Physical and social Disability among the Aging (노인의 신체적ㆍ사회적 능력장애에 관한 조사연구 -서울시내 거주노인을 대상으로-)

  • This study was designed to investigate the nature and magnitude of disability among non-institutionalized elderly in urban area(Seoul city). From November 1987, through December 1987, 1,054 individuals aged over 65 years from the sampled of clustered random sampling method were interviewed in person. The physical disability findings support the well known relationship between physical disability and age. The magnitude of disability, however, is not as great as conventional wisdom might suggest. And this paper reports results on the prevalence of social disability : housekeeping transportation, food prepatation, grocery shopping, social interaction, intellectual activity, and social role. The magnitude of social disability increase with advancing age. Women, in general, report a higher degree of social disability than man. These findings are compared with other investigations of physical and social disability among the elderly.

Changes in Physiological Characteristics of Trapezius, Multifidus and Hamstring Muscles According to Tilt Angle of Exercise Bed in Elderly (운동침대 틸트 각도 변화에 따른 노인의 승모근, 다열근 및 슬괵근 근육의 생리학적 특성 변화 연속 증례 연구)

  • Purpose: This study investigated the effects of changing the tilt angle of the exercise bed on physiological characteristics of trapezius, multifidus, and hamstring muscle function. Methods: Twenty elderly who complains of back and lumbar pain or discomfort participated in this study (average age: $64.70{\pm}4.55$ years). The physiological characteristics, including muscle tone (F), dynamic stiffness (S), and elasticity (D) of trapezius, multifidus, and hamstring muscles were measured by a Myoton device at three tilt angles (0, 15, and 30 degrees) during exercise. Results: The muscle tone, dynamic stiffness, and elasticity of the right trapezius and left hamstring muscle showed significant differences (p<0.05) according to exercise bed tilt angle, while the other muscles showed no significant differences. Conclusions: Our results demonstrate that physiological characteristics of the upper and lower extremity muscles are affected ipsilaterally, according to the tilt angles of the exercise bed.

The Effects of Discharge Planning for the Elderly with Pulmonary Disease in the Emergency Room (퇴원계획중재가 응급실 내원 호흡기 질환 노인의 퇴원준비도, 간호서비스 만족도, 재입원율에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of discharge planning on patient satisfaction, the readmission rate and preparedness for discharge in the elderly admitted to the emergency room (ER) for pulmonary disease. Methods: A quasi-experimental intervention study design was used. Older adults with pulmonary health problems in the ER in one general hospital were randomly allocated to either an experimental (n=21, 74.2 years) or control group (n=19, 70.7 years). The experimental group participated in a discharge planning program by a geriatric nurse practitioner. Data were collected from medical records, physical measurements and structured questionnaires including information on demographics, patient satisfaction, readmission, and preparedness for discharge. Results: Participants in the experimental group had significantly better outcomes with regard to patient satisfaction with nursing services (p=.003) and preparedness for discharge (p=.034). However, there was no significant effect on the readmission rate (p=.392) Conclusion: The results suggested that a discharge planning program could bolster nursing service satisfaction and preparedness for discharge in older patients admitted to the ER for pulmonary health problems. To clarify the effects of discharge planning on older patients admitted to the ER, a larger sample population, better instruments for various measures, a new manual on discharge planning and frequent follow-up will be necessary.

Gender Analysis in Elderly Speech Signal Processing (노인음성신호처리에서의 젠더 분석)

  • Changes in vocal cords due to aging can change the frequency of speech, and the speech signals of the elderly can be automatically distinguished from normal speech signals through various analyzes. The purpose of this study is to provide a tool that can be easily accessed by the elderly and disabled people who can be excluded from the rapidly changing technological society and to improve the voice recognition performance. In the study, the gender of the subjects was reported as sex analysis, and the number of female and male voice samples was used equally. In addition, the gender analysis was applied to set the voices of the elderly without using voices of all ages. Finally, we applied a review methodology of standards and reference models to reduce gender difference. 10 Korean women and 10 men aged 70 to 80 years old are used in this study. Comparing the F0 value extracted directly with the waveform and the F0 extracted with TF32 and the Wavesufer speech analysis program, Wavesufer analyzed the F0 of the elderly voice better than TF32. However, there is a need for a voice analysis program for elderly people. In conclusions, analyzing the voice of the elderly will improve speech recognition and synthesis capabilities of existing smart medical systems.

