Incidence and Related Factors of the Metabolic Syndrome in a University Hospital (대학병원 종합검진센터 수진자의 대사증후군 발생과 관련요인)

  • Objectives: The purpose of this research was to investigate incidence and related factors of the MS in a university hospital Methods: This survey was conducted from January in 1997 to March in 2008. This study is a cohort study. The baseline subjects were 529 men and 609 women without metabolic syndrome(MS) who visited the health promotion center of Chungnam National University Hospital for general health check-up. The MS was diagnosed according to the definition by the NCEP ATP III. The abdominal obesity guidelines for waist circumference applied the Korean Society for the Study of Obesity Guideline Results: The survey exhibited that baseline risk factors that mainly affect odds ratio(OR) of the MS at follow-up were sex, ages, heavy drinking, uric acid and transpeptidase(GGT) in women. Incidence of the MS was 10.6% in total cases, 18.1% in men and 4.0% in women. The risk of the MS was significantly lower in women than men(OR=0.4, 95% confidence interval(CI: 0.2-0.9), significantly higher 50-59 Ages than < 40 ages(OR=3.4, 95% CI: 1.9-6.4), significantly high in women with heavy drinkers(OR=14.3, 95% CI: 1.2-177.5), significantly high in whole, men and women with high uric acid group(whole, OR=2.6, 95% CI: 1.6-4.2. men, OR=2.1, 95% CI: 1.3-3.5. women, OR=14.2, 95% CI: 3.0-68.3) and significantly high in women with high GGT(OR=1.8, 95% CI: 2.4-12.6). Conclusion: Risk factors of incident MS were baseline sex, ages, heavy drinking in women, uric acid and GGT in women. These can be utilized as an important index to determine risk factors of incident MS and will be basic data in part of management, education, countermeasure and selection test of the MS.

Association between Health Risk Factors and Mortality over Initial 6 Year Period in Juam Cohort (주암 코호트에서 초기 6년간 건강위험인자와 사망의 관련성)

  • Objectives: This study was conducted to investigate the association between health risk factors and mortality in Juam cohort. Methods: The subjects were 1,447 males and 1,889 females who had been followed up for 68.5 months to 1 January 2001. Whether they were alive or not was confirmed by the mortality data of the National Statistical Office. A total of 289 persons among them died during the follow-up period. The Cox's proportional hazard regression model was used for survival analysis. Results: Age, type of medical insurance, self cognitive health level, habit of alcohol drinking, smoking, exercise and BMI level were included in Cox's proportional hazard model by gender. The hazard ratio of age was 1.07(95% CI: 1.05-1.10) in men, 1.09(95% CI: 1.06-1.12) in women. The hazard ratio of medical aid(lower socioeconomic state) was 1.43(95% CI 1.02-2.19) in women. The hazard ratios of current alcohol drinking and current smoking were respectively 1.69(95% CI: 1.01-2.98), 1.52(95% CI: 1.02-2.28) in women. The hazard ratio of underweight was 1.56(95% CI 1.08-2.47) in men. The hazard ratios of underweight, normoweight, overweight, and obesity were respectively 1.63(95% CI: 1.02-2.67), 1.0(referent), 0.62(95% CI: 0.32-1.63), 1.27(95% CI: 0.65-3.06), which supported the U-shaped relationship between body mass index and mortality among the men over 65. Conclusions: The health risk factors increasing mortality were age, underweight in male, age, lower socioeconomic state, current alcohol drinking, current smoking in female. To evaluate long-term association between health risk factors and mortality, further studies need to be carried out.

