Intra-tracheal Administration of the Disinfectant Chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT) in a Pregnant Mouse Model for Evaluating Causal Association with Stillbirth (가습기살균제 CMIT/MIT의 기도 점적투여를 통한 임신마우스의 사산에 대한 영향)

  • Objectives: Recently, a report was published that the humidifier disinfectant CMIT/MIT did not cause developmental toxicity and was not detected in systemic circulation as a result of an inhalation toxicity test. Therefore, this study was carried out to investigate any associations between CMIT/MIT exposure and developmental toxicity using the in vivo apical toxicity test method. Methods: Groups of pregnant ICR mice were instilled in the trachea with chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT) using a visual instillobot over a period of seven days from days 11 to 17 days post-coitum. For the in vivo apical toxicity test method, an $LD_{50}$-based dose-range finding model was applied to decide the dose range for inducing developmental toxicity. Results: Among the groups of 0, 0.1, 0.5, 1.0, and 1.5 mg ai/kg/day CMIT/MIT, the exposure groups of 0.5 mg and 1.0 ai/kg/day CMIT/MIT were estimated to reflect the thresholds for the stillbirth and death of pregnant mice, respectively. The groups of 0.5, 1.0, and 1.5 mg ai/kg/day CMIT/MIT induced stillbirth rates of 2.57, 10, and 53.8%, respectively. Another exposure group of 0.75 mg ai/kg/day CMIT/MIT did not induce any deaths of pregnant mice and resulted in a stillbirth rate of 8% in only one of six pregnant mice. Conclusions: CMIT/MIT can induce stillbirth in pregnant mice. It was also concluded that CMIT/MIT moves through the pulmonary circulation system and then continues on through systemic circulation and the placenta. There is a possibility of stillbirth and other health causalities in humans beyond the lungs caused by CMIT/MIT exposure.

A Study on Microbial Contamination and Disinfection of Ultrasonic Probe in Metropolitan Area (수도권 지역의 초음파 프로브의 미생물 오염도와 소독에 관한 연구)

  • There was a shortage of research reports on sterilization criterion and contamination of ultrasonic probes. Therefore, in this study, we were going to provide a basic study to measure the level of microbial contamination in ultrasonic probes and to investigate the radiographer's awareness of infection. After the scan, samples were collected from the rubber part of the probe by opening a sterile swab (Transport Medium AM608-1S) for medical bacteria collection with the remaining gel removed with a paper towel. Also, the collected samples of bacteria were grown for seven days and then the laboratory was analyzed. Among the total 29 types of microorganisms, Micrococcus luteus 21(26%), Moraxella species 16(20%), Coagulase negative staphylococcus 8(10%), Bacillus species 5(7%), Bicillus circulans 3(5%), Acinetobacter lwoffii 2(2%), and 1 other Candida parapsilosis (1%) a number of bacteria and fungus, was detected. In a disinfectant experiment using LuciPac Pen on the Lumitester PD-30s, we cultured the rubber part of the probe two to three times to measure the bacteria. Bacteria decreased to 97% with Aquanax (alkaline reduced water 100%), 99% with Klarion wash (0.01% sodium hydroxide), 94% with Klarion disinfection (0.01% nitrous acid water), Sterilization was best with Klarion wash (0.01% sodium hydroxide). Therefore, guidelines for cleaning and disinfection of ultrasonic probes was required, and further development of probe-only disinfectants is required.

  • $Listeria$ $monocytogenes$, a fatal food-borne pathogenic bacteria, can form a biofilm on many different supports. The biofilm gives $L.$ $monocytogenes$ more viability and resistance to disinfectants and sterilization procedures.$L.$ $monocytogenes$ formed biofilms on various culture vessels tested in this experiment and showed the maximum amount of biofilm when it was cultured for 4 days at $30^{\circ}C$ in BHI broth. In this study, biofilm formation was stimulated or inhibited by addition of different Kimchi samples. That was not in accordance with the effect of Kimchi on the growth of $L.$ $monocytogenes$.

  • Out of 50 seed samples of Sesamum indicum L. tested, Alternaria sesami and Corynespora cassiicola were detected in 29 samples and 38 samples, respectively. Heavy infection of A. sesami and C. cassiicola caused seed rot and seedling blight on water agar as well as in pots. In case of light infection the seed germinated showing elongation of the radicle and hypocotyl, but soon light brown or dark brown lesions appeared at certain parts of the shoot and root where sporulation of the fungi was observed after prolonged incubation in moisture. Four seed disinfectants were compared for the control of seed-borne infection of A. sesami and C. cassiicola. Complete control of seed-borne infection of C. cassiicola was obtained by seed treatment with Benlate T and Homai and seed treatment with Busan 30 was found superior to control seed-borne infection of A. sesami.

