Cancer Unit Nurses' End-of-Life Care-Related Stress, Understanding and Training Needs (암병동 간호사의 임종간호 스트레스와 인지 및 교육요구도)

  • Purpose: This study was performed to identify the level of stress perceived by nurses who attend dying patients in the cancer care unit; their understanding regarding end-of-life care and related training needs. Methods: A cross-sectional descriptive study was conducted with 151 nurses stationed at the cancer care units of four general hospitals located in Seoul and Gyeonggi province in Korea. Data were collected using self-reported questionnaires and the response rate was 96%. The data were analyzed using t-test, ANOVA and Pearson's correlation analysis. SPSS 12.0 was used for data analysis. Results: Nurses experienced a high level of stress in the end-of-life care settings. Their understanding of end-of-life care was above the mid-point of the scale while their training needs for end-of-life care was relatively high. The more experienced the nurses were, the more stressed they were, particularly due to excessive workload. Nurses who served longer in the cancer unit tended to show greater needs for end-of-life care training. Conclusion: This study found nurses perform end-of-life care with a high level of stress but with insufficient understanding, and thus, showed great needs for related training. Such findings can be useful to develop an end-of-life care training program for nurses.

Relationship between usage of removable denture and depression in Korean adults with loss of multiple teeth: A cross-sectional study using the seventh Korea National Health and Nutrition Examination Survey (다수 치아를 상실한 한국 성인에서 가철성 의치의 장착 여부와 우울증의 연관성: 국민건강영양조사 제 7기 자료를 활용한 단면 연구)

  • Purpose: The purpose of this study is to evaluate the relationship between removable denture and depression. This study aims to investigate the relationship between depression and each group by dividing them into a group that were using removable dentures and a group that needed removable dentures but were not using removable dentures. Materials and methods: This study was conducted using raw data from the 7th Korea National Health and Nutrition Examination Survey (KNHNES). By using variables related to the state of the prosthesis and the need for prosthesis among oral examination data, the oral examination data were classified into two groups; group using removable denture and group in need of removable dentures. In addition, the variables of depression were classified into normal (0-4), mild depression (5-9), moderate depression (10-19), and severe depression (20-27) using the values of PHQ-9. For statistical analysis, logistic regression analysis was performed using SAS 9.4. Results: In the group in need of removable dentures, the risk of severe depression was statistically and significantly higher than the group using removable dentures. Conclusion: In patient who have lost multiple teeth, rehabilitation of the missing area with removable dental prosthesis is expected to decrease the occurrence of severe depression.

Effects of Old-age Income Level on Life Satisfaction: Focusing on Effects of the Relative Income of Reference Groups (노후 소득수준이 생활만족도에 미치는 영향: 준거집단의 상대소득 효과를 중심으로)

  • In this study, the author utilizes longitudinal data of the recent 12 years to verify whether the absolute versus relative income of old age has a significant effect on their life satisfaction of life. The findings of this study show that there is a significant difference in life satisfaction of old age according to respondents' relative income of three reference groups based on their asset quintile and residental area and education. In the correlation analysis using the cross-sectional data, the absolute and relative income level was closely related to the life satisfaction. In the panel regression analysis using the 12 years longitudinal data from the Korea National Labour Panel dataset, the income level of counterparts in those reference groups appeared highly significant in determining the level of life satisfaction of the elderly. However, some demographic variables such as age and marital status did not have a statistically significant effect in the long-term perspective. The income of reference groups in terms of asset levels, education and region as well as their own past life satisfaction level had a significant effect on the elders' life satisfaction determination. These results suggest that the relative income hypothesis in terms of life satisfaction among the elderly is more valid than the previous absolute income hypothesis.

Classification Tree Analysis to Assess Contributing Factors Influencing Biosecurity Level on Farrow-to-Finish Pig Farms in Korea (분류 트리 기법을 이용한 국내 일괄사육 양돈장의 차단방역 수준에 영향을 미치는 기여 요인 평가)

