보건(지)소 치과위생사의 구강보건직무교육실태에 관한 단면적 조사연구 (A Cross-Sectional Study on Job Training Course of the Dental Hygienists at the Public Health (sub)Centers)

  • 본 연구는 구강보건관련 직무교육에 대한 실태를 파악하고 효율적인 직무교육프로그램 개발에 도움이 될 수 있는 기초자료를 제공하고자 2004년 4월부터 5월까지 전국 보건(지)소에 근무하는 1,255명의 치과위생사 중 층화무작위추출법에 의해 추출된 380명의 대상자들에게 2003년도 구강보건사업관련 직무교육실태 및 교육과정내용의 필요에 대한 인식도를 분석하기 위하여 대상자들에게 설문을 실시하여 다음과 같은 결론을 도출하였다. 1. 보건(지)소의 치과위생사 집단의 일반적 특성에서 보건지소 근무치과위생사가 61.1%이었고, 보건소 근무 치과위생사가 35.3%이었으며, 연령은 36~40세가 62%로 높은 비중을 차지하였고 경력은 11~15년이 48.4%, 16년 이상이 35.7%, 10년 이하가 15.9%순으로 나타났다. 또한 가장 높은 직급은 의료기사 7급이 85.5%로 나타났다. 2. 2003년 구강보건관련 직무교육 수료자율은 28.9%, 미수료자율은 71.1%이었고 '직무교육이 업무수행에 도움이 되었다'가 65.6%, '보통이다'가 34.4%, '도움이 되지 않았다'가 0%로 업무수행에 도움이 되는 의견이 높았다. 또한 교육받지 않은 이유로는 '행정상 시간상 이유로 신청하지 않았다'가 44.6%, '신청을 하여도 선정이 안 되었다'가 29.3%순이었으며, 치과위생사업무가 아닌 타업무에 종사하여서 교육을 받지 않은 경우도 13.4%로 나타났다. 3. 직무교육의 교육과정내용별 필요에 대한 인식도는 초등학교 예방진료가 4.42, 구강보건교육이 4.41, 유아구강보건사업과 노인구강보건사업이 4.04, 장애인구강보건사업이 3.92, 구강보건기획 평가가 3.85, 구강보건진단이 3.69순으로 나타났다. 그러나 근무기간에 따른 인식도의 차이는 통계적으로 유의하지 않았으며(p > 0.05), 근무지에 따른 인식도의 차이는 구강보건 기획 평가와 구강보건진단에서 보건소와 보건지소 근무치과위생사간에 유의성이 나타났다(p < 0.001). 4. 직무교육의 적당한 시기로는 전반기가 39.8%, 연중 무관하다가 34.4%, 중반기가 12.2% 로 나타났고, 전문치위생사 양성과정이 필요하다는 의견도 79.2%로 나타났다.

치과위생사 직무와 개인의 창의성에 의한 자기평가 연구 (A study on the self-evaluation of job performance and creativity of dental hygienists)

  • Objectives : The awareness of health care need increasing attention from the public along with changing professional human-resources activity. As there are increasing social demand for the roles and professionalism of dental hygienists, the purpose of this study was to examine the job performance, creativity and organizational climate of dental hygienists by their own self-diagnostic evaluation in an attempt to help enhance their work ability. Methods : According to the data released as August 2008 by the Dental Association and public health centers in Gwangju, 735 dental hygienists worked at 425 dental clinics. The subjects in this study were 156 dental hygienists at 85 dental clinics who were selected by proportionate sampling method from among the dental hygienists. After an interview survey was conducted in person, the answer sheets from 132 respondents were analyzed except 24 incomplete ones. The questionnaire used in this study consisted of 49 items. A five-point Likert scale was used and SPSS 12.0 program was analyzed the data. Results : 1. As a result of checking the types of works, treatment cooperation(54%), and oral health education often(45%), They weren't responsible for preventive treatment(42%). 2. When they made a self-diagnostic evaluation of creativity and organizational climate, they put the highest value on their autonomy(2.21), followed by positive attitude(2.10). Out of seven organizational climate, challenging spirits(2.81) ranked highest, followed by reliability(2.66). 3. Major management eopmubyeol individual creativity compared with three business relationships and a positive attitude in the case of the high group received oral health education and care management services in a group doing the most was General office work and office assistance work in the medical assistance group that was doing business primarily. 4. In the relationship between self-assesment score and three performance duties, Positive difference was noted in the group with positive attitude and elevated autonomy in terms of performing oral health education. Conclusions : As a result, individual creativity and organization of the atmosphere, each detail, through analysis of the self-assessment model, developed guide on the investigation by doing a dental hygienist, a unique business promotion and Enlargement of the study suggested the need to be considered is.

