목적: 최근 치과 영역에서 CAD-CAM (computer aided design-computer aided manufacturer) 시스템의 이용과 디자인 소프트웨어가 늘어남에 따라 쉽고 빠른 수복물의 디자인이 가능해졌다. 과거에는 치과 기공사의 wax up 기술의 숙련도에 따라 금관의 외형이 결정 되었으나, CAD-CAM 시스템에서는 디자인 소프트웨어에 대한 숙련도가 더 중요한 요인이 되어가고 있다. 이에 치과 기공사와 치과의사간의 결과물의 차이를 알아보고자 한다. 재료 및 방법: 대합치와 인접치가 양호한 환자의 하악 우측 제1대구치를 금관 지대치 형성 한 후 polyether로 인상채득 하였다. 인상재에 초경석고를 이용하여 모형을 제작 하였으며, Trios 구강내 스캐너를 이용하여 STL (stereolithography) 파일을 추출하였다. 3shape dental designer 상에서 5년 이상 경력의 치과 기공사 1명, 2년 이하 경력의 치과 기공사 3명, 4명의 보철과 1년차 전공의가 동일한 STL 파일을 이용하여 금관을 디자인 하였다. CAD-CAM 시스템 경력이 가장 오래된 기공사의 디자인을 기준으로 하여 치과 기공사 그룹 3명(group DT)과 1년차 전공의 그룹 4명(group RT) 의 금관 외형을 비교하였다. 디자인된 금관의 근원심폭, 협설폭, 높이, 교두간 거리, 대합치와의 접촉점의 수를 측정, 비교 하였다. 독립표본 t검정을 시행하여 그룹간의 차이를 비교하였고, 대합치와의 접촉점의 수는 Wilcoxon signed-rank test 를 이용하였다. 결과: 정규성 검정 결과 근원심폭, 협설폭, 높이, 교두간 거리는 정규분포를 보였다. Group DT와 group RT의 금관 디자인상에서 근원심폭, 협설폭, 높이, 교두간 거리에서 통계적으로 유의할만한 차이가 없었다. 대합치와의 접촉점 수는 정규분포를 보이지 않았으며, wilcoxon signed-rank test 결과 통계적으로 유의한 차이를 보이지 않았다. 결론: 금관의 외형의 크기에 있어서는 group DT와 group RT 사이에 통계학적으로 유의할만한 차이가 없었지만 수치화하기 어려운 금관의 해부학적 형태나 주변 치아와의 형태적 유사성 등은 차이가 있었다. 디자인 소프트웨어에 대한 숙련도가 높아진다면 각 술자가 가지고 있는 교합에 대한 개념을 더 구현하기 쉬울 것이다.
PURPOSE. The purpose of this study was to evaluate the marginal discrepancy of heat-pressed ceramic veneers manufactured using a CAD/CAM system. MATERIALS AND METHODS. The ceramic veneers for the abutment of a maxillary left central incisor were designed using a CAD/CAM software program. Ten veneers using a microstereolithography apparatus (AM group), ten veneers using a five-axis milling machine (SM group), and ten veneers using a traditional free-hand wax technique (TW group) were prepared according to the respective manufacturing method. The ceramic veneers were also fabricated using a heat-press technique, and a silicone replica was used to measure their marginal discrepancy. The marginal discrepancies were measured using a digital microscope (${\times}160$ magnification). The data were analyzed using a nonparametric Kruskal-Wallis H test. Finally, post-hoc comparisons were conducted using Bonferroni-corrected Mann-Whitney U tests (${\alpha}=.05$). RESULTS. The $mean{\pm}SD$ of the total marginal discrepancy was $99.68{\pm}28.01{\mu}m$ for the AM group, $76.60{\pm}28.76{\mu}m$ for the SM group, and $83.08{\pm}39.74{\mu}m$ for the TW group. There were significant differences in the total marginal discrepancies of the ceramic veneers (P<.05). CONCLUSION. The SM group showed a better fit than the AM and TW groups. However, all values were within the clinical tolerance. Therefore, CAD/CAM manufacturing methods can replace the traditional free-hand wax technique.
