Therapeutic potential of eccentric exercises for age-related muscle atrophy

  • Recent studies have focused on evidence-based interventions to prevent mobility decline and enhance physical performance in older adults. Several modalities, in addition to traditional strengthening programs, have been designed to manage age-related functional decline more effectively. In this study, we reviewed the current relevant literatures to assess the therapeutic potential of eccentric exercises for age-related muscle atrophy (sarcopenia). Age-related changes in human skeletal muscle, and their relationship with physical performance, are discussed with reference to in vitro physiologic and human biomechanics studies. An overview of issues relevant to sarcopenia is provided in the context of the recent consensus on the diagnosis and management of the condition. A decline in mobility among the aging population is closely linked with changes in the muscle force-velocity relationship. Interventions based specifically on increasing velocity and eccentric strength can improve function more effectively compared with traditional strengthening programs. Eccentric strengthening programs are introduced as a specific method for improving both muscle force and velocity. To be more effective, exercise interventions for older adults should focus on enhancing the muscle force-velocity relationship. Exercises that can be performed easily, and that utilize eccentric strength (which is relatively spared during the aging process), are needed to improve both muscle force and velocity.

유우(乳牛)의 혈청(血淸)과 유즙중(乳汁中)의 호르몬수준(水準)에 의한 임신진단(姙娠診斷)에 관한 연구(硏究) (Studies on Pregnancy Diagnosis of Dairy Cows based on Hormone Concentrations in Blood Serum and Milk)

  • The study was carried out to find out the changes of hormone levels in blood serum and milk of Holstein cows during the estrous cycle. The progesterone, estradiol-$17{\beta}$ from the blood serum and milk samples were assayed by radioimmunoassay methods. The results of this study were summarized as follows; 1. The progesterone levels in blood serum during the estrous cycles began to decline rapidly at 2 days before estrus, decreased to $0.27{\pm}0.18ng/ml$ at on the day of estrus, and reached a peak mean level of $3.33{\pm}0.47ng/ml$ at 15 days after estrus. 2. The progesterone levels in milk during the estrous cycles began to decline rapidly at 2 days before estrus, decreased to $0.80{\pm}0.18ng/ml$ on the day of estrus, and increased a peak mean level of $3.80{\pm}0.36ng/ml$ at 15 days after estrus. 3. The estradiol-$17{\beta}$ levels in blood serum during the estrous cycles showed a peak mean level of $9.79{\pm}1.72pg/ml$ on the day of estrus, and decreased from $4.79{\pm}1.82pg/ml$ to $5.73{\pm}0.96pg/ml$ at luteal phase. 4. The estradiol-$17{\beta}$ levels in milk during the estrous cycles showed a peak mean level of $36.80{\pm}2.04pg/ml$ on the day of estrus, and decreased from $18.93{\pm}0.84pg/ml$ to $19.50{\pm}1.12pg/ml$ at luteal phase. 5. During 20 to 25 days after artificial insemination, the accuracy of pregnancy diagnosis from the blood serum progesterone levels were 87.5% for non pregnant cows (<2.0ng/ml), and 83.3% for pregnant cows ($${\geq_-}$$3.0 ng/ml). The accuracy of pregnancy diagnosis from the milk progesterone levels were 75.0% for non-pregnant cows (<2.4 ng/ml), and 94.4% for pregnant cows ($${\geq_-}$$3.2 ng/ml).

  • Objectives: To examine the effect of long-term Korean medicine treatment on mild cognitive impairment (MCI). Methods: This study was conducted on two patients who were diagnosed with mild cognitive impairment and received long-term Korean medicine treatment. In order to evaluate the degree of cognitive decline, SNSB and K-MMSE were performed 2~3 times at the first visit and during the treatment process. Results: Both cases 1 and 2 were diagnosed with MCI with a high possibility of developing dementia. In case 1, MCI progressed to early dementia at one year after the diagnosis of MCI. However, in case 2, that MCI status was maintained without progressing to dementia for 4 years after the diagnosis of MCI. The cause of such difference in the course of MCI might be attributed to the fact that Case 1 had lower K-IADL level but higher SGDS level than Case 2 at the time of initial diagnosis, with weak family support and irregular herbal medicine intake. Conclusions: Korean medicine treatment could improve and manage symptoms of cognitive decline due to MCI.