맞춤형 방문구강보건사업 현황조사 (A study on the state of customized visiting oral health programs)

  • Objectives : This study was to examine the state of customized visiting oral health programs in a bid to help facilitate the unified operation of the programs and the development of required guidelines. Methods : The subjects in this study were 49 dental hygienists who were professionals responsible for customized visiting health care programs across the nation. Results : 1. Regarding the form of employment of the dental hygienists were investigated many contract and daily workers. 2. As to the possession of equipment necessary for visiting oral health programs, denture cleaners(12.2%) were most widely possessed in some regions, followed by mobile scalers(10.2%) and mobile suctions(8.2%). In terms of expendable devices and materials, dental mirrors, pincettes and explorers were the most widely possessed dental checkup devices, and the most widely possessed oral hygiene supplies were toothbrushes, interdental brushes and denture cleaners. Those devices and materials were in more possession than the other types of devices and materials. The most widely possessed equipment for educational purpose was laptop computers, followed by beam projectors and screens. The most widely possessed teaching materials were dentiform, followed by CD-ROMs. 3. Those whom they visited the most for oral health care service were elderly people, followed by the disabled and patients with chronic diseases. The dental hygienists who went out to visit those people outnumbered the others who stayed at public health centers. Concerning the types of visiting oral health care service, the most prevalent service provided to the elderly included denture cleaning/management, oral massage and preventive treatment against dental caries. The most dominant service provided to the disabled involved education of the oral health care act, preventive treatment against dental caries and toothbrushing by professionals. The most common service offered to patients with chronic diseases was education of the oral health care act and oral health education. The dental hygienists paid a visit to a mean of 5.8 households a day. The average weekly number of households cared by the dental hygienists was 27.3. It took a mean of 37.1 minutes for them to take care of each household. 4. As for satisfaction level with the implementation of the visiting oral health programs, they expressed the greatest satisfaction at teamwork with professionals($3.56{\pm}0.94$), followed by the professionalism of their work($3.21{\pm}0.94$) and workload($3.08{\pm}0.94$). Their satisfaction level with the work conditions required for creative job performance($2.75{\pm}0.98$) and partnership with other institutions($2.64{\pm}1.03$) was below 3.0. In regard to the impact of their characteristics, marital status made a statistically significant difference to satisfaction level with workload. The unmarried dental hygienists were more pleased with their workload than the married ones(p<0.05). 5. As to needs for education for professionalism improvement, they asked for education about visiting oral health care skills the most, followed by education about oral health care for patients with chronic diseases, education of planning/evaluation and education of oral health care for the disabled. Conclusions : The top priority for the vitalization of the programs was the procurement of budget, followed by the procurement of equipment and educational media and the procurement of human resources.

지역사회치위생학 현장(보건소)실습 실태 (Current education status of the community dental hygiene practice)

  • Objectives: This study was obtained to identify current education status of the community dental hygiene practice. Methods: It was designed cross section and self-reported on-line questionnaire(Survey monkey). It was performed probability sampling by targeting 82 dental hygiene schools(each one faculty member) in charge of community dental hygiene curriculum and 254 community health centers's community dental hygienists whom was working at oral health section. The response rate was 60% and 53%, respectively. The questionnaire consisted of time, duration, practice group, evaluation method, and practice contents including 63 learning objectives of dental hygiene. Results: Nearly half of these schools conduct such community field work practice in the spring semester of the junior year. This practice was mainly progressed based on average 4 students as one team per each one school for 7-8 hours a day during the period of more than 5 weeks(p<0.05). However, in case of both school and community health center, almost half of feedback after practice was not achieved and there was a difference in needs for practice education between schools and community health center. Conclusions: We should be considered that a sufficient consultation for the practice environment and its contents between schools and community health centers. It was considered that development of a standardized practice manual reflecting such requirement.

