액와동맥-양측대퇴동맥우회술후 발생한 인조혈관 감염의 치료 (Management of Infected Axillo-bifemoral Graft; A Case Report)

  • Infection after reconstructive surgery is one of the most catastrophic postoperative complication in vascular surgery. Mortality rates reported from a world-wide experience range between 25 and 88 percent. The surgeon faced with such a complication must choose among many diagnostic and management options to maximize limb salvage and survival based on the presentation and site of the infectiota the degree of ischemia of the lower extremities, and the overall medical condition of the patient. We successfully managed with descending thoracic aorta-to-bifemoral arteries bypass after the entire removal of the infected axillo-bifemoral graft because of bypass graft infection.

한방의료기관 감염관리에 대한 한의사의 인식도와 수행도 (Perception and Practice level of Korean Medical Doctors on Infection Control and Prevention in Korean Medicine Facilities)

  • Purpose : This study was designed to identify the perception and practice level of infection control among korean medical doctors and to identify factors that may influence the performance and practice level. Methods : Data were collected using the on­line survey method. Seven hundred and eighty four Korean medical doctors(KMD) participated the survey. The study was conducted from December 2018 to January 2019. Results : The results of this study are as follows. 1. Participants who experienced infection risk by needles or sharp instruments were 596(76%). and participants who had experienced blood or body fluid contact with the mucous membrane or skin of the patient during treatment were 226(28.8%) of them. 2. The degree of perception and practice of the infectious guideline was higher in the group over 50 years, in the doctor group, in the group with more than 6 years experience in clinic and in the group who work in the hospital. (p < 0.05) 3. In the performance of the infection control management related to the Korean medical treatment, the practice level of the article 'Discard the remaining needle that used for one patient' was the lowest at 4.02, 'Identify the patient and check the validity period of sterilization of medicines or instruments' was the second lowest in 4.16. 4. Among the contents of "Prevention of Nosocomial infection and Sanitary Safety Guidelines" issued by the Korean Medical Association in 2008, the guideline "Prevention of infection by pathogens such as HIV, MRSA, SARS" were lowest article in the perception and practice level of participants. 5. Regression analysis was performed to find out the factors affecting perception and performance of the participants. The regression model showed significant difference in the regression model of the working years. (p < 0.05) 6. In order to examine the effect of the variables on the perception and practice of the infectious guideline, the mediated effect of the knowledge and education level according to the years of working, age, education degree was found to be significant only in the education degree variable. (p < 0.05) In conclusion : in order to improve the perception and practice of infectious control of Korean medicine doctors, it is necessary to include the contents of infection management as essential education during the continuing education of Korean medicine association.

요양병원 간호사의 병원감염관리에 대한 인지도와 수행도 (Recognition and Performance Level of Hospital Infection Control in Nurses of Long-term Care Hospital)

  • The purpose of this study was to investigate the long-term care hospital nurse's recognition and performance level of hospital infection control. The subjects of the study were 147 long-term care hospital nurses. The period of data collection was from April 1 to 30, 2013. The data were analyzed by SPSS 19.0 program. The result are as followed; First, the total average scores of the recognition and performance by long-term care hospital nurses of hospital infection control were $4.64{\pm}0.32$ and $4.21{\pm}0.23$. Second, recognition of hospital infection control was significantly different according to position and hospital infection control education experience. Performance of hospital infection control was significantly different according to education level and hospital infection control education experience. Third, there was a positive correlation between the degree of recognition and performance of hospital infection control. Therefore, it is suggested to apply the concrete education program to enhance the recognition in order to improve the performance of hospital infection control of the Long-term hospital nurses.

국내 감염관리간호사의 핵심역량과 영향요인 분석 (Factors Affecting Core Competencies among Infection Control Nurses in Korea)

  • Purpose: This study was conducted to describe core competencies and identify factors affecting core competencies among infection control nurses (ICN). Methods: Infection control nurses from hospital with more than 200 beds comprised the sample. Questionnaires were sent to the sample via e-mail. One hundred and three questionnaires were returned. Data were analyzed with descriptive statistics and stepwise multiple regression via SPSS/WIN 21.0. Results: The core competency level of healthcare workers area was the highest that of the education and research was the lowest. There were significant differences in core competencies related to demographics such as age, clinical carrier, infection control carrier, position, academic degree, infection control specialist license, hospital location, and hospital type. The explained variances for the core competency were 38.3% and the contributing factors to core competencies were infection control carrier and infection control specialist license. Conclusion: It showed various levels of core competencies depending on infection control nurses' demographics and hospital traits, therefore consistent management efforts for the licensing and career path of infection control would be required.

