Objectives : The purpose of this study is to investigate splenic constipation syndrome(脾約證) in Shanghanlun by comparing 179 & 247 provisions. Methods : Including Shanghanlun, several texts which contain Cheng Wu-ji, Wan Mi-zhai, Qian Huang, Zhu Gong, Fang You-zhi, Yu Jia-yan, Zhang Lu, Cheng Ying-mao, Zhang Xi-ju, Chen Xiu-yuan, Wu Qian, Xu Ling-tai, You Zai-jing, Zhu Dan-xi's opinion about the splenic constipation syndrome were chosen and comparative consideration was carried out. Results & Conclusions : The following conclusions could be drawn. 247 provisions in Shanghanlun is different from 179 provisions because the beginning of the each splenic constipation syndrome is different. The difference between 179 & 247 provisions are proved by comparing Mild Purgative Decoction which is used Taiyangyangming splenic constipation syndrome and Fructus Cannabis Pill which is used Dorsal pedal pulse splenic constipation syndrome. Cheng Wu-ji's opinion that 179 & 247 provisions is different is meaningful because he is first investigated the different beginning of the splenic constipation syndrome. Several annotator's opinion which oppose Cheng Wu-ji is majority and they set a high value on Jangbu(臟腑) factor. Several annotator's opinion which agree Cheng Wu-ji is minority and they set a high value on splenic constipation syndrome also begins from Taiyangbing. The concept of overlapping of Taiyangyangming is different from the concept of Taiyangyangming. Consequently 179 & 247 provisions has to be distinguished.
Yoshimasi Todo (吉益東洞), who thought that the books on herbal medicines are the reason for the complicated explanation of the effects of herbs, thought that the articles from Treatise on Cold Damage Diseases (傷寒論) and Synopsis of Prescriptions of the Golden Chamber (金匱要略) are the only books that can identify the effects of herbs. He also thought that the practitioners from the later period have added too much on the original text, which led to his thought that the original text of Treatise on Cold Damage Diseases (傷寒論) and Synopsis of Prescriptions of the Golden Chamber (金匱要略) did not have the explanation that involves the theory of yin-yang(陰陽), the five phases(五行), and three yin and three yang(三陰三陽). From this, he investigated the standards of using the herbs in the Synopsis of Prescriptions of the Golden Chamber (金匱要略) by comparing the articles. In this point of view, Yoshimasi Todo (吉益東洞) said that Poria Scleotium (茯笭) treats palpitations (悸) and 肉瞤筋惕. This meant the symptom of unvoluntary palpitations and the trembling of muscles with wrenching pain. Also, he thought that the symptom of Poria Scleotium (茯笭) in the formula of decoction is shown by the symptoms of dizziness, palpatations, vomiting, trembling of muscles, and cramps. This paper studies on the symptoms that seems close to the meaning of the symptom of Poria Scleotium (茯笭) that Yoshimasi Todo (吉益東洞) meant, which can be summarized to palpatations and trembling of muscles. I wish this paper is of help to those who study Treatise on Cold Damage Diseases (傷寒論), Synopsis of Prescriptions of the Golden Chamber (金匱要略), and the medicine of Yoshimasi Todo (吉益東洞).
Objectives : The purpose of this study was to investigate the interpretation of the Pyeongwi-san(PWS) prescription in order to obtain the evidence for clinical applications. Methods and Result : We interpreted on the PWS-related contents based on 15 classic books, analyzed it according to followed categories ; origin, indication and precaution, usage, meaning of name, physiological and pathological situation of PWS, explain about each herbs. Conclusion : 1. PWS was first mentioned in the Bakjebang, its application was enhancing appetite. Application of PWS not only expanded digestive disease, infectious disease, and pain in articular, but also used to toniyfing drug. 2. Generous application method of PWS was ‘Sujunbok(水煎服)’. Pill preparation(丸劑) and decoction with salt(鹽湯劑) were also used 3. "Pyeongwi(平胃)“ in PWS means "flatten the stomach". There are some theory about methodology to "Pyeongwi (平胃)“, one is "drain the pathogen in stomach(敦阜說)”, other is "tonifying stomach's healthy qi(卑監說)“, the other is compromise theory. 4. Most medical literature related application situation of PWS to dampness(濕邪) in stomach. In Gangsulwongobangsunju, there are more detailed explain focused on metal qi(金氣), and fire qi(火氣) 5. Objectives : Gwakhyangjeonggi-san (GJS) which consists of 13 herbal medicines has been used to treat gastrointestinal disorders caused by common cold. This study was performed to compare GJS decoctions produced using different pressure levels for various extraction times. Methods : Decoctions were prepared by the pressure levels of $0kgf/cm^2$ (non-pressurized) or $1kgf/cm^2$ (pressurized) for 30-180 min. The extraction yield, total soluble solid content (TSSC), and hydrogen ion concentration (pH) were measured, and the contents of the nine marker compounds were determined using high performance liquid chromatography. Results : The higher pressure and longer extraction time significantly increased TSSC value, while decreased the pH value. However, only extraction time affected the extraction yield of pressurized decoction. Variation of the amounts of chemical compounds was shown in pressurized and non-pressurized decoctions during extraction time. The result of regression analysis showed that pressure and extraction time can influence to extraction yield, TSSC, pH, and the content of chemical compounds. Conclusions : This study suggests that the pressure and extraction time can significantly affect the extraction efficiency of components from GJS decoctions.
