수면호흡장애의 진단과 수면다원검사 (The diagnosis of sleep related breathing disorders and polysomnography)

  • Sleep related breathing disorders(SRBDs) are a group of diseases accompanied by difficulties in respiration and ventilation during sleep. Central sleep apnea, obstructive sleep apnea(OSA), sleep-related hypoventilation, and hypoxemia disorder are included in this disease entity. OSA is known to be the most common SRBDs and studies show its significant correlation with general health problems including hypertension, arrhythmia, diabetes, and metabolic syndrome. The diagnostic process of OSA is composed of physical examinations of the head and neck area and also the oral cavity. Radiologic studies including cephalography, CT, MRI, and fluoroscopy assist in identifying the site of obstruction. However, polysomnography(PSG) is still considered the gold standard for the diagnosis of OSA since it offers both qualitative and quantitative recording of the events during a whole night's sleep. The dentist who is trained in sleep medicine can easily identify patients with the risk of OSA starting from simple questions and screening questionnaires. Diagnosis is the first step to treatment and considering the high rate of under-diagnosis for OSA the dentist may play a substantial role in the diagnosis and treatment of OSA which will eventually lead to the well-being of the patient as a whole person. So the objective of this article is to assist dental professionals in gaining knowledge and insight of the diagnostic measures for OSA including PSG.

문항반응이론을 적용한 융합적 사고 및 문제해결 역량진단 도구의 병렬 단축형 개발 : H 대학교를 중심으로 (Development of Parallel Short Forms of the Convergent Thinking and Problem Solving Inventory Utilizing Item Response Theory : A Case Study of Students in H University)

  • The study was conducted to develop two parallel short forms for the Convergent thinking and Problem solving questionnaires which are part of H University's core competency diagnostic tools, based on Multi-Item Response Theory. Item responses of 2,580 students were analyzed using Graded Response Model(GRM) to determine item difficulty and discrimination of each item. The research results are as follows. Two parrallel short tests were developed for the Convergent thinking questionnaire consisting of 12 items which were originally 17 items. Likewise, the Problem solving questionnaire, which originally consisted of 15 questions, was divided into two parallel short forms, each consisting of 9 items. The reliability of the shortened parallel tests was confirmed through internal consistency analysis, and their similarity to the original tests was established through correlation analysis. This study contributed to quality management of competency-based education and programs at H University by developing shortened tests. Based on the results, implications were presented as well as limitations and discussions.

  • This study examines a Song-dynasty book entitled TaeYiKukZieKuaZungMun Kyuk (A Collection of Imperial Medical Service Examination Questions and Answers 太醫局諸科程文格), which is the collection of questions and answers in the state examinations on medicine. This book was compiled by Hah Dae-yim (何大任) who was the vice-principal of the TaeYiKuk (The Imperial Medical Service 太醫局). The book consists of nine chapters. The present study reviewed all the chapters and found a number of significant issues. First, test-takers were required to be highly proficient in the fundamental knowledge of canonic texts of East Asian medicine. Second, pulse diagnosis was emphasized among the four diagnostic methods (四診). Third, herbal medicine formulas are organized according to the fixed structures of Ki Bang (奇方), which contained an odd number of herbs and Wu Bang (偶方), which contained an even numbered herbs), and fixed ratios for mixing various herbs. Fourth, there is a theory for division of therapies in which acupuncture is used for meridian diseases and herbal medicine for organ diseases. Fifth, herbal medicine formulas based on Unki theory (運氣學) are simpler than those of the previous generations. Sixth, the knowledge on the place of origin of herbs was emphasized. Seventh, knowledge of the relationship between herbs was also emphasized. Eighth, Tang (湯) and San (散) were used most frequently as forms of medicine.

