Outcomes for Employment of a Trauma Clinical Nurse Specialist in the Treatment of Trauma Patients (외상환자의 치료에서 외상 전문간호사 도입 결과)

  • Purpose: Ongoing treatment and care, as well as initial stabilization, are required for trauma patients. With increasing number of sickest trauma patients and shortage of surgeons, the need for advanced practice nurse to provide and coordinate trauma care has been greater. The purpose of this study is to analyze the effect of hiring a trauma clinical nurse specialist and its influence on the treatment of trauma patients. Methods: Based on the employment of the clinical nurse specialist in December 2010, the patients were divided into two groups: patients admitted from January 1, 2010 to November 30, 2010 and patients admitted from December 1, 2010 to December 31, 2011. Retrospectively, data were collected using electronic medical records. The general characteristics, clinical courses, and ICU re-admission rates, collaboration (transfers to other departments and collaborative surgery) were compared. Results: To have a clinical nurse specialist on the trauma team resulted in a statistically significant reductions in the length of general ward hospital stay (p<0.05), the ICU re-admission rate, (p<0.03), the lead-time before transfer to other departments (p<0.05). Conclusion: The clinical nurse specialist, as a professional practitioner, improved the quality of treatment through early detection and management of problems. In addition, as a coordinator, the clinical nurse specialist maintained a cooperative relationship with multi-disciplinary medical personnel. The trauma clinical nurse specialist contributed to the treatment of trauma patients positively through a decrease in ICU re-admission rate and length of hospital stay.

The Role and Responsibilities of Coordinators in Korean Gifted Classes by Examining the Coordinators of Foreign Gifted Programs (외국 영재교육 프로그램 코디네이터의 역할을 통해 고찰한 한국 영재학급 업무 담당자의 역할과 대안)

  • The purpose of this study is to find out the role and responsibilities of coordinators for gifted classes in Korean schools, by examining the role and responsibilities, and qualifications of coordinators' at gifted classes in America and England. To do this, first, we examine the tasks and role of Korean gifted classes in elementary and secondary schools. Then we look into the coordinators of gifted programs of public schools and university programs in America and England. The coordinators of America and England are experts in gifted programs who have understanding and knowledge on gifted education, and they work for everything regarding gifted programs, such as designing the gifted education program, dealing documents regarding gifted programs, and assisting school personnel in the identification, development, teaching and evaluation of gifted students. The coordinators are the most important and core person in gifted programs. However, in Korean school, the role of coordinators is one of the school tasks of which any teacher has to take charge and there is no regulation on the qualification of coordinators. Therefore we suggest to set up the regulations of qualification, role and responsibilities of gifted programs' coordinators, by understanding that the role of coordinators is important.

Pastor's Expectations from Parish Nurses (목회자의 목회간호에 대한 역할기대)

  • Parish nursing is a community health nursing role developed in 1983 by Lutheran chaplain Granger Westberg. An increasing emphasis on holistic care, personal reseponsibility for a healthy lifestyle, and changes in healthcare delivery systems have undoubtedly facilitated the establishment and nurturance of an innovative nursing role in the community. Parish nurses are functioning in a variety of church congregations of various denominations. The parish nurse is a educator, a personal health counselor, a coordinator of volunteers. The parish nurses helps people relate to the complexed medical care system and assists people to integrate faith and health. The autors conducted a study on pastor's expectations from parish nurses. Results of this study will be useful to those instrumental in planning, initiating, supporting, and evaluating a parish nurses program The research was done on 130 pastors in Taegue and Kyong Sang Buck Do, of various ages ranging from their 20's to 60's: and pastoring churches of various sizes, ranging from under 100 to over 300 members. 94.6% agreed that they needed a parish nurse on their staff; and 86.2% said they wanted to start a parish nurse program in their churches if certain basic conditions were met. The pastors responded that some would hire the nurses on a full-time basis(22.3%), a part -time basis (37.7%) or use volunteer nurses (40%). The pastors said they would expect the following from a parish nurse: health counselling (80.0%) regular health check-ups (78.5%) health care for the elderly (78.5%) health information and education (72.3%) hospice care (72.3%) visiting sick church members at home (69.2%) arranging and training volunteers to help the seek (59.2%) health care for expectant mothers (50.0%) introducing and taking people to health care facilities (46.2%) The pastors were surveyed about specific areas of health education they would want the parish nurse to teach(for example, high blood pressure and heart disease prevention and management(76.2%) ; stress management(74.6%); and diabetes prevention and management(73.8%). The pastors were surveyed about specific areas of health counselling they would expect the parish nurse to do (for example, drug abuse, (73.1), alcohol abuse(64.6%), marriage conflict(60.0%), recovery after the loss of a loved one(56.9%), and women's conflict with parents-in-law(53.8%). The pastors were surveyed about types of things they would want included in regular health check-ups, what they would want a parish nurse to do on home visits, and what they would want included in home care for the elderly. They were also surveyed on what kind of spiritual care they would like parish nurses to give. Most (90.7%) wanted their parish, parishioners to be involved in the parish nurses program as volunteers, and in a variety of ways(such as visiting sick in their homes(68.5%) and helping with housework(63.1%) and taking sick people to health facilities(60%). Parish nurses role, activities, and boundaries of practice should be continuously monitored and refined and a 'case manager' should be conceptualized as an additional or all-encompassing role. An initial parish / community needs and readiness assessment should be done prior to establishing a program to detemine if the congregation is ready, willing, and able to support such a position for at least a 2 to 3 year period.

