A Case of Vivax Malaria Complicated by Adult Respiratory Distress Syndrome and Successful Management with Extracorporeal Membrane Oxygenation

  • Complicated malaria is mainly caused by Plasmodium falciparum, but, increasingly, Plasmodium vivax is also being reported as a cause. Since the reemergence of indigenous vivax malaria in 1993, cases of severe malaria have been steadily reported in Korea. Herein, we report a case of vivax malaria complicated by adult respiratory distress syndrome (ARDS) that was successfully managed with extracorporeal membrane oxygenation (ECMO). A 59-year-old man presented at our hospital with fever and abdominal pain, which had persisted for 10 days. On admission, the patient had impaired consciousness, shock, hypoxia and haziness in both lungs, jaundice, thrombocytopenia and disseminated intravascular coagulation, metabolic acidosis, and acute kidney injury. A peripheral blood smear and a rapid diagnostic test verified P. vivax mono-infection. Ten hours after admission, hypoxia became more severe, despite providing maximal ventilatory support. The administration of antimalarial agents, ECMO, and continuous venovenous hemofiltration resulted in an improvement of his vital signs and laboratory findings. He was discharged from the hospital 7 weeks later, without any sequelae.

  • 임상에서 사용하는 진단 검사 장치인 전산화 단층촬영기(CT)를 이용하여 Dicom 파일을 획득하였습니다. Dicom 파일과 3D 프로그램, 3D 프린터로 손가락 보조기(Finger brace)를 제작하였습니다. 손가락 보조기(Finger brace)는 인체를 대상으로 하기 때문에 형태의 정밀도가 매우 중요합니다. 3D Print는 정밀도가 우수하고, 재료가 다양하고, 출력 시간이 짧은 이점이 있습니다. 임상에서는 알루미늄 보호대나 의료기 업체의 손가락 보호대가 한정적입니다. 3D 프린터로 손가락 보조기(Finger brace)를 제작하면, 환자의 손가락 외상, 질병, 변형의 치료에 이용할 수 있는 정밀한 형태의 맞춤형 손가락 보조기(Finger brace)를 환자에게 적용할 수 있을 것이라고 기대합니다.

한국 성인에서 고요산혈증 위험을 예측하기 위한 중성지방-혈당 지수의 유용성 (Usefulness of Triglyceride and Glucose Index to Predict the Risk of Hyperuricemia in Korean Adults)

  • 본 연구는 한국 성인을 대상으로 고요산혈증 위험을 예측하기 위한 중성지방-혈당 지수(triglyceride and glucose index, TyG index)의 유용성을 평가하였다. 서울지역 종합병원에서 2017년부터 2019년까지 건강진단을 실시한 20세 이상 남성 14,266명, 여성 9,033명을 대상으로 하였다. TyG 지수에 따른 고요산혈증 발생 위험도는 로지스틱 회귀분석을 실시하였으며, TyG 지수의 고요산혈증 위험 예측능력을 확인하기 위해 ROC 곡선을 구하였다. 고요산혈증을 예측하기 위한 TyG 지수의 정확도는 0.68이며, 남성 0.61, 여성 0.67이었다(각각 p<0.001). TyG 지수의 고요산혈증 발생 위험은 1사분위수보다 4사분위수에서 1.69배 높았으며, 남성은 2.03배, 여성은 2.07배 높았다(각각 p<0.05). 따라서 TyG 지수는 고요산혈증의 선별검사로서 진단적 유용성은 높지 않았으나, TyG 지수와 고요산혈증간에는 관련이 있었다.

A novel method of objectively detecting tooth ankylosis using cone-beam computed tomography: A laboratory study

  • Purpose: The aim of this study was to objectively detect simulated tooth ankylosis using a novel method involving cone-beam computed tomography (CBCT). Materials and Methods: Tooth ankylosis was simulated in single-rooted human permanent teeth, and CBCT scans were acquired at different current levels (5, 6.3, and 8 mA) and voxel sizes (0.08, 0.125, and 0.2). In axial reconstructions, a line of interest was perpendicularly placed over the periodontal ligament space of 21 ankylosed and 21 non-ankylosed regions, and the CBCT grey values of all voxels along the line of interest were plotted against their corresponding X-coordinates through a line graph to generate a profile. The image contrast was increased by 30% and 60% and the profile assessment was repeated. The internal area of the resulting parabolas was obtained from all images and compared between ankylosed and non-ankylosed regions under different contrast enhancement conditions, voxel sizes, and mA levels using multi-way analysis of variance with the Tukey post hoc test(α=0.05). Results: The internal area of the parabolas of all non-ankylosed regions was significantly higher than that of the ankylosed regions(P<0.05). Contrast enhancement led to a significantly greater internal area of the parabolas of non-ankylosed regions (P<0.05). Overall, voxel size and mA did not significantly influence the internal area of the parabolas(P>0.05). Conclusion: The proposed novel method revealed a relevant degree of applicability in the detection of simulated tooth ankylosis; increased image contrast led to greater detectability.

