Clinical Practice Guidelines for Acute Pancreatitis (급성 췌장염 진료 권고안)

  • Acute pancreatitis is a common gastrointestinal disease that is associated with significant morbidity and consumes enormous health care resources. As such, it requires up-to-date evidence-based diagnosis and standard treatment guidelines with broad support from the clinician. Korean Pancreatobiliary Association has developed clinical practice guidelines for the diagnosis and treatment of acute pancreatitis to provide a framework for clinicians to manage acute pancreatitis and to improve national health care. The guidelines were divided into four parts: the diagnosis of acute pancreatitis, the assessment of the severity, the initial management, and the treatment of necrotizing pancreatitis and local complications of acute pancreatitis. In this article, we summarize and present the diagnosis and treatment guidelines for acute pancreatitis established in Korea.

  • The purpose of this study is to discuss several approaches to addressing naso-orbito-ethmoidal (NOE) fracture. Orbital fracture, especially infraorbital fracture, can be treated through the transconjunctival approach easily. However, in more severe cases, for example, fracture extending to the medial orbital wall or zygomatico-frontal suture line, only transconjunctival incision is insufficient to secure good surgical field. And, it also has risk of tearing the conjunctiva, which could injure the lacrimal duct. Also, in most complex types of facial fracture such as NOE fracture or panfacial fracture, destruction of the structure often occurs, for example, trap-door deformity; a fracture of orbital floor where the inferiorly displaced blowout facture recoils to its original position, or vertical folding deformity; fractured fragments are displaced under the other fragments, causing multiple-packed layers of bone.

  • Pollen-food allergy syndrome (PFAS) is an immunoglobulin E-mediated immediate allergic reaction caused by cross-reactivity between pollen and the antigens of foods-such as fruits, vegetables, or nuts-in patients with pollen allergy. A 42.7% prevalence of PFAS in Korean pediatric patients with pollinosis was recently reported. PFAS is often called oral allergy syndrome because of mild symptoms such as itching, urticaria, and edema mainly in the lips, mouth, and pharynx that appear after food ingestion. However, reports of systemic reactions such as anaphylaxis have been increasing recently. This diversity in the degree of symptoms is related to the types of trigger foods and the characteristics of allergens, such as heat stability. When pediatric patients with pollen allergy are treated, attention should be paid to PFAS and an active effort should be made to diagnose it.

  • It is necessary to produce properly educated Manual Lymph Drainage (MLD) specialists conducted by certified instructors. This study was conducted to introduce a comprehensive understanding, application, historical background, and effect of the MLD and to present a basis for using the frequency MLD as a basic resource for training professional. MLD, a field of manual therapy, is used to treat a variety of diseases such as joint disease, circulatory system disease, lymphedema, and pain et al. However, since there is no proper education for MLD in Korea, there is a need to provide education that meets internationally accepted standards. The Physical Therapists Association and related academic societies will need to prepare a system to train specialists on MLD and to recognize medical insurance through it.

An Evaluation of Thermal Comfort of New Towns in Seoul Metropolitan Area (수도권 신도시의 열쾌적성 평가)

  • This study assessed the thermal comfort of new towns in the Seoul Metropolitan Area (Ilsan, Bundang, Dongtan1) using PET (Physiologically Equivalent Temperature) which refers to real human heat stress. The relationship between PET and urban spatial elements was also analyzed using multiple regression analysis. The study results show that the thermal comfort of Dongtan 1, which is considering a reduction of the urban heat island effect in the planning phase, is higher than other cities. In addition, through regression results, the impervious ratio, floor area ratio, commercial area ratio, and residential area ratio were found to be major factors increasing PET. Moreover, the river area ratio and NDVI were found to be major factors decreasing PET. This study has scientific significance as research that focuses on the assessment of thermal comfort scientifically and definitely, by estimating PET for an entire urban area using GIS analysis that included remote sense analysis and the wind field model. The results of this study can be used in preparing more effective urban plans for the promotion of citizen thermal comfort.

  • The purpose of this study is to increase the usage of satisfaction of outdoor exercise equipment installed at parks when plans and designs are made for renovation or formation of parks. In order to do so, the study surveyed and analyzed the usage satisfaction of 150 men and women who use outdoor exercise equipment in neighborhood parks located in Dongtan New Town. There were four factors that affect the variables of usage satisfaction. They can be defined as the main facility factor, awning factor, maintenance factor, and additional facility factor. From the result regression analysis of the total satisfaction rate and the factors, the main facility factor was the most influential of the variables. This could indicate that the facility factor affects the satisfactory rate of outdoor exercise equipment. This research is done only with the examples of neighborhood parks, so it limits the possibility of analysis of seasonal use. It is necessary to conduct analysis research of parks which installed many outdoor exercise equipment, such as a vest-pocket park, children's garden, and pedestrian/urban/metropolitan neighborhood parks.