A Study on the Medical Tourism and Activation : With focus on Medical Communication (의료관광의 활성화를 위한 제언: 의료커뮤니케이션을 중심으로)

  • This paper is concerned with medical tourism, which is expected to become the new growth engine of the 21st century. Medical tourism is a collaborative and synergistic composition of Healthcare and Tourism, and has led to a remarkable growth in 2012 since the year 2009, exhibiting huge growth potential. This paper reviews the emerging market of the medical tourism with five major hospitals in Korea from the perspective of medical tourists, and makes suggestions for the global health care and the sustainable development in Korean medical tourism as a growth market: the construction of websites for active services and transparent management of medical expenses with proper arrangement of medical consulting and advices, cooperation of the health care center and the medical tourism industry, and the activation of healthcare communication. For the smooth progress of medical tourism, this paper suggests two separate communication channels: one for the patient tourists and the other for the medical tourism coordinators. The former needs accuracy with professional knowledge on the healthcare and communication, which should be classified as medical interpreters working with disease-oriented medical tourists. The latter refers to international medical coordinators dedicated to the smooth progress of medical tourism and services. This paper also points out the creative efforts to improve the relatively poor infrastructure of tourism industry to accommodate the medical tourists, and improve the medical tourism industry.

A Study on a Role of a Nurse for the Community Welfare Center Being Recognized by Nurses for the Community Welfare Center and Social Workers (사회복지관 간호사와 사회복지사가 인식하는 사회복지관 간호사의 역할)

  • As this study is the descriptive research study that tries to present the basic data aiming to establish a role of nurses for the Community Welfare Center, by surveying the role expectation and the degree of role performance towards nurses for the Community Welfare Center that nurses for the Community Welfare Center and social workers recognize, it carried out the Questionnaire research targeting nurses who are working for the Community Welfare Center located in Seoul and social workers of institutions where nurses are working, and then analyzed the results, and the study results are as follows. Both the group of nurses and the group of social workers were highly recognizing the necessity and the importance of nurses for the Community Welfare Center, and expected that more effective services will be possible to be offered by means of that nurses do work for the Community Welfare Center. However, compared to the degree that social workers recognize a nurses specialty, the nurses thought that their specialty is not acknowledged in the Community Welfare Center, and even in case of duty friction between groups being felt while social workers and nurses are working together, nurses were experiencing further friction of duties. While nurses are well recognizing their roles, they are statistically and significantly recognizing the degree of role performance lower than expected, thus the difference between the role expectation and the role performance could be seen. Also, the group of nurses showed the higher role expectation towards nurses than the group of social workers, and in relation to roles of a counselor, referral resource, an advocator, an educator, a case manager, a and a coordinator, the degree of role expectation by the group of nurses is statistically and significantly higher than the group of social workers, thus it could be seen that there is difference in the expectation degree between both groups, as to a role of nurses for the Community Welfare Center. In particular, as to a role of universally medical services, the expectations between nurses and social workers showed mutual agreement, but in relation to a role of nurses for community in the more expanded sphere, social workers did not show a receptive attitude, thus it was indicating that there exists an element of role conflict. In relation to the role performance of nurses for the Community Welfare Center, nurses were feeling the disorder degree more than social workers, in all items of disorder factors, and there was difference in recognition between nurses and social workers, as to the priority of disorder factors. Because of, through this study, having been found the difference in recognition of role expectation, the degree of role performance, and a disorder factor between nurses and social workers, as to the nurses for the Community Welfare Center, it is required a study with a more diversified method on a role of nurses for the Community Welfare Center.

