  • FHIR is a protocol that enables easy data exchange from the time the event occurred in health care settings as a standard for next-generation message exchange of HL7. Create a meaningful message from the ECG and medical equipment, and express the messages generated by standardized FHIR message it will be used in various medical institutions to ensure delivery to EMR, such as hospital information systems can query the results via a smartphone.

  • The purpose of this paper, as a part of healthcare research, is to design and development Patient-aware System that will support EMR(Electronic Medical Record) in hospital. A mobile device-based system that can use database of existing EMR, replace existing paper-type chart, and identify patient fast and correctly was developed. To identify patient, RFID(Radio Frequency Identification) was used, and through interworking RFID and the system, it is possible to identify patient automatically. The developed system was tested in the test bed, and the possibility of faster diagnosis and treatment than existing paper-type chart was tested.

  • EMR(Electronic Medical Records) 시스템은 기존의 의무 기록을 전산화 한 것으로 의료 서비스 시용자의 신체정보 및 진료정보 등을 전산화 하여 저장하는 시스템이다. 노령화 사회로 들어서게 되면서 많은 의료 소비자들이 의료 서비스 기관들에 발걸음이 잦아지고 각 환자마다 늘어나게 될 데이터를 좀 더 효율적으로 관리할 수 있는 EMR의 필요성이 대두되고 있다. 본 논문은 현재 IT트렌드의 흐름을 살펴보고 의료 정보 기술과 접목한 새로운 차세대 EMR 기술을 분석하고 그 발전 방향을 제시하고자 한다.

  • Radiofrequency electromagnetic radiation (RF-EMR) from various sources may impact health due to the generation of frequency bands. Broad pulses emitted within frequency bands can be absorbed by cells, influencing their function. Numerous laboratory studies have demonstrated that mobile phones-generally the most widely used devices-can have harmful effects on sex cells, such as sperm and oocytes, by producing RF-EMR. Moreover, some research has indicated that RF-EMR generated by mobile phones can influence sperm parameters, including motility, morphology, viability, and (most critically) DNA structure. Consequently, RF-EMR can disrupt both sperm function and fertilization. However, other studies have reported that exposure of spermatozoa to RF-EMR does not affect the functional parameters or genetic structure of sperm. These conflicting results likely stem from differences among studies in the duration and exposure distance, as well as the species of animal used. This report was undertaken to review the existing research discussing the effects of RF-EMR on the DNA integrity of mammalian spermatozoa.

Development of Guideline on Electronic Signatures for Electronic Medical Record (전자의무기록에 대한 공인전자서명 적용 지침 개발)

  • One of the most secure ways of maintaining the confidentiality and integrity of electronic information is to use electronic signatures. So, in this paper, we developed guideline on electronic signatures for EMR(electronic medical record) based on the Medical Law and the Electronic Signature Act. This guideline is intended to introduce EMR easily in the medical field and to facilitate the promotion of EMR. We developed it through consulting from the advisory committee that was made up of experts in the fields of medical record, EMR system and electronic signatures. The contents of the guideline consist of subject and time stamp of electronic signatures, validity of a certificate, management of electronic signatures and custody and management of EMR. In the future, we will develop practical cases and promote educations and publicities of them to use in the medical institutes and EMR system related industries.

  • Medical Gateway(MG), that integrates medical devices with its unique data format in various hospital information systems, provides effectiveness in the cost of the management unlike existing systems that uses PC as a gateway to hospital information systems because MG enables to interface medical devices with itself. However, in spite of using MG for integrating medical devices, we need additional programs or systems to interwork with EMR system of each hospital in accordance with the configuration of DB. In this paper, we design and implement the format of medical information and server system for interworking MG with EMR system. The proposed system makes easy exchanging medical data between MG and EMR system. Thus, the proposed system can contribute to a national project to export abroad advanced hospital setup converged with IT.

  • The rapid development and distribution of information communication industry facilitates the changes of hospital administration, introducing EMR(Electronic Medical Record) instead of paper-based medical record in the medical field. The developed countries such as U.S. have established EMR system after in the middle of 1970s because the primary advantages of EMR is to store and handle vast amounts of records efficiently and increase the quality of health care. Most of health organizations in Korea also apply medical record system to their administration. As the result, they have accomplished a scientific administration system through the use of medical record to handle a variety of patient's information including patient's confidentiality and privacy such as family history, social status, income level, and so on. However, access to and the misuse of EMR causes illegal infringement of patient's information and finally it becomes a very serious medical issue. Potential leakage and misuse of records may seriously infringe patient's privacy rights. In this respect, the related agencies in the public and private sector have been making efforts to prevent patient's records leakages. Especially, the revision bill of Medical Law in 2002 establishes the ways on the security and standards of electronic records. However, it does not provide the proper guidelines which is applied to the rapid changes of the medical environment. One of the most priorities in the hospital administration is the production and maintenance of an accurate medical records fulfilled by medical recorders. Therefore, it is very important for health care providers to hire ethical-based medical recorders. But, unfortunately most of hospitals overlook the importance of their roles. All parts including government, physician and patient must have more concerns on the problems related to EMR. Therefore, this study aims to propose the proper ways to resolve the problems coming from EMR.