Properties of Blood Pressure and Routine Laboratory Test Results by the Status of Smoking and Alcohol Intakes in Male Workers (일부 산업장 남성근로자들의 흡연,음주실태에 따른 혈압 및 혈액검사치의 특성)

  • This study was conducted to estimate the incidence and the degree of cigarette smoking and drinking among working men, and then to investigate the effects on blood pressure, various hematological indices and blood chemistry. The sample consisted of 2,287 male workers who had undertaken a general health check-up during the two-year period from January, 2000 to December, 2001. Such factors as blood pressure, blood glucose, lipid profiles, and liver function tests were determined and then analyzed with respect to the subjects smoking and/or drinking status. The major findings from this study are: 1. The drinking and smoking status have shown that 52.7% of participants were in the habit of both drinking and smoking while 11.6% were not associated with either. On the other hand 25.4% were involved only in drinking and 10.2% only in smoking. In the group smoking over 21 cigarettes per day over 30, the age group occupied the largest proportion at 20.1%. 2. Regarding the relationship between smoking and/or drinking status, and blood pressure, hematology and blood chemistry, the smoking and/or drinking group had significantly higher levels of blood pressure, both systolic and diastolic, Hb & Hct, TG, LDL-C, SGOT, and ${\gamma}$-GTP, than the non-smoking and/or non-drinking group. But HDL-C was significantly lower in the smoking group and significantly higher in the drinking group than the non-smoking/non-drinking group. 3. Regarding amount smoked, a larger number of cigarettes per day was significantly associated with the higher levels of blood pressure, systolic and diatolic, TG, TC, LDL-C, Hb, Hct, and ${\gamma}$-GTP. As for the amount druck, an increasing amount of alcohol intake was associated with rising levels of blood pressure, systolic and diatolic, TG, TC, LDL-C, HDL-C, Hb, SGOT, and ${\gamma}$-GTP. 4. Regarding the correlation among all the variables stated above, the smoking and drinking amount was shown to be in the positive correlation with blood pressure, both systolic and diastolic, TG, TC, Hb, and ${\gamma}$-GTP. On the contrary, LDL-C and HDL-C was in a positive correlation only with the amount drunk amount, and Hct only with the amount smoked. 5. As with systolic and diastolic blood pressure, the odds ratio of the smoking group was 2.35 and 2.58 compared to the non-smoking/drinking group. whereas it was 1.47 and 1.75 in the smoking/drinking group. Concerning serum lipids, the smoking/drinking group had 1.97 times the levels of TG in the non-smoking/non-drinking group, though the smoking group had 1.55 times the levels of HDL-C in the non-smoking/non-drinking group. As with liver function test results, the drinking group had 2.50 times and the smoking/drinking group had 4.41 times the levels of ${\gamma}$-GTP in the non-smoking/drinking group. respectively. The above results revealed that smoking and alcohol intake were effected the results of blood pressure and laboratory tests. Specifically, not only the smoking/drinking group but also those only smoking or only drinking were not as desirable as the non-smoking and non-drinking group to the results of blood pressure and laboratory tests.

Comparison of Solo Eating Perception and Dietary Behaviors According to Gender in the Seoul and Gyonggi-do regions (성별에 따른 혼자 식사에 대한 인식과 식행동 비교 - 서울·경기지역을 중심으로 -)

  • This study was conducted to compare solo eating perceptions and dietary behaviors in the Seoul and Gyonggi-do regions according to gender. The study focused on the dietary perceptions, solo eating, eating together, dietary habits, food intake patterns and 38 keywords pertaining to solo eating. Of 411 subjects, 207 (50.4%) were males and 204 (49.6%) were females. The biggest problem that respondents perceived in their dietary life was irregular eating. Women prepared meals more often than men (p<0.001) and ate more instant and frozen food than men (p<0.05). Both males and females missed breakfast most, and the amount of the population eating breakfast alone ranked second highest. Women had lunch alone more often than men, while men ate dinner alone more often than women (p<0.01). All respondents spent 5~15 min when eating alone. Women avoided excessive smoking and drinking more than men (p<0.001). Men related key words regarding solo eating with negative emotions such as 'lonely', 'boring', and 'feel sorry when I see someone eating alone in a restaurant' more than women. Conversely, women related more positive key words with eating alone such as 'free', 'joy', and 'my own time'. Dietary lives of Koreans have rapidly changed along with social changes. Our research findings will contribute to increased comprehensive menu development and HMR development in the foodservice industry.