A Study on Water Quality Management Methods of Waterscape Facilities in Accordance Legislation of Water Quality Criteria (수질기준 법제화에 따른 물놀이형 수경시설의 수질관리방안 연구)

  • This study was conducted to propose measures of water quality management as the water criteria for waterscape facilities which have been highlighted as alternatives to wading pools in summer season was legislated. The number of public waterscape facilities has reached 290 sites in Gyeounggi province and 971 sites in South Korea in 2017. The water tank capacity of 80.3 % public waterscape facilities was less than $100m^3$. Facilities with disinfection system were only 6.5 % and facilities with filtration function were also very low at 6.9 %. Most of the waterscape facilities, about 93%, are expected to be vulnerable to complying with revised water quality criteria because they have to be disinfected by handling. Chlorine disinfectants, which are more persistent than ozone or ultraviolet sterilization methods, are more preferred. However, care should be taken when adding disinfectans because hypochlorous acid, which is an effective component of chlorine disinfectant, remains after the disinfection, but it is easily decomposed with time. For this study, ${\bigcirc}{\bigcirc}$ park floor fountain with a capacity of $63m^3$ was selected and the amount of free residual chlorine concentration was measured by injecting a certain amount of chlorine bleach. As a result, it took 5 hours to decrease from the water quality standard of 4 mg/L to 0.04 mg/L. If the waterscape facility is operated for 7 to 8 hours, the chlorine bleach should be re-injected after 5 hours. In addition, the problem of pH increase due to the input of chlorine disinfectant is expected, and the neutralization method using vinegar was proposed.

Characteristics of Natural Organic Matter (NOM) on Han River and Criterion of Enhanced Coagulation (한강원수 자연유기물의 특성분석 및 강화응집 기준 평가)

  • The Disinfectants/Disinfection By-products (D/DBP) Rule proposed by the US Environmental Protection Agency requires the implementation of enhanced coagulation as a control strategy for natural organic matter (NOM) removal and as a means of limiting the formation of all DBPs, i.e., not just the trihalomethanes and haloacetica acids. To control DBP formation, several best available technologies (BATs) were determined for removal of DBPs and DBP precursors. The enhanced coagulation is one of the BATs for DBP precursors removal. Treatment facilities that achieve a specified percent removal of total organic carbon (TOC) prior to the application of a continuous disinfectant or that achieve a residual TOC concentration < 2mg/L prior to the application of a continuous disinfectant are considered to be in compliance with enhanced coagulation. The enhanced coagulation was applied to raw water in Korea, the Han River. Raw water were examined and effects of different raw water qualities on enhanced coagulation were investigated. Three analyses were used for raw water characteristics, water quality measurement, molecular weight distributions, hydrophobic/hydrophilic fractionation. The Han River had the relatively low alkalinity and low organic carbon concentration. The results of molecular weight distributions showed significant portions of low molecular weight organics, which is very different from most water in USA. The alum doses for the required TOC removal guided from USEPA manual were quite low (i.e. 10~30 mg/L alum) for the water, probably due to the specific water quality of the Han River.

The Humidifier Disinfectant Disaster and the Culture of Bureaucratic Organizations (가습기살균제 참사와 관료적 조직 문화)

  • This paper discuses how toxic humidifier disinfectants were produced and distributed by bypassing tightly regulated laws from 1994 when the first humidifier disinfectant was made until 2011 when it was found to be the cause of the untreated interstitial lung disease. Rather than presupposing immoral entrepreneurs or incompetent civil servants as the cause of this disaster, this paper focuses on the bureaucratic character of the corporate organization that sold the humidifier disinfectant and the government that approved it, and shows that communication in these bureaucratic organizations was hampered and thus defective. It emphasizes the fact that the bureaucracy and secrecy of these organizations are the major cause of the humidifier disinfectant disaster. An analysis of the bureaucratic characteristic of the organization can reveal what improvements should be made in the corporation and in the government in order not to recur this disaster.