  • The objective of this study was to determine potential contributing factors associated with biosecurity level of farrow-to-finish pig farms and to develop a classification tree model to explore how these factors related to each other based on prediction model. To this end, the author analyzed data (n = 193) extracted from a cross-sectional study of 344 farrow-to-finish farms which was conducted between March and September 2014 aimed to explore swine disease status at farm level. Standardized questionnaires with information about basic demographical data and management practices were collected in each farm by on-site visit of trained veterinarians. For the classification of the data sets regarding biosecurity level as a dependent variable and predictor variables, Chi-squared Automatic Interaction Detection (CHAID) algorithm was applied for modeling classification tree. The statistics of misclassification risk was used to evaluate the fitness of the model in terms of prediction results. Categorical multivariate input data (40 variables) was used to construct a classification tree, and the target variable was biosecurity level dichotomized into low versus high. In general, the level of biosecurity was lower in the majority of farms studied, mainly due to the limited implementation of on-farm basic biosecurity measures aimed at controlling the potential introduction and transmission of swine diseases. The CHAID model illustrated the relative importance of significant predictors in explaining the level of biosecurity; maintenance of medical records of treatment and vaccination, use of dedicated clothing to enter the farm, installing fence surrounding the farm perimeter, and periodic monitoring of the herd using written biosecurity plan in place. The misclassification risk estimate of the prediction model was 0.145 with the standard error of 0.025, indicating that 85.5% of the cases could be classified correctly by using the decision rule based on the current tree. Although CHAID approach could provide detailed information and insight about interactions among factors associated with biosecurity level, further evaluation of potential bias intervened in the course of data collection should be included in future studies. In addition, there is still need to validate findings through the external dataset with larger sample size to improve the external validity of the current model.

Decisional balance corresponding to the Stage of Change of Smoking Cessation in Middle Aged Men (중년 흡연남성의 금연 변화단계에 따른 의사결정 균형에 대한 연구)

  • Purpose : This study was done to identify the factors of decisional balance for smoking cessation among middle aged men in Korea. This was composed of a set of variables at the level of decision making when middle aged men decide to stop smoking. It was used to classify and identify the characteristics of the stages of change for smoking cessation to which the middle aged men belong, and was used to identify the variables of decisional balance which influenced the stages of change for smoking cessation in middle aged men. Methods : A convenience sample of 169 men who had smoked and were smoking, were between age 30 and 60 and lived in Seoul(mean age=44), were selected from the community. The data was collected from December 10, 1999 to February 28, 2000. The research instrument was the Decisional Balance Measure for Smoking Cessation (Velicer et al., 1985). and Stage of Change Measure(DiClemente et al., 1991). The data were analyzed using the SAS Program. Results : 1. According to stage of change measure, the 169 subjects were distributed in each stage of change for smoking cessation: in the pre-contemplation stage 63 subjects(32.7%), contemplation stage 60 subjects (35.5%), preparation stage 17 subjects(10.1%), and maintenance stage 29 subjects(17.2%). 2. Factor analysis identified 4 factors of decisional balance as appropriate factors for smoking cessation of middle aged men. There were named by the researchers; 1)'Perceived Burden to Self', 2)'Perceived Benefit to Others' 3)'Perceived Approval to Others', and 4)'Perceived Disapproval to Self'. 3. Analysis of variance showed that the three components, 1)'Perceived Burden to Self(F=8.50, P=.0001)', 2)'Perceived Benefit to Others(F=3.19, P=.025)' and 3)'Perceived DisApproval to Self(F=2.87, P=.038)were significantly associated with stage of change. 4. Through discriminant analysis, it was found that 'Perceived Burden to self' was the most influential variable in discriminating the four stages of change(pre-contemplation, contemplation, preparation, and maintenance). Conclusion : The results are consistent with the application of the Transtheoretical model, which has been used to understand how people change health behavior. Even though this study is a cross-sectional, not a longitudinal study, the findings of this study give useful information for smoking cessation intervention for the middle aged men.

A Study on the Determinants of Export Diversification and Adoption of Competitive Advantage Strategy Using Hierarchical Analysis - A case of Automotive Parts enterprises (계층분석기법(AHP)을 이용한 수출다변화의 결정요인과 경쟁우위 전략 채택에 관한 연구 - 자동차부품 기업 사례)

  • This study analyzed factors that should be considered in the process of export diversification strategy formulation and priorities of strategies. We reviewed the extant literature and interviewed six experts. Cross-sectional analysis of a literature review and interview results were used to determine the influencing factors of export. The data for this study were collected from a survey of 68 export experts in Automotive Parts enterprises. Data were analyzed using AHP. As a result of the analysis, the order of importance appeared in the order of internal capacity, correlation buyer effect, entry barriers, technology response, and competition environment. The regions with a high overseas level were product differentiation, centralization, and low cost, whereas those with a low level were in the order of low cost, product differentiation, and concentration. It is meaningful to review the alternatives for adoption of the export diversification strategy and establish a strategy appropriate to the situation. It is necessary to establish a customized strategy considering the characteristics of the global region, and not only internal competence but also mutual relationship with local buyers are important.

