캠벨얼리(Vitis labruscana B.) 잎 에탄올 추출물이 신경세포에서 아밀로이드 전구 단백질의 발현과 아세틸콜린에스테라제 활성에 미치는 영향 (The Effect of Vitis labruscana B. Leaves Ethanol Extract on the Expression of Amyloid Precursor Protein in Neuroblastoma Cells and on the Acetylcholinesterase Activity)

  • Alzheimer's disease (AD) is the most common form of dementia, and the accumulation of β-amyloid (Aβ) in the brain triggers AD, followed by hyperphosphorylation of tau protein, neurofibrillary tangles, and synapses loss, neuronal cell death, and cognitive decline occur in a chain. In APPswe neuronal cell line, 50 ㎍/ml of Campbell early (Vitis labruscana B.) leaves 50% ethanol extract (VLL) treatment inhibited the secretion of Aβ1-42 by about 63% and the secretion of Aβ1-40 by about 50%. VLL did not target the enzymatic activity of the amyloidogenic pathway and decreased the protein expression of APP. As a result of RT-qPCR (Reverse transcription-quantitative real-time PCR) of the APPswe cell line treated with VLL, it is thought that the protein expression of APP was reduced by inhibiting the transcription process of the APP gene. In addition, VLL inhibited acetylcholinesterase (AChE) enzyme activity in vitro by 27.6% and 54.7%, respectively, at 50 and 100 ㎍/ml concentrations. We found that VLL inhibited the production of Aβ, a dementia-inducing substance, by suppressing the transcription of the APP gene, and that VLL inhibited AChE activity. We suggest that VLL has the potential as a natural drug material that modulates the alleviation of dementia symptoms.

Harnessing the Power of Voice: A Deep Neural Network Model for Alzheimer's Disease Detection

  • Background and Purpose: Voice, reflecting cerebral functions, holds potential for analyzing and understanding brain function, especially in the context of cognitive impairment (CI) and Alzheimer's disease (AD). This study used voice data to distinguish between normal cognition and CI or Alzheimer's disease dementia (ADD). Methods: This study enrolled 3 groups of subjects: 1) 52 subjects with subjective cognitive decline; 2) 110 subjects with mild CI; and 3) 59 subjects with ADD. Voice features were extracted using Mel-frequency cepstral coefficients and Chroma. Results: A deep neural network (DNN) model showed promising performance, with an accuracy of roughly 81% in 10 trials in predicting ADD, which increased to an average value of about 82.0%±1.6% when evaluated against unseen test dataset. Conclusions: Although results did not demonstrate the level of accuracy necessary for a definitive clinical tool, they provided a compelling proof-of-concept for the potential use of voice data in cognitive status assessment. DNN algorithms using voice offer a promising approach to early detection of AD. They could improve the accuracy and accessibility of diagnosis, ultimately leading to better outcomes for patients.

Mitochondrial dysfunction and Alzheimer's disease: prospects for therapeutic intervention

  • Alzheimer's disease (AD) is a multifactorial neurodegenerative disease and has become a major socioeconomic issue in many developed countries. Currently available therapeutic agents for AD provide only symptomatic treatments, mainly because the complete mechanism of the AD pathogenesis is still unclear. Although several different hypotheses have been proposed, mitochondrial dysfunction has gathered interest because of its profound effect on brain bioenergetics and neuronal survival in the pathophysiology of AD. Various therapeutic agents targeting the mitochondrial pathways associated with AD have been developed over the past decade. Although most of these agents are still early in the clinical development process, they are used to restore mitochondrial function, which provides an alternative therapeutic strategy that is likely to slow the progression of the disease. In this mini review, we will survey the AD-related mitochondrial pathways and their small-molecule modulators that have therapeutic potential. We will focus on recently reported examples, and also overview the current challenges and future perspectives of ongoing research.

  • 인구 고령화가 급속히 진행되면서 치매인구가 빠른 속도로 증가하고 있다. 치매는 완벽한 치료법이 없기 때문에 경도인지 장애 평가를 통해 조기 발견 및 예방 훈련하는 것이 중요하다. 하지만 경도인지 장애 평가는 수기형태로 평가하기 때문에 도출된 데이터를 활용하기에 한계가 있다. 따라서 기존 방식의 결과 데이터를 통합적으로 관리하고 사용하는 분석 및 저장 시스템이 필수적이다. 본 논문은 경도인지 장애 평가 데이터를 분석 및 저장하기 위한 시스템을 설계하고자 한다.

