  • This study presents a case of diffuse large B cell lymphoma (DLBCL) in a 58-year-old man showing unusual manifestations mimicking chronic osteomyelitis. In this case review, we describe the imaging findings of DLBCL which mimics chronic osteomyelitis and review existing reports regarding the differential diagnosis of bone involvement of lymphoma and osteomyelitis through imaging and laboratory findings and diffusion-weighted magnetic resonance imaging (DWI) such as the advanced MRI sequence.

  • Clear cell sarcoma is rare and difficult to diagnose. Herein, we present a case of clear cell sarcoma in the dorsum of the wrist with MRI findings, including diffusion-weighted imaging, and histopathologic correlation, which was initially diagnosed as giant cell tumor of tendon sheath.

  • Purpose: Advances of magnetic resonance imaging (MRI), especially that of the Ultra-High Field (UHF) MRI will be reviewed. Materials and Methods: Diffusion MRI data was obtained from a healthy adult young male of age 30 using a 7.0T research MRI scanner (Magnetom, Siemens) with 40 mT/m maximum gradient field. The specific imaging parameters used for the data acquisition were a single shot DW echo planar imaging. Results: Three areas of the imaging experiments are focused on for the study, namely the anatomy, angiography, and tractography. Conclusion: It is envisioned that, in near future, there will be more 7.0T MRIs for brain research and explosive clinical application research will also be developed, for example in the area of connectomics in neuroscience and clinical neurology and neurosurgery.

  • Mapping brain structural and functional connections through the whole brain is essential for understanding brain mechanisms and the physiological bases of brain diseases. Although region specific structural or functional deficits cause brain diseases, the changes of interregional connections could also be important factors of brain diseases. This review will introduce common neuroimaging modalities, including structural magnetic resonance imaging (MRI), functional MRI (fMRI), diffusion tensor imaging, and other recent neuroimaging analyses methods, such as voxel-based morphometry, cortical thickness analysis, local gyrification index, and shape analysis for structural imaging. Tract-Based Spatial Statistics, TRActs Constrained by UnderLying Anatomy for diffusion MRI, and independent component analysis for fMRI also will also be introduced.

확산강조영상과 현상확산계수(ADC) 영상을 통한 고환 표피 낭종의 진단: 증례 보고 (Testicular Epidermoid Cyst on Diffusion-Weighted MR Imaging and ADC Map : A Case Report)

  • 고환 표피 낭종은 드문 양성 종양으로, 모든 고환 종양의 1-2%를 차지한다. 양성 질환으로 고환의 보존이 가능하므로 수술 전에 정확한 진단이 가능하다면 불필요한 고환 절제술을 피할 수 있다. 저자는 오른쪽 음낭의 무통성 종괴로 내원한 18세 남자의 고환 표피 낭종 한 례를 경험하여 보고하고자 한다. 종괴는 초음파상 양파환 징후를 보였고 컴퓨터 단층 촬영에서는 종괴 내부에 곡선의 석회화가 관찰되었다. 자기공명영상 T2 강조영상에서 종괴는 내부에 교차하는 저 신호의 환을 동반하는 고신호 강도로 보였다. 확산강조 영상에서 이 종괴는 고신호 강도를 보이고 현상확산계수(ADC) 영상에서는 낮은 값을 보였으며 이와 같은 소견은 두개내 표피 낭종에서 보고된 바와 동일한 것이다. 기존의 자기공명영상에 확산강조 영상과 현상확산계수 영상을 추가로 시행한다면 고환 표피 낭종의 진단에 도움이 될 것으로 기대된다.

Recurrence and Metastasis of Lung Cancer Demonstrate Decreased Diffusion on Diffusion-Weighted Magnetic Resonance Imaging

  • Background: Diffusion-weighted magnetic resonance imaging (DWI) is reported to be useful for detecting malignant lesions. The purpose of this study is to clarify characteristics of imaging, detection rate and sensitivity of DWI for recurrence or metastasis of lung cancer. Methods: A total of 36 lung cancer patients with recurrence or metastasis were enrolled in this study. While 16 patients underwent magnetic resonance imaging (MRI), computed tomography (CT) and positron emission tomography-computed tomography (PET-CT), 17 underwent MRI and CT, and 3 underwent MRI and PET-CT. Results: Each recurrence or metastasis showed decreased diffusion, which was easily recognized in DWI. The detection rate for recurrence or metastasis was 100% (36/36) in DWI, 89% (17/19) in PET-CT and 82% (27/33) in CT. Detection rate of DWI was significantly higher than that of CT (p=0.0244) but not significantly higher than that of PET-CT (p=0.22). When the optimal cutoff value of the apparent diffusion coefficient value was set as $1.70{\times}10^{-3}mm^2/sec$, the sensitivity of DWI for diagnosing recurrence or metastasis of lung cancer was 95.6%. Conclusions: DWI is useful for detection of recurrence and metastasis of lung cancer.

Current Status of Magnetic Resonance Imaging in Patients with Malignant Uterine Neoplasms: A Review

  • In this study, we summarize the clinical role of magnetic resonance imaging (MRI) in the diagnosis of patients with malignant uterine neoplasms, including leiomyosarcoma, endometrial stromal sarcoma, adenosarcoma, uterine carcinosarcoma, and endometrial cancer, with emphasis on the challenges and disadvantages. MRI plays an essential role in patients with uterine malignancy, for the purpose of tumor detection, primary staging, and treatment planning. MRI has advanced in scope beyond the visualization of the many aspects of anatomical structures, including diffusion-weighted imaging, dynamic contrast enhancement-MRI, and magnetic resonance spectroscopy. Emerging technologies coupled with the use of artificial intelligence in MRI are expected to lead to progressive improvement in case management of malignant uterine neoplasms.

Intravoxel Incoherent Motion Magnetic Resonance Imaging for Assessing Parotid Gland Tumors: Correlation and Comparison with Arterial Spin Labeling Imaging

  • Objective: To compare and correlate the findings of intravoxel incoherent motion (IVIM) magnetic resonance (MR) imaging and arterial spin labeling (ASL) imaging in characterizing parotid gland tumors. Materials and Methods: We retrospectively reviewed 56 patients with parotid gland tumors evaluated by MR imaging. The true diffusion coefficient (D), pseudo-diffusion coefficient (D*), and fraction of perfusion (f) values of IVIM imaging and tumor-to-parotid gland signal intensity ratio (SIR) on ASL imaging were calculated. Spearman rank correlation coefficient, chi-squared, Mann-Whitney U, and Kruskal-Wallis tests with the post-hoc Dunn-Bonferroni method and receiver operating characteristic curve assessments were used for statistical analysis. Results: Malignant parotid gland tumors showed significantly lower D than benign tumors (p = 0.019). Within subgroup analyses, pleomorphic adenomas (PAs) showed significantly higher D than malignant tumors (MTs) and Warthin's tumors (WTs) (p < 0.001). The D* of WTs was significantly higher than that of PAs (p = 0.031). The f and SIR on ASL imaging of WTs were significantly higher than those of MTs and PAs (p < 0.05). Significantly positive correlation was found between SIR on ASL imaging and f (r = 0.446, p = 0.001). In comparison with f, SIR on ASL imaging showed a higher area under curve (0.853 vs. 0.891) in discriminating MTs from WTs, although the difference was not significant (p = 0.720). Conclusion: IVIM and ASL imaging could help differentiate parotid gland tumors. SIR on ASL imaging showed a significantly positive correlation with f. ASL imaging might hold potential to improve the ability to discriminate MTs from WTs.
