Fall Risk in Low-Income Elderly People in One Urban Area (도시 빈곤 노인의 낙상발생 위험요인에 관한 연구)

  • Purpose: The purpose of this study was to investigate the factors that increase of the risk for falls in low-income elders in urban areas. Methods: The participants were elderly people registered in one of public health centers in one city. Data were collected by interviewing the elders, assessing their environmental risk factors, and surveying relevant secondary data from the public health center records. For data analysis, descriptive statistics and multiple logistic regression were performed using SPSS version 14. Results: Stroke, diabetes, visual deficits, frequency of dizziness, use of assistive devices and moderate depression were statistically significant risk factors. The comorbidity of chronic diseases with other factors including depression, visual deficit, dizziness, and use of assistive devices significantly increased the risk of falls. From multiple logistic regression analysis, statistically significant predictors of falls were found to be stroke, total environmental risk scores, comorbiditiy of diabetes with visual deficits, and with depression. Conclusion: Fall prevention interventions should be multifactorial, especially for the elders with stroke or diabetes, who were identified in this study as the high risk group for falls. A fall risk assessment tool for low-income elders should include both the intrinsic factors like depression, dizziness, and use of assistive devices, and the extrinsic factors.

  • The purpose of this study was to develop systematic diagnostic system testing easily, rapidly vestibular function of patients suffered from vestibular syndrome such as nausea vomiting, dizzness, ataxia. Diagnostic system composed of rotatory chair system which rotated sinusoidally patients against their vertical axis for purpose of invoking eye movement by vestibulo-ocular reflex and the softwares which storaged eye movement into computer and analyzed eye movement. Rotatory chair system consisted of comfortable chair and DC servomotor with reducer(1:80) by controlled servo in field of nonlinear motor control, double feedback loops system containing velocity feedback loop and position feedback loop was applied to this sever controlled rotatory chair system. Maximum rotatory velocity of rotatory chair was upto 60 degree per second and frequency range was 0.01 to 0.64 Hz. These above results suggest that clinical rotatory chair system may test easily, rapidly vestibular function and diagnose etiology of dizziness, thus giving effective assistance on the treatment of dizziness patients.

A Case Report of a Patient with Lateropulsion in Lateral Medullary Infarction Improved by Korean Medicine Treatment Including Scalp Acupuncture (가쪽쏠림보행을 동반한 외측 연수경색 환자의 두침요법을 병행한 한의치료: 증례보고)

  • The aim of this study was to report the effect of Korean medicine treatment with scalp acupuncture of patient with lateropulsion, dizziness, headache caused by lateral medullary infarction. We treated the patient with scalp acupuncture, acupuncture, herbal medicine. Numeric rating scale (NRS), Korean version of berg balance scale (K-BBS), gait balance evaluation, dizziness handicap inventory (DHI), activities-specific balance confidence scale (ABC) and vestibular disorders activities of daily living scale (VADL) were used to evaluate symptoms. NRS of Lateropulsion, dizziness, headache were decreased. K-BBS score increased from 4 to 56, and in the gait balance evaluation, it returned to normal. DHI, VADL score were decreased and ABC score increased from 0 to 1,300. This case report shows that Korean medicine treatment with scalp acupuncture can be effective in lateral medullary infarction. However, further controlled studies are needed to confirm the effect of scalp acupuncture on such patients.

A Case Report of a Patient with Olivopontocerebellar Atrophy Improved with Chengsimyeonja-tang (청심연자탕으로 호전된 올리브교소뇌위축증 환자 치험 1례)

  • Objectives This study aimed to report significant improvement of dizziness, gait disturbance and dysarthria after treatment with Sasang constitutional medicine treatment on a Taeeumin patient with olivopontocerebellar atrophy. Methods A 53-year-old female diagnosed olivopontocerebellar atrophy about one year ago suffered from dizziness, gait disturbance and dysarthria. The patient was identified as Taeeumin Joyeol pattern and treated with Cheongsimyeonja-tang and Taegeuk acupuncture. The patient's main symptoms were observed using Global Assessment Scale (GAS) during the treatment period. In addition, the Unified Multiple System Atrophy Rating Scale (UMSARS) was used to assess the overall function of patient. Results We observed a significant improvement in symptoms of dizziness, gait disturbance and dysarthria using GAS, and UMSARS score decreased in Part I and II. Conclusions This case showed that Sasang constitutional medicine treatment can be effective treatment for Taeeumin patient with olivopontocerebellar atrophy. We consider that consistent treatment can contribute to improve the patient's quality of life.