A Review on the Correlation between the Pathology of Alzheimer's Disease and microRNA

  • The purpose of this study was to explain the pathology of Alzheimer's disease (AD) and to investigate the correlation between AD and microRNA. AD is the most common type of dementia, accounting for about 80% of all types of dementia, causing dysfunction in various daily activities such as memory loss, cognitive impairment, and behavioral impairment. The typical pathology of AD is explained by the accumulation of beta-amyloid peptide plaques and neurofibrillary tangles containing hyperphosphorylated tau protein. On the other hand, microRNA is small non-coding RNA 22~23 nucleotides in length that binds to the 3' untranslated region of messenger RNA to inhibit gene expression. Many reports explain that microRNAs found in circulating biofluids are abundant in the central nervous system, are involved in the pathogenic mechanism of AD, and act as important factors for early diagnosis and therapeutic agents of AD. Therefore, this paper aims to clarify the correlation between AD and microRNA. In this review, the basic mechanism of miRNAs is described, and the regulation of miRNAs in the pathological processes of AD are highlighted. Furthermore, we suggest that miRNA-based system in development of therapeutic and diagnostic agents of AD can be a promising tool.

Patterns of Depressive Symptoms on Cognitive Function Decline: An Investigation in Middle-Aged Koreans Based on the Korean Longitudinal Study of Aging (KLoSA)

  • Background: Numerous studies have consistently demonstrated that depression can be associated with cognitive function decline, primarily focusing on older adults due to the neurodegenerative characteristics of dementia. With persistent depression frequently reported in patients with early-onset or young-onset dementia, this study aimed to assess the impact of depression, specifically the changes in depressive symptoms over time, on the risk of cognitive function decline in middle-aged adults in Korea. Methods: This retrospective study utilized data from the first four waves (2006-2012) of the Korean Longitudinal Study of Aging (KLoSA), focusing on middle-aged adults with normal cognitive function at baseline. Changes in depressive symptoms were categorized into four groups based on the CES-D score, and their association with cognitive function decline was evaluated using a multivariate logistic regression model. Results: Of the initial 10,254 participants, 3,400 were included in the analysis. Depressive status, particularly newly onset (adjusted odds ratio [aOR] 1.96; 95% confidence interval [CI] 1.32-2.93) and persistent depression groups (aOR 5.59; 95% CI 2.90-10.78), were significantly associated with cognitive function decline. In contrast, recovery from depressive symptoms was not significantly associated with cognitive function decline (p=0.809). Conclusions: Our study showed a significant association between changes in depressive symptoms and cognitive function decline in middle-aged Korean adults. This suggests that management of depressive symptoms could be crucial for the prevention of cognitive function decline in this population.

면 단위에 거주하는 영유아 자녀를 둔 여성의 분만 후 시기별 모유수유 실천 및 모성적응 (Breast Feeding Practice and Maternal Adaptation of Infant and Early Childhood Mothers)

  • Purpose: This study was conducted to assess the breast feeding rate and maternal adaptation of mothers with infants and children in early childhood in a community. Methods: This descriptive study was conducted from November to December 2015, and included a total of 283 mothers of infants and children in early childhood. The data were analyzed using descriptive analysis, the ${\chi}^2-test$, and one way ANOVA. Results: The rates of breast feeding after birth were 76.3% (1 month postpartum), 69.3% (3 months postpartum), 53.4% (6 months postpartum), 32.2% (9 months postpartum), and 22.6% (12 months postpartum). The level of maternal adaptation of subjects was $3.78{\pm}0.54$. Subjects who were breastfed until 9 months postpartum had a higher level of maternal adaptation than those who stopped breast feeding at 1 month postpartum (F=3.926, p<.002). The breast feeding rate of subjects who were educated about breast feeding after childbirth was significantly higher than that of those who did not receive breast feeding education after delivery. Conclusion: To increase the breast feeding rate and maternal adaptation, community health nurses should develop and provide breast feeding programs to mothers soon after childbirth.

노래를 통한 초기 알츠하이머 치매환자의 자서전적 기억 (Song-Induced Autobiographical Memory of Patients With Early Alzheimer's Dementia)