Comparison of the Degree of Bacterial Removal by Hand Hygiene Products

  • Background: The coronavirus disease (COVID-19) pandemic increased awareness regarding the importance of hand hygiene in infection prevention. Although social distancing and vaccination are the strongest ways to prevent infection, personal hand hygiene is the most basic and easiest way to maintain public health. However, in addition to hand washing using running water, sanitizing tissues, and disinfection products are convenient for hand hygiene, especially outdoors. Therefore, it is necessary to improve the appropriateness of individual hand hygiene methods. In this study, we investigated the degree of hand hygiene offered by various hygiene products and hand drying methods for maintaining hand hygiene. Methods: An LED UV light kit was used for fluorescent observation of hand contamination. Bacteria from the hands were cultured to compare the degree of hand hygiene offered by various hygiene products. Bacteria were cultured in a hand-shaped medium dish to identify areas vulnerable to hand hygiene. Moreover, the degree of hand hygiene was observed according to the drying method using bacterial cultures. Results: We confirmed that hand washing under running water with antibacterial soap, sanitizing with alcohol gel disinfectant, and wiping with antibacterial wet wipes was effective for hand hygiene compared to washing under running water alone. However, for all hygiene products, a large number of bacteria were detected on the fingertips. We verified that natural drying, rather than rubbing, is effective in maintaining hand hygiene. Conclusion: These results suggest that hand hygiene products and drying methods are critical in hand hygiene management. Therefore, these results provide a basis for determining whether an individual's hand hygiene management method is appropriate.

Clinical Factors Related with Helicobacter Pylori Infection - Is there an Association with Gastric Cancer History in First-Degree Family Members?

  • Background: The aim of this study was to assess clinical factors associated with Helicobacter pylori positivity and to evaluate the incidence of gastric carcinoma in first-degree family members of infected patients. A total of 580 patients (mean age:$38{\pm}17$) with gastrointestinal complaints underwent C-14 urea breath test (UBT). Patients were grouped as: Group-1, untreated patients (n:384); and Group-2, patients who previously treated with eradication triple therapy (n:196). C-14 UBT was performed 1-2 months after the completion of eradication therapy. Associations of H pylori positivity with age, gender, ABO and Rhesus groups, smoking, dietary habits, and history of gastric cancer in first-degree family members were evaluated. The frequency of H pylori positivity was significantly higher in group-1 (58%) compared to group-2 (20%), p=0.001. There were no correlations between H pylori positivity and age, gender, ABO groups, Rhesus subgroups, smoking and dietary habits in both patient groups. The frequency of gastric cancer in family members was significantly higher in patients with H pylori infection among group-1, compared to infected patients among group-2 (56% vs. 28.6% respectively, p=0.03). We observed a significant association between H pylori positivity and the presence of gastric cancer in first-degree relatives of group-1 patients. Our results provide some confirmation of the presence of a link between gastric cancer development and H pylori. C-14 UBT is a sensitive, reliable and a widely recommended test for the detection of H pylori infection and recurrence. We suggest that detection and eradication of H pylori may contribute to a reduced risk of gastric cancer in the family members of infected patients.

입원 아동 보호자의 병원감염예방 지식과 실천 (Knowledge and Practice on Infection Prevention of Caregivers of Hospitalized Children)

  • Purpose: The purpose of this study was to identify the knowledge and practice on hospital infection prevention of caregivers of hospitalized children. The results will be the basic data of future policy and education program for hospital infection prevention. Methods: A descriptive survey design was utilized, The subjects included 151 caregivers from a university hospital in Gyeongnam Province, Korea. The data was collected using a self-reported questionnaire which included 17 items for knowledge, 18 items for practice, developed by researchers from March 2005 to January 2006. The collected data was analyzed with descriptive statistics, t-test, and ANOVA using SPSS program. Results: The mean score of knowledge on infection prevention was .87 of 1 (especially low in method of hand washing item), and practice, 2.60 of 3 (especially low in protection from other people items). There were differences in practice by admission frequency (F=2.83, p = .04), but there were no differences in knowledge by any general characteristics. Conclusion: The degree of knowledge and practice on caregiver's infection prevention was not enough to prevent hospital infection of hospitalized children. Therefore, effective strategies should be developed to prevent hospital infection of hospitalized children.

