식품중 총 아플라톡신 분석법 개선 (Advances in the Analysis of Total Aflatoxins in Foods)

  • 총 아플라톡신 분석을 위한 최적의 추출 및 용출조건을 확립하여 최적의 전처리 조건을 확립하였다. 추출용매는 70% methanol 용액에 NaCl을 각각 1% 첨가한 용액을 사용하였고, 정제용 컬럼인 immunoaffinity column을 이용하여 acetonitrile 3 mL를 용출용매로 사용하여 아플라톡신 혼합용액의 분리능을 좋게 하였다. 검출한계는 0.05 ng/g 수준이였고, 식품의 유형별 총 아플라톡신($B_1,\;B_2,\;G_1,\;G_2$)의 회수율은 곡류 $74.1{\sim}95.5%,\;83.7{\sim}98.8%,\;80.4{\sim}102.4%$$72.8{\sim}76.5%$, 두류 $85.8~87.5%,\;83.8{\sim}90.7%,\;92.0{\sim}94.5%$$60.6{\sim}65.6%$, 견과류 $84.6{\sim}97.1%,\;86.0{\sim}94.1%,\;95.5{\sim}111.5%$$71.0{\sim}89.9%$, 가공식품 $81.5{\sim}87.1%,\;82.8{\sim}85.8%,\;85.4{\sim}92.7%$$68.9{\sim}76.4%$, 건조과실류 $83.6{\sim}93.5%,\;78.1{\sim}90.4%,\;93.0{\sim}108.5%$$64.9{\sim}78.5%$, 기타식품 $72.5{\sim}98.3%,\;73.1{\sim}96.4%,\;83.5{\sim}107.2%$$64.2{\sim}75.8%$를 보여주었다.

영역조절모형(RAM)을 활용한 신약개발 국가연구개발사업의 효율성 분석 (The Efficiency Analysis of National R&D Programs for Drug Development Using Range Adjusted Measure)

  • 신약개발은 국민의 건강권과 직결되며 국가 차원의 제약산업 육성을 위해 매우 중요하다. 그동안 신약개발 분야와 관련하여 다양한 연구가 수행되었지만, 신약개발 국가연구개발사업에 대한 효율성 분석은 매우 부족한 실정이다. 특히 R&D 분야는 연구개발시차, 일출효과(spill-over effect) 등으로 인해 투입과 산출의 일정한 방향성과 등비율적인 증감을 가정하는 CCR 모형이 적합하지 않다. 또한 가변규모수익을 가정하는 BCC 모형도 여유분(slack)으로 인해 의사결정단위 간의 명확한 우선순위 도출이 어려운 한계점이 있다. 최근에는 R&D 분야의 특수성을 고려해서 자료포락분석 모형 중 영역조절모형(Range Adjusted Measure: RAM)을 활용한 분석방법이 제시되고 있다. RAM 모형은 가변규모수익 가정 하에 투입지향이나 산출지향처럼 방향성에 대한 사전적인 가정 없이 비효율성을 최대한 제거하는 방향으로 효율성을 측정하므로 R&D 분야의 효율성 분석에 적합하다. 특히 RAM 모형은 매우 강건한 단조성으로 인해 의사결정단위 간 명확한 순위 구분도 용이하다. 따라서 본 연구에서는 자료포락분석의 RAM 모형과 토빗 회귀분석이 결합된 2단계 접근법(two-stage approach)을 활용해서 범부처전주 기신약개발사업을 중심으로 신약개발 국가연구개발사업에 대한 효율성을 분석하고 주요 시사점을 논의하였다.

  • The health industry of China is faster than before, so it will be rank 5th at world-wide markets in 2010. Firstly, the custom tariff was lower at 15% in 2000 and at 10.4% on June 2005. And, it will be lower into 9.8% in 2010. Secondly, this is because of expansion of purchase power on increase in economic growth, extended human life expectancy. As it is entered the WTO in January 2002, it will positively affect on export of Korea for China. This paper focuses on the analysis of export increase caused by reduction of custom tariff in China. As above mentioned, the time schedule on average reduction of custom tariff was 15% in 2000, 10.4% in 2005. Then, it will be lower into 9.8% in 2010. As the empirical test related health industry, it presents 19.80% export increase rate for China of Korea on reduction of custom tariff from 2001 to 2010. The exporting of drug for China will be up to 17.85% for 10 years. Also, the exporting of Biomedical will be up to 20.99%, and respectively 22.95%, 22.60% in Cosmetics and food industry. Conclusionally, the exporting of this health industry will be increase greatly, compared with any other industries. So, it is necessary that firms increase in R&D investment and government support as policy for health industry.

