Through an examination of future societal development, it was found that the demographic structure is gradually aging, thus placing higher demands on elderly welfare centers. This study conducted surveys on three elderly day care centers in the entire region, aiming to enhance the pivotal role of elderly day care centers in the welfare facilities for the elderly. The current usage status, interior architectu/ral evaluations, and questionnaire surveys of these centers were analysed. The results of investigation into the architectural environment of the elderly day care centers showed that in terms of physical environment, smaller-scale centers did not lag behind larger ones in internal facilities, but larger-scale centers excelled in spatial utilization. Furthermore, the results of questionnaire survey results revealed correlations between family relationships, health status, level of assistance required, and satisfaction with facility usage, while no correlation was found between usage time, health status, and satisfaction with facility usage. This study conducted basic investigations such as architectural and physical surveys, and questionnaire surveys on elderly day care centers. Based on the survey results, future design practices should continuously explore and innovate to meet the needs of an aging society, further improve the community care system, provide more comprehensive and meticulous services for the elderly, and lay the foundation for enhancing the quality of life for the elderly and providing comfortable services.
Purpose: The purpose of this study is to identify levels of quality of health-related life (EQ-5D) and its related factors among vulnerable elders receiving home visiting health care services in some rural areas. Methods: The subjects of this study were vulnerable elders aged 65 or higher receiving visiting health care benefits by a public health center in a county. Results: Levels of health-related EQ-5D were high when the subjects were males, their ADL and IADL were high. The EQ-5D of the vulnerable elders and social supports had a positive correlation, while their IADL and depression had a negative correlation with the EQ-5D. We can see that the EQ-5D related factors are variables that have significance influence on gender, subjective health status, BMI, IADL, depression levels, and social support. Conclusion: To improve the EQ-5D of the vulnerable elderly, it needs maintenance of independent IADL levels, social supporting systems using small meeting of the elderly in community. It is necessary to maintain independent IADL levels, enhance social supporting systems including small group gatherings for elders living alone by means of places like community halls, and develop specific intervention programs for the prevention and management of depressive elders.
Purpose: This study was conducted to identify health problems and support received from the health and welfare service using MDS-HC(Minimun Data Set for Home Care) in the aged living at home. Method: Eighty-one elderly persons were selected from those listed in community-welfare service centers in Seoul and Kyunggi Province between December 2002 and January 2003. Result: Eleven health problems per elderly person on average were identified, and the frequent care needs were in order: preventive health measure, health promotion, visual function, depression & anxiety, communication disorders, social function, pain, environmental assessment, oral health, cognition and falls. The number of health problems by the level of ADL was ‘ADL 1(Independence)’ 9.87, ‘ADL 2(Partial independence)’ 12.78, ‘ADL 3(Dependence)’ 13.73. Utilization of formal health & welfare services among the elderly was ‘meals on wheels’ 40.7%, ‘home helper’ 38.2%, ‘visiting of social welfare worker’ 21.0%, ‘physical therapy’ 19.6%, ‘day care center’ 12.3%, ‘volunteer's service’ 9.9%, ‘home visiting care’ 3.7%, ‘occupational therapy’ 3.7%, and ‘speech therapy’ 2.5%. Conclusion: The results suggest that using the MDS-HC 2.0 is applicable to help decide criteria for both health and welfare service supplied to the elderly.
Purpose: This study tries to explore and analyse Service Attached Elderly Housing of Japan in order to prepare new elderly housing type effectively for aged society of Korea. Service Attached Elderly Housing was introduced in 2011 and it includes existing elderly housing of Japan such as Designated Elderly Housing, Elderly Friendly Housing and Superior Elderly Housing. Methods: Historical transition of Japanese elderly housing and background of the introduction of Service Attached Elderly Housing have been reviewed first. Thereafter research team visited to 3 elderly housings of Japan providing elderly services such as elderly care, meal, cleaning, washing, safety check, entertainment, etc. Based on the literature survey and field trip, the characteristics of Service Attached Elderly Housing have been drew out. Results: As the characteristics of Service Attached Elderly Housing of Japan, mixed use facility, community based housing, generation mix, barrier free design, and provision of daily life service for the elderly have been identified. Implications: Not only physical consideration including Universal Design but also elderly services such as elderly care, meal, housework, safety check, entertainment and so on are necessary for the supply of elderly housing.
본 논문은 한국노년학회지의 노인보건정책연구와 관련된 게재논문의 내용을 분석하여 그 유형과 주요 정책제언의 내용을 정리하고, 이를 바탕으로 향후 우리나라의 노인보건정책에서 추구하여야 할 정책과제를 도출, 제시하는데 있다. 지난 한국노년학회지가 발간된 이후부터 2007년도까지 노인보건과 관련된 게재논문수는 총 61편이었고, 이 중에서 정신보건분야, 구강보건분야를 제외한 질병, 의료비 등 일반보건의료와 관련한 연구가 대부분을 차지하고 있다. 연구방법론을 보면, 특정지역에 거주하는 노인을 대상으로 하되 표본조사를 통해서 구축된 자료와 노인복지기관이나 병원 등을 이용한 노인을 분석대상으로 한 논문이 대부분을 차지하고 있다. 이를 바탕으로 제시할 수 있는 향후 정책과제는 다음과 같다. 첫째, 노인의 건강수준에 적합한 특화된 건강증진프로그램의 개발 및 활성화방안을 강구하여야 할 것이다. 둘째, 지역사회중심의 노인 일차보건의료체계의 구축이 강구되어야 할 것이다. 셋째, 급성기 치료 이후의 회복기 치료를 저렴한 비용으로 제공할 수 있는 체계를 구축하여야 할 것이다. 마지막으로 장기요양서비스를 필요로 하는 노인환자에 대한 의료적 관리체계를 구축하여야 할 것이다.
