A Bibliographic Study on the cause and Symptom of Hae Su (咳嗽) caused by a cold (외감(外感)으로 인(因)한 해수(咳嗽)의 원인(原因), 증상(症狀), 치료(治法)에 대(對)한 문헌적(文獻的) 고찰(考察))

  • This study has been carried out to investigate the cause and symptom of Hae su (咳嗽) cause by a cold referring to 25 literature. 1. Cause : wind (風), cold (寒), heat (署), moisture (濕), dryness (燥), fire (火). It appears to cause double and above. 2. Symptoms : PunghanHaeSu (風寒咳嗽) ; HaeSu (咳嗽), Balyul (發熱), DamHueSaegBaeg (痰稀色白), Hu Yang Sung Tag (喉痒聲濁), BiSaegYuChe (鼻塞流涕), OhPungHan (要風寒), DooShinTong (頭身痛), TaeBaqBaeg (笞薄白), Maeg Bu Gin (脈浮緊). PungYeulHaeSu (風熱咳嗽) ; HaeSu (咳嗽), DamJo (痰稠), SaegHwang (色黃), KaegChulBulSang (喀出不痰), KuKallnTong (口瀉咽痛), BalYeul (發熱), OhPungYuHan (要風有肝), HogYuDuTong (或有頭痛), TaeBagHwang (笞薄黃), Maegbusag (脈浮數). JoYeulHaeSu (燥熱咳嗽) ; KunHaeMuDarn (乾咳無痰), HogDamJungDaeHyulSa (或痰中帶血絲), HaelnHungTong (咳引胸痛), BiJolnKun (鼻燥咽乾), pliogyuOhPungBalYeul (或要風發熱), TaeBagHqangIKun (笞薄黃而乾), MaegSeSag (脈細數). 3. Treatment ; PungHanHaeSu ; SoPungSanHan (疏風散寒), Sun PaeJiHae (宣肺止咳), PungYeulHaeSu ; SoPungchungYeul (疏風淸熱), SunPaeJiHae (宣肺止咳), JoYeulHaeSu; CheongPoeYunJo (淸肺潤燥), SaengJinJiHae (生津止咳). Basing on the literature research, I found that Hae Su (咳嗽) caused by a cold belong to category of YugEum(六淫).

Analysis of herbal formulation about a series of Chijasi-tang in Dongeuibogam (동의보감(東醫寶鑑)에 수록된 치자시탕(梔子豉湯) 가미방(加味方) 분석(分析))

  • In this study, we selected some herbal formulation about a series of Chijasi-tang in Dongeuibogam by using web-based open program;Prescription Lineage Graph (http://164.125.206.43/PrescriptionLineageGraph.aspx). And we compared and analyzed the changes of efficacy, major target symptoms of each herbal formulation according to compositional variation of each herbal formulation. Chijasi-tang, first appeared in Sanghanlun, consists of Capejasmine and Fermented soybean, and it is mainly used to treat insomnia due to vexation, heartburn and yellow greasy tongue fur. Capejasmine can clear away irritable feverish sensation in chest by flowing downward the heat, and Fermented soybean can disperse stagnated heat throughout the chest by expelling stagnated heat from the exterior In the case of the heat stagnation caused by relapsing of disease due to overstain, Chisijisil-tang can be used. And if symptom appear more on the upper or exterior than a case of Chisijisil-tang, Seosisi-tang could be more suitable, if half exterior and half interior, Omae-tang could be for it. In addition, if symptom caused by relapsing due to improper diet, Chisijihwang-tang could be proper formulation. In the case of the heat stagnation body inside and jaundice, if it is caused by alcohol, Chijadaehwang-tang could be used for the purpose of urgent purgation, Galchul-tang would be suitable for helping the function of the spleen and the stomach and for treatment of damp-heat of the spleen and the stomach. And if it is caused by pandemic infection, Jangdal-hwan would be good formulation for it. Samhwangseokgo-tang and Yangdokchija-tang could be appropriate formulation for the raging of noxious heat and pathogenic fire caused by febrile disease with toxic yang. Daehwangeum-ja is for severe constipation due to heat-dryness with stagnated fever, Haebaek-tang is appropriate for severe diarrhea due to heat type change of Soeum. According to the result of our investigation, although there are various target causes and symptoms of each herbal formulations, whatever pathogenetic cause is, the stagnated heat in interior side is the basis of symptoms. Therefore, the purpose of including Chijasi-tang in composition of each herbal formulation is treatment of the stagnated heat. For such reason, on the fundamental or ancillary basis of Chijasi-tang plus some herbs for each therapeutic purpose.

