Case Report; Esthetic Restorations for Immediate Implantation and Delayed loading on Maxillary Anterior Region (상악전치부임플란트에서의 즉시식립 후 심미수복증례)

  • Successful osseo-integration of dental implants that Dr. Bronemak reported in 1965 had been ground-breaking research in the restorative dentistry for the missing dentition. Clinical application of dental implants in the restorative dentistry has begun with the role of retention and support for the complete denture, beyond the functional recovery in partially missing area, and succeeds in the cosmetic recovery for anterior missing area. Recently, immediate implantation and loading after the extraction have been preferred by many excellent clinicians especially on maxillary anterior missing area, because they want to prevent from the absorption of residual alveolar bone. But it is hard to decide immediate loading for common clinicians also, because it is difficult for them to convict proper osseo-integration. In this article, immediate implantation and delayed loading case on maxillay anterior region have been introduced and predictable prosthetic procedure for the esthetic result has suggested.

Esthetic Restoration of Deciduous Teeth Using Prefabricated Zirconia Crown in Children with Early Childhood Caries: A Report of Three Cases

  • Early childhood caries is a widespread condition that requires attention; however, its treatment remains a challenge in terms of child behavior management. This study describes the usefulness of customized zirconia crowns for the restoration of deciduous teeth through the evaluation of some cases. Three cases are described: a 29-month-old girl who presented with severe early childhood caries affecting anterior tooth, a 50-month-old boy who presented with extensive caries of his anterior tooth, and 70-month-old girl who presented with extensive caries of his primary posterior dentition. These prefabricated, zirconium-based ceramic crowns ($Nusmile^{TM}$ NuSmile) are available in various sizes, shapes and colors. Before the treatment, radiographs and intraoral photographs were taken to evaluate the appropriateness of treatment. Our findings indicate that customized zirconia crowns may be appropriate for restoring the deciduous teeth. Further long-term clinical studies are required to clarify the usefulness of this restorative method.

  • Since January 2019, insurance coverage for caries treatment of permanent teeth using composite resin in children aged 12 and under has started in South Korea. The purpose of this study was to compare the pattern of permanent molar composite resin restoration aged 12 and under before and after the insurance coverage from January 2009 to March 2020 in the Department of Pediatric Dentistry at Kyung Hee University Dental Hospital at Gangdong. Since the insurance coverage was started in 2019, the frequency of permanent molar composite resin restoration aged 12 and under has increased more than twice. There was a significant change in frequency of resin restoration by shape of cavity comparing before and after the insurance coverage, whereas there was no significant difference in changes by patient age, gender and position of tooth. As accessibility to the permanent tooth composite resin restoration is increased by the insurance coverage, which shows a fair survival rate, it is expected that it could be able to secure oral health in the early permanent dentition aged 12 and under.

  • Despite dentistry's attempts to improve the dental health of the public and to minimize the effects of caries, many children still present with extensive destruction of primary anterior teeth. One of dentistry's most challenging tasks is to repair these teeth with restoration which are durable, retentive, and esthetic. Esthetic restoration can often be achieved with polycarbonate crowns, strip crowns, conventional S-S crowns, open-faced S-S crowns, commercially veneered S-S crowns. But, all of these have limitation. Advances in restorative materials and metal-bonding procedures have made possible new restorative techniques that combine the advantages of S-S crowns with the cosmetics of composite restoration methods. The described technique of bonding composite to trimmed and fitted S-S crowns offers many advantages over other techniques currently used to restore primary anterior teeth. 1. If S-S crowns are accurate trimmed and contoured, good retention of crowns is achieved. 2. The patient time required is similar to that of conventional S-S crowns. 3. Good esthetics and high bond strengths are achieved. 4. It is possible to use this veneering technique intraorally on crowns that have fractured veneers.

  • The purpose of this study were to investigate the patterns of prosthetic restorations according to sex and age. We had surveyed the out-patents of dental clinics it Jeon-Ju, Iri and Gun roan area. The abtained result were as follow ; 1. The patterns of prosthetic restoration in female was higher than male and 20$\sim$30 age groups was higher than others. 2. The crown was higher in prosthetic restoration. 3. The procelain appliance area was high in male and female on 20$\sim$29 age. 4. The partial denture was higher than full denture in both sex. 5. The crown appliance area was not different in both sex. 6. The 3 unit bridge was higher than other bridge in both sex. 7. The porcelain appliance area was high in male and female on upper anterior. 8. The inlay appliance area of lower posterior was higher than upper posterior. 9. The upper posterior was high in male, and the lower posterior was high in female in partial denture. 10. The full dentur was high in old age group.