Food Habits and Nutritional Status of Men working at Industry (산업체 근로자의 식습관과 영양상태에 관한 연구)

  • This study was carried out to obtain the information concerning nutritional status, including factors of food habits of 293 men working at industry in Ulsan area. The subjects of this study were composed of 152 clerical workers and 141 physical workers. The results of this study can be summarized as follows: Average age of this subjects was 34.7 years (ranging from 21 to 52), the mean height and weight were 171.5 cm and 66.9 kg, corresponding to the BMI of 22.7. The mean food habit score was 4.4 (ranging from 0 to 9.5) and the mean nutrition knowledge score was 4.9 (ranging from 0 to 10.0) which showed significant correlations with food habit score. Dietary intake data showed the average daily intake of energy, protein, calcium and iron were higher than Korean recommended dietary allowances (RDA), vitamin A, $B_1,\;B_2$, niacin, ascorbic acid were lower than RDA. Among the various influential factors, nutrition knowledge, drinking, smoking and prevalence symptoms of subjects correlated with food habits significantly.

The Effects of Dietary Patterns and Apolipoprotein E Phenotype on the Blood Lipid Profiles of Individuals from Cheju Area (제주지역 성인의 Apolipoprotein E Phenotype 분포와 식생활 및 혈청지질 농도의 관련인자 연구)

  • The purpose of this study was to determine the relation between serum lipid profiles, apolipoprotein E phenotype, and dietary patterns in a cross-section of healthy individuals from Cheju-Do. Age, gender, anthropometric measurements, blood pressure, dietary consumption, drinking / smoking habits and menopausal status were surveyed. Total cholesterol, LDL-cholesterol, HDL-cholesterol, triglyceride, fasting blood glucose, and insulin levels were measured from overnight fasting blood. The study involved a total of 286 individuals(147 men and 139 women) between the ages of 20 and 60 years old. All of the subjects were recruited from a population of healthy individuals living in Cheju-Do. The results of the study are as follows : 1) Among the males, those in their 20's had the maximum food intake, while those in their 40's had the minimum food intake. For the females, food intake was the highest for those in their 30's. Energy and nutrient intakes were directly proportional to the amount of food intake. Men in their 30's were heavier than other men and women in their 40's were heavier than other women. The activity index for men in their 20's and 30's appeared to be lower than that of men above 40. The activity index of women in their 20's appeared to be lowest among all aged groups, and the index appeared to increase from the age of 30 onwards. 2) In terms of changes In serum constituents with age, men in their 40's appeared to have the highest levels of serum constituents such as lipids, glucose, and insulin. Men in their 50's showed the highest levels of serum LDL-cholesterol and glucose. Men in their 30's showed peak levels of serum triglycerides. On the other hand, women in their 50's appeared to have peak levels of serum total cholesterol, LDL-cholesterol, and triglycerides. There was no ch:ange with age in HDL-cholesterol and insulin levels for men and women. The percentage of the subjects had the following apo E phenotypes : E3/3, 91.3% ; E3/2, 5.4% ; E4/3, 2.5% ; E4/2, 0.7%. Lee's reserch with Korean female college students showed that the percentage of ApoE3/3, E3/2, E 4/2, E4/3, and E4/4 were 84.8%, 6.7%, 6.7%, 0.9%, 0.9%, respectively. The number of samples with ApoE mutation was so small that there was no statistical significance in the relation between apolipoprotein E phenotype and se겨m lipids. 3) To investigate the relati onship between weight and serum constituents, the subjects of this study were divided into three groups by BMI underweight, normal weight, and overweight. The serum constituents of men and women below the age 40 in the overweight groups belonged to the normal domain. On the other hand, serum cholesterol levels of both men and women above the age 40 in the overweight groups remained in the borderline-high region(above 200mg/dl), and the mean value of LDL-cholesterol(above 130mg/dl) and triglycerides of men were above normal. Fasting blood glucose levels also remained in the borderline-high region. Total cholesterol levels of women above the age 40 in the overweight group was in the borderline-high region. Influence of Physical Activity on the Prevalence of Diabetes Mellitus in Korean Male Adults (신체활동이 당뇨병 유병률에 미치는 영향: 성인 남성을 중심으로)