Effects of Mori Ramulus on Collagen-induced Arthritis Rat - Expression of Immunocells in Draining Lymph Node - (상지가 콜라겐 유발 관절염 랫트에 미치는 영향 - 배액림프절의 면역세포 발현 -)

  • Mori Ramulus has multiple applications in Korean traditional medicine prescription because it has antioxidant and anti-inflammatory effects by reducing macrophage activities. Yet, no studies on the anti-arthritic activity of EMR (extract of Mori Ramulus) have been reported in vitro and in vivo. Rheumatoid arthritis (RA) is a systemic autoimmune disease with chronic inflammation characterized by hyperplasia of synovial cells in affected joints, which ultimately leads to the destruction of cartilage and bone. Because collagen-induced arthritis (CIA) is similar to RA in pathological symptoms and immune reactions, there have been several reports concerning RA using CIA mouse model. Here, we investigated the effects of Mori Ramulus on RA using CIA mice. The importance of CD4+ Th1 cells in RA progress was previously indicated and studies further showed that Th17 cells play a prime role in severity of disease. Accordingly, the present study was focused on CIA associated with CD4+ Th1 cells and Th1 7 cells. DBA/1OlaHsd mice were immunized with bovine type II collagen (CII). After a second collagen immunization, mice were treated with EMR once a day for 4 weeks. The severity of arthritis within the paw joints was evaluated by histological assessment of cartilage destruction and pannus formation. Immune cells in peripheral blood mononuclear cells (PBMC), draining lymph node (DLN) and paw joints, cytokine production and gene expression were assessed from CIA mouse using ELISA, FACS and real-time PCR analysis. Administration of EMR significantly suppressed the progression of CIA and inhibited the production of TNF-$\alpha$, IL-6 and IL-17 in the serum. The erosion of cartilage was dramatically reduced in mouse knees after treatment with EMR. In conclusion, our results demonstrate that EMR significantly suppressed the progression of CIA and that this action was mediated by the decreased production of TNF-$\alpha$, IL-6, IL-17 and collagen II-specific antibody in the serum. EMR suppressed Th17 cells and reduced level of IL-6 via B cell suppression, and thus, the levels of autoantibodies produced from B cells were decreased. Furthermore, EMR suppressed NKT cells which directly stimulate B cells and develop imbalance of Th1/Th2 cell. Oral administration of EMR (100 mg/kg or 200 mg/kg) significantly suppressed the progression of CIA, which is comparable to that of methotrexate (MTX, 0.3 mg/kg) used as a positive control. We are currently studying the mechanism underlying the therapeutic role for EMR in CIA mice.

Smart Tongue Electronic Chart System (스마트 설진 전자챠트 시스템)

  • These days it is becoming more and more common to find electronic medical screening systems installed in Oriental hospitals and clinics. This is a relatively new development for the practice of traditional Oriental medicine. Specifically, Pulse detection machines are being utilized in order to help determine a patient's disease scientifically. However, identifying and diagnosing the specific disease correctly for each patient is still very difficult in Oriental medicine. The intention of this paper is to propose a solution which uses two separate Electronic systems working together to produce a better likelihood of finding the correct diagnosis for each patient. It is proposed that an EMR intelligent electronic chart system be developed and employed, which would utilize both Pulse wave system and a tongue detection system at the same time, in order to solve the problem. Computer simulation results have proven to show that EMR systems used in hospitals and clinics are more efficient and yield a more accurate diagnosis than traditional methods.

A Secure Medical Information Management System for Wireless Body Area Networks

  • The wireless body area networks (WBANs) consist of wearable computing devices and can support various healthcare-related applications. There exist two crucial issues when WBANs are utilized for healthcare applications. One is the protection of the sensitive biometric data transmitted over the insecure wireless channels. The other is the design of effective medical management mechanisms. In this paper, a secure medical information management system is proposed and implemented on a TinyOS-based WBAN test bed to simultaneously address these two issues. In this system, the electronic medical record (EMR) is bound to the biometric data with a novel fragile zero-watermarking scheme based on the modified visual secret sharing (MVSS). In this manner, the EMR can be utilized not only for medical management but also for data integrity checking. Additionally, both the biometric data and the EMR are encrypted, and the EMR is further protected by the MVSS. Our analysis and experimental results demonstrate that the proposed system not only protects the confidentialities of both the biometric data and the EMR but also offers reliable patient information authentication, explicit healthcare operation verification and undeniable doctor liability identification for WBANs.
