대한민국에서 2019년 1월부터 만 12세 이하 어린이에 대한 우식 영구치 복합 레진 수복 치료가 급여화 되었다. 이 연구의 목적은 2009년 1월부터 2020년 3월까지 급여화 적용 전후의 강동 경희 치과대학병원에서의 만 12세 이하 영구치 복합 레진 치료 패턴을 분석하는 것이다. 급여화가 시작된 2019년에 만 12세 이하 어린이 대구치 복합 레진 수복량은 2배 이상 증가하였다. 급여화 전후 치료 나이, 성별, 치아의 위치에는 유의한 변화가 없었으나 와동의 형태에서 유의한 변화가 관찰되었다. 수용할 만한 생존율을 보이는 영구치 복합 레진 수복에 대한 접근성이 급여화로 증가함에 따라, 만 12세 이하 어린이의 초기 영구치열에서의 구강 건강이 증진될 것으로 보인다.
소아 및 청소년에서 유구치 및 대구치는 인접면이나 평활면에 광범위한 치아우식증으로 치질이 많이 파괴되거나, 치수치료로 인하여 치질 부위가 약해져서 파절되기 쉬운데, 치료방법으로 기성금속관이 사용되고 있다. 기성금속관은 1950년대 Humphrey에 의해서 소개되었고, 장점으로 내구성이 강하고, 임상적 수명이 길며, 성공률이 높지만, 치질 삭제로 인한 치수노출 위험성과 인접 치아의 삭제의 가능성이 있고, 다수의 치아 수복 시에 교합면 높이를 맞추기 어려우며, 큰 크기의 기성금속관을 선택한 경우 금속관의 변연에 의해 인접 치아의 맹출을 방해할 수 있다는 단점이 있다. 기성금속관의 단점을 보완하기 위한 방법으로 구치부 수복 시 교정용 밴드를 사용하는 것을 고려해 볼 수 있는데, 치아우식증으로 파괴된 치질 부위를 교정용 밴드와 복합레진, 또는 글래스아이오노머, 아말감 등으로 수복하는 방법이다. 교정용 밴드를 이용한 구치부 수복은 치경부와 교합면에 치아우식증이 있어 치질이 많이 파괴되거나, 치수치료한 치아, 법랑질 저형성된 대구치 등에서 사용될 수 있고, 시술 시간이 짧아 행동조절이 어려운 어린이에서 유용하다. 그러나, 치료 후 구강위생상태를 청결히 유지시키는 것이 중요하다. 이 증례들에서는 유구치와 대구치에 광범위한 치아우식증의 치료를 위해 내원한 환아에서 교정용 밴드를 이용한 보존적 수복치료를 시행하여 양호한 결과를 보여 이를 보고하는 바이다.
심한 치아 우식증, 외상, 발육성 결함으로 인한 근관치료 및 광범위한 수복이 필요한 소아, 청소년 시기의 유전치 및 영구 전치부 치료는 소아치과 의사에게 어려운 문제이다. 전치부에 사용되어온 기존의 수복 방법은 치질 손상이 광범위한 경우 유지력과 심미적 한계를 가지고 있으며, 특히 청소년기에 영구 전치를 수복할 경우 심미성과 유지력 뿐 아니라 성장 완료 후 영구 보철물 수복을 위한 전 준비 단계를 고려해야 한다. 본 증례는 소아, 청소년기의 광범위한 전치부 치관 손상에서 섬유 강화형 포스트를 이용하여 심미적 수복을 시도하였으며 양호한 결과를 얻을 수 있었다.
This case report describes a technique in which endodontic treatment and permanent indirect restoration were completed in the same clinical appointment with the aid of a computer-aided design/computer-aided manufacturing (CAD/CAM) system. Two patients were diagnosed with irreversible pulpitis of the mandibular first molar. After access preparation, root canals were located, irrigation was performed until bleeding ceased, and the coronal tooth structure was prepared for indirect restoration. Then, utilizing an interim 3-mm build-up of the endodontic access cavity, a hemi-arch digital scan was performed with an intraoral scanner. Subsequent to digital scanning, restoration design was performed simultaneously with the endodontic procedure. The root canals were shaped using the Race system under irrigation with 2.5% sodium hypochlorite followed by root canal filling. The pulp chamber was subsequently filled with a 3-mm-thick composite resin restoration mimicking the interim build-up previously utilized to facilitate block milling in the CAD/CAM system. Clinical try-in of the permanent onlay restoration was followed by acid etching, application of a 5th generation adhesive, and cementation of the indirect restoration. Once the restoration was cemented, rubber dam isolation was removed, followed by occlusal adjustment and polishing. After 2 years of follow-up, the restorations were esthetically and functionally satisfactory, without complications.