Three Cases of Cervical Vertigo Improved by Traditional Korean Medicine Treatment (한의 치료로 호전된 경추성 현훈 환자 치험 3예)

  • The purpose of this study is to evaluate the effectiveness of traditional Korean medicine treatment of cervical vertigo. Three patients were diagnosed as cervical vertigo with correlating symptoms of imbalance and dizziness with neck pain. The diagnosis of cervical vertigo is also dependent on excluding other vestibular disorders on the basis of history, examination, and vestibular function tests. They were treated by acupuncture, transcutaneous electrical nerve stimulation therapy and low-intensity pulsed ultrasound at acupoints and sympathetic ganglion chain of their cervical and upper thoracic region. The evaluation of clinical outcome was done by numeric rating scale (NRS), dizziness handicap inventory (DHI) and neck disability index (NDI), EuroQol-five dimensions questionnaire (EQ-5D) index. After the treatment, the value of their NRS, NDI, DHI was significantly decreased and their EQ-5D index was significantly increased. The traditional Korean medicine treatment at cervical and upper thoracic region could be an effective way to treat cervical vertigo.

A Case Study of Soyang Patient with Post COVID-19 Syndrome Treated with Hyeongbangjihwang-tanggamibang (형방지황탕가미방으로 호전된 코로나-19 후유증 소양인 환자 치험 1례)

  • Objectives This study was aimed to report a significant improvement of patient with post COVID-19 syndrome, diagnosed as 'Soyang type lesser yang wind damage patterns' based on Sasang Constitutional Medicine. Methods The patient was diagnosed as Soyang type and had treatment with Sasang Constitutional Medicine three times everyday from June 13th to July 16th. Clinical improvement was evaluated with the degree of dizziness, migraine, palpitation and nausea caused by COVID-19. Results The symptoms of dizziness, migraine, palpitation, nausea caused by COVID-19 were improved after treated with Hyeongbangjihwang-tanggamibang. Conclusions After the treatment with Hyeongbangjihwang-tanggamibang, the symptoms of dizziness, migraine, palpitation, nausea caused by COVID-19 were disappeared. This suggests that Hyeongbangjihwang-tanggamibang is effective in recovering post COVID-19 syndrome for patient who diagnosed as 'Soyang type lesser yang wind damage patterns' based on Sasang Constitutional Medicine.

Rasch Analysis of the Clinimetric Properties of the Korean Dizziness Handicap Inventory in Patients with Parkinson Disease (파킨슨병 환자에서 한국어판 Dizziness Handicap Inventory의 라쉬 분석에 의한 임상측정 특성 평가)

  • Objectives: The Korean Dizziness Handicap Inventory (KDHI), which includes 25 patient-reported items, has been used to assess self-reported dizziness in Korean patients with Parkinson disease (PD). Nevertheless, few studies have examined the KDHI based on item-response theory within this population. The aim of our study was to address the feasibility and clinimetric properties of the KDHI instrument using polytomous Rasch measurement analysis. Methods: The unidimensionality, scale targeting, separation reliability, item difficulty (severity), and response category utility of the KDHI were statistically assessed based on the Andrich rating scale model. The utilities of the orderedresponse categories of the 3-point Likert scale were analyzed with reference to the probability curves of the response categories. The separation reliability of the KDHI was assessed based on person separation reliability (PSR), which is used to measure the capacity to discriminate among groups of patients with different levels of balance deficits. Results: Principal component analyses of residuals revealed that the KDHI had unidimensionality. The KHDI had satisfactory PSR and there were no disordered thresholds in the 3-point rating scale. However, the KDHI showed several issues for inappropriate scale targeting and misfit items (items 1 and 2) for Rasch model. Conclusions: The KDHI provide unidimensional measures of imbalance symptoms in patients with PD with adequate separation reliability. There was no statistical evidence of disorder in polytomous rating scales. The Rasch analysis results suggest that the KDHI is a reliable scale for measuring the imbalance symptoms in PD patients, and identified parts for possible amendments in order to further improve the linear metric scale.