With the advances of CAD-CAM (computer-aided design and computer-aided manufacturing) technology, the field of modern clinical dentistry has been dramatically changed. The first step in the digital workflow for tooth-supported dental prosthesis is a data acquisition with intraoral digital or conventional impression techniques. For the accuracy of intraoral digital impression data, the basic principles of conventional impression should be applied. It is necessary to obtain a good visibility with properly-dried field and well-exposed margin of the prepared abutment. Currently, the equi- or supra-gingival finish line can be recommended as an indication for intraoral digital impression. The scan data are generally exported to '.stl' file format, which has only morphological information of black and whitem while '.obj' file format can store data on color and texture.
PURPOSE. The purpose of this study was to compare computer-aided design/computer-aided manufacturing (CAD/CAM) abutment and prefabricated abutment in Morse taper internal connection type implants after cyclic loading. MATERIALS AND METHODS. The study was conducted with internal type implants of two different manufacturers (Group Os, De). Fourteen assemblies were prepared for each manufacturer group and divided into 2 groups (n=7): prefabricated abutments (Os-P, De-P) and CAD/CAM abutments (Os-C, De-C). The amount of axial displacement and the removal torque values (RTVs) were measured before and after cyclic loading (106 cycles, 3 Hz with 150 N), and the tensile removal force to dislodge the abutments was measured after cyclic loading. A repeated measures ANOVA and a pattern analysis based on the logarithmic regression model were conducted to evaluate the effect of cyclic loading on the axial displacement. The Wilcoxon signed-rank test and the Mann-Whitney test was conducted for comparison of RTV reduction% and tensile removal forces. RESULTS. There was no significant difference between CAD/CAM abutments and prefabricated abutments in axial displacement and tensile removal force; however, significantly greater RTV reduction% after cyclic loading was observed in CAD/CAM abutments. The correlation among the axial displacement, the RTV, and the tensile removal force was not significant. CONCLUSION. The use of CAD/CAM abutment did not significantly affect the amount of axial displacement and tensile removal force, but presented a significantly greater removal torque reduction% than prefabricated abutments. The connection stability due to the friction at the abutment-implant interface of CAD/CAM abutments may not be different from prefabricated abutment.
The computer-aided design/computer-aided manufacturing (CAD/CAM) system was introduced to shorten the production time of all-ceramic restorations and the number of patient visits. Among these types of ceramic for dental CAD/CAM, they have been processed into inlay, onlay, and crown shapes using leucite-reinforced glass-ceramics to improve strength. The purpose of this study was to observe the mechanical properties and microstructure of leucite-reinforced glass-ceramics for dental CAD/CAM. Two types of leucite-reinforced glass-ceramic blocks (IPS Empress CAD, Rosetta BM) were prepared with diameter of 13 mm and thickness of 1 mm. Biaxial flexural testing was conducted using a piston-on-three-ball method at a crosshead speed of 0.5 mm/min. Weibull statistics were used for the analysis of biaxial flexural strength. Fracture toughness was obtained using an indentation fracture method. Specimens were observed by field emission scanning electron microscopy to examine the microstructure of the leucite crystalline phase after acid etching with 0.5% hydrofluoric acid aqueous solution for 1 minute. The results of strength testing showed that IPS Empress CAD had a mean value of $158.1{\pm}8.6MPa$ and Rosetta BM of $172.3{\pm}8.3MPa$. The fracture toughness results showed that IPS Empress CAD had a mean value of $1.28{\pm}0.19MPa{\cdot}m^{1/2}$ and Rosetta BM of $1.38{\pm}0.12MPa{\cdot}m^{1/2}$. The Rosetta BM sample exhibited higher strength and fracture toughness. Moreover, the crystalline phase size and ratio were increased in the Rosetta BM sample. The above results are expected to elucidate the basic mechanical properties and crystal structure characteristics of IPS Empress CAD and Rosetta BM. Additionally, they will help develop leucite-reinforced glass-ceramic materials for CAD/CAM.
