임플란트 인상채득 방법과 인상용 코핑 연결 고정에 따른 정확성 비교 (Comparative accuracy of implant impression techniques with different splinting materials)

  • 목적: 하악 부분 무치악 모형에서 pick-up type 인상용 코핑을 사용한 개방형 인상 채득법과 transfer type 인상용 코핑을 사용한 폐쇄형 인상 채득법, 개방형 인상 채득법에서 pick-up type 인상용 코핑 고정 재료에 따른 인상 채득 정확성을 비교 하고자 하였다. 연구 재료 및 방법: 하악 부분 무치악 모형의 좌측 구치부에 직경 4.5 mm, 길이 11.5 mm의 임플란트 고정체 두 개 식립 후 3D 프린터를 이용하여 개방형 개인 트레이 30개 폐쇄형 개인 트레이 10개를 제작하였다. 제작한 개인 트레이를 이용하여 PN군: 개방형 인상 채득법(non-splint), PG군: 개방형 인상 채득법(splint GC pattern resin), PH군: 개방형 인상 채득법(splint Herifix), TN군: 폐쇄형 인상 채득법으로 나누어 각 군당 10회씩 총 40회의 인상 채득을 진행하였다. 인상채득 후 석고 모형을 제작하여 모델 스캐너로 스캔 후 역설계 프로그램으로 스캔오차를 측정하여 95% 유의수준으로 평가 하였다. 결과: 폐쇄형 인상 채득법 TN군이 개방형 인상 채득법 PN군 보다 더 낮은 오차값을 보였다. 개방형 인상 채득법(splint)에 사용된 인상용 코핑 고정 재료 간의 비교 결과에서는 PH군에서 가장 낮은 오차값을 보였으며 PN군, PG군 순으로 정확한 인상 채득 정확성을 보여주었다. 결론: 임플란트 인상 채득 방법이나 인상용 코핑의 연결 고정 유무에 따른 각 군 간의 정확성에 유의한 차이는 없다.

임플란트 피개의치에서 지대주와 골의 변형률에 관한 연구 (Strains of abutment and bones on implant overdentures)

  • 연구목적: 임플란트 피개의치에서 연결고정형과 비연결고정형 보철물 지대주와 골의 변형률을 비교하고 임플란트 식립 경사에 따른 변형률을 비교 분석하고자 하였다. 연구 재료 및 방법: 2개의 아크릴릭 레진 모형의 좌측과 우측 견치 부위에 2개의 외부연결형 임플란트를 각각의 모형에 식립하였다. 한 모형에는 두 개의 임플란트를 평행하게 식립하였고, 다른 모형에는 순측과 설측으로 10도의 이개도(divergence)를 가지게 임플란트를 식립하였다. 모형에서 바를 이용한 연결고정형 지대주와 볼을 이용한 비연결고정형 지대주를 고정체에 번갈아 연결하고, 각각의 경우 대응하는 유지장치가 부착된 피개의치를 제작하여 지대주 상에 안착시켰다. 전기저항 스트레인 게이지(strain gauge)를 이용하여 피개의치에 100 N에서 200 N의 부하를 가하였을 때 지대주와 가상골 표면의 변형률(strain)을 측정하였다. 결과 및 결론: 측정된 값들을 통계분석하여 얻은 결과는 다음과 같다. 바를 이용한 연결고정형 보철물은 비연결형 보철물 보다 높은 변형률 값을 보였다. 그러나 방향은 주로 압축력이었으며 부하는 분산되었다(P<.05). 볼을 이용한 비연결고정형 보철물은 연결고정형보다 낮은 변형률 값을 보였다(P<.05). 순측경사된 임플란트에서는 비연결고정형 보철물이 연결고정형 보철물보다 더 높은 변형률 값을 보였다. 설측경사된 임플란트는 순측경사된 임플란트보다 낮은 변형률을 보였다(P<.05). 임플란트가 평행하게 식립된 경우보다 평행하지 않게 식립된 경우 지대주와 가상골의 변형률 값이 더 높았다(P<.05).