비처방의약품 허가 제도의 국가별 비교 연구 및 고찰 (Comparison of Approval Process for Nonprescription Drugs in Different Countries)

  • Nonprescription drugs have become increasingly important in Korean healthcare. By leveraging lower-cost drugs and reducing expenditure associated with fewer physician visits, the nonprescription segment can deliver tremendous value to individual consumers and the Korean healthcare system. Many countries have provided simpler and more rapid routes to market entry for qualifying nonprescription drug products, using the established data on drug safety and efficacy, as well as public and professional opinion. In US, the FDA waived the pre-approval process for over-the-counter (OTC) drugs marketed through the OTC Monograph Process. In Australia and Canada, different OTC product application levels are defined, with a reduced level of assessment required when the risks to consumers are considered low. Japan established a new OTC evaluation system in 2014 to facilitate the Rx-to-OTC switch process. The legislative framework for medicinal products in the European Union allows for drugs to be approved with reference to appropriate bibliographic data for old active substances with well-established uses. Through a comparison of the regulatory framework and the requirements for nonprescription approval process in different countries, several ways to improve regulatory practice for the evaluation of nonprescription drugs in Korea have been suggested.

영동지방 의약분업 예외지역 약국들에서의 감기 증상 모의 환자에 대한 약물 처방 현황 (Prescription Pattern for a Simulated Patient With the Common Cold at Pharmacies in a Region in Korea Without Separation of Dispensary From Medical Practice)

  • Objectives: Korea has been practicing the separation of dispensary from medical practice since 2000 as a national policy to prevent misuse or overuse of medicines. This study aimed to investigate prescription patterns from except pharmacies in order to determine the appropriateness of drug usage among those patients. Methods: Thirty-two pharmacies in the Yeongdong area of Gangwon Province were examined in this study. The same simulated patient complaining of cold symptoms for 3 days visited each pharmacy to obtain a prescription for medication. Results: At pharmacies prescribing medicine, steroids (53.1%) and antibiotics (50.0%) were used to treat the common cold. Duplicate prescriptions of drugs, such as antihistamines (47.0%) and decongestants (31.3%) were common. The average number of drug prescriptions was 6.59, and 53.2% of pharmacies had prescribed more than seven drugs. The average total cost of the prescriptions was 6,093 won, and the daily cost was 2,544 won. Conclusions: Steroids and antibiotics were frequently abused among patients whose medications had been prescribed by pharmacies. Also, there were a considerable number of drugs and duplicate prescriptions. The prices of the drugs were somewhat high.

영국의 일반의약품(Over-the-counter drugs) 관리법의 의료정책적 함의 (The NHS Over-the-Counter Drugs Policy in UK: Its Experiences and Implications)

  • Changes in a drug's availability from prescription only to over-the-counter (OTC) status is of concern to physicians from both public health and individual patient perspectives. Government has generally been supportive of changes in medications from prescription(Rx) to over-the-counter (OTC) status in Korea, however, recognizing that there are both benefits and risks to any health care intervention, health care professionals are conservative in implementing changes to either the process or structure of health care. Changes in status of a drug from Rx to OTC can represent a change in both structure and process. Cost and convenience seem to be major factors in determining whether, given the choice, patients purchase a medicine over the counter or obtain it on prescription. With current arrangements, exemption from prescription charges provides an incentive to continue to obtain products on NHS prescription even when they are available over the counter. There is therefore no simple relation between the availability of over the counter medicines and the level of prescribing of deregulated products. The appropriate use of over the counter medicines-particularly those that have only recently been deregulated-places a burden of care on community pharmacists and calls for closer working relationships with general practitioners. In particular, systems for referral and for recording details of both prescribed and over the counter medicines need to be developed, and a direct route needs to be established for community pharmacists to report adverse drug reactions to over the counter products. Reclassification of prescription medicines-by making them available through pharmacies without a prescription-provides the opportunity for consumers to purchase a wider range of medicinal products without making a demand on NHS resources. There is, however, no simple relation between availability of over the counter medicines and demand for NHS prescriptions. In the late 1980s the UK government fuelled the over the counter market by making it easier to reclassify certain medicines from prescription only status to allow over the counter sale in pharmacies. To explore the influence of deregulation of medicines on NHS prescribing, this article presents analyses of consumer behaviour in using medicines and prescribers' attitudes to over the counter medication and collates findings from research. Policy makers should be aware that patients' expectations in relation to OTC medicines may be in conflict with evidence-based practice.

고속도로 휴게소 영양표시 인식 여부에 따른 영양표시 만족도, 구매행동 변화 및 식생활관리에 관한 연구 (A Study on the Association among Satisfaction of Nutrition Labeling, Change of Purchase Behaviors, and Dietary Life Care, based on the Awareness of Nutrition Labeling at Expressway Rest Areas)