  • 본 연구의 목적은 초기 알츠하이머 치매환자를 대상으로 본인이 선곡한 노래 조건과 치료사가 선곡한 노래 조건에서 자서전적 기억 회상의 내용, 회상하는데 걸린 시간과 기분변화를 측정하여 자서전적 기억회상에 효과적인 노래조건을 알아보는 것이다. 실험 동안 각 피험자는 자신이 선곡한 친숙한 노래(M-AD)와 치료사가 선곡한 노래(M-MT) 조건에서 노래를 듣다가 자신이 직접 경험한 기억이 떠오르면 손을 들게 하였고, 노래가 끝나면 인출된 기억을 이야기하도록 하였다. 피험자가 이야기한 내용은 녹음 전사하였고, 기억의 주체성과 내용, 회상 시간과 기분 변화 등과 관련하여 분석하였다. 본 연구 결과 M-AD 조건이 M-MT 조건에서 제공된 노래에 대한 친숙도와 선호도가 높았으며, 회상한 기억과 노래의 밀접함의 정도가 높았다. 또한 구체적으로 기억을 회상할 수 있었으며, 회상하는 데 걸린 시간도 짧았다. 또한 실험 전후의 기분변화 폭이 컸으며, 회상한 기억과 기분의 관련성에도 영향을 미쳤다. 본 연구는 초기 알츠하이머 환자 본인이 선곡한 노래가 자서전적 기억을 회상하는 단서가 되어 구체적이고, 긍정적인 기억을 촉진하며 회상시간이 단축됨을 보여주어 임상에서 초기 알츠하이머 환자의 자서전적 기억을 회상할 때, 본인이 선곡한 친숙한 노래가 효과적으로 사용될 수 있음을 보여줄 수 있음을 시사한다.

초기 단계의 피질하 혈관성 치매와 알쯔하이머병에서 Tc-99m HMPAO SPECT 영상 소견 차이 (Differences of Tc-99m HMPAO SPECT Imaging in the Early Stage of Subcortical Vascular Dementia Compared with Alzheimer's Disease)

  • 목적: 본 연구는 초기의 피질하 혈관성치매와 알쯔하이머병 환자에서 Tc-99m HMPAO 단일광자방출단층촬영술을 이용하여 국소 뇌 혈류 흐름의 특징적인 양상을 평가하고 두 질환에서의 차이를 비교하고자 하였다. 대상 및 방법: 피질하 혈관성치매 환자 16명, 알쯔하이머병 46명, 12명의 대조군이 포함되었다. 피질하 혈관성치매와 알쯔하이머병은 NINCDS-ADRDA 와 NINDS-AIREN 기준에 합당하였고, 이들의 연령, 교육연수, 한국형 간이정신상태검사 점수와 치매임상척도 점수를 일치시켰다. 세 군에서 국소 뇌혈류 흐름을 측정하고자 SPM 분석을 이용한 TG-99m HMPAO 단일 광자방출단층촬영술을 실시하였다. 피질하 혈관성치매와 알쯔하이머병에서의 단일광자방출단층촬영술 결과를 정상 대조군과 비교하였고, 이어서 두 질환 간의 차이를 비교하였다. 결과: 대조군과의 SPM분석 결과 피질하 혈관성치매에서는 우측 관자엽, 시상, 좌측 뇌섬엽, 상부 관자이랑, 양측 띠이랑, 이마엽에서 의미 있는 관류저하가 있었고, 알쯔하이머병에서는 좌측 모서리위이랑, 상부 관자이랑, 중심뒤이랑, 하부 마루소엽, 우측 뒤통수관자이랑, 양측 띠이랑에서 관류저하을 보였다(uncorrected p<0.01). 피질하 혈관성치매 환자에서 알쯔하이머병 환자에 비해 우측 해마이랑, 띠이랑, 좌측 뇌섬엽, 양측 이마소이랑 부위에 의미있는 관류저하를 보였다(uncorrected p<0.01). 결론: 본 연구는 피질하 혈관성치매와 알쯔하이머병 환자에서의 관류의 특징과 차이를 보여주었고, 이러한 결과는 질병의 초기단계에서 두 질환을 구별하는데 도움을 줄 수 있을 것으로 생각한다.

Olfactory neuropathology in Alzheimer's disease: a sign of ongoing neurodegeneration

  • Olfactory neuropathology is a cause of olfactory loss in Alzheimer's disease (AD). Olfactory dysfunction is also associated with memory and cognitive dysfunction and is an incidental finding of AD dementia. Here we review neuropathological research on the olfactory system in AD, considering both structural and functional evidence. Experimental and clinical findings identify olfactory dysfunction as an early indicator of AD. In keeping with this, amyloid-β production and neuroinflammation are related to underlying causes of impaired olfaction. Notably, physiological features of the spatial map in the olfactory system suggest the evidence of ongoing neurodegeneration. Our aim in this review is to examine olfactory pathology findings essential to identifying mechanisms of olfactory dysfunction in the development of AD in hopes of supporting investigations leading towards revealing potential diagnostic methods and causes of early pathogenesis in the olfactory system.