  • Objectives: This study investigated the effect of physical activity of adult Korean males on the prevalence of diabetes mellitus. Methods: This cross-sectional study used the raw data from the 2017 Community Health Survey. The subjects of this study were 74,494 men aged 19-64 years old. The data was analyzed applying the complex sampling design and using SPSS 23.0 Program. Descriptive statistics, Rao-Scott Chi-square tests and multiple logistic regression analysis were all performed. Results: The prevalence of diabetes mellitus was 6.4%. The walking activity was 58.5% and the over moderate intensity physical activity was 27.5%. After adjusting for age, marital status, educational level, household monthly income, drinking and smoking status, and BMI, those subjects without moderate intensity physical activity had a 1.14 times higher prevalence of diabetes mellitus (95% CI = 1.05-1.24) than those men with moderate intensity physical activity. Conclusion: In order to decrease the prevalence of diabetes mellitus among adult men, regular physical activities over a moderate intensity level should be emphasized and encouraged in daily life.

The Effects of Dietary Carbohydrate on Serum Triglyceride Concentrations in Korea (우리나라 성인의 당질섭취가 혈청 중성지방수준에 미치는 영향)

  • The relationship between dietary carbohydrate (CHO) and fasting triglyceride (TG) concentrations was examined in 2,689 men, 1,908 premenopausal women, and 966 postmenopausal women aged 20-69 years in the 1998-Korean Health and Nutrition Survey. The serum TG concentrations in fasting blood samples were measured and dietary CHO intake was assessed by using 24-hour recall method. As CHO intake increased, the TG concentrations were significantly affected (127.4 mg/dl, 132.4 mg/dl, 134.8 mg/dl, 142.6 mg/dl, 147.2 mg/dl) in postmenopausal women, while CHO intake was not significantly associated with TG concentrations in men and in premenopausal women. For the lowest to the highest quintiles of CHO, the mean TG concentrations were 120.0 mg/dl and 140.3 mg/dl in postmenopausal women with body mass index (BMI) <25 kg/m$^2$ and 135.6 mg/dl & 155.3 mg/dl in postmenopausal women with BMI$\geq$25 kg/m$^2$. In multivariate analysis, BMI, waist hip ratio (WHR), glucose, smoking, and drinking habit were associated with serum TG concentrations in men. In premenopausal women, BMI, WHR, and glucose levels were associated with serum TG concentrations, while age, BMI, WHR, glucose levels, and CHO intake were associated with serum TG concentrations in postmenopausal women. In conclusion, CHO intake is related to serum TG concentrations in postmenopausal women in Korea.

Comparison of Physical, Psychosocial and Cognitive Factors, and Depression between Middle-aged Women and Middle-aged Men (중년남성과 중년여성의 우울과 신체적, 사회·심리적, 인지적 요인 비교)

  • Purpose: The purpose of this study is to provide basic data for nursing programs in order to reduce depression among middle aged adults by looking at gender differences. Method: A comparative descriptive design was used. Subjects were 315 adults from February. 15 to March. 2, 2005, using a structured questionnaire. The data was analyzed using $\chi^{2}$-test, Pearson correlation analysis and stepwise multiple regression. Results: $\chi^{2}$-test revealed significant gender differences in socio-demographic variables for career, drinking, smoking, and satisfaction with the spouse. The factors that influenced depression for men were stress, fatigue, job satisfaction and self-efficacy. For women, those factors were stress, family support, coping skills, and well-being. Conclusions: The results show that gender differences existed in terms of depressions, in terms of factors influencing depression. As a result, when middle aged adults are provided nursing programs, gender differences should be considered. To decrease the depression of men, nursing interventions are needed to decrease their stress and fatigue, and to improve their job satisfaction and self- efficacy. To decrease the depression of women, it is also needed to decrease their stress, to elevate their family support and coping skills, and to support them so that their well-being is perceived good.