Like all other European countries, as a result of gradual industrialization, Germany is just before the start of new social system; the so-called 'aged society'. Over half of the century, Germany focused their welfare system on facilities. However, with the start of the new 'aged society', they introduced a new concept which consists mainly on home care and community care. These new concepts resulted from the general idea that even elderly have the right to live an independent quality life. Therefore, to satisfy the elderly with such life it has been seen as necessary not only to offer them efficient protection but to introduce an economical welfare system and facility scheme. The regional integration plan for welfare is one of the open schemes in which the lifestyle of the elderly are seen as the whole. Participated by several members and specialists of all social standings, necessary facilities are planned. By looking into a project example carried out in Germany, this study is aimed to increase the quality of the elderly welfare system and facility construction plan.
Purpose: The purpose of this study was to investigate the effects of an essential oil mouthrinse with gingival massage on oral health in community indwelling elderly. Method: The subjects were composed of 61 healthy elderly at a Senior Welfare Center in J city. Thirty subjects in the experimental group were given toothbrushing education ongingival massage toothbrushing with an essential oil mouthrinse for 2 weeks(4 times per day, 3 minutes per session). The 31 subjects in the control group were given toothbrushing education ongingival massage toothbrushing. The effects of the treatment were measured by salivary pH, salivary IgA level, halitosis, oral subjective symptoms, and oral self care behavior scores before, right after and 2 weeks after the experiment. Results: Salivary pH was significantly increased(p=0.018) in the experimental group. Salivary IgA levels were not significantly different between the experimental and control groups; however, IgA levels of the experimental group were significantly increased(p=0.006) after time had passed. Halitosis was significantly decreased(p=0.002) in the experimental group. Oral subjective symptom scores were significantly decreased(p=0.000) and oral self care behavior scores were significantly increased(p=0.000) in the experimental group. Conclusion: Regular gingival massage toothbrushing with an essential oil mouthrinse could be an effective oral health nursing intervention for the elderly.
Purpose: The purpose of this study was to find out lower urinary tract symptoms, daily life inconvenience, and sleep quality in elderly community woman. Methods: Participants were 140 elderly women. Data collection was conducted from September 1 to October 30, 2017. Data were analyzed using descriptive statistics and t-test with the SPSS program. Results: The rate of urge incontinence was 25.0%, stress incontinence 36.4%, unexplained incontinence 15.7%, and nocturnal incontinence 9.3%. Those participants with urge incontinence presented significantly higher daily life inconvenience scores; urge incontinence, stress incontinence, and unexplained incontinence were significantly associated with lower sleep quality scores. Furthermore, frequent urination, nocturia, and incomplete emptying were significantly higher daily life inconvenience scores; and frequent urination, nocturia, hesitancy, straining, intermittent stream, burning, incomplete emptying, bladder pain, and abnormal strength of stream were significantly related to lower sleep quality scores. Conclusion: It is necessary to develop intervention strategies considering incontinence and urination symptoms to reduce daily life inconvenience caused by lower urinary tract symptoms and improve sleep quality in elderly women.
The purpose of this research is to explore the best examples from Japan to derive spatial and institutional characteristics. This study used the field survey method for 6 elderly welfare facilities in Japan. The characteristics of local friendly facility are as follows; First of all, since elderly care facilities in South Korea are separated from local community and facility plans are large-scaled and unified which are disconnected from local area, this research set aging in community, multi-function, diversity, and intergeneration into the concept that is necessary for local friendly facilities. Secondly, residential environment plan's basic directions are 1) plan that minimizes facilities-like atmosphere, 2) spatial plan that focuses on the elderly dignity and privacy, 3) comfortable and enjoyable communal living space plan, 4) local friendly and communicating plan, 5)plan that minimizes staff's care giving burden. Thirdly, the rooms necessary for local friendly facility model are composed of bedroom, dining room, kitchen, living room, garden, toilet, laundry room, bathroom, corridor, and office based on the legal installation standards.
Purpose: This study was performed to investigate the influencing factors of cognitive function and depression in elderly. Method: From 3 provinces, 282 elders who resided in community and facilities completed structured questionnaires, including cognitive functions, depression, self-esteem, ADL, and IADL. Data were analyzed by applying ${\chi}^2$-test, ANOVA, partial correlation coefficient, and stepwise multiple regression analysis with SAS 8.12. Results: Community dwelling aged people showed higher scores in MMSE-K, self-esteem, ADL and IADL, and showed lower scores in depression than facility elders. MMSE-K was positively correlated in self-esteem, ADL, and personal cognition of health, except depression. Major factors that affect cognitive function of elderly were residual type, age, and IADL. In addition, major factors that affect depression of elderly were self-esteem, personal cognition of health, and marital status. Conclusions: Based on the results above, it is necessary to identify the status of cognition and depression in the elderly, and to develop nursing intervention programs, which improve cognitive function and reduce depression for aged, especially for the facility admitted aged.