  • According as zone of life of big city is expanded by new city development etc.., need special skill master plan compensation which can be systematic for wide area traffic problem solution and confusion expense minimization in the metropolitan area because wide area of capital region traffic is gone continuously and continues. Accordingly, Kyonggi Province suggested rapid-transit railway achievement that can connect Seoul Gangnam in 20 minutes with Dongtan new city recently. MLTM(Ministry of Land Transport and Maritime Affaris) announced "Capital region railway network improvement plan research services (2007.12)" result that Gangnam High-speed railway route (Suse $\sim$ Dongtan $\sim$ Pyeongtaek) construction for offer High-speed railway service to capital region and Kyonggi southern part area inhabitantses and need to line capacity tribe solution by Seoul-Busan high-speed railway second-stage and Honam high speed railroad completion. Is judged that need examination about wide area rapid-transit railway combined application operation that take advantage of rail track reserve capacity of High-speed railway for utilization efficiency elevation of country and efficient investment of national finance according as High-speed railway and Gangnam rapid-transit railway route that is suggested in Kyonggi Province are overlaped. Therefore, in this research, I wish to quote investment efficiency plan of railroad business by that different kind's train is run in uniformity track by presenting combined application operation plan and working expenses curtailment effect etc. that is optimized through analysis of roadbed and E & M technology condition, intermediate station plan, train operation planning etc. in case of wide area rapid-transit railway and high-speed railway run combined application.

Development of the Korean Developmental Screening Test for Infants and Children (K-DST)

  • Background: Most developmental screening tools in Korea are adopted from foreign tests. To ensure efficient screening of infants and children in Korea, a nationwide screening tool with high reliability and validity is needed. Purpose: This study aimed to independently develop, standardize, and validate the Korean Developmental Screening Test for Infants and Children (K-DST) for screening infants and children for neurodevelopmental disorders in Korea. Methods: The standardization and validation conducted in 2012-2014 of 3,284 subjects (4-71 months of age) resulted in the first edition of the K-DST. The restandardization and revalidation performed in 2015-2016 of 3.06 million attendees of the National Health Screening Program for Infants and Children resulted in the revised K-DST. We analyzed inter-item consistency and test-retest reliability for the reliability analysis. Regarding the validation of K-DST, we examined the construct validity, sensitivity and specificity, receiver operating characteristic curve analysis, and a criterion-related validity analysis. Results: We ultimately selected 8 questions in 6 developmental domains. For most age groups and each domain, internal consistency was 0.73-0.93 and test-retest reliability was 0.77-0.88. The revised K-DST had high discriminatory ability with a sensitivity of 0.833 and specificity of 0.979. The test supported construct validity by distinguishing between normal and neurodevelopmentally delayed groups. The language and cognition domain of the revised K-DST was highly correlated with the K-Bayley Scales of Infant Development-II's Mental Age Quotient (r=0.766, 0.739), while the gross and fine motor domains were highly correlated with Motor Age Quotient (r=0.695, 0.668), respectively. The Verbal Intelligence Quotient of Korean Wechsler Preschool and Primary Scales of Intelligence was highly correlated with the K-DST cognition and language domains (r=0.701, 0.770), as was the performance intelligence quotient with the fine motor domain (r=0.700). Conclusion: The K-DST is reliable and valid, suggesting its good potential as an effective screening tool for infants and children with neurodevelopmental disorders in Korea.

Rationale, Design, and Interim Observations of the Steady Movement With Innovating Leadership for Heart Failure (SMILE HF) Registry: A Multicenter Prospective Cohort Registry for Patients With Acute Heart Failure

  • Background and Objectives: Heart failure (HF) is a leading cause of hospitalization and death worldwide. The Steady Movement with Innovating Leadership for Heart Failure (SMILE HF) aims to evaluate the clinical characteristics, management, hospital course, and long-term outcomes of patients hospitalized for acute HF in South Korea. Methods: This prospective, observational multicenter cohort study was conducted on consecutive patients hospitalized for acute HF in nine university hospitals since September 2019. Enrolment of 2000 patients should be completed in 2024, and follow-up is planned through 2025. Results: Interim analysis of 1,052 consecutive patients was performed to understand the baseline characteristics. The mean age was 69±15 years; 57.6% were male. The mean left ventricular ejection fraction was 39±15%. The prevalences of HF with reduced ejection fraction, HF with mildly reduced ejection fraction, and HF with preserved ejection fraction were 50.9%, 15.3%, and 29.2%. Ischemic cardiomyopathy (CMP) was the most common etiology (32%), followed by tachycardia-induced CMP (12.8%) and idiopathic dilated CMP (9.5%). The prescription rate of angiotensin-converting enzyme inhibitor/angiotensin receptor blockers/angiotensin receptor/neprilysin inhibitor, beta-blockers, spironolactone, and sodium-glucose cotransporter-2 inhibitors at discharge were 76.8%, 66.5%, 50.0%, and 17.5%, respectively. The post-discharge 90-day mortality and readmission rates due to HF aggravation were 2.0% and 6.4%, respectively. Our analysis reveals the current state of acute HF in South Korea. Conclusions: Our interim analysis provides valuable insights into the clinical characteristics, management, and early outcomes of acute HF patients in South Korea, highlighting the current state and treatment patterns in this population.

  • The purpose of this research is to analyze the emotional characteristics of community space with recently constructed apartment complexes. The designated subjects of this research are located in Dongtan the first New Town, with 500 to 1000 households and constructed as single buildings for community space are 14 apartment. The results are as follows: 1)The space for day care planned for children's emotional comfort and stability measured by symbolizing image of nature. Also, allowing children to recognize their own space by decorating the walls with the works of themselves and their friends was shown to break down extraneous feelings against other spaces, and allow intimacy for spaces. 2)The space for the aged was shown to give a secure and friendly feeling by considering the psychological and physical traits of the elderly. It can replenish the depressed bodies of aged people with vigor and stimulate their emotion. 3)The space for indoor exercise utilized various visual facilities, such as graphics and images, to bring out specific areas and create affiliation in stimulating the emotion of residents.