  • This study was to investigate the association among satisfaction of nutrition labeling, change of purchase behaviors, and dietary life care, based on the awareness of nutrition labeling at expressway rest areas. The subjects (n=903) were divided into two groups, according to the awareness of nutrition labeling: Awareness of Nutrition Labeling (ANL) group, n=367; Non Awareness of Nutrition Labeling (NANL) group, n=536. Effort of health care and identification of nutrition labeling was significantly higher in the ANL group, compared with the NANL group. As for the main reason for not identifying nutrition labeling, 'not interested in nutrition labeling' was the highest in the ANL group, and 'Don't know nutrition labeling is provided' in the NANL group. Identification ratio of nutrition labeling in future was significantly higher in the NANL group, compared with the ANL group among the subjects who didn't identify nutrition labeling before. After their becoming aware, a change of food purchase after reading the nutrition labeling was significantly higher in the ANL group, compared with the NANL group. Health beliefs on the nutrition labeling were significantly higher in the ANL group, compared with the NANL group. Satisfaction of nutrition labeling was also significantly higher in the ANL group, compared with the NANL group. The ANL group also expressed a necessity of expansion of nutrition labeling, compared with the NANL group. In the ANL group, identification of nutrition labeling was significantly higher in the hard effort group, compared with the little effort group of dietary life care. Therefore, improvement and campaign of nutrition labeling for consumers at expressway rest areas, especially for the NANL group, will be effective in identifying nutrition labeling for their health care.

건강증진 프로그램 효과에 영향을 미치는 프로그램 관련 요소 분석 - 문헌고찰을 중심으로 - (A Review for the Factors Affecting the Effects of Health Promotion Programs)

  • To find out more efficient ways of implementing health promotion programs and to determine the factors affecting the results of various interventions, we reviewed 73 articles un the effectiveness of health promotion programs. These include the papers on the smoking, alcohol, drug abuse, nutrition(obesity) and stress management etc. Specific interventions evaluated in this review are education based on the lecture or video shows, health-related event activities, modifications of policy or the environment, health risk appraisal etc. By using KIHASA Line of Korea Institute of Health and Social Affairs, National Congress Library Database MEDLINE, we identified 201 articles published from 1980 to Jun. 1979 and finally selected 73 papers which contain the implementation process, and result of each program. The factors used in the analysis of the programs are (1) characteristics of participants (2) interventions evaluated (3) research design (4) length of programs and evacuation point (7) outcome indices (6) effect of program evaluated by each author. The study results did not prove positive effect of education based on lectures or video shows etc. Rather, it was suggested that lecture-based education has negative effect on the result. Event activity such as contest or health festivals has positive effect, and policy change or environmental change is closely related to the event activity. Also, the result shows that the overall effect of programs fur the students is less than that of the programs for the others. The programs conducted over, 1-year are mere likely to have positive outcomes than shorter ones. And, the outcomes of the programs with controlled research design such as experimental or quasi-experimental study tend to be evaluated inferiorly to those with non-experimental design.

델파이법을 적용한 암연구수준의 평가 (An Application of Delphi Method to the Assessment of Current Status of Cancer Research)

  • Globally, cancer research has been considered one of the most important field of biomedical researches. Recently, in Korea, there are increasing concerns about cancer research and the development of national cancer control programme. For the efficient investment in cancer research at the national level, strategic approach is needed based on the nationwide information about current status of research. However even the basic data on cancer research have not been systematically collected, and are not available when necessary. The aim of this study is to assess current status of cancer research. For this purpose, this study applied two round Delphi method in which fifteen experts in cancer research fields participated. They rated each items on the initial list at the first round, and modified their responses at the second round. Panels responded that pathogenesis of cancer, research & development of cancer drug, and oncogene, etc. are the most urgent and important research Holds. They assessed national level of cancer research as being 49.6% of the world highest level. Coefficient of variation tended to be lowered with the iteration. Predictive stability was evaluated to be lower in items of urgency than in items of importance and research level. Although this study shares the same limitations in the selection of the experts with many other Delphi studies, it provides a primary data that would be required to plan the national strategy of the cancer research.

의약품 선별등재제도 도입 전후 건강보험 등재의약품의 특성별 현황 (Changes in the List of Drugs Covered by National Health Insurance after the Introduction of Positive List System in Korea)

  • This study aimed to identify the changes in the list of drugs covered by national health insurance(NHI) after the introduction of positive list system (PLS) in Korea in December, 2006. Six-year (pre-policy:2004-2006, post-policy:2007-2009) lists of the NHI reimbursable drugs filed from Health Insurance Review and Assessment Service (HIRA) were analyzed. The total number of listed drugs as well as drug ingredients, and the average number of listed drugs per manufacturer decreased annually after the introduction of PLS. More than 8,000 drugs were delisted in the year 2007 right after the policy change. Prescription-only drugs occupied more than 85% of NHI listed drugs. The percentage of oral type of listed drugs has been increased to more than 60% after the policy change. Korean pharmaceutical manufacturers occupied more than 90% of listed drugs than multinational firms. The gap between Korean and foreign manufacturer in terms of the average number of newly listed drugs per manufacturer in each year has decreased two years after the PLS (Korean 7.7 vs. foreign 6.3 in 2009) as the price negotiation power of foreign firms has increased. The total number of listed drugs is expected to decrease in the future as the Korean government makes an effort to delist the unnecessary drugs that do not show cost-effectiveness.
