Gastric cancer (GC) is one of the most common cancers, with high incidences in East Asia countries. Most GC patients have been reported with low early diagnosis rate and show extremely poor prognosis. Therefore, it is necessary to develop novel and more sensitive biomarkers to improve early diagnosis and therapy in order to provide longer survival and better quality of life for gastric cancer patients. MicroRNAs (miRNAs) play crucial roles in GC development and progression. miRNAs have emerged as a novel molecular biomarker for cancer diagnosis, prognosis and therapy with surprising stability in tissues, serum or other body fluids. This review summarizes major advances in our current knowledge about potential miRNA biomarkers for GC that have been reported in the past two years.
Purpose: To estimate the incidence of breast cancer in Yemeni female patients presenting with a breast mass. Materials and Methods: This retrospective study was carried out with 595 female patients with palpable breast lumps, attending to Alkuwait university hospital, Sana'a, Yemen. Triple assessment, including breast examination, mammography and biopsy (FNAC, core needle, or excision), for all patients were performed. Results: The incidences of benign and malignant lesions was calculated. Some 160 (26.9%) of 595 patients had malignancies; 213 (35.8%) were fibroadenomas; 12 (2.0%) were fibrocystic change; 143 (24.03%) were inflammatory lesions (including mastitis and ductectasia); 62 (10.4%) were simple cysts, while 5 (0.8%) were phyllodes tumors. The mean age of patients with malignant lumps was 44.3 years. Conclusions: Among Yemeni female patients with palpable breast lumps, the rate of breast cancer is high, with occurrence at an earlier age than in Western countries. Improving breast cancer awareness programs and increasing breast cancer screening centers inb different areas of Yemen are needed to establish early diagnosis and offer early and optimal treatment.

Purpose: Traumatic pancreatic injury is not common in abdominal trauma injury. However, the morbidity and the mortality rates of patients with pancreatic injury, which are related with difficulties of initial assessment, establishment of diagnosis, and treatment are relatively high. The aim of this study is to review our institution's experience and suggest a diagnosis and therapeutic algorithm for use in cases involving traumatic pancreatic injury. Methods: Eighteen(18) patients with blunt pancreatic injury from January, 2004 to October 2012 were included in this study. We analyzed treatment and diagnosis method, other organ injury, treatment interval, hospital stay and complications retrospectively. Results: Nine patients were treated with conservative medication and another nine patients were treated surgically. Complications occurred in nine patients, and one patient died due to intraventricular hemorrhage and subdural hemorrhage with multiple organ failure. Delayed surgery was performed in three cases. The early and delayed surgery groups showed difference in hospital stay and intensive care unit stay. Delayed surgery was associated with a longer hospital stay (p=0.007) than immediate surgery. Conclusion: In blunt pancreatic trauma, proper early diagnosis and prompt treatment are recommended necessity. Based on this review of our experience, we also suggest the adoption of our institution's algorithm for cases involving traumatic pancreatic injury.
We aim to monitor vascularization of early bone perfusion following rabbit lumbar intertransverse bone graft fusion surgery using magnetic resonance imaging assessment. Correlation with graft survival status was evaluated by histological method. Experimental animals were randomly divided into three groups and the model was established by operating bilateral lumbar intertransverse bone graft with different types of bone graft substitute material. The lumbar intertransverse area of three groups of rabbits was scanned via MRI. In addition, histological examinations were performed at the $6^{th}$ week after surgery and the quantitative analysis of the osteogenesis in different grafted area was carried out by an image analysis system. The MRI technique can be used for early postoperative evaluation of vascularized bone graft perfusion after transplantation of different bone materials, whereas histological examination allows direct visualization of the osteogenesis process.

Purpose: This study was done to compare the comprehension and knowledge about delirium in nurses working at long-term care hospitals (NLH) with nurses working at a general hospital (NGH). Method: The participants were 191 nurses from a general hospital and 7 long-term care hospitals in Chungnam. The tool for comprehension consisted of the needs and importance of delirium education and the confidence of management for early detection, caring and prevention of delirium. The tool for knowledge of delirium developed by the author consisted of causing factors, symptoms, caring and prevention of delirium. Result: The comprehension of need and importance of delirium education was higher than the confidence for delirium management in participants. NLH nurses had a higher level in comprehension of the need and importance of delirium education and a lower level in confidence of delirium management than NGH nurses. The mean for delirium knowledge was 75.32% for all participants. The knowledge of NLH nurses was significantly lower than those of NGH nurses. All nurses showed low scores in confidence and knowledge of delirium prevention. Conclusion: A systematic educational program for nurses to enhance the ability for assessment, early detection and prevention of delirium for institutionalized elderly patients needs to be developed.