  • Application of ceramic materials for fabrication of dental restoration materials has been a focus of interest in the field of esthetic dentistry. The ceramic materials of choice are glass ceramics, spinel, alumina, and zirconia. The development of yttrium tetragonal zirconia polycrystal (YTZP)-based systems is a recent addition to all-ceramic systems that have high strength and are used for crowns and fixed partial dentures. Computer-aided design/computer-aided manufacturing (CAD/CAM)-produced, YTZP-based systems are popular with respect to their esthetic appeal for use in stress-bearing regions. The highly esthetic nature of zirconia and its superior physical properties and biocompatibility have enabled the development of restorative systems that meet the demands of today's patients. Many in vitro trials have been performed on the use of zirconia; however, relatively fewer long-term clinical studies have been published on this subject. The use of zirconia frameworks for long-span fixed partial dentures is currently being evaluated; in the future, more in vivo research and long-term clinical studies are required to provide scientific evidence for drawing solid guidelines. Further clinical and in vitro studies are required to obtain data regarding the long-term clinical use of zirconia-based restorations.

Understanding and trends of esthetic treatment in prosthodontics : IPS e.max (심미보철 치료의 경향과 이해 : part 1. IPS e.max)

  • With the increase of esthetic demands, most patients want to have restorations which are not only functional but also esthetic. For the esthetic restoration, many ceramic systems have been introduced and applied in dentistry. Among those ceramic restorations, IPS e.max system composed of lithium disilicate glass ceramic is one of the most commonly used systems because it has strength and esthetic characteristics. IPS e.max system is divided into IPS e.max Press and IPS e.max CAD according to the manufacturing methods. IPS e.max Press is fabricated through heat-pressed technique with ceramic ingot, which is very simple. The restorations which are made using IPS e.max system can apply to 3 units restoration for the anterior teeth and premolar, and single posterior tooth restoration. Cementation is one of the most important clinic procedure for the longevity of the restorations. All ceramics are bonded by resin cements, it is classified into three groups including adhesive, self-adhesive, and conventional. Variolink N, which is an adhesive resin cement and manufactured by same company with IPS e.max, is recommended for the bonding of IPS e.max restoration. Conventional and self-adhesive resin cement is also available. The aim of this review article is to provide the understanding of material properties, production procedure and clinical application of IPS e.max system.

  • Conventionally, when a zirconia fixed dental restoration is planned, the interim restoration is made manually and the final restoration is fabricated by the silicone impression taking at the prosthodontic stage. This conventional workflow does not provide direct relation between interim and final restorations. Moreover, the predictability of the final restoration could be low. Nowadays, the CAD/CAM based restoration fabrication and related digital techniques are developed and being applied in dentistry in multiple ways. This case report introduces a digital workflow for fabricating an optimal gingival adaptation and predictability of monolithic zirconia restoration by using CAD/CAM interim restoration, intra oral scan, and image superimposition technique in a case that required tooth extraction.

  • Traditionally, tooth restoration has been carried out by replicating teeth using plaster-based materials. However, recent technological advances have simplified the production process through the introduction of computer-aided design(CAD) systems. Nevertheless, dental restoration varies among individuals, and the skill level of dental technicians significantly influences the accuracy of the manufacturing process. To address this challenge, this paper proposes an approach to designing personalized tooth restorations using Generative Adversarial Network(GAN), a widely adopted technique in computer vision. The primary objective of this model is to create customized dental prosthesis for each patient by utilizing 3D data of the specific teeth to be treated and their corresponding opposite tooth. To achieve this, the 3D dental data is converted into a depth map format and used as input data for the GAN model. The proposed model leverages the network architecture of Pixel2Style2Pixel, which has demonstrated superior performance compared to existing models for image conversion and dental prosthesis generation. Furthermore, this approach holds promising potential for future advancements in dental and implant production.

  • Currently, digital technology is being used in various fields of dental treatment. In particular, in the case of anterior esthetic restoration, the traditional restoration method cannot contain facial information and it is difficult for the patient to predict the treatment result. However, in the case of esthetic restoration through digital design, the visualization of the prosthesis design and the ease of reflecting patient feedback, and expecting the treatment result is available. In this case, the patient confirmed the results of restoration treatment using a digital method before treatment and obtained consent for treatment in an anterior tooth trauma patient. In addition, since the conventional digital smile design method uses only the patient's facial and smile information, the design was made on a two-dimensional plane, and its application was somewhat limited. However, in this case, a three-dimensional virtual patient was created and thus the designed restoration was viewed from various angles. Through this case, it was possible to obtain a high degree of satisfaction with the ease of communication with the patient and the technician during the esthetic restoration using the digital method, the simplicity of the procedure, and the treatment result.