치아 임플란트 재료에 따른 자기공명영상의 SNR 비교 (Comparison of the SNR in the MR images on dental implant material)

  • 치아 임플란트는 구강 내에 위치하여 목, 뇌 기저부와 안면부 MR 영상에 영향을 미친다. MR 검사에 사용되어지는 자장의 비균일성은 주로 주파수 부호화 방향으로 신호강도의 변화 및 기하학적인 왜곡으로 인공물을 발생시킨다. 본 연구는 치아 임플란트에 의한 왜곡 정도를 자기공명영상의 신호대 잡음비(singal to noise ratio:SNR)를 평가하여 임플란트 영향 정도를 확인하고자 하였다. 이를 위하여 인체와 유사하게 지방, 근육, 물로 구성된 돼지고기 팬텀을 이용하여 신호대 잡음비를 측정하고, 실험을 위해 제작한 고정형 물 팬텀을 이용하여 신호대 잡음비를 측정하여 치아 임플란트 재료 종류에 따른 신호대 잡음비의 크기를 비교 분석하였다. 돼지고기 팬텀의 Brushite, HSA, Metal, RBM 은 T1 SE에서 2.76, 2.22, 1.88, 1.57 이었고, T2 FLAIR에서 1.88, 1.78, 1.65, 1.79, T2 FSE에서 2.28, 2.25, 2.88, 2.05 T2 GRE에서 2.74, 1.94, 1.67, 1.48의 신호대 잡음비가 측정되었다. 고정형 물 팬텀의 Brushite, HSA, Metal, RBM 은 T1 SE에서 1.93, 1.87, 1.93, 2.06, T2 FLAIR에서 1.83, 1.76, 1.82, 1.92 T2 FSE에서 1.85, 1.79, 7.86, 1.97 T2 GRE는 1.97, 1.93, 1.99, 2.06의 신호대 잡음비가 측정되었다. 이러한 결과를 고려해 볼 때, 치아 임플란트의 재료를 선택할 경우 환자와 치과 의사는 치아의 수복이 주된 목적이지만 여러 분야의 검사에 미치는 영향도 고려해야한다. 또한 환자의 치아 임플란트 재료에 따라 검사 프로토콜을 선택하는데 이 연구 결과가 기초자료로 활용될 수 있다.

양극 산화와 불소 화합물로 처리한 임플랜트의 표면 특성 및 골유착 안정성에 관한 연구 (A Study on Surface Characteristics and Stability of Implants Treated with Anodic Oxidation and Fluoride Incorporation)

  • State of problem : A number of investigation about increase of surface area via various surface treatments and modification of surface constituent have been carried out. Purpose : The surface characteristics and stability of implants treated with anodic oxidation, fluoride ion incorporation, and groups treated with both methods were evaluated. Material and method : Specimens were divided into six groups, group 1 was the control group with machined surface implants, groups 2 and 3 were anodic oxidized implants (group 2 was treated with 1M $H_2SO_4$ and 185V, group 3 was treated with 0.25M $H_2SO_4$ and $H_3PO_4$ and 300V). Groups 4, 5 and 6 were treated with fluoride. Group 4 was machined implants treated with 0.1% HF, and groups 5 and 6 were groups 2 and 3 treated with 10% NaF respectively. Using variable methods, implant surface characteristics were observed, and the implant stability was evaluated on rabbit tibia at 0, 4, 8 and 12 weeks. Result : 1. In comparison of the surface characteristics of anodic oxidized groups, group 2 displayed delicate and uniform oxidation layer with small pore size containing Ti, C, O and showed mainly rutile, but group 3 displayed large pore size and irregular oxidation layer with many crators. 2. In comparison of the surface characteristics of fluoride treated groups 4, 5, 6 and non-fluoride treated groups 1, 2, 3, the configurations were similar but the fluoride treated groups displayed rougher surfaces and composition analysis revealed fluoride in groups 4, 5, 6. 3. The fluoride incorporated anodic oxidized groups showed the highest resonance frequency values and removal torque values, and the values decreased in the order of anodic oxidized groups, fluoride treated group, control group. 4. According to implant stability tests, group 2 and 3 showed significantly higher values than the control group (P<.05). The fluoride treated groups showed relatively higher values than the non fluoride treated groups and there were significant difference between group 4 and group 1 (P<.05). Conclusion : From the results above, it can be considered that the anodic oxidation method is an effective method to increase initial bone stability and osseointegration and fluoride containing implant surfaces enhance new bone formation. Implants containing both of these methods should increase osseointegration, and reduce the healing period.