  • The results of the study about comparison of Eastern-Western medicine on the Atopic dermatitis were as follows. 1. Atopic dermatitis is chronic eczematous dermatosis which have severe itching, characteristic eruption, easily occur region and pass on chronic relapsing progress and have family history of Atopic disease and hereditary disposition. 2. Atopic dermatitis assume an remarkable clinical aspect and it's diagnosis depends on family history and clinical symptom. 3. In all cases of Western medical treatment is nothing but a symptomatic treatment because can not find out certainly the cause of Atopic dermatitis. 4. Atopic dermatitis is belong to the category of the 'Naesun(내癬)', 'Taeryumchang(胎斂瘡)', 'Samanpoong(四灣風)' etc. in Oriental medicine. 5. The etiology and pathogenesis of Atopic dermatitis in oriental medicine are congenital defect(稟賦不足), internal accumulation of damp and heat(濕熱內鬱), improper diet(飮食不節), exogenous pathogenic factors(外邪侵襲), etc.. 6. The treatments of Atopic dermatitis in oriental medicine are thought effective clear up heat and remove dampness with febrifugal arld diuretic drug(淸熱利濕), invigorate the spleen to resolve dampness(健脾燥濕), nourish Um and blood to relave dryness(滋陰養血潤燥), etc..

A Clinical Study on Differentiation of Syndromes of Amenorrhea or Oligomenorrhea with DSOM (한방진단시스템 DSOM을 이용한 무월경 및 희발월경의 변증진단 연구)

  • Purpose: In oriental medicine, doctors have mainly made diagnosis and treatment with amenorrhea or delayed menstrual period based on overall analysis of symptoms and signs patients have. We think patients with amenorrhea or delayed menstrual period to have symptoms that are classified into one differentiation of syndromes, and then studied to make the index. This study has been carried out to investigate pattern identifications and classify symptoms according to them. Methods: We examined 52 patients who visited Dong-eui university oriental medical center from June 2005 to February 2009 for undergoing treatment for amenorrhea or delayed menstrual period and made OB & GY questionnaires up Results: We investigated whether the patients had symptoms concerned with symptom types by analyzing the result of DSOM(Diagnos System of Oriental Medicine, hearafter DSOM). It came out 51 cases among 52 with pathogenesis that was related to the symptom types. The symptom types of were surveyed into Insufficiency of Kidney and Liver(肝腎不足), Insufficiency of Qi and Blood(氣血虛弱), Insufficiency of Yin & Dryness of blood(陰虛血燥), stagnation of Qi and Blood(氣滯血瘀), phlegm and damp(痰濕阻滯), coldness of Blood(血寒), Heat of Heart(心火). Conclusion: As a result of the investigation, one case did not have symptoms of differentiation of syndromes of amenorrhea or delayed menstrual period. 29 cases had 1 or 2 differentiation of syndromes. 21 patients had such complex symptoms of diverse differentiation of syndromes that it was difficult to diagnose a differentiation of syndromes in clinical survey. It is necessary to put the priority among the differentiation of syndromes in diagnosis in the future.

A Study about trends of using Shihogayonggolmoryo-tang granule (시호가용골모려탕 과립제의 임상경향 분석 및 활용)

  • Objectives : This study was carried out to investigate trends of using Shihogayonggolmoryo-tang granule on neuropsychiatry fields. And suggest that using herbal extracts change non-prescription medicine to prescription medicine. Methods : I researched 86 outpatients who treated with Shihogayonggolmoryo-tang granule at oriental medicine hospital. And I analyzed their medical records of chart review study. Results : 1. Average medication time in this research was 14.9 days and the main usage was supplementary to herbal medicine, such cases amounted to 62%. 2. There are 52 patients out of 86 who are much better, such cases amounted to 55%. 3. The improved diagnosis is Syndrome of heart-fire flaring up, Syndrome of upper energizer dryness-heat, headache, and tinnitus. The improved symptom is anxiety, headache, dizziness, tic disorders, and chest discomfort. Conclusions : I expect to using herbal extracts change non-prescription medicine to prescription medicine based on this study.