전문대학생용 학습전략 진단 척도의 온라인 활용을 위한 재타당화 연구 (Scale Revalidation Study for Online Use of the Learning Strategy Diagnostic Scale for Junior College)

  • 이 연구의 목적은 전문대학생용 학습전략 척도(K-Learning Strategy Scale for College)의 온라인 활용을 위해 학습인지와 학습정서 요인의 문항을 추가하고 재타당화하는 것이다. 전문대학생의 학습전략에 영향을 미칠 수 있는 학습인지, 학습행동, 학습정서의 하위요인을 각각의 항목별로 구체적으로 탐색하고 분석하는 것은 자기성찰과 학업성취도 향상을 위해 중요하다. 추가된 문항은 학습인지 요인의 학습정보처리 과정에서의 주의집중을 진단하는 2개 문항과 학습정서 요인의 타인에 대한 불안수준을 진단하는 대인불안 요인 2개 문항이다. 연구대상 지역은 전국 5개 지역으로 실시하였으며, 연구대상자는 불성실 응답 327명을 제외한 923명의 전문대학생이다. 수정된 K-LSS_r 척도는 학습인지(18문항), 학습정서(15문항), 학습행동(19문항)의 3개 영역 하위요소로 구성된 총 52문항의 전문대학생용 학습전략 진단 척도이며, 본 연구에서 일반화를 위한 신뢰도 검증 결과 척도 전체 Cronbach의 α 계수는 .896이었고, 3개 요인의 Cronbach의 α 계수는 .876에서 .910이었다. 척도의 반분신뢰도 계수는 전체가 .858이었고 3개 요인의 반분신뢰도계수는 .792에서 .843이었다. 5개 지역 소재 대학생 350명을 대상으로 3주간의 검사-재검사 신뢰도 검증 결과는 전체가 .884이었으며, 일반화를 위한 타당도 검증 결과에서 구인타당도가 통계적으로 유의한 정적상관을 나타내었다.

전국 소동물 병원의 초음파 진단기기 및 활용도 현황조사 (Survey on Utilization of Ultrasonographic Machine in Small Animal Clinics in South Korea)

  • This study was carried out to analyze the utilization of ultrasound machines in animal hospitals and focused on surveying the present condition of diagnostic ultrasound on veterinary medicine in South Korea. Total 279 veterinary hospitals were surveyed with e-mail questionnaires or telephone survey. E-mail questionnaires consist of 17 items of questions including existence of ultrasound machine, types of ultrasound machine, ultrasound examination costs, frequency, purposes, other diagnostic imaging equipments, and referring system of ultrasound. Telephone surveys asked about the existence of the ultrasound machine and types of the ultrasound machine to 279 animal hospitals. Two hundred and seventy-one out of 279 animal hospitals holds ultrasound machine. Seventy-two percents clinics purchased used ultrasonographic machines and mean years after the date of manufacture is 7.5 years and the proportion of superannuated machines are relatively high. Also many clinicians prefer single organ scanning rather than general scan technique and more than 60% of clinics perform ultrasonographic examination less than 5 times a week. Clinics located in Seoul area tend to have more expensive and brand-new ultrasonographic machines and the distribution of radiology specialist are higher in this area. Problems associated with the present condition were oversupply of machines, unequal distribution of the medical equipment in different localities, ineffective use of the medical equipment, and high percentage of old poor-quality medical equipments. There should be a viable alternative proposal to control amount and quality of the ultrasound machines. Also, the improved management system for the ultrasound machine is required.

퍼지추론규칙을 이용한 적응형 평가시스템 (An Adaptive Evaluation System Using Fuzzy Reasoning Rule)