처리 방법이 다른 표면이 임플랜트의 골유착 및 안정성에 미치는 영향 (THE EFFECT OF DIFFERENT SURFACE TREATMENT ON THE OSSEOINTEGRATION AND STABILITY OF IMPLANTS)

  • Purpose: This experiment examined the effect of different surface treatment on the osseointegration and stability of implants. Material and methods: In this study, 40 each of machined, SLA and RBM implants, which are the most commonly used implants, were implanted into the tibia of 20 normal rabbits using $OsseoCare^{TM}$. The rabbits were sacrificed after 1 week, 4 weeks, 8 weeks and 12 weeks for implant stability analysis, removal torque analysis, histologic and histomorphometric analysis. Result : ISQ showed significant difference between Machined and RBM at first week and at 4 weeks. There was significant difference between Machined and both SLA and RBM(p<0.05) but after 8 weeks there were no significant difference between each group. In the removal torque, RBM showd significantly higher values than SLA and Machined surface at 1st week. At 4th and 12th week, there was significant difference between Machined and SLA, RBM(p<0.05). In the bone to implant contact variable, there was no significant difference between each surface treatment method. In the Machined surface group, there was no significant difference between each time interval. but in SLA group, there were significant differences between the 1st week and 12th week and in RBM group, there were significant differences between the 1st week and 8th, 12th week and between 4th and 12th week(p<0.05). The bone area showed significantly higher values in SLA and RBM compared to Machined surface 1st and 8th week and significantly higher values in SLA than Machined surface at the 4th week(p<0.05). Conclusion: The roughened surface of implants showed positive effect in the early stages of implantation and assisted in bone formation After the bone formation stage, there was no statistical difference between Machined and roughened surface groups. In dental implantation, where initial stability is critical to the success of implants, the use of roughened surface implants should assist in reducing the healing period after implantation.

$Smartpeg^{TM}$의 고압멸균소독 및 재사용이 임플랜트 안정성 지수(ISQ) 측정에 미치는 영향 (THE EFFECT OF AUTOCLAVE STERILIZATION AND REUSE OF $SMARTPEG^{TM}$ ON THE IMPLANT STABILITY QUOTIENT (ISQ) MEASUREMENT)

  • Statement of problem: Resonance frequency analysis is evaluated as the non-invasive and objective method for the evaluation of implant stability and has been increasingly used. It is necessary to evaluate the factors affect the ISQ measurement stability. Purpose: This study was performed to evaluate the effect of the autoclave sterilization and reuse of $Smartpeg^{TM}$ on ISQ measurement. Material and methods: $SmartPeg^{TM}$ (Integration Diagnostics Ltd., $G\ddot{o}teborg$, Sweden) of autoclave group (A) was autoclave sterilized 9 times and $Smartpeg^{TM}$ of reuse group (R) was reused 9 times. Ten $SmartPeg^{TM}s$ were allocated to each group and after each autoclave sterilization and reuse, implant stability quotient (ISQ) values were measured 3 times from the two directions a and b at a right angle. $Osstell^{TM}$ mentor (Integration Diagnostics Ltd. $G\ddot{o}teborg$, Sweden) was used and type 1 (article no. 100353) $Smartpeg^{TM}$ was selected according to $Smartpeg^{TM}$ reference list. Osstem Implant US II future (Osstem Co., Seoul, Korea) in $4.0mm{\times}11.5mm$ was embedded in the self-curing acrylic resin ($Orthojet^{(R)}$, Lang Dental, U.S.A.). Data was statistically analyzed by one-way ANOVA $({\alpha}=.05)$ and scheffe test was done where a significant difference exist. Correlation test was also done between ISQ value and the number of autoclave sterilization or reuse. Results: 1. In autoclave group, the means and sd. of ISQ value before autoclave sterilization were $84.97{\pm}0.41,\;84.93{\pm}0.74$ at direction a and b. There was significant differences between autoclave groups at direction a and b (P=.000). 2. In reuse group, the means and sd. of ISQ value before reuse were $85.40{\pm}0.62,\;85.50{\pm}0.57$ at direction a and b. There was no significant difference between reuse groups at direction a and b (P>.05). 3. There was a weak positive correlation between the number of reuse and ISQ value at direction a and b (${\gamma}=.207$ and .246, P<.01). Conclusion: Within the limitations of this study, the following conclusions were drawn. Till ninth reuse of $Smartpeg^{TM}$, the ISQ measurement stability did not be affected. After twice autoclave sterilization of $Smartpeg^{TM}$ the ISQ measurement stability was affected.