Objectives: This study was carried out to analyze the single dose toxicity of Mecasin(Gami-Jakyak Gamcho buja Decoction) pharmacopuncture in muscle of Sprague-Dawley rats. Methods: All experiments were performed at the Medvill, an institution acknowledged to conduct non-clinical studies, under the Good Laboratory Practice (GLP) regulations. Sprague-Dawley rats were chosen in this pilot study. The reason Sprague-Dawley rats were chosen is that they have been widely used in safety test in the field of medicine, so the results can be easily compared with many other databases. Doses of Mecasin pharmacopuncture, 0, 500, 1,000, and 2,000mg/kg, were registered to the experimental groups, and a dose of normal saline solution, 10 ml/kg, was registered to the control group. Mecasin pharmacopuncture and normal saline were injected into the thigh of the rats by disposable syringes at intervals of six hours twice a day. This study was performed under the approval of the Institutional Animal Ethic Committee. Results: There is no death or abnormality in any of the four groups. No significant changes in weight, hematological parameters or clinical chemistry between the control group and the experimental groups were observed. To inspect abnormalities in organs and tissues, we used microscopy to examine representative histological sections of each specified organ; the results showed no significant differences in any of the organs or tissues. Conclusion: The above outcomes suggest that treatment with Mecasin pharmacopuncture is relatively safe. Further evaluations and studies on this subject are needed to prove more concrete evidence.
Objectives: The purpose of this study was to understand the status of reporting and characteristics of adverse drug reactions (ADRs) induced by herbal drugs and to make a suggestion for the domestic pharmacovigilance system on herbal medicine. Methods: We carried out a hospital-based observational study at Dongguk University Ilsan Oriental Hospital from April 2012 to December 2014. We reviewed all the herbal-drug-associated ADRs reports registered to the spontaneous ADR reporting system in electronic medical records of the hospital in the period. Results: We found out 101 reports including 163 herbal-drug-associated ADRs from 97 patients. Females (69.3%) outnumbered males and the most frequent age group was the 50s (44, 27.0%). No serious adverse event was observed. The most commonly reported ADR was gastro-intestinal system disorders (68, 41.5%) followed by skin-related disorders (42, 25.8%). Diarrhea (29, 17.8%) was the most frequently referred clinical manifestation. Most ADRs were induced by internal medicines (160, 98.2%) including manufactured (36, 22.1%) and self-prepared decoction (160, 76.1%). The pairs of Igi-hwan-diarrhea, gamiboa-tang-vomiting, and Magnoliae Flos-gastro-intestinal-system-related ADRs were observed twice each and the others appeared only once. Conclusions: We propose Korean government to take an initiative in national pharmacovigilance system for herbal medicine. To perform the surveillance on herbal drugs, the Association of Korean Medicine (AKOM) should set up a nationwide network by designating centers connecting the Korean medical hospitals, local Korean medicine clinics, and the public health centers. The government and AKOM should also educate and encourage them to understand the pharmacovigilance system and report the ADRs actively.
Objectives: This study aimed to learn what should be considered in [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer)] by analyzing the existing guidelines and clinical trials. Methods: The development committee searched guidelines for herbal medicinal product or gastric cancer developed already. Then, clinical trials for gastric cancer using herbal medicine were searched. The searched trials were analyzed in terms of inclusion and exclusion of participants, intervention, comparator, outcomes and trial design. Then, we compared the results of analysis with the regulations and guidelines of Ministry of Food and Drug Safety to suggest the issue that we will have to consider when making the [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer]. Results: As a result, few guidelines for anti-tumor agent and clinical trial with herbal medicinal product were searched in the national institution homepage. In addition, 10 articles were searched by using the combination following search term; 'stomach neoplasm', 'herbal medicine', 'Medicine, Korean traditional', 'Medicine, Chinese Traditional', 'TCM', 'TKM', 'trial'. Most trials included gastric cancer participants with medical history of operation. The type of intervention was various such as decoction, granules, and fluid of intravenous injection. Comparators were diverse such as placebo, conventional treatment including chemotherapy and nutritional supplement. The most frequently used outcome for efficacy was quality of life. Besides, the symptom score, tumor response, and survival rate were used. Safety was investigated by recording adverse events. Conclusion: We found out some issue by reviewing the existing guidelines and comparing it with clinical trials for gastric cancer and herbal medicinal products. These results will be utilized for developing [Guideline of Clinical Trial with Herbal Medicinal Product for Gastric Cancer].