  • 본 논문에서는 기존의 LCMS에서 사용되는 평가시스템에 퍼지 추론 규칙을 이용한 적응형 퍼지평가시스템(AFES ; Adaptie Fuzzy Evaluation System)을 제안한다. AFES 는 학습자가 하나의 학습코스(learning course)에 들어가기 전에 퍼지진단평가(fuzzy diagnostic evealuation)를 통해 학습자에게 코스수준(course level)을 부여한다. 학습자는 코스수준에 따른 맞춤식 학습경로(learning path)로 학습을 종료한 후, 퍼지최종평가(fuzzy final evaluation)를 통해 최종성적(final grade)을 AFES 으로부터 부여 받는다. AFES의 가장 큰 특징은 최종성적의 점수 부여 규칙에 있는데, 만약 서로 다른 학습자가 동일한 문제 수에 대하여 같은 수의 정답을 냈더라도, AFES 는 125 가지 퍼지 추론 규칙(fuzzy reasoning rule)에 의거하여 탄력적으로 서로 다른 최종성적을 학습자에게 부여한다.

Evaluation of Anxiety and Depression in Patients with Disc Displacement according to Diagnostic Criteria for Temporomandibular Disorders

  • Purpose: The etiology of temporomandibular disorders (TMDs) is complex and multifactorial including trauma, anatomical, pathophysiological, social and psychological factors. Psychological factors can induce or sustain TMDs in various ways. And psychological problems, such as anxiety and depression can be occurred due to TMDs. Therefore, evaluation of psychological factors in patients with TMDs is important. Although disc displacement (DD) is crucial in clinical situation, most of studies have focused on the relationship of psychosocial factors and myofascial pain. And also, Subtypes of DD can cause different degrees of discomfort, it is necessary to evaluate the psychological states of the patients according to the subtype. The Hospital Anxiety Depression Scale (HADS) is one of the self-report questionnaire to evaluate the psychological factors. HADS-Anxiety (HADS-A) and HADS-Depression (HADS-D) are assessed through 14 questions. The purpose of this study was to evaluate anxiety and depression assessed by HADS in patients diagnosed with subtypes of DD according to diagnostic criteria for TMDs. Methods: Four hundred thirty nine patients were diagnosed as one of the subtypes of DD. One hundred fourty nine subjects with no symptoms were set as control groups. All of them answered the HADS for Koreans. The cut-off score for anxiety and depression was set a score of 8. The chi-square test was performed to evaluate association between DD and anxiety/depression. Results: There was a significant difference in HADS-D between five groups (p<0.01). However, there was no significant difference in HADS-A. All the DD groups showed a significant difference in HADS-D compared with the control group except the DD without reduction without limited opening group. The DD without reduction with limited opening group showed the highest rates in HADS-D (40.4%). Conclusions: Based on the above results, it is necessary to consider the depression in treatment of the patients with DDs.

구강건조증 증상 평가를 위한 설문지의 신뢰도에 관한 연구 (Reliability of a Questionnaire for Evaluation of Dry Mouth Symptoms)