노인 구강건강증진 활동에 대한 치과위생사의 학습경험과 수행경험에 따른 수행가능성 및 필요성 조사 (A study on the performance and necessity of dental hygienists for oral health promotion activities for the elderly according to their learning experiences and performance experiences)

  • Objectives: The purpose of this study was to investigate the awareness of the performance and necessity of oral health promotion activities and seek ways to revitalize the professional role of dental hygienists in oral healthcare for the efficient oral care of the elderly. Methods: Eighty-five dental hygienists in charge of oral health promotion projects at public health centers and 38 dental hygienists in network dental clinical trials were investigated for their learning experience, performance experience, feasibility, and necessity of dental hygienists for general and oral health service items for the elderly. The collected data were analyzed using frequency analysis, chi-square test, and Mann-Whitney U test. Results: The degree of performance possibility according to the learning experience and performance experience of the dental hygienist for the whole body and oral health promotion activity items for the elderly showed that the degree of performance possibility was higher among those with experience compared to the non-experienced person, and it was statistically significantly higher (p<0.05). Conclusions: The dental hygienist's professional oral health service is a necessary system to improve practical knowledge and skills and to provide a wide range of professional oral health services for the elderly.

치과위생사의 루페 사용에 대한 인식 및 태도 (Perceptions and Attitudes toward Use of Loupes among Dental Hygienists)

  • 이 연구의 목적은 치과용 루페 사용에 대한 치과 위생사의 인식과 태도를 조사하는 것이었다. 치과 의료기관 및 보건소에서 일하는 180명의 치과위생사를 대상으로 설문조사를 수행하였으며, 치과임상에서의 루페에 대한 치과위생사의 인식 및 태도에 대해 분석하였다. 전체 응답자 중 4.6%가 현재 루페를 사용한다고 응답했으며, 38.9%는 치과 의료기관에 루페도입이 필요하다고 응답했다. 약 절반에 해당하는 대상자는 루페가 시력 향상(53%)과 선명도 향상 (44%)에 도움이 된다고 응답하였다. 루페 착용에 있어 가장 일반적으로 응답한 단점은 제한되는 시야 (39%)와 전반적인 의존성(38%)이었다. 대부분의 치과위생사들은 치과 진료에서 루페를 사용하지 않았음을 확인했으며, 루페에 대한 관심도는 매우 낮았다. 치위생과정에서의 루페 적용 및 그 효과를 입증하기 위한 더 많은 연구가 필요할 것으로 사료된다.

일부 치과위생사의 대학졸업연한 차이에 따른 희망진로와 현 근무처와의 일치도 및 직업 만족도 조사 (Job consistency and occupational satisfaction of dental hygienists by educational period of college)

  • Objectives : Five years have passed since the departments of dental hygiene with four-year term released their graduates. It is necessary to investigate concordance between desired career and current job and job satisfaction in dental hygienists according to graduation term. Accordingly, a survey on concordance between desired career and current job and job satisfaction in dental hygienists according to graduation term was conducted to obtain a basis necessary for providing right job consciousness to students of departments of dental hygiene. Methods : Of departments of dental hygiene nationwide, a total of four departments consisting of two departments with 3-year term and two departments with 4-year term were randomly selected. Of 683 graduates for 2006-10 of the selected departments, 163 graduates who agreed to participate in this study through telephone and e-mail were chosen as subjects. The data of 159 graduates were analyzed except for 4 graduates who did not respond to job satisfaction. The questionnaire consisted of a total of 17 questions: 5 general questions such as graduation term, and current job and salary, 2 questions for desired career at the time of entrance and graduation, and 10 questions for the satisfaction of current job and working environments. Concordance between the desired career at the time of entrance and graduation and current job was calculated using Kappa-value. A cross analysis was conducted to investigate job satisfaction according to general features and graduation term. A statistical significance was examined using $X^2$-test (p<0.05). Results : The Kappa-value for concordance between the desired career at the time of graduation and the current job was 0.288, showing slightly low concordance. In particular, the Kappa-value was 0.089 in the graduates of departments of dental hygiene with 3-year term, which showed significantly low concordance. The job satisfaction of the graduates was 49.1%, which was higher in the graduates with 4-year term than in the graduates with 3-year term, but was statistically insignificant (p>0.05). The detailed job satisfaction was all higher in the graduates with 4-year term than in the graduates with 3-year term except for distance to a working place, but a statistical significance was only found in working place recognition and regional status (p<0.05 ). Conclusions : This study showed differences in concordance between desired career and current job and job satisfaction between the graduates of departments of dental hygiene with 3-year term and the graduates of departments of dental hygiene with 4-year term. Therefore, the curriculum of department of dental hygiene is required to be more specifically applied in accordance with graduation term. In addition, a further study is required to develop specialized curriculums in accordance with graduation term.