Objectives : This study was done to determine the effect of Korean medical treatment to patient diagnosed as cauda equina syndrome(CES) suffering from both lower limb dysesthesia, saddle dysesthesia, buttock dysesthesia, dysuria, dyschezia and gait disturbance. Methods : A patient with CES was treated with acupuncture, sweet bee venom pharmacopuncture, herbal medicine from February 2 to April 9 at Department of Acupuncture & Moxibustion, Daejeon Korean Hospital of Daejeon University. Patient received acupuncture treatment($CV_1$, $CV_3$, $CV_4$, $CV_6$, $ST_{36}$, $BL_{31}$, $BL_{32}$, $BL_{33}$, $BL_{34}$) 12 times a week, bee-venom pharmacopuncture($BL_{23}$, $BL_{28}$) was conducted twice a week, herbal medicine(Samilsingihwan decoction) was applied to patient thrice a day for nine weeks. The following symptoms were observed saddle, and lower limb dysesthesia, dysuria, dyschezia and gait disturbance. Results : After treatment, dysesthesia, dysuria, dyschezia and gaiting developed good outcome. But anal reflex remained still. Conclusions : Acupuncture, bee-venom pharmacopuncture, herbal medicine combined treatment might be effective to patients with CES.

Angelica gigas Nakai (danggui) is a popular herb which has been used as a blood-building decoction for recovery from weakness in the Chinese medicine. Its demand increased in functional foods and pharmaceutical industries. For its hygiene, fumigation has been used, but the use of fumigants are going to be prohibited for food processing. In order to investigate gamma irradiation technique for hygiene of danggui, the immunomodulation activity of danggui after irradiation was examined. The water extract of irradiated danggui showed a strong mitogenic effect on splenocytes in vitro to the same level of lipopolysaccharide (LPS) and phytohemagglutinin (PHA). The effect was not different from that of non-danggui. It was tested whether there was any difference between irradiated and non-irradiated danggui in effects on the secretion of antibodies and graft versus host reaction in vivo. It turned out that intraperitoneal (i.p.) administration of the extract of irradiated danggui for 4 days remarkably increased the number of antibody-secreting cells in mice injected with sheep red blood cells (SRBC). Splenomegaly, due to graft versus host reacton, was also increased after 7 days i.p. administration of the extract of danggui in mice injected with allogeneic splenocytes. In these two in vivo test, the effect were not different from those of non-irradiated danggui. These results indicated that immunomodulation activity of danggui might be preserved after irradiation. In the other experiments (data not shown), the irradiated danggui was stable in active component analysis and safe in genetic toxicity test. In further research, the stability in other physiological activity of irradiated danggui will have to be proved before practical application of irradiation for hygiene.
This is a study on the methods of taking prescriptions in SANGHANRON (傷寒論), and after this, numbers of results have been obtained. SANGHANRON was written by Chang-Ki (張機) in the 2nd century, so it reflexes the usage of prescriptions of previous age indirectly. And gave affects on the methods of taking prescriptions to the oriental medicine doctors of next generation. Before Han-Dynasty (漢代), there were not so many publications connected with Oriental Medicine. Besides, some books couldn't hand down to next generation due to the gap of time and space. As time goes by, letters in medical books changed little by little, so contents connected with taking decoctions changed too. The effects of decoction and herb tea can be changed by the flexibility of methods of taking medicines, so we have to decide what kinds of taking methods should be taken and adapted to patients by the most effective way. There are many kind of methods of taking decocted prescriptions in SANGHANRON, so Chang-Ki selected the most appropriate method considering degree and position of disease and condition of patient. But nowadays, due to inconvenient procedure of taking medicines, some methos are not in common in clinical medicin. So this study was started to look back upon the changes of taking prescriptions and gave effort to find out the propriety of variation of taking prescriptions. In SANGHANRON, many kinds of taking prescriptions appeared from once a day to six times a day, except these, some prescriptions have to be taken little by little. These methods of taking prescriptions simplified as time pass by, but this change may give influence to the effect of medicine and finally we can't gain expected therapeutic value. So we have to distinguish the methods -though complicated and troublesome- in clinical medicine to make perfection more perfect in treating patients, and further studies have to be followed to prove the propriety of these methods.