이중나사산 임플란트의 안정성에 대한 평가 - 오스텔 멘토를 이용한 초기 안정성 ; PART I (Evaluation of Stability of Double Threaded Implant-Emphasis on Initial Stability Using Osstell MentorTM; Part I)

  • Fixture의 안정도를 평가하는 장비인 Osstell Mentor와 기존 Osstell과의 비교 시험 및 측정환경에 따른 노이즈 발생 시험 결과를 통해 다음과 같은 결과를 얻었다. 1. US II Fixture와 SS II Fixture의 ISQ 값을 측정한 1차 검증에서는 Osstell과 Osstell Mentor의 ISQ 값 경향의 차이가 크게 나타나지 않음을 확인하였다. 2. SS II Fixture를 얕게 매식한 것과 정상적인 매식한 것의 ISQ 값 비교를 한 2차 검증에서는, Osstell과 Osstell Mentor 모두 매식 깊이에 따른 동일한 경향의 결과를 보였다. 3. 3차 검증에서는 골질별로 Osstell과 Osstell Mentor의 ISQ 값이 일정한 경향을 보이는 것을 확인하였고, 특히 Osstell Mentor 장비는 Osstell 장비보다 사용함에 있어서 편리성이 뛰어나다고 판단된다. 또한 ISQ 값을 100% 신뢰하기는 어렵지만 Osstell Mentor를 임상 및 실험에 적용하는데 있어서는 문제가 없을 것으로 판단된다. 4. 측정환경에 따라 전자파를 발생시키는 환경이 존재하면 노이즈가 발생할 수 있으나, 실제 측정결과 휴대폰이 작동 중인 환경을 제외하고는 노이즈가 발생하지 않았다. 실제 치과 내에서의 사용에서도 노이즈는 발생하지 않음을 확인하였다. 5. 비교적 안전하게 장치를 장착 및 탈착 할 수 있어 측정과정에 의해 발생되는 임플란트와 주위골에 대한 잠재적 위험도가 낮고 측정위치에 따른 오차가 적어 임상적으로 불리한 상황과 다양한 환경에서의 임플란트 안정성 측정이 가능해 임상적 활용도가 높다고 판단된다.