Objectives The purpose of this case study is to report significant improvement of headache, constipation and a slight improvement of tremor after treatment with Hyeongbangjihwhang-tang in a Soyangin patient with Parkinson's disease. Methods This Soyangin patient is treated by Hyeongbangjihwhang-tang and acupuncture therapy. In this study, the authors introduced DASH (Disabilities of the arm, shoulder, and hand), which had been developed for evaluating the functional impairment in the movement of upper extremities in regular daily activities, work ability and sports/performing arts ability. It is an ergonomic risk assessment tool used for industrial workers and also a disability measurement tool for upper extremity disorders arising from musculoskeletal disorders and symptom. This study intended to examine the applicability of DASH in occupational health field. Firstly, DASH development process and composition were reviewed through The DASH outcome measure user's manual and early articles. Secondly, reliability, validity, and responsiveness of the DASH in various languages at the application stage as well as its reliability and validity at the early stage of development were investigated. Thirdly, focusing on the application of DASH to clinical cases, workers with musculoskeletal symptoms, healthy workers, workers with other diseases, and general population were discussed besides workers with major musculoskeletal disorders. Lastly, DASH questionnaire was examined for its potential as a reference for assessing the functional impairment in the movement of upper extremities of workers with musculoskeletal symptoms in industrial workers in Korea.
Ataxia-telangiectasia (AT; OMIM 208900) is a rare autosomal recessive inherited progressive neurodegenerative disorder, with onset in early childhood. AT is caused by homozygous or compound heterozygous mutations in ATM (OMIM 607585) on chromosome 11q22. The average prevalence of the disease is estimated at 1 of 100,000 children worldwide. The prevalence of AT in the Republic of Korea is suggested to be extremely low, with only a few cases genetically confirmed thus far. Herein, we report a 5-year-old Korean boy with clinical features such as progressive gait and truncal ataxia, both ankle spasticity, dysarthria, and mild intellectual disability. The patient was identified as a compound heterozygote with two novel genetic variants: a paternally derived c.5288_5289insGA p.(Tyr1763*) nonsense variant and a maternally derived c.8363A>C p.(His2788Pro) missense variant, as revealed by next-generation sequencing and confirmed by Sanger sequencing. Based on claims data from the Health Insurance Review and Assessment Service Republic of Korea, we calculated the prevalence of AT in the Republic of Korea to be about 0.9 per million individuals, which is similar to the worldwide average. Therefore, we suggest that multi-gene panel sequencing including ATM should be considered early diagnosis.
Purpose: This study aimed to review recent findings from birth cohort studies on maternal and child environmental health. Methods: Birth cohort studies regarding environmental health outcomes for mothers and their children were investigated through a systematic review. A literature search was conducted in PubMed, CINAHL, the Cochrane Library, Embase, and RISS to identify published studies using the keywords using a combination of the following keywords: maternal exposure, environmental exposure, health, cohort, and birth cohort. Articles were searched and a quality appraisal using the Newcastle-Ottawa Scale for cohort studies was done. Results: A review of the 14 selected studies revealed that prenatal and early life exposure to environmental pollutants had negative impacts on physical, cognitive, and behavioral development among mothers and children up to 12 years later. Environmental pollutants included endocrine disruptors, air pollution (e.g., particulate matter), and heavy metals. Conclusion: This systematic review demonstrated that exposure to environmental pollutants negatively influences maternal and children's environmental health outcomes from pregnancy to the early years of life. Therefore, maternal health care professionals should take steps to reduce mothers' and children's exposure to environmental pollutants.
Postoperative critical care management for lung transplant recipients in the intensive care unit (ICU) has expanded in recent years due to its complexity and impact on clinical outcomes. The practical aspects of post-transplant critical care management, especially regarding ventilation and hemodynamic management during the early postoperative period in the ICU, are discussed in this brief review. Monitoring in the ICU provides information on the patient's clinical status, diagnostic assessment of complications, and future management plans since lung transplantation involves unique pathophysiological conditions and risk factors for complications. After lung transplantation, the grafts should be appropriately ventilated with lung protective strategies to prevent ventilator-induced lung injury, as well as to promote graft function and maintain adequate gas exchange. Hypotension and varying degrees of pulmonary edema are common in the immediate postoperative lung transplantation setting. Ventricular dysfunction in lung transplant recipients should also be considered. Therefore, adequate volume and hemodynamic management with vasoactive agents based on their physiological effects and patient response are critical in the early postoperative lung transplantation period. Integrated management provided by a professional multidisciplinary team is essential for the critical care management of lung transplant recipients in the ICU.
