자노인을 대상으로 24시간 회상법으로 에너지섭취량을 조사하고자 하는 경우, 남자와는 다른 별도의 접근법이 강구되어야 할 것이다. 성별이 24시간 회상법의 정확도에 영향을 미치는 주요한 요인으로 보고된 만큼 향후 연구 대상자 수를 증가하여 더 다양한 연령에서 성별에 따른 차이를 평가하는 지속적인 연구가 필요하다고 사료된다.

재가노인들을 위한 무료급식소의 식단 평가 (Evaluation of the Menus of Free Meal Service Centers for Home-bound Elderly)

  • The purpose of this study was to evaluate the diet quality of the menus delivered by 17 free meal service centers for the low-income home-bound elderly in Chung- cheong buk-Do. Statistical data analysis was compleleted using the SPSS package program for descriptive analysis, T-test, and ANOVA. The meals offered by free meal service centers were not met the 1/3 recommended dietary allowances in calcium and vitamin $B_2$. There were significant differences between dependent variables(nutrient content, nutrient density, nutrient deficiency, NAR, MAR, food group intake patterns) and independent variables (operation type, operation status, operation period, nutritionist, food cost).

한국형 DASH 교육 프로그램이 여성노인의 산화스트레스, 항산화능력과 호모시스테인 농도에 미치는 효과 (The Effects of Korean DASH Diet Education Program on Oxidative Stress, Antioxidant Capacity, and Serum Homocysteine Level among Elderly Korean Women)

  • Purpose: The purpose of this study was to investigate the effects of the Korean Dietary Approaches to Stop Hypertension (DASH) education program on oxidative stress, antioxidant capacity, and serum homocysteine (Hcy) level in elderly Korean women. Methods: This was a quasi-experimental study employing non-equivalent control group pretest-posttest design. The DASH group (n= 27) underwent the DASH education program for 12 weeks, whereas the control group (n= 21) was provided one education session. The survey was conducted before and after the intervention period. Results: In DASH group, monounsaturated fatty acid (MUFA), eicosapentaenoic acid (EPA), and docosahexaenoic acid (DHA) intakes significantly increased after the intervention as compared to prior to the intervention. No significant differences were found in the control group. After the intervention, thiobarbituric acid reactive substance (TBARS) and Hcy levels significantly decreased in the DASH group but not in the control group. Conclusion: The DASH education program was effective in reducing oxidative stress and Hcy levels in elderly Korean women.

빈곤층 노인의 건강상태, 건강인식 및 건강증진행위 실천 간의 관계 -보건소 방문간호 대상자 중심으로 - (Health Status, Health Perception, and Health Promotion Behaviors of Low-income Community Dwelling Elderly)

  • Purpose: The purpose of the study was to investigate the health status(present illness, ADL and IADL), health perception, and health promotion behaviors of low-income elderly who are receiving the visiting nurse service in the community. Method: The sample of the study was 735 elderly over 65years old with basic livelihood security, who were conveniently selected from 245 public health centers nation-wide. Data collection was done using a structured questionnaire through interviews by visiting nurses. Result: The average number of present illnesses in the study subjects was 4.18. The average scores of ADL and IADL were 15.903.39and 9.772.97 respectively, which indicates a relatively independent everyday life. However, $64.2\%$ of the subjects perceived their health status as 'not healthy'. In terms of health promotion behaviors, $77.8\%$ of the subjects had ceased smoking, $83.9\%$ stopped drinking, $56.4\%$ had a regular diet, $45.8\%$ received regular physical check-ups during the past two years, and $66\%$ received flu shots. Approximately $50\%$ of the subjects were practicing 3-4 health promotion behaviors. Significant factors associated with health promotion behaviors were ADL, IADL and self-efficacy. Conclusion: Health promotion programs which focus on regular diet, exercise, and regular physical check-ups should be developed to improve independence of everyday life and quality of life among low-income elderly.