  • 구강건조증은 입안이 마르는 느낌의 주관적 증상을 의미하는 것으로 타액분비의 감소에 따른 불충분한 구강점막의 습윤도에 의해 발생할 수 있다. 그러나 구강건조증의 정도는 실제 측정된 객관적인 타액분비율과 상관관계를 보이지 않는 경우도 있어 그 진단과 치료에 어려움을 야기한다. 본 연구에서는 이러한 구강건조증의 진단과 평가를 위해 구강건조증 환자의 문제 해결을 위한 설문지를 개발하고 설문지의 문항들이 갖는 신뢰도를 분석하여 향후 구강건조증의 연구 및 진단 도구로서의 활용가능성을 알아보고자 하였다. 이를 위하여 구강건조감의 정도를 Visual Analogue Scale(VAS) 척도로 답하는 6 가지 문항과 구강건조감에 따른 행동을 파악하기 위한 4 가지 문항을 개발하고 문항들이 갖는 신뢰도를 평가하기 위하여 구강건조증의 증상을 호소하지 않는 건강한 성인 남녀 88 명(남자 44 명, 평균 $25.6{\pm}3.1$세, 여자 44 명, 평균 $24.3{\pm}2.1$세)을 대상으로 2 회에 걸쳐 질문에 답하도록 하여 다음과 같은 결과를 얻었다. 1. 구강건조감의 정도를 묻는 6 가지 문항의 급내상관계수(intraclass correlation coefficient; ICC) 값은 낮 시간의 구강건조감의 정도(Dry-day)가 0.767, 입안에 침이 적다고 느끼는 정도(Am-sal)가 0.850, 구강건조감으로 인한 일상 생활의 불편감 정도(Eff-life)가 0.791로 매우 높은 신뢰도를 나타내었고, 밤 또는 아침 기상시의 구강건조감의 정도(Dry-PM)가 0.563, 식사시의 구강건조감의 정도(Dry-eat)가 0.674, 음식물을 삼키기 힘든 정도(Dif-swal)가 0.641로 높은 급내상관계수 값을 나타내었다. 2. 6 개의 문항들의 내적 일치도를 평가하는 크론바흐의 알파(Cronbach's alpha) 값은 0.928로 매우 높게 나타났다. 3. 구강건조감에 따른 행동을 파악하기 위한 4 가지 문항들의 코헨의 카파(Cohen's kappa) 값은 잠자리 옆에 물을 준비하게 되는 빈도($H_2O$-bed)는 0.850으로 매우 높은 신뢰도를 나타내었고, 입이 말라 잠을 깨는 빈도(Night-awake)가 0.506, 마른 음식 섭취시에 물을 마시게 되는 빈도(Sip-liq)가 0.419, 껌이나 사탕을 먹게 되는 빈도(Gum-candy)가 0.407로 높은 신뢰도를 나타내었다. 이러한 결과로부터 VAS 척도로 답변하는 주관적 구강건조감의 정도를 묻는 6 가지 질문과 주어진 보기를 선택하여 답변하는 구강건조증에 따른 행동을 묻는 4 가지 질문으로 구성된 설문은 중등도 이상의 신뢰도를 가지고 있고 VAS 척도로 답변하는 6 가지 질문은 매우 높은 내적 일치도를 가지고 있음을 알 수 있었다.

Are Filipino Women in Korea Qualified English Teachers?

  • As the demand of English education is increasing, the demand for Native English speaking teachers (NEST) is rising, especially in Asian countries. However, due to the low number of NEST, the Korean government is suggesting that Filipino Women be used as English teachers as an alternative. This study aims to answer three questions: (1) are Filipino women in Korea qualified to teach English based on the error analysis of their written essays? (2) what are the linguistic features found in their diagnostic essays? and (3) is their written English better than Korean college students' written English based on the comparison of the two groups? The findings from the Filipino participants show the most frequently occurring errors are related to punctuation usage (commas and hyphens), vocabulary (word choice), verb usage, redundancy, and even as basic as capitalization usage. The results from the comparison of the two groups show that the percentage of the Filipino participants' written error was 14% while the percentage of the Korean participants was 17%. The findings would give us some ideas on the qualification of Filipino women in Korea as English teachers.

부산 및 울산지역 일부 대학생들의 에이즈에 대한 지식과 태도 조사 (College Students Knowledge and Attitudes Toward AIDS in Pusan and Ulsan Areas)

  • Given the global impact of the AIDS pandemic, it is necessary to take every measure to prevent an epidemic of this disease in Korea. The only available strategy is prevention. Considering college students as a potential risk group, this study examined their knowledge of, and attitudes toward AIDS. In this study, 399 students from three colleges in Pusan and Uoolsan were examined for their attitudes to, and knowledge of AIDS. Findings indicate that the majority of the students(90%) had basic knowledge on the transmission and the prevention of the disease. However, most of them, including nursing students, lacked knowledge as to the etiological agent, major symptoms and available diagnostic techniques. To the questions designed to examine social attitude towards AIDS patients, the students showed a tendency to view AIDS patients as responsible for their own illness. Interestingly, they showed sympathy to their imaginary friends and relatives suffering from AIDS. The study findings suggest a need for the development of AIDS educational programs for college students.