Relationship between the Subjective-Objective Oral Health Status and Oral Health Related Quality of Life in the Elderly

  • The purpose of this study was to analyze the relationship between objective oral health status determined by dentists, self-perceived subjective oral health status, and oral health related quality of life (OHRQoL) in the elderly. The related factors affecting OHRQoL in the elderly were also surveyed. Four hundred and thirty elderly individuals who visited the three public health centers and four dental clinics in Busan were selected by convenience sampling. Twelve dental hygienists investigated the subjective oral health status and OHRQoL using the 14-item Oral Health Impact Profile (OHIP-14) and twentyone dentists examined the objective oral health status, including healthy remaining teeth, treated remaining teeth, functional remaining teeth, missing teeth, and non-treated missing teeth. Data were analyzed using SPSS ver. 12.0. OHRQoL was higher when oral and periodontal status was perceived as healthy, when there was no toothache, no interference in mastication, and when study subjects had the ability of food softening. It was also higher when study subjects had ${\geq}20$ remaining teeth and <9 missing teeth, and were wearing denture. The related factors affecting OHRQoL of the elderly were the type of medical insurance, toothache, ability of food softening, perception of periodontal status, and the number of healthy remaining teeth. There was a significant relationship between the subjective-objective oral health status and OHRQoL in the elderly. A continuous oral health care system aimed at retaining ${\geq}20$ healthy remaining teeth is needed to improve oral health and OHRQoL for the elderly, especially for the elderly receiving medical aid.

Dental Hygienist Coordination System 개발 교육주제 요구조사 (A Survey on the Demand of Education topic for the Development of Dental Hygienist Coordination System)

  • 의료시장의 개방과 더불어 의료기관의 경쟁력확보를 위한 핵심전략으로 환자관리의 중요성이 강조되고 있다. 이 연구는 치과위생사의 업무능력향상과 전문성을 높이기 위해 수행업무인식과 교육요구주제를 조사 분석하여 Dental Hygienist Coordination System을 개발하는데 기초자료로 활용하고자 수행하였다. 비례표본추출법을 이용하여 광주광역시 85개 치과의원에 근무하는 치과위생사 156명을 대상으로 직접 방문하여 설문법을 이용하여 조사 분석한 바, 다음과 같은 결론을 얻었다. 1. 코디네이터교육경험 유무에 따른 교육과정별 우선순위 요구과목으로 서비스분야에서는 코디네이터이론과 실무(유무:상담 및 교육실무), 코디네이터를 위한 기초(유무:의학용어), 코디네이터를 위한 원무관리(유무:건강보험), 시설 및 환경관리(유:환경관리, 무:감염관리)이고, 매니저분야에서는 의료서비스마케팅(유무:의료환경과 고객특성, 의료서비스마케팅), 조직인사관리(유무:병원조직), 회계 및 재무관리(유무:병원회계), 건강보험실무(유:진료비청구관리, 무:진료비산정)으로 조사되었다(p>0.05). 2. 코디네이터교육경험 유무에 따른 교육요구과목 차이를 분석한 결과, 코디네이터이론과 실무 0.14, 건강보험실무와 의료서비스마케팅 0.13이고, 인사관리 - 0.23, 코디네이터를 위한 의료기초 -0.19로 조사되었다(p>0.05). 3. 코디네이터교육경험 유무에 따른 교육요구주제는 경험자군과 비경험자군 모두 상담 및 교육실무로 나타났으며, 교육경험자군과 비교육경험자군의 차이를 보면 고객접점관리 0.25, 고객응대기법 0.24로 조사되었다(p>0.05). 이상과 같은 결론에서 우선순위가 높은 교육과목인 코디네이터 이론과 실무, 건강보험 실무에서 진료비 청구관리를 중심으로 체계적인 교육과정을 포함한 Dental Hygienist Coordination System(D.H.C.S)개발에 필요한 근거자료로 활용될 수 있을 것으로 사료된다.