A Case Report of a Disorientation and Dizziness with Acute Anterior Circulation Region Cerebral Infraction Improved by Korean Medical Treatment: Focusing on Sunghyuangjunggi-san-gamibang (Xingxiang zhengqi san) (성향정기산가미방을 중심으로 한 한의약 치료를 통해 단기간에 호전을 보인 전순환계 뇌경색 후 발생한 지남력장애 및 현훈을 주소로 한 환자 1례에 대한 보고)

  • Introduction: The aim of this study was to report the effect of Korean medicine treatments on a stroke patient with acute cerebral infarction. Method: A 63-year-old male with anterior and middle cerebral artery infarction had symptoms of dizziness and disorientation. The patient was treated with Korean medicine therapy, including the herbal medicine "Sunghyuangjunggi-san-gamibang." Treatment progress was assessed using the Korean version of the Modified Barthel Index (K-MBI) and the Korean dizziness handicap inventory (K-DHI). Results: After 14 days of treatment, the K-DHI score decreased from 78 to 7 and the K-MBI score increased from 18 to 24. The patient's main symptoms were improved after the treatment, and no side effects were observed. Conclusion: Korean medicine treatment, including Sunghyuangjunggi-san, might be a recommended therapeutic option for dizziness and disorientation in stroke patients.

Korean Medicine Treatment for Anterior Inferior Cerebellar Artery Infarction: A Case Report (어지럼증을 주소로 하는 전하소뇌동맥경색 환자에 대한 한방치료 증례보고 1례)

  • Objective: The aim of this study is to report the effectiveness of Korean medicine for a patient with dizziness following anterior inferior cerebellar artery infarction. Methods: The patient was treated with traditional Korean methods including acupuncture, herbal therapy, moxibustion, and vestibular rehabilitation exercise during an admission period of seven days. The patient's dizziness was assessed using a numeric rating scale (NRS) and the Korean Dizziness Handicap Inventory (K-DHI), Korean Activities-specific Balance Confidence (K-ABC), Korean Vestibular Disorders Activities of Daily Living (K-VADL), the modified Rankin scale (mRS), and the Korean version of the Modified Barthel Index (K-MBI). Results: After seven days of combined treatment with traditional methods and vestibular rehabilitation, the patient's dizziness was reduced from NRS 6-7 to NRS 2. In addition, K-DHI decreased from 84 to 22; K-ABC improved from 52% to 78.125%; K-VADL reduced from 175 to 37; the mRS score changed from 4 to 1; and the K-MBI score increased from 86 to 98. No adverse events were observed during treatment. Conclusion: This study suggests that combined therapy of Korean medicine and vestibular rehabilitation can be effective treatment for anterior inferior cerebellar artery infarction patients.

  • ■ Objectives The purpose of this case report is to show the effect of korean medicine on a progressive supranuclear palsy patient with gait disturbance and dizziness. ■ Methods A patient with gait disturbance and dizziness diagnosed as progressive supranuclear palsy was treated with herbal medicine, acupuncture, electro-acupuncture, moxibustion. The period of admission is 16 days and we evauated the improvements of symptoms by the Unified Parkinson's Disease Rating Scale(UPDRS), Numeric Rating Scale(NRS) and Global Assessment(G/A). ■ Results After taking Modified Bosimhwan andbeing treated by acupuncture therapy, improvements of gait disturbance and dizziness are observed. ■ Conclusion This case report proved the effect of Korean medicine on progressive supranuclear palsy.