  • Since the treatment of edentulous patients with osseointegrated implant was first introduced more than 30 years ago, implant therapy has become one of the most important dental treatment modalities today. Based on the previous experience and knowledge, $Br{\aa}nemark\;Novum^{(R)}$ protocol was introduced with the concept of simplifying surgical and prosthetic technique and reducing healing time recently. This protocol recommends the installation of three 5mm wide diameter futures in anterior mandible and the prefabricated titanium bars for superstructure fabrication. This study was designed to analyze the stress distribution at fixture and superstructure area according to changes of fixture number, diameter and superstructure materials. Four 3-dimensional finite element models were fabricated. Model 1 - 5 standard fixtures (13mm long and 3.75mm in diameter) & superstructure consisted of type IV gold alloy and resin Model 2- 3 wide diameter fixtures (13mm long and 5.0mm in diameter) & superstructure consisted of type IV gold alloy and resin Model 3-3 wide diameter fixtures (13mm long and 5.0mm in diameter) & superstructure consisted of titanium and resin Model 4-3 wide diameter fixtures (13mm long and 5.0mm in diameter) & superstructure consisted of titanium and porcelain A 150N occlusal force was applied on the 1st molar of each model in 3 directions - vertical($90^{\circ}$), horizontal($0^{\circ}$) and oblique($120^{\circ}$). After analyzing the stresses and displacements, following results were obtained. 1. There were no significant difference in stress distribution among experimental models. 2. Model 2, 3, 4 showed less amount of compressive stress than that of model 1. However, tensile stress was similar. 3. Veneer material with a high modulus of elasticity demonstrated less stress accumulation in the superstructure. Within the limites of this study, $Br{\aa}nemark\;Novum^{(R)}$ protocol demonstrated comparable biomechanical properties to conventional protocol.

Ti-8Ta-3Nb 합금의 표면처리에 의한 백서 두개관 세포의 반응 (Biological response of primary rat calvarial cell by surface treatment of Ti-8Ta-8Nb alloy)

  • Purpose: Ti-6Al-4V alloy is widely used as an implant material because of its good biocompatibility and good mechanical property compared with commercial pure titanium. Otherwise, toxicity of aluminum and vanadium in vivo has been reported. Ti-8Ta-3Nb alloy is recently developed in the R&D Center for Ti and Special Alloys and it was reported that this alloy has high mechanical strength, no cytotoxicity and similar biocompatibility to commercial pure titanium, but many studies are needed for its clinical use. In these experiment, we carried out different surface treatment on each Ti-8Ta-3Nb alloy disks, then cultured cell on it and assessed biological response. Materials and Methods: cpTi, Ti-6Al-4V, Ti-8Ta-3Nb alloy disks were prepared and carried out sandblasting and acid etching (SLA) or alkali-heat treatment (AH) on the Ti-8Ta-3Nb alloy disks. We cultured primary rat calvarial cells on each surface and assessed early cell attachment and proliferation by scanning electron microscopy, cell proliferation, alkaline phosphatase activity. Result: The rates of cell proliferation on the cpTi, Ti-8Ta-3Nb AH disks were higher than others (p<0.05) and alkaline phosphatase activity was significantly enhanced on the Ti-STa-8Nb AH disks(p<0.05). Conclusion: Most favorable cell response was shown on the Ti-8Ta-3Nb AH surfaces. It is supposed that alkali-heat treatment of the Ti-8Ta-3Nb alloy could be induced earlier bone healing and osseointegration than smooth surface.

구치부에 발치 후 지연 식립된 표준 직경의 임플란트에 대한 임상적, 방사선학적 평가 (CLINICAL & RADIOGRAPHIC EVALUATION FOLLOWING DELAYED IMPLANTATION USING REGULAR DIAMETER IMPLANTS IN THE POSTERIOR REGION)

  • The purpose of this study was to evaluate clinical and radiographic changes of regular diameter implants placed in the posterior region. A total of 47 standard diameter implants were consecutively placed in the posterior region of 20 partially edentulous patients. The diameters of the implants were 4.1mm (N=35, 74%) and 4.3mm (N=12, 26%), respectively. Peri-implant bone loss and clinical parameters such as mobility, suppuration, swelling, bleeding on probing (BOP) were evaluated at the baseline and?the final follow-up visit. The age of the patients ranged between 24~82 years (mean age: 54.7 years). The cumulative survival rate of the regular diameter implants loaded for a period of 3-24 months (Mean: $11.7{\pm}7.9$ month) was 100%. The average bone loss over the follow-up was $0.36{\pm}0.67\;mm$. Success rate was 95.7%. Only two implants failed (bone loss exceeding 1mm after 1 year of placement). Some prosthetic complications occurred, such as screw loosening (N=1) and dissolution of cementation material (N=2). The present study describes successful outcome following the use of standard-diameter-implants placed in the posterior region, and further comprehensive maintenance practices and follow-up schedules are required.