Occupational Therapy in Long-Term Care Insurance For the Elderly Using Text Mining (텍스트 마이닝을 활용한 노인장기요양보험에서의 작업치료: 2007-2018년)

  • Objective : The purpose of this study is to quantitatively analyze the role of occupational therapy in long - term care insurance for the elderly using text mining, one of the big data analysis techniques. Method : For the analysis of newspaper articles, "Long - Term Care Insurance for the Elderly + Occupational Therapy for the Elderly" was collected after the period from 2007 to 208. Naver, which has a high share of the domestic search engine, utilized the database of Naver News by utilizing Textom, a web crawling tool. After collecting the article title and original text of 510 news data from the collection of the elderly long term care insurance + occupational therapy search, we analyzed the article frequency and key words by year. Result : In terms of the frequency of articles published by year, the number of articles published in 2015 and 2017 was the highest with 70 articles (13.7%), and the top 10 terms of the key word analysis showed the highest frequency of 'dementia' (344) In terms of key words, dementia, treatment, hospital, health, service, rehabilitation, facilities, institution, grade, elderly, professional, salary, industrial complex and people are related. Conclusion : In this study, it is meaningful that the textual mining technique was used to more objectively confirm the social needs and the role of the occupational therapist for the dementia and rehabilitation in the related key keywords based on the media reporting trend of the elderly long - term care insurance for 11 years. Based on the results of this study, future research should expand research field and period and supplement the research methodology through various analysis methods according to the year.

The Relationships among Coffee Consumption, Blood Pressure, and Serum Lipids in Korean Adult Men and Women (성인남녀의 커피 섭취 상태 및 혈압, 혈중 지질과의 관련성)

  • The purpose of this study was to estimate coffee consumption and it's relationship to blood pressure and blood lipid profiles in Korean adult men and women. Antheropometrics, dietary intake by 24-hour recall, and serum lipid profiles were measured in 354 subjects. The average subject ages for the age ranges of $20{\sim}49$, $50{\sim}64$, and over 65 years, were 39.7, 57.5, and 70.6 years for the men, and 40.7, 56.9, and 70.9 years for the women, respectively. Mean height and weight were significantly higher in the men and decreased with increasing of age. Thirty percent and 28% of the men and women, respectively, consumed coffee. The proportions of coffee consuming subjects in the age rangers of $20{\sim}49$ years(34.9%) and $50{\sim}64$ years(35.6%) were significantly higher than in the group over 65 years(11.5%). The mean daily intakes of food and energy were showed the same results according to sex and age. Mean daily coffee intake was not significantly different between the men and women, but it was significantly lower in the over 65 age group. Systolic blood pressure was significantly higher in the men and increasing age. The atherogenic index was significantly higher in the $20{\sim}49$ year-old men, and in the women over 65 years. There were no significant correlations among coffee consumption, blood pressure, and the serum lipid profile. In conclusion, the daily coffee consumption by adult men and women, with the exception of the elderly, was $50{\sim}90m{\ell}$, and it was not significantly correlated to blood pressure or blood lipids.

Coronary Artery Bypass Graft Surgery in Patients 70 Years of Age and Older (70세 이상 고령 환자에서의 관상동맥우회술)

  • Background: There has been an increase in the number of elderly patients considered for coronary artery bypass grafting (CABG). Recently, there were many satisfactory reports of coronary artery bypass grafting (CABG) in old age due to the development in operative technique and postoperative management. We evaluated operative and follow-up results of patients 70 years of age and older compared to 60 years old. Material and Method: We retrospectively studied the cases of 74 consecutive patients 70 years or older (group A) who underwent a elective CABG from January 2000 to December 2003 and compared that of relatively young age group (group B, 60-69 years old). We compared preoperative characteristics, operation technique, postoperative results that effect outcome, also we investigated late mortality and cardiac events at follow-up periods. Result: Preoperative demographic and clinical characteristics of two groups were not different, except preoperative renal dysfunction(serum creatinine: $\geq$1.4 mg/dl) (group A 17, 23$\%$ vs group B 14, 9$\%$) (p=0.024). There was no difference of the mean number of distal anastomosis and the left ventricular ejection fraction in group A decreased significantly from 53.7$\pm$13$\%$ preoperatively to 49.9$\pm$ 12$\%$ postoperatively (p=0.02), but not changed in group B. There was no difference at operative mortality rate and postoperative major morbidity rate, but wound problem of saphenous vein harvest site was significantly higher in group A than group B (6.8$\%$ vs 0.7$\%$, p=0.02). The mean follow up duration was 24.3$\pm$13 months and the cumulative survival were 95.4$\%$ at 2 year and 79.9$\%$ at 4 year in group A and 95.4$\%$ at 2 year and 90.1$\%$ at 4 year in group B (p=ns). Conclusion: We conclude that age is not a factor of determination when we decide about operation because coronary artery bypass grafting in elderly more than 70 years old can be performed with a low mortality rate and acceptable morbidity rate.