A home-based exercise program for temporomandibular joint osteoarthritis: pain, functionality, and joint structure

  • Objectives: Osteoarthritis (OA) is the most prevalent and disabling joint disease in the world. Temporomandibular joint (TMJ) exercise is a widely used treatment and could be a beneficial and long-term tool for treating TMJ OA. The present study aims to evaluate the effects of therapeutic exercise in the conservative treatment of TMJ OA. Materials and Methods: A single-group experimental pre-post test was performed. We included patients who met the diagnostic criteria for TMJ OA. Outcome variables were pain intensity (visual analogue scale), functionality (Helkimo index), and structural changes (ultrasound). Follow-up periods were at months 1, 3, and 6. The intervention included a home-based program with thermotherapy, manual therapy, and therapeutic exercise during the entire follow-up period. Results: We included 15 patients and 26 joints, all women with a median age of 57 years (range, 49-62 years). Median change in pain intensity on joint palpation, mouth opening, and at rest at the first month was 47.5 mm, 51 mm, and 60 mm, respectively, and 48 mm, 49.5 mm, and 42.5 mm, at six months (P=0.001). The Helkimo index showed significant improvement in medians from baseline severe dysfunction (17 points) to minimal dysfunction at three and six months (2 points) (P=0.001). Ultrasound showed improved disc position. Conclusion: This study demonstrated significant improvements in pain, function, and joint disc position and represents a valuable tool for the long-term treatment of patients with TMJ OA.

Evaluation of Proposed Diagnostic System for Detection of Pan-enterovirus Using Reverse Transcription Nested PCR from Water Environment

  • Pan-Enterovirus (Pan-EV) infects millions of children and infants worldwide every year. As severe infections have recently been reported, the need for monitoring has consequently intensified. Pan-EV is a categorical name for waterborne enteroviruses belonging to the Picornaviridae family, and includes a wide range of pathogens including Coxsackievirus (CoxV), Echovirus (EcoV) and Enterovirus (EV). In this study, we proposed an optimal RT-nested PCR method for diagnosis of various types of Pan-EV in an aquatic environment and developed a positive control. Considering detection sensitivity, specific reaction, and final identification, one condition capable of amplifying 478 bp among the four candidates in the 1st round PCR (RT-PCR) and one condition in the 2nd round PCR (nested PCR) were selected. Through the detection of nucleic acids extracted from 123 groundwater samples and the detection sensitivity test based on artificial spiking in the sample, the methods are optimal for non-disinfected water samples such as groundwater. We developed a positive control for Pan-EV detection that can be amplified to different sizes under the two conditions. Accuracy could be further improved by testing for contamination from the control group. The method proposed in this study and the positive control developed are expected to be used in monitoring Pan-EV in aquatic environments including groundwater through future research using more samples.

Could Urinary Copper/Zinc Ratio Be a Newer Tool to Replace 24-Hour Urinary Copper Excretion for Diagnosing Wilson Disease in Children?