Bibliometric Analysis of the Journal of the Korean Society of Clinical Toxicology (대한임상독성학회지에 발표된 연구 문헌의 통계적 분석)

  • Purpose: The purpose of this study was to examine the research characteristics and the trend of the Journal of the Korean Society of Clinical Toxicology by bibliometric analysis. Methods: This study was a retrospective quantitative literature review of the publications. We collected data from the internet homepage of the Korean Society of Clinical Toxicology. Among 228 publications, a total of 225 articles were included in this analysis. The data were analyzed from different perspectives, including article types, study design, number of authors, type of toxic material, and the top five ranking prolific authors and the affiliated organization were identified. Results: A total of 225 articles were analyzed; 98(43.6%) were original articles, 115(51.1%) were case reports, and 12(5.3%) were reviews. Among the original articles, nine were prospective studies and 89 were retrospective studies, which were assorted according to study design; there were two(2.0%) cross sectional studies, 93(94.9%) cohort studies, and three(3.1%) etc. The median number of authors per article was five and the top five ranking authors and affiliated organizations published 31.1% and 32.8% of total articles, respectively. The most abundant topic was pesticides, followed by natural poisons and poisons encountered in the work place. Conclusion: Since its foundation, the Journal of the Korean Society of Clinical Toxicology has published 19 issues and 228 articles and has played a key role in development of toxicology research in Korea. However, low ratio of original articles and a decrease in the number of recent articles indicates that greater effort is needed in clinical research. In addition, further interest of many experts and various institutions is necessary.

An Efficiency Analysis of Integrated Online and Offline Operations of Listed Retail Companies -Focusing on 28 listed retail companies in China- (상장 소매업체의 온라인·오프라인 통합 운영 효율성 분석 -중국 28개 상장 소매업체를 중심으로-)

  • The purpose of this paper is to analyze the efficiency of on-off-line integrated operations of listed retailers, propose ways to identify and improve the problems of inefficient enterprises, and accelerate the integration of on-off-line. The 28 listed retail companies selected total capital, management expenses, number of employees and wage rates as input factors as of 2018 and total operating income and operating profit were selected as output indicators to analyze efficiency using DEA. The results show that the integrated enterprise has a higher overall level of operational efficiency, but it is still in a state of stagnation. The pure technical efficiency of the integrated enterprise is generally higher, and the scale efficiency is the main reason that the overall efficiency of the enterprise is not improved. This study can help retail companies adjust their development strategies of online and offline integration according to their own degree of online and offline integration. This study has limitations in explaining the change in efficiency of retailers by conducting a cross-sectional analysis using data limited to 2018. It is necessary to utilize data over the next several years to conduct a longitudinal analysis.

Association between Sleep duration and Grip strength in Korean adults Using Convergence Survey Data (융복합조사자료를 활용한 수면시간과 악력 간 관련성 연구)

  • The purpose of this study was to investigate the relationship between sleep duration and muscle strength in Korean adults aged 19 years and older. The cross-sectional analysis was conducted using the 2016 National Health and Nutrition Examination Survey data and Chi square test and multiple regression analysis were used. As a result of the analysis, the grip strength of those with more than weekday average sleep duration of 9 hours was found to be -1.267kg compared with those with weekday average sleep duration of 7 hours. The grip strength of those with more than weekend average sleep duration of 9 hours was found to be -0.879kg compared with those with weekend average sleep duration of 7 hours. In model simultaneously adjusting for both the average weekday and weekend average sleep duration, weekday average sleep duration of 9 hours was found to be -1.034kg compared with those with weekday average sleep duration of 7 hours. Therefore, careful observation will be required in light of the fact that both sleep duration and grip strength can predict future health conditions.

Child Abuse Recognition and Related Factors among Korean Nursing Students (간호대학생의 아동학대 인식과 관련요인)

  • Objectives: This study measures nursing students' ability to recognize child abuse and identifies the factors related to varying levels of recognition. Methods: In this cross-sectional study, data were collected from 370 third and fourth year nursing students using a self-reported questionnaire during November 15-30, 2011. The measuring tool for child abuse recognition used in the study was developed by Ozasa (2011) and is composed of 44 items including physical, mental, and sexual abuse, and neglect. For data analysis, descriptive statistics, two sample t-tests, and regression analysis were evaluated with the SPSS/PC ver20.0 program. Results: Nursing students were concerned about child abuse(85.4%), but knew little about related laws and regulations(14.3%), and they had almost no formal education or training regarding how to recognize child abuse. They only 1.6% reported child abuse even if they encountered such incidents; however, they correctly recognized even infrequent incidents of child abuse. Recognition of sexual abuse ranked highest, while recognition of neglect ranked lowest. Those with higher levels of concern over child abuse showed higher recognition scores. Regression analysis revealed that physical abuse, mental abuse, and neglect had different related factors, while sexual abuse had none. Conclusion: Education and training on the subject of child abuse is strongly recommended in nursing curriculums so that nurses will be able to appropriately respond to and report suspected child abuse.