컴퓨터 지원 설계 및 제조 기법(CAD-CAM)은 치과 영역에서 급속도로 발전해온 분야로, 다양한 보철 치료 영역에 적용되고 있다. 이 중 절삭형 방식(Subtractive Manufacturing)으로 분류되는 절삭가공 기법을 이용한 의치 제작은 상용화된 디지털 총의치 제작 방식 중 하나이며, 동시에 첨가형 방식(Additive Manufacturing)으로 분류되는 레이저 소결 혹은 용융 기법을 활용하여 피개의치를 위한 금속구조물을 보다 효율적으로 제작할 수 있게 되었다. 본 증례에서는, CAD-CAM을 통해 절삭형 방식으로 제작한 상악 총의치와 3D 금속 프린팅을 이용한 금속구조물과 절삭형 방식으로 제작된 인공치를 접목한 하악 임플란트 피개의치를 제작하였다. 이를 통해 기능적, 심미적으로 적절한 임상적 결과를 얻었으므로 이를 보고하는 바이다.
PURPOSE. The purpose of this in vitro study was to investigate the wear resistance and surface roughness of three interim resin materials, which were subjected to chewing simulation. MATERIALS AND METHODS. Three interim resin materials were evaluated: (1) three-dimensional (3D) printed (digital light processing type), (2) computer-aided design and computer-aided manufacturing (CAD/CAM) milled, and (3) conventional polymethyl methacrylate interim resin materials. A total of 48 substrate specimens were prepared. The specimens were divided into two subgroups and subjected to 30,000 or 60,000 cycles of chewing simulation (n = 8). The wear volume loss and surface roughness of the materials were compared. Statistical analysis was performed using one-way analysis of variance and Tukey's post-hoc test (α=.05). RESULTS. The mean ± standard deviation values of wear volume loss (in mm3) against the metal abrader after 60,000 cycles were 0.10 ± 0.01 for the 3D printed resin, 0.21 ± 0.02 for the milled resin, and 0.44 ± 0.01 for the conventional resin. Statistically significant differences among volume losses were found in the order of 3D printed, milled, and conventional interim materials (P<.001). After 60,000 cycles of simulated chewing, the mean surface roughness (Ra; ㎛) values for 3D printed, milled, and conventional materials were 0.59 ± 0.06, 1.27 ± 0.49, and 1.64 ± 0.44, respectively. A significant difference was found in the Ra value between 3D printed and conventional materials (P=.01). CONCLUSION. The interim restorative materials for additive and subtractive manufacturing digital technologies exhibited less wear volume loss than the conventional interim resin. The 3D printed interim restorative material showed a smoother surface than the conventional interim material after simulated chewing.
목적: 본 연구는 수복물의 적합도를 계측하는 두 가지 측정방법을 평가하기 위해 CAD-CAM (computer aided design-computer aided manufacturing)과 열가압 소성법으로 전부 도재관을 제작하고 micro CT와 cross-section technique으로 전부 도재관의 적합도를 비교하고자 하였다. 재료 및 방법: 하악 제 1대구치 레진 치아를 전부 도재관을 위한 지대치를 형성하고 PMMA (polymethylmethacrylate) 레진 블록을 가공하여 10개의 시편을 복제하였다. 복제한 레진 다이의 인상을 채득하고 모형을 제작하여 5개는 열가압 소성법으로 IPS e.max Press (Ivoclar Vivadent, Schaan, Liechtenstein) 도재관을 제작하였다. 다른 5개의 시편은 CAD-CAM 가공법(Ceramil motion 2, Amann Girrbach, Koblach, Austria)을 이용하여 IPS e.max CAD 도재관을 제작하였다. 각 레진 다이에 전부 도재관을 인산아연 시멘트(Fleck's zinc cement, Keystone industries, Gibbstown, NJ, USA)로 손가락 압력에 의해 합착하였다. 변연 및 내면 적합도를 측정하기 위해서 합착된 시편 10개를 먼저 micro CT로 촬영하여 협-설 시상면의 영상정보를 획득하였다. 그 후 절단 시편을 만들어 주사전자현미경으로 micro CT와 동일한 지점의 간극을 측정하였다. SPSS 22.0 프로그램을 이용하여 각 측정 지점에서 제작 방식별, 측정 방식별로 Mann-Whitney 검정 방법을 통해 유의차 검증을 시행하였다. 결과: Micro CT와 주사전자현미경으로 측정한 변연 및 내면 적합도는 유사하였으며, 통계적으로 유의한 차이를 보이지 않았다. 두 가지 제작법의 도재관은 변연 및 내면 적합도에 유의한 차이가 없었다. 결론: CAD-CAM과 열가압소성으로 제작한 전부 도재관은 적합도에 차이를 보이고 있지 않으며, Micro CT와 주사전자현미경을 이용한 측정 방식 모두 임상적으로 이용 가능할 것으로 사료된다.