  • Purpose: Although the 24-hours urinary copper excretion is useful for the diagnosis of Wilson disease (WD), there are practical difficulties in the accurate and timed collection of urine samples. The purpose of this study was to verify if the spot morning urinary Copper/Zinc (Cu/Zn) ratio could be used as a replacement parameter of 24-hours urinary copper excretion in the diagnosis of WD. Methods: A cross-sectional study was conducted at the Department of Pediatric Gastroenterology and Nutrition, Bangabandhu Sheikh Mujib Medical University (BSMMU), Dhaka, Bangladesh, from June 2019 to May 2021 on 67 children over three years of age who presented with liver disease. Twenty-seven children who fulfilled the inclusion criteria for WD were categorized into the test group, and the remaining forty children were considered to have non-Wilsonian liver disease and were categorized into the control group. Along with other laboratory investigations, spot morning urinary samples were estimated for the urinary Cu/Zn ratio in all patients and were compared to the 24-hour urinary copper excretion. The diagnostic value of the Cu/Zn ratio was then analyzed. Results: Correlation of spot morning urinary Cu/Zn ratio with 24-hours urinary copper excretion was found to be significant (r=0.60). The area under ROC curve with 95% confidence interval of morning urinary Cu/Zn ratio measured using 24-hours urine sample was 0.84 (standard error, 0.05; p<0.001). Conclusion: Spot morning urinary Cu/Zn ratio seems to be a promising parameter for the replacement of 24-hours urinary copper excretion in the diagnosis of WD.

Accuracy and time efficiency of conventional and digital outlining of extensions of denture foundation on preliminary casts

  • PURPOSE. The purpose of this diagnostic study was to assess the accuracy and time efficiency of a digital method to draw the denture foundation extension outline on preliminary casts compared with the conventional technique. MATERIALS AND METHODS. A total of 28 preliminary edentulous casts with no anatomical landmarks were digitized using a laboratory scanner. The outlining of the entire basal seat of the denture was performed on preliminary casts and digitized. Casts with no extension outline were digitized and outlines were drawn using software (DWOS, Straumann). The accuracy of the extension outlined between both techniques was evaluated in the software (GOM Inspect; GOM GmbH) by file superimposition. Specificity and sensitivity tests were applied to measure accuracy. The paired t-test (95% CI) was used to compare the mean total area and the working time. RESULTS. The accuracy ranged from 0.57 to 0.92. The buccal and labial frenulum showed a lower value in the maxilla (0.57); while the area between the retromolar pad and buccal frenulum (0.64) showed a lower score in the mandible. The maxillary denture foundation and the working time for both arches were significantly longer for the digital method (P < .001). CONCLUSION. The denture foundation extension outline exhibited a sufficiently excellent accuracy for the digital method, except for the maxillary anterior region. However, the digital method required a longer working time.

Noninvasive Prenatal Diagnosis using Cell-Free Fetal DNA in Maternal Plasma: Clinical Applications

  • 현재 사용되고 있는 침습적 산전진단법(양수천자, 융모막샘플링)은 1-2%의 태아 손실이 초래되어, 비침습적 산전진단법이 산전진단의 궁극적인 목표로 대두되어 왔다. 1997년 Dr. Lo에 의해서 임신부 혈장 내에 세포 유리 태아 DNA (cffDNA)의 존재가 발견된 후 비침습적 산전진단의 새로운 가능성이 열렸으며, 과거 10년간 이에 대한 연구의 많은 진전을 보여주고 있다. 최근에 cffDNA를 이용한 Hemophilia A와 듀센형 근이영양증 등 반성 유전병(X-linked disorders) 진단에 필수적인 산전태아의 성 판정과 RhD-음성 임신부에서 태아의 RhD유전자 핵형 분석 등이 이미 외국에서 임상적으로 적용되고 있으나, 한국에서는 아직 실용화되지 않고 있다. CffDNA의 임상 사용에는 여전히 많은 제약점이 있으며, 이는 임신부 혈장 내 cffDNA 양에 비해 많은 양의 모태 DNA가 존재하고, 종래에 사용되었던 특이적인 Y염색체 유전자(Y-specific gene)는 남아 태아 임신 시에만 적용된다는 것에 기인한 다. 따라서 모든 태아에 적용할 수 있는 태아 성과 무관한 마커(sex-independent universal fetal marker as internal positive controls)가 요구되며, 이를 이용하여 정확한 태아 DNA를 검출할 수 있다. 본 연구진은 국내 처음으로 임신부 혈장 내에 cffDNA를 이용하여 SRY 유전자, RhD-exon 7, 태아 성과 무관한 DNA마커(universal fetal DNA marker)로써 RASSF1A 유전자를 실시간 중합효소연쇄반응(RT- PCR)을 사용하여 뛰어난 결과를 얻었다. 이는 한국에서 처음으로 성공적으로 시도된 것이다. 연구결과에서 산전 태아 성 판별과 산후 태아의 성이 100% 일치하였으며, 임신 주기별 SRY 수치는 임신이 진행할수록 증가함을 확인할 수 있었다. 따라서 이러한 방법은 혈우병 A, 듀센형 근이영양증, 선천성 부신증식증과 연골 무형성증의 진단과 치료 상담에 이용할 수 있으며 50%에서 침습적인 방법을 줄일 수가 있다. 또한, RhD-음성 임신부 대상으로 태아의 성 판정과 RhD 태아 유전자형을 분석한 결과 RhD-음성 태아를 정확히 검출함으로써 앞으로 기존 양수천자 등 침습적 검사를 대체할 수 있을 것이다. 특히 이는 치료가 필요 없는 RhD-음성 태아에서 RhD-면역글로불린의 예방적 치료를 사전에 막을 수 있어, 임신부 건강을 보호하고 의료 비용을 줄일 수 있는 큰 장점을 가진다. 한국에서 최초로 시도된 임신부 혈장 내 cffDNA를 이용한 본 연구의 성공은 비침습적 산전진단 임상 적용의 새 길을 제시하였다. 따라서 이를 각 유전질환의 산전진단에 유용하게 활용하는 것은 태아와 임신부의 건강 증진과 의료비용 절약 등 개인과 국가에 많은 기여를 할 것으로 사료된다.