  • A symptoms of Diabetic is finded in many recordes, that is said to Diabetic or dryness or migratory arthralgia or flaccid paralysis of limb or abscess or arthralgia, but Diabetic is most closely symptom. So, study the 3-symptoms of Diabetes mellituscause, thses are polyuria and polyphasia and thirst, and this is similar symptoms of So-gal. So, bibliographic study or pathological approach of symptoms of So-gal, and So-gal in oriental medicine has come to the following conclution. The symptoms of So-gal is polyuria and polyphasia and thirst, is due to Simsinbulgyo, that is mean to breakdown of the normal physiological coordination between the heart and the kidney. A thirst is due to heat-transformation have an effect to the upper energizer and the heart and the lung and the stomach in Simsinbulgyo. A polyuria is due to failure of the kidneys Yang, that is due to failure of Yang of the heart in Simsinbulgyo.

  • In oriental medicine, 'chi(痔)' is 'the prolapsed nodule' in 'Ku-gyu(九竅)', but in this paper, I will write about prolapsed nodule only in anus or around it, chi-jil(痔疾), it called hemorrhoid in western medicine. So in the literatual studies on chi(痔) in anus or around it, the results are as follows. 1. The etiology and pathogenesis of ch.i-jil(痔疾) is wind, wetness, dryness, and heat caused by inrregular diet habit, severe drinking and sexual action, deficiency of ki(氣) and hyeol(血). 2. Characteristic symptoms of chi-jil(痔疾) is the prolapsed nodule in the anus or around it, and general symptoms are hematochezia, pain, hernia, swelling, abcess, and mucosal secretion. 3. Chi-jil(痔疾) is classified eight types by characteristic symptom, shape, etiology and pathogenesis. They are mac-chi(脈痔), jang-chi(腸痔), ki-chi(氣痔), hyeol-chi(血痔), joo-chi(酒痔), mo-chi(牡痔), bin-chi(牡痔), and loo-chi(屢痔)(or young-chi(영痔), choong-chi(蟲痔)). Additionally, they are divided into two parts, internal and external chi-jil(痔疾), as likely as classification of internal and external hemorrhoid in western medicine. 4. Treatment of chi-jil(痔疾) is two methods, internal treatment and external treatment. Internal treatment is per os herb-pharmacotheraphy, external treatment is surgical or the other external pharmacotheraphy. There are several external treatment, these are fumigation-theraphy(熏痔法). irrigation-theraphy(洗痔法), paint-theraphy(塗痔法). withering-theraphy(枯痔法), bending-therphy(結紮法) and incisal -theraphy(切開法).

  • In the literatual studies on the yupung(油風), the results were as follows. 1. Yupung(油風) was called 'Quijidu(鬼지頭)', 'Quichedu(鬼剃頭)', 'Ballak(髮落)' etc. It was a localized loss of hair in round or oval ares without any subjective symptom 2. The etiology and pathogenesis of Yupung were disorder of vital energy& blood circulation caused by deficiency of blood and wind-dryness syndrome, energy-stagnation and blood stasis, impairment of the liver & kidney. The treatments of Yupung were invigorating the liver & kidney, clearing away heat-evil and cooling blood, nourishing the liver & kidney, activating blood circulation to dissipate blood stasis etc, 3. In the frequency of prescription, the most numerous prescription is Shineuingyangjindan(神應養眞丹) and the next are Tonggyuhwalhyultang(通竅活血湯) & Chilbomiyumdan(七寶美髥丹), 4. In the frequency of medicine, the most numerous medicine is Radix Angelicae Sinensis(當歸) and the next are Rhizoma Rehmanniae Praeparatae(熟地黃) & Rhizoma Ligusiici Chuanxiong(川芎). 5. In classification of drug action, medicines of clearing away wind-heat evil and invigorating yin used to be very busy. 6. In classification of four characters, the most parts are warm medicine. 7. In classification of five tastes, the most numerous tastes are sweet and bitter tastes. 8. In classification of toxicity, the most is non-toxic medicines. 9. In classification of the channel distribution, the most is the medicine that belongs to liver channel. 10. In acupuncture theraphy, it was used 'GV20(百會)', 'G20(風池)', 'GV16(風府)', 'GV14(大椎)', 'LI4(合谷)' etc. in body acupuncture, was used 'S25(天樞)', 'GV14(大椎)', 'B13(肺兪)', 'LI11(曲池)' etc. in moxibustion.