A Model to Explain Temperature Dependent Systemic Infection of Potato Plants by Potato virus Y

  • The effect of temperature on the rate of systemic infection of potatoes (Solanum tuberosum L. cv. Chu-Baek) by Potato virus Y (PVY) was studied in growth chambers. Systemic infection of PVY was observed only within the temperature range of $16^{\circ}C$ to $32^{\circ}C$. Within this temperature range, the time required for a plant to become infected systemically decreased from 14 days at $20^{\circ}C$ to 5.7 days at $28^{\circ}C$. The estimated lower thermal threshold was $15.6^{\circ}C$ and the thermal constant was 65.6 degree days. A systemic infection model was constructed based on experimental data, using the infection rate (Lactin-2 model) and the infection distribution (three-parameter Weibull function) models, which accurately described the completion rate curves to systemic infection and the cumulative distributions obtained in the PVY-potato system, respectively. Therefore, this model was useful to predict the progress of systemic infections by PVY in potato plants, and to construct the epidemic models.

요양병원 간호사의 도덕적 민감성, 감염관리 조직문화가 감염관리 수행도에 미치는 영향 (The Effects of Moral Sensitivity and Organizational Culture for Infection Control on Infection Control Performance of Long-Term Care Hospital Nurses)

  • Purpose: This study investigated the organizational culture in hospital for infection control, moral sensitivity, and the degree of infection control among long-term care hospital nurses, and to identified the factors associated with infection control perfomance. Methods: 186 nurses who directly care for patients at seven Long-Term Care Hospitals in D Metropolitan City participated in the survey. Data were collected using self-reported questionnaires and analyzed using the IBM SPSS 26.0 software. Results: Moral sensitivity showed statistically significant differences in age (F=5.473, p=.065), clinical experience (F=8.890, p=.031), nursing hospital work experience (F=6.520, p=.038), religion (t=-2.01, p=.046) and position (t=-2.96, p=.003). Correlation analysis revealed that with moral sensitivity and effect of organizational culture on infection control, there was a positive correlation between infection control and patient-centered nursing (r=.201, p<.006), professionalism (r=.149, p<.042), benevolence (r=.303, p<.001), infection control organizational culture (r=.556, p<.001). Benevolence of moral sensitivity (β=.21, p=.001) and infection control organizational culture (β=.54, p<.001) were associated with infection control perfomance. Moral sensitivity (including patient-centered nursing, professionalism, and benevolence) and infection control organizational culture explained 33.8% of the variance in infection control (F=24.57, p<.001). Conclusion: It is important to improve the moral sensitivity of nurses and a positive organizational culture for better infection control. We need to develop intervention strategies and establish systematic and administrative support.

치과내원환자의 치과감염관리에 대한 인식 및 요구도에 관한 연구 (A study on the perception and needs about dental infection control of the dental patients)

  • Objectives: The purpose of this study was to provide a satisfactory and reliable dental care service for the patients and to establish a plan to raise the perception about dental infection control. Methods: The perception of infection control in the dental clinic of the patients was examined who visited to dental clinics, over 20 years old and living in Seoul and Gyeonggi province since December 2016. The questionnaire was used as the survey tool, and 274 questions in the questionnaire were analyzed. Results: Results of examined factors affecting the perception of dental infection control in dental clinics show that the perception of infection control in dental clinics is affected by the degree of university graduates (p<0.05), full-time workers (p<0.01), housewives (p<0.001), and oral health concerns (p<0.05), the perception of infection control of dental medical personnel (p<0.001) in order. Conclusions: Dental medical personnels must thoroughly wash their hands, wear uniforms, wear personal protective equipment, and thoroughly clean and disinfect the interior of a dental clinic.