흉수의 감별 전단으로 Cyfra 21-1의 진단적 가치 (Diagnostic Value of Cyfra 21-1 in Differential Diagnosis of Pleural Effusion)

  • 연구배경: 양성 질환과 악성 질환의 감별 전단으로 혈청에서의 cyfra 21-1 에 관한 연구는 많이 되고 있으나, 흉수내 cyfra 21-1 의 진단적 의의에 관한 연구는 거의 없는 실정이다. 저자들은 흉수의 원인의 감별 진단으로 흉수의 cyfra 21-1 이 진단적 의의가 있는지를 알아보기 위하여 흉수내 cyfra 21-1 을 측정하였으며 혈청내 cyfra 21-1과도 비교하여 흉수내 cyfra 21-1의 진단적 의의를 알아보았다. 연구방법: 1994년 8월부터 1996년 6월까지 삼출성 흉막염으로 내원하였던 환자 92례를 대상으로 흉수와 혈청의 cyfra 21-1을 측정하였다. 흉수의 원인은 생화학적 검사, 세포학적 검사, 그리고 기관지 내시경 등응로 확진 되었으며, 흉수의 원인 질환에 따라 흉수와 혈청 cyfra 21-1 을 비교하였으며 각각의 민감도와 특이도는 ROC 곡선으로 분석하였다. 결과: 혈청 cyfra 21-1은 폐암군에서 가장 높았으며 양성질환군 및 전이암군과 비교할 때 양성 질환군과는 통계학적 의의가 있었으나, 전이암군과는 통계학적 의의가 없었다. 폐암군의 흉수 cyfra 21-1은 혈청보다 높았고, 각 질병군을 비교하였을 때 혈청에서와 같은 통계학적 결과를 보였다. 혈청과 흉수의 cyfra 21-1의 상관관계는 양성 질환군의 경우 통계학적 의의가 없었으나(r=-0.044, p=0.866), 악성 질환군은 유의한 상관 관계가 있었다(r=0.837, p=0.001). 전체 악성질환군인 경우 혈청 cyfra 21-1의 cut off 치를 2.7ng/mL로 하였을 때 민감도와 특이도는 각각 72.7%, 72.4%였으며, 폐암군은 cut off 치를 2.39ng/mL로 하였을 때 민감도와 특이도는 각각 73.9%, 72.4%로 두 군사이의 cut off 치, 민감도, 특이도는 차이가 없었다. 흉수 cyfra 21-1은 전체 악성 질환군의 경우 cut off 치가 10.74ng/mL 일 때 민감도 및 특이도는 각각 78.7%, 69.6% 이었으며, 폐암군은 cut off 치가 22.25 ng/mL 일 때 민감도 및 특이도는 각각 81.8%, 78.7%였다. 결론: 흉수의 cyfra 21-1 치는 양성 질환보다 악성 질환에 의한 흉막염에서 유의하게 높았으며 특히 원발성 폐암에 의한 악성 흉막염에서 민감도와 특이도가 높아 원발성 폐암에 의한 흉수의 감별진단에 cyfra 21-1을 유용하게 이용할 수 있을 것으로 생각한다.