Study on the 'Diagnosis and Treatment of Terror and Palpitation due to Fright and the Several Hemorrhagic Diseases' in Synopsis of Golden Chamber (금궤요략${\cdot}$경계토뉵하혈흉만어혈병맥증병치 제십육(第十六)에 대(對)한 연구(硏究))

  • This Chapter mentioned Terror and Palpitation due to Fright(驚悸) and Hemorrhagic disease(血證). Terror and Palpitation due to FrightAcctually Terror(驚) is different from Palpitation(悸). Terror(驚) is one of the seven emotions. But in this case, It refer to the Palpitation and the uneasiness of mind due to one's hearing a strange sound of seeing a strange. Tremulous Pulse can be appear. So Terror(驚) is caused by Exopathic Factors(外因) and belongs to Excess syndrome(實證). Palpitaion(悸) is the sensation of plamus, palpitation and unrest not because of being frightened. It is usurally caused by the deficiency of Ki(氣) and blood(血). So Deep, Thready and weak pulse can be appear. So Palpitaion(悸) is caused by Endopathic Factors and belongs to Deficiency Syndrome. In this Chapter, Terror and Palpitation due to Fright(驚悸) treat with the Kyeji-ke-jakyak-ka-chokchil-moryu-yongkol-kuyuk-tang(桂枝去芍藥加蜀漆牡蠣龍骨救逆湯) and Banha-mahwang-hwan(半夏麻黃丸). There are two type in Hemorrhagic disease(血證). One is bleeding(出血) and another is blood stasis(瘀血). The contents which relate with the Hemorrhagic disease(血證) are Hematemisis(吐血), Rhinorrhagia, Hemafecia(下血). In hemorrhage pathological mechanisms, there are two mechanisms. One is that Fire and Heat(火熱) pressure blood. Another is that cold and deficiency(虛寒) disable Ki(氣) from keeping blood flowing within vessels. Blood stasis(瘀血) can be called Extravasated Blood(惡血), Coagulated Blood, Blood retention(蓄血,積血), Dead Blood(死血) and Emaciation due to Blood disorder(乾血). It refer to a morbid state of unsmooth circulation and blood stagnancy often resulting from Ki(氣) stagnation, Ki(氣) deficiency and accumulation of pathogenic coldness. The symptom of Blood stasis are 'Fullness sensation in the chest, Lip Flaccidity, Cyonotic Tongue and Dryness of Mouth'. And the man who have Blood stasis, want to rinse his mouth with the water, but he can't drink the water because there isn't interior Heat of Excess Type. The symptom of Cyonotic Tongue(舌靑) had influence on diagnosing Blood stasis(瘀血) in offspring.

Safety and antifatigue effect of Korean Red Ginseng: a randomized, double-blind, and placebo-controlled clinical trial

  • Background: Korean Red Ginseng (KRG) is widely used for strengthening the immune system and fighting fatigue, especially in people with deficiency syndrome. However, there is concern that the long-term application or a high dose of KRG can cause "fireness" (上火 in Chinese) because of its "dryness" (燥性 in Chinese). The aim of this study was to assess the safety and efficacy of a 4-week treatment with KRG in participants with deficiency syndrome. Methods: This was a 4-week, randomized, double-blind, placebo-controlled clinical trial. A total of 180 Chinese participants were randomly allocated to three groups: placebo control group, participants were given a placebo, 3.6 g/d; KRG 1.8 g and 3.6 g groups. The primary outcomes were the changes in fireness and safety evaluation (adverse events, laboratory tests, and electrocardiogram). The secondary outcomes were the efficacy of KRG on fatigue, which include the following: traditional Chinese medicine (TCM) symptom scale and fatigue self-assessment scale. Results: Of the 180 patients, 174 completed the full study. After 4 weeks of KRG treatment, the Fire-heat symptoms score including Excess fire-heat score and Deficient fire-heat score showed no significant change as compared with placebo treatment, and no clinically significant changes in any safety parameter were observed. Based on the TCM syndrome score and fatigue self-assessment score, TCM symptoms and fatigue were greatly improved after treatment with KRG, which showed a dose- and time-dependent effect. The total effective rate was also significantly increased in the KRG groups. Conclusion: Our study revealed that KRG has a potent antifatigue effect without significant adverse effects in people with deficiency syndrome. Although a larger sample size and longer treatment may be required for a more definite conclusion, this clinical trial is the first to disprove the common conception of "fireness" related to KRG.