농촌지역 주민들의 구취실태와 유발요인 (Halitosis and Related Factors among Rural Residents)

  • 본 연구는 농촌지역 주민들의 구취실태를 파악하고 구취에 관련된 요인을 알아봄으로써 구취예방 및 효율적인 구취제거 방안을 마련하는데 기초 자료를 제공하고자 일부 농촌지역의 주민 293명을 대상으로 2006년 1월 4일부터 1월 21일까지 면접설문조사(구강위생관리 행태, 구취관련 질병력, 구취실태), 구취측정, 구강검사, 치아우식활성검사(스나이더검사, 타액분비율검사, 타액완충능검사)를 실시한 결과 다음과 같은 결론을 얻었다. 1. 잇솔질 횟수는 1일 2회가 46.1 %로 가장 많았고, 여자가 남자보다 잇솔질 횟수가 많았다. 매일 혀솔질을 하는 군은 25.6%이었고, 보조 구강위생용품을 사용하는 군은 9.2 %이었다. 2. 평상시 구취를 자각하고 있는 사람은 62.5 %이었고, 구취를 가장 심하게 자각하는 시기는 기상 후가 72.7 %, 구취를 자각하는 부위는 잇몸에서 23.0 %, 구취의 유형으로는 구린 냄새가 37.2 %로 높게 나타났다. 3. 구취측정 결과 OG는 50 ppm미만이 54.3 %, $50{\sim}100ppm$ 범위에 41.6 %로 나타났고, $NH_3$$20{\sim}60ppm$ 범위에 52.6 %로 가장 높았다. 4. 구취관련 질병력별 OG는 치아우식증으로 인한 식편압입, 당뇨병과 구취에 대한 가족력군에서 $50{\sim}100ppm$ 범위에 유의하게 높았으며, $NH_3$는 호흡기계 질환군에서 유의한 차이가 있었다. 5 평상시 구취 자각정도별 OG는 '냄새가 나지 않는다'는 군과 '가끔 냄새가 난다'는 군에서 50 ppm 미만에 각각 55.9 %, 57.5 %로 나타났고, '본인이 느낄 정도로 항상 냄새가 난다'는 군과 '항상 심하게 냄새가 난다'는 군에서 $50{\sim}100ppm$ 범위에 각각 52.0 %, 63.6 %로 높게 나타났으며, $NH_3$는 모두 $20{\sim}60ppm$ 범위에 높게 나타났다. 6. 구강검사별 OG는 치수노출치와 식편압입이 많을수록, 설태지수가 높아질수록 $50{\sim}100ppm$ 범위에 OG값이 증가되었고, $NH_3$는 보철치가 많을수록, 설태지수가 높아질수록 유의하게 증가되었으며, 하악 국소의치군에서 60 ppm 이상으로 유의하게 증가되었다. 7. 스나이더검사는 고도활성이 43.0 %로 가장 높았고, 산 생성균의 활성이 높을수록 OG값이 증가되었다. 자극성 타액분비율 검사는 8.0 ml 이하에서 62.5 %로 가장 높았고, 타액분비율이 많을수록 OG값이 감소된 분포를 보였으며, 타액완충능검사는 0.1N 유산용액의 방울 수가 $6{\sim}10$ 방울에서 58.7 %로 가장 높았고, 타액완충능이 증가될수록 OG값이 냄새를 느끼지 못하는 50 ppm 미만에서 증가되었다. 8. 구강환경과 구취와의 상관관계에서 OG는 타액분비율, 보철치와 음의 상관관계를, 치수노출치, 충전치, 현존치, 설태량, 식편압입과 양의 상관관계를 보였으며, $NH_3$는 우식치와 음의 상관관계를, 보철치, 잇솔질 횟수와 양의 상관관계를 보였다. 9. 다중회귀분석 결과에서 OG에 영향을 주는 요인으로는 여자, 치수노출치, 보철치, 식편압입, 타액분비율, 설태지수, 스나이 더검사의 고도활성이 선정되었고 이들의 설명력은 45.1 %이었으며, NH3에 영향을 주는 요인으로는 여자, 치수노출치, 설태지수, 보철치가 선정되었으며 이들의 설명력은 6.6 %이었다. 이상의 결과를 볼 때, 조사대상 농촌지역 주민들의 구취실태는 구강환경 및 구취관련 요인, 치아우식활성검사의 스나이더 검사, 타액분비율검사와 밀접한 관련이 있음을 시사한다. 따라서 이들 주민들의 구취예방을 위해서는 식후에 올바른 잇솔질 방법 및 혀솔질과 더불어 보조 구강위생용품을 사용하여 식편압입과 설태제거를 해야 할 필요성이 강조된다. 구취의 원인과 그 성분은 매우 복잡하고 다양하므로 개인별 구취발생 요인을 정확하게 분석하기 위해서는 추후 계속적이고, 체계적인 연구가 필요하며, 보건(지)소의 치과위생사를 활용하여 지역사회 주민들에게 계속적인 구강보건교육 프로그램이 제공되어야 한다고 생각된다.