Long-Term Estimation of 119 Ambulance Services by Demographic Changes in Korea (인구 변동에 따른 119 구급 서비스의 장기 추계)

  • Based on the Statistical Yearbook of 119 Emergency Medical Services in 2017 (National Fire Agency, 2018) and Population Projection by Province: 2015~2045 (Statistics Korea, 2017), this study analyzed the effects of population changes, such as low fertility and aging on the use of ambulance services in the future. The population of Korea is expected to decline after peaking in 2031, but the number of elderly people aged over 70 is expected to continue to increase. The rates of emergency ambulance transport (REAT, the number of patients transferred per 1,000 inhabitants) are not changed significantly, compared to the recent trends, but the ambulance services for elderly over 70 years of age will increase exponentially. Therefore, the population changes due to low fertility and aging is accompanied by a quantitative and qualitative change in ambulance services, and from the long term perspectives, it is necessary to consider not only the effective response of ambulance services due to aging, but also the adjustment of existing jurisdictions due to population changes and the relocation of manpower and resources.

Analysis on the Effects and Influences of Mindfulness Based Breathing and Body Scan for Elderly Women's Attentive Concentration (마음챙김 호흡과 신체이완이 여성노인의 주의집중력에 미치는 영향 분석)

  • This study is analized with how Mindfulness Based Breathing and Body Scan influences to elderly women's attentive concentration measured by 2-channel system EEG(Electroencephalogram) which method is objectively and determinately. The subjects in this study are 60 aged over 65years of elderly women in K city, who were divided into two groups; such as 30 of the experimental group and 30 of the control group checked by EEG before and after the treatment. The study started from March to July in 2013. The treatment was conducted by ones a week, 60minutes a time for 16 weeks. The result in experimental group is indicated as followed. The Attention Quotient Lt(69.61/70.85, p<.024) showed higher score after treatment, and Attention Quotient Rate Lt(4.22/3.75, p<.037), Attention Quotient Rate Rt(4.29/3.70, p<.019) did show statistically lower score after treatment. Attention Quotient is shown attentive concentration and awareness in the brain. Therefore, Mindfulness Based Breathing and Body Scan is proved the value and efficiency for activating elderly women's attentive concentration by a way of practical using in neuroscience.

Perioperative Risk Factors Related to Lumbar Spine Fusion Surgery in Korean Geriatric Patients

  • Objective : Life expectancy for humans has increased dramatically and with this there has been a considerable increase in the number of patients suffering from lumbar spine disease. Symptomatic lumbar spinal disease should be treated, even in the elderly, and surgical procedures such as fusion surgery are needed for moderate to severe lumbar spinal disease. However, various perioperative complications are associated with fusion surgery. The aim of this study was to examine perioperative complications and assess risk factors associated with lumbar spinal fusion, focusing on geriatric patients at least 70 years of age in the Republic of Korea. Methods : We retrospectively investigated 489 patients with various lumbar spinal diseases who underwent lumbar spinal fusion surgery between 2003 and 2007 at our institution. Three fusion procedures and the number of fused segments were analyzed in this study. Chronic diseases were also evaluated. Risk factors for complications and their association with age were analyzed. Results : In this study, 74 patients experienced complications (15%). The rate of perioperative complications was significantly higher in patients 70 years of age or older than in other age groups (univariate analysis, p=0.001; multivariate analysis, p=0.004). However, perioperative complications were not significantly associated with the other factors tested (sex, comorbidities, operation procedures, fusion segments involved). Conclusion : Increasing age was an important risk factor for perioperative complications in patients undergoing lumbar spinal fusion surgery whereas other factors were not significant. We recommend good clinical judgment and careful selection of geriatric patients undergoing lumbar spinal fusion surgery.