Objectives: The purpose of this study was to investigate the perception and satisfaction of caregivers and parents after being covered by health insurance for light-curing composite resin fillings, and to contribute to the settlement of the expansion and coverage of health insurance. Methods: A survey was conducted on caregivers who visited two dental institutions in the Gyeonggi-do area, and parents who joined internet cafes (online communities) in the Gyeonggi-do area from June 2 to September 2, 2020. 225 responses were collected eventually, and 212 valid responses were used as analysis data. Results: 67.0% of the study subjects were aware of the permanent teeth resin fillings being covered by health insurance and the satisfaction was relatively high (91.5%). There was a statistically significant difference in the perception of permanent tooth resin filling benefits according to the child's oral condition (p=0.025) and the parents' own interest in oral health (p=0.039). Conclusions: Based on the results of this study, it is necessary to increase the accessibility to the initial treatment for dental care of children through active promotion of detailed items of permanent teeth resin fillings.
The purpose of this study were to investigate the chief complain and dental treatment needs in handicapped patient. This study examines treatment records of 1025 patients in free dental clinic for handicapped patients during 10 years from 1999 to 2008. The results were as follows : 5.8 average visit per patient; mean patient age was 25; 544 patients was younger than 20. Handicapped patients classified according to types of disability. Crippled disorder were 19.1%, brain disorder were 4.5%, visual disorder were 3.1%, auditory disorder were 4.1%, speech disorder were 0.9%, mental retardation were 67.1%, and developmental disorder were 25.1%. Performed treatments were 322 scaling, 13 fluoride varnish, 727 preventive resin restoration, 1296 resin restoration, 600 amalgam restoration, 46 GI restoration, 612 extraction, 289 pulp treatment of primary teeth, 75 pulp treatment of permanent teeth and 138 stainless steel crown restoration. Many handicapped patients have some difficulty to dental treatment. They have limited access to dental care, which is compounded by a shortage of skilled dental professionals who are willing to treat these population and financial problems.
The infraocclusion is defined as a condition, in which teeth are present with their occlusal surface below the neighboring teeth, and long after, they reach occlusion. The most common cause of infra-occlusion is thought to be ankylosis. The treatment options for patient with infraocclusion of primary molars are observation, restoration and surgical removal of the affected teeth. We report a case of 8 - year - old boy who visited our clinic for consultation of orthodontic treatment. The patient was diagnosed by multiple infraoccluded primary molars with permanent successors. Surgical extraction were performed on primary molars. After follow-up of 7 months period, #44, 45 were erupted and the degree of posterior open bite was decreased.
The objective of this case report is to introduce a simple technique for simultaneously taking a closed-mouth impression and functionally generated path (FGP) for a full coverage crown restoration. A monolithic zirconia crown was the restoration of choice. An alginate impression of the abutment tooth was taken to fabricate a custom-made closed-mouth impression tray covering the abutment tooth and the adjacent teeth. The tray had an FGP table and an abutment tray in cameo and intaglio surfaces, respectively. The impression was taken with silicone impression material after adjusting the abutment tray and inscribing the FGP using self-curing acrylic resins. Plaster casts were made from the impression, and a zirconia crown was fabricated. The crown was cemented to the abutment tooth with minimal adjustments. This simple technique resulted in a well-fitting crown that accounted for mandibular movements. Using the custom closed-mouth impression tray incorporating an FGP table simultaneously aids in fabricating an accurately fitting restoration that incorporates harmonious mandibular movements using a single impression capture.
A 8-year-old patient presented with a crown-root fracture of the maxillary right central incisor with an oblique subgingival fracture line. A multidisciplinary treatment approach including endodontic treatment, surgical extraction and intraalveolar repositioning was used to gain sufficient crown length of the fractured maxillary incisor. The coronally repositioned maxillary right central incisor was stabilized by a resin wire splint. Apexification using MTA was performed. Resin core and direct resin restoration(Cl IV) on fractured teeth was built up. Clinical and radiographic follow-up of the maxillary right central incisor after 36 months showed no signs of root resorption or pathology and acceptable aesthetics and functions were maintained. Surgical extrusion can be considered as a good treatment modality for young patients.
최근 수복용 치과재료는 물리적 성질의 개선과 함께 빠르게 발전하였고 이에 따라 다양한 재료의 선택이 가능하게 되었다. 유구치 우식 수복을 위하여 아말감, 복합레진, 그리고 기성금속관이 널리 사용되어 왔으며, 최근 물성이 강화된 글라스아이오노머 시멘트가 수복재로서 사용되고 있다. 유구치부 인접면 우식증 치료에 사용되고 있는 수많은 치과용 재료들은 각각 뚜렷한 장단점을 가지고 있지만 이의 생존율에 관한 연구는 많지 않다. 이에 본 논문은 유구치의 2급 수복물에 있어서 생존율을 평가하고, 각 수복물의 종류에 따른 합병증들을 조사하여 비교 해보고자 한다. 본 논문은 700개가 넘는 표본을 조사하여, 현재 소아치과의사들에게 가장 큰 고민 중 하나인 구치부 2급 수복물의 선택에 대해 고려해볼 수 있는 결과를 도출하였다.