PURPOSE. To determine the shear bond strengths of different denture base resins to different types of prefabricated teeth (acrylic, nanohybrid composite, and cross-linked) and denture teeth produced by computer-aided design/computer-aided manufacturing (CAD/CAM) technology. MATERIALS AND METHODS. Prefabricated teeth and CAD/CAM (milled) denture teeth were divided into 10 groups and bonded to different denture base materials. Groups 1-3 comprised of different types of prefabricated teeth and cold-polymerized denture base resin; groups 4-6 comprised of different types of prefabricated teeth and heat-polymerized denture base resin; groups 7-9 comprised of different types of prefabricated teeth and CAD/CAM (milled) denture base resin; and group 10 comprised of milled denture teeth produced by CAD/CAM technology and CAD/CAM (milled) denture base resin. A universal testing machine was used to evaluate the shear bond strength for all specimens. One-way ANOVA and Tukey post-hoc test were used for analyzing the data (α=.05). RESULTS. The shear bond strengths of different groups ranged from 3.37 ± 2.14 MPa to 18.10 ± 2.68 MPa. Statistical analysis showed significant differences among the tested groups (P<.0001). Among different polymerization methods, the lowest values were determined in cold-polymerized resin.There was no significant difference between the shear bond strength values of heat-polymerized and CAD/CAM (milled) denture base resins. CONCLUSION. Different combinations of materials for removable denture base and denture teeth can affect their bond strength. Cold-polymerized resin should be avoided for attaching prefabricated teeth to a denture base. CAD/CAM (milled) and heat-polymerized denture base resins bonded to different types of prefabricated teeth show similar shear bond strength values.
최근 디지털 기술과 Computer-aided design/Computer-aided manufacturing (CAD/CAM) 작업 환경은 의치 제작에 있어 임상의의 진료 과정의 변화를 가져왔다. CAD/CAM 기술을 이용한 의치 제작 방식은 진료 및 기공 과정을 단순화하여 오류의 발생을 감소시키며, 임상적인 효율성과 편리함을 제공한다. 본 증례는 하악 편위를 가진 상·하악 완전 무치악 환자에서 광경화 SLA 기반의 3D 프린팅을 이용한 디지털 제작 방식의 양악 총의치 수복 증례이다. 구강 스캐너로 채득한 디지털 모형에서 기록상을 제작하였고 교합제에 악간관계를 기록한 후 부가 중합형 실리콘 인상재로 폐구 정밀 인상을 채득하였다. 또한 교합제를 장착하고 촬영한 안면 스캔 데이터를 추가로 획득하여 기공 작업 시 교합 평면 결정이나 인공치 배열에 참고할 수 있도록 하였다. 인상체를 3차원 스캔 후 CAD 프로그램을 통해 인공치를 배열하였고, 연마면을 형성하여 의치 디자인을 완성하였다. 시적 의치 및 최종 의치는 미국식품의약국(Food and Drug Administraion, FDA) 승인을 받은 액체 광경화성 레진을 이용하여 stereolithography (SLA) 기반 3D 프린터로 출력하였고, 최종 의치는 적절한 안정과 유지, 지지를 보였으며 기능적, 심미적으로 만족스러운 결과를 얻을 수 있었다.