  • 본 연구의 선행 연구에서는 시군구단위 도시쇠퇴를 진단할 수 있는 시스템을 개발하였다. 본 연구에서는 시군구 단위보다 더 세밀한 읍명동 단위 및 지구단위의 도시쇠퇴 현황을 파악할 수 있는 시스템을 개발하였다. 개발된 지구단위 쇠퇴진단 모듈을 기존의 도시재생 종합정보시스템과 통합하여 도시의 세밀한 쇠퇴현황을 파악할 수 있게 하였다. 개발된 시스템은 중앙정부차원의 도시재생 사업 의사결정 담당자뿐만 아니라 지자체의 도시재생 사업 담당자들이 사업을 계획하고 우선순위 대상지를 선정하는 업무 등에 활용 가능할 것으로 판단된다.

Multimodal Imaging of Sarcopenia using Optical Coherence Tomography and Ultrasound in Rat Model

  • Sarcopenia, or reduced muscle mass and volume, is due to various factors such as senile change, neuronal degeneration, drug, malignancy, and sepsis. Sarcopenia with the aging process has been evidenced by the decline in muscle mass by 0.5 to 1% per year with 3-5% reduction in muscle strength for 10 years between the ages of 40 and 50, and a 1-2% of decline of mass every year in people aged 60-70. Therefore, early diagnosis and understanding the mechanism of sarcopenia are crucial in the prevention of muscle loss. However, it is still difficult to image changes of muscle microstructure due to a lack of techniques. In this study, we developed an animal model using denervated rats to induce a rapid atrophy in the tibialis anterior (TA) and imaged its structural changes using optical coherence tomography (OCT) along with histologic and ultrasound analyses. Ultrasound showed changes of overall muscle size. Histology revealed that the atrophic TA muscle displayed an increased size variability of muscle fiber and inflammatory changes. Three dimensional OCT imaged the changes of perimysial grid and muscle fiber structure in real time without sacrifice. These observed advantages of multimodal imaging using OCT and ultrasound would provide clinical benefits in the diagnosis of sarcopenia.

  • Evaluation of dementia in patients with early symptoms of cognitive decline is clinically challenging, but the need for early, accurate diagnosis has become more crucial, since several medication for the treatment of mild to moderate Alzheimer' disease are available. Many neurodegenerative diseases produce significant brain function alteration even when structural imaging (CT or MRI) reveal no specific abnormalities. The role of PET and SPECT brain imaging in the initial assessment and differential diagnosis of dementia is beginning to evolve vapidly and growing evidence indicates that appropriate incorporation of PET into the clinical work up can improve diagnostic and prognostic accuracy with respect to Alzheimer's disease, the most common cause of dementia in the geriatric population. in the fast few years, studios comparing neuropathologic examination with PET have established reliable and consistent accuracy for diagnostic evaluations using PET - accuracies substantially exceeding those of comparable studies of diagnostic value of SPECT or of both modalities assessed side by side, or of clinical evaluations done without nuclear imaging. This review deals the role of functional brain imaging techniques in the evaluation of dementias and the role of nuclear neuroimaging in the early detection and diagnosis of Alzheimer's disease.

Value of Fecal Calprotectin Measurement During the Initial Period of Therapeutic Anti-Tubercular Trial

  • Background/Aims: The diagnosis of intestinal tuberculosis (ITB) is often challenging. Therapeutic anti-tubercular trial (TATT) is sometimes used for the diagnosis of ITB. We aimed to evaluate the changing pattern of fecal calprotectin (FC) levels during TATT in patients with ITB. Methods: A retrospective review was performed on the data of 39 patients who underwent TATT between September 2015 and November 2018 in five university hospitals in Daegu, South Korea. The analysis was performed for 33 patients with serial FC measurement reports. Results: The mean age of the participants was 48.8 years. The final diagnosis of ITB was confirmed in 30 patients based on complete mucosal healing on follow-up colonoscopy performed after 2 months of TATT. Before starting TATT, the mean FC level of the ITB patients was 170.2 ㎍/g (range, 11.5-646.5). It dropped to 25.4 ㎍/g (range, 11.5-75.3) and then 23.3 ㎍/g (range, 11.5-172.2) after one and two months of TATT, respectively. The difference in mean FC before and one month after TATT was statistically significant (p<0.001), and FC levels decreased to below 100 ㎍/g in all patients after one month of TATT. Conclusions: All ITB patients showed FC decline after only 1 month of TATT, and this finding correlated with complete mucosal healing in the follow-up colonoscopy after 2 months of TATT.