Characteristics of Bioaerosol Generation of Household Humidifiers by User Practices (가정용 가습기의 사용자 습관에 따른 실내공기 중 바이오에어로졸의 발생특성)

  • Objectives: This study was performed in order to evaluate the generation characteristics of airborne bacteria and fungi while operating a household humidifier, in consideration of user habits. Methods: Microbial samples were collected in a closed chamber with a total volume of 2.76 $m^3$, in which a humidifier was operated according to experimental strategies. A cultivation method based on the viable counts of mesophilic heterotrophic bacteria and fungi was performed. Experimental strategies were divided into three classes: the type of water in the water reservoir (tap water, cooled boiled water); the frequency of filling the reservoir (refill every day, no refill); and the sterilization method (sterilization function mode, humidifier disinfectants). Results: Significant increases in the concentration of airborne bacteria were observed while the humidifier was in operation. The concentration had increased to 2,407 $CFU/m^3$ by 120 hours when tap water filled the reservoir without any application of sterilization, while for cooled boiled water, it was merely 393 $CFU/m^3$ at a similar time point. Usages of disinfectant in the water tank were more effective in decreasing bioaerosol generation compared to sterilization function mode operation. Generation characteristics of airborne fungi were similar to those of bacteria, but the levels were not significant in all experiments. Calculated exposure factor can be used as an indicator to compare biorisk exposure. Conclusion: This study identified the potential for bioaerosol generation in indoor environments while operating a household humidifier. User practices were critical in the generation of bioaerosol, or more specifically, airborne bacteria. Proper usage of a humidifier ensures that any biorisks resulting from generated bioaerosol can be prevented.

Regenerative Endodontic Treatment Without Discoloration of Infected Immature Permanent Teeth Using Retro MTA : Two Case Reports (치수 괴사된 미성숙 영구치에서 Retro MTA를 이용한 변색 없는 재생적 근관치료 : 증례 보고)

  • Regenerative endodontic treatment has the potential to heal a necrotic pulp, which can affect root development in immature teeth. However, several drawbacks and unfavorable outcomes are associated with regenerative endodontic treatment, of which the most significant is coronal discoloration due to the presence of minocycline in triple antibiotic paste and mineral trioxide aggregate (MTA). To prevent tooth discoloration following pulp treatment, the modified triple antibiotics (ciprofloxacin, metronidazole, clindamycin) were used as canal disinfectants and Retro MTA, a $ZrO_2$-containing calcium aluminate cement, was used to seal the canal. Following access cavity acquisition, the canal was copiously irrigated with 2.5% sodium hypochlorite. A modified triple antibiotic paste was then applied to the canal. Once the tooth was asymptomatic (after between 3 and 8 weeks), Retro MTA was carefully placed over the blood clot or a collagen plug. Follow-up radiographs revealed normal periodontal ligament space and root development. In two cases, successful regenerative endodontic treatment of the infected immature tooth, without discoloration, was achieved with disinfection using modified triple antibiotics and Retro MTA sealing.

THE REVIEW OF TRANSMISSION OF INFECTIOUS DISEASE IN HUMAN TISSUE TRANSPLANTATION: PHASE II. ALLOGENIC SOFT TISSUES (동종조직이식술시 전염성질환의 이환가능성에 대한 고찰 II: 동종연조직)

  • Implantation of allografts has increased widely with not only the availability of many allogenic bone but also allogenic soft tissues. The aim of tissue banking is to provide surgeons with safe tissues compatible with their intended clinical application. The incidence of tissue transplant-transmitted infection is unknown and can only be inferred from prospective studies. The possibility of donor-to-recipient disease transmission through soft tissue transplantation can be considered by reviewing the risk associated with other transplanted hard tissues. Viral, bacterial, and fungal infections have been transmitted via transplantation of soft tissue allografts such as skin, cornea, dura, pericardium. fascia lata, and heart valves. Corneas have transmitted rabies, Creutzfeldt-Jakob disease (CJD), hepatitis B (HBV), cytomegalovirus (CMV), herpes simplex virus (HSV), bacteria, and fungi. Heart valves have been implicated in transmitting tuberculosis, hepatitis B. HIV-1 and CMV. CJD has been transmitted by dura and pericardium transplants. Skin has transmitted CMV, bacteria, and fungi. Cadaveric skin, pericardium, dura, and fascia lata have been used in dental patients with intra-oral soft tissue injuries and GBR. This study is review of the considering transmission of infectious disease in allogenic soft tissues and guidelines of reducing the risk. Prior to use, many tissues are exposed to antibiotics, disinfectants, and sterilants, which further reduce or remove the risk of transmitted disease. Because some soft tissue grafts cannot be subjected to sterilization steps, the risk of infectious disease transmission remains and thorough donor screening and testing is especially important.