쇠퇴지역 재난·재해 종합진단지수 개발과 지역간 위험성 비교·분석 - 서울시 사례 - (Development of A Comprehensive Diagnosis Index for Disasters in Declining Areas and Comparison of Risks between Regions: A case of Seoul)

  • 도시 쇠퇴지역은 인구가 감소하고, 건축물과 시설물 등 건조환경은 노후화되고 있다. 따라서 재난·재해 발생 시 취약하며, 복구에도 많은 시간과 비용이 소요된다. 본 연구의 목적은 쇠퇴지역의 재난·재해를 종합적으로 진단하기 위한 평가기법을 개발하고 사례분석을 통해 시사점을 제시하는 것이다. 재난·재해 종합진단지수를 산출하기 위한 평가지표를 선정하고, AHP 분석으로 유형, 구성요소, 평가지표를 포함한 계층별 가중치를 산정하였다. 산정된 가중치로 유형별 종합진단지수를 산출하고, 도시쇠퇴 수준에 따른 위험성을 분석하였다. 서울시를 사례지역으로 분석한 결과 서초구, 동작구, 금천구, 강서구 등 남부지역에서 재난·재해 종합 위험도가 높고, 서울시 도심지와 북부지역 그리고 공원 및 녹지지역에서 상대적으로 낮은 것으로 분석되었다. 본 연구의 결과는 자연재난과 사회재난을 포함해 재난·재해 유형별 종합진단지수 평가체계와 기법을 제시한 점에서 학술적 의의가 있다.

알츠하이머병의 최신지견 (Recent Advances in Diagnosis and Treatment of Alzheimer's Disease)

  • Alzheimer's disease (AD) is a neurodegenerative disorder in which neuronal loss causes cognitive decline and other neuropsychiatric problems. It can be diagnosed based on history, examination, and appropriate objective assessments, using standard criteria such as the Diagnostic and Statistical Manual of Mental Disorders or the National Institute of Neurological and Communicative Disorders and Stroke and the Alzheimer's Disease and Related Disorders Association (NINCDS-ADRDA). Brain imaging and biomarkers are making progress in the differential diagnoses among the different disorders. The cholinesterase inhibitors, donepezil, rivastigmine and galantamine and N-methyl-D-aspartate receptors antagonist memantine are approved by the US Food and Drug Administration for AD. Recently some acetylcholinesterase inhibitors gained approval for the treatment of severe AD and became available in a higher dose formulation or a patch formulation. Optimal care in AD is multifactorial and it should include early diagnosis and multidisciplinary care with pharmacological and nonpharmacological interventions including exercise interventions, cognitive interventions and maintenance of social networks.

Medical Imaging and Nuclear Molecular Imaging Probes for Pulmonary Fibrosis Diagnosis

  • Idiopathic pulmonary fibrosis (IPF) is a progressive disease caused by some risk factors, including smoking, viral infection, toxic substances, and radiation, that decline lung function of fresh oxygen and blood delivery throughout the body. Patients with pulmonary fibrosis have suffered from breathing and cough and the average survival rate is only 3 years after diagnosis. Therefore, it is significant to diagnose IPF and start treatment in enough time. Usually, lung biopsy is available to diagnose localized pulmonary fibrotic sites directly. However, it is insufficient to visualize whole lung tissue, and also it has a risk of infection for patients. In the clinic, medical imaging systems can diagnose pulmonary fibrosis non-invasively without infection. In this review, we introduce current medical imaging systems used to diagnose pulmonary fibrosis, including CT, MRI, and nuclear medicine. Further, we introduce several molecular imaging probes targeting specific biomarkers which are expressed in pulmonary fibrosis. Through this paper, it is expected that it would be helpful to understand the latest knowledge and research trends on pulmonary fibrosis diagnostic imaging.