  • This study investigates normative role model of the journalists under the changing environment. Firstly, this article explores what pressure the new media environment gives to the journalists in their routine of news production and distribution. These are stated from the angle of epistemological, professional and interactive pressure. Next, as a reference for the standard journalism in the age of mass media, the idea of Rhee Young-hee, a late journalist who won respects from many Korean journalists, is studied. His firm belief in the pursuit of hard facts, rigorous investigative writing and expertism are spelt out. Then, this study explores how, in real term, this pressure changes the journalistic value, norm and practices in the newsroom. Ten of Koran journalists are interviewed in order to get their idea about the emerging journalistic standards under the digital environment. From this in-depth interviews, it is conclued that the pursuit of hard fact, investigative writing, expertism of Rhee Young-hee are, nonetheless the change of the media technology, still effective and provide good reference points for the enhancement of the standard of journalism in Korea. However, it is also suggested that the methods to fulfil desirable journalism in the digital age should be different from that of the mass communication age. The interviewees make propose that the journalist, as a network node, news curator or coordinator, should actively interact with the audiences facilitating their enhanced potential as a news 'prosumer'(producer and consumer).

DACUM Job Analysis of Die and mold makers for Apprenticeship education of Specialized high school (특성화고 산학일체형 도제교육을 위한 금형제작원의 DACUM 직무분석)

  • The purpose of this study is to analyze job of die and mold makers for apprenticeship education of specialized high school using DACUM method. The contents of this study were to identify the duties, specific tasks performed and the level of importance, difficulty, frequency and entry level on each task. This study also make out a DACUM chart of die and mold makers for apprenticeship education of specialized high school. The DACUM committee, which consisted of one facilitator, nine die and mold experts, one recorder and one coordinator, was established to conduct DACUM. The conclusions are as follows; First, this study defines a die and mold makers as 'a person who make it possible to manufacture/assemble/inspect die and mold parts using various machine tool'. Second, duties in job of die and mold makers for apprenticeship education were total 12 and total tasks in job of die and mold makers for apprenticeship education were 86. Third, this study determine the level of importance, difficulty, frequency and entry level about each task. Finally, this study make out a DACUM chart of die and mold makers for apprenticeship education of specialized high school based on the results of DACUM job analysis. And knowledge, skills, tools, and positive behaviors, future trends/concerns about die and mold makers for apprenticeship education of specialized high school were presented.

  • One of most important problems in the Monsoon Asia today is the production of rice paddy to meet the needs of the ever increasing population. Diversemeans are being employed to meet this demand, both by increasing productivity of existing farm land and by bringing further areas into cultivation. The primary step in either field is to ensure that there is sufficient moisture in the soil to suit the paddy, and at the same this means that excess moisture has to be drained off the land, while in others irrigat ion has to be employed to bring sufficient water to an area. In view of the fact that the project comprises a huge amount of earthwork, it can be carried out by extensive use of construction machinery in order to shorten the period. As farm ditch has a comparatively small section with shallow cutting depth, inaddition, there is lack of access road in the field, the excavation equipment with bulldozer or tracter-shovel (backhoe) type are not applicable because there are mostly adapted for the excavation of deep and wide section. Mini-backhoe with its bucket width not larger than 0. 3m, and width of blade not larger than 1. 00m seems to be more adaptable. About 80% of excavation of ditch section will be done by the machinery while the other 20% of excavation together with the finishing of the section are supposed to be done by man-power. The embankment of ditch section can be compacted by the crawler of backhoe when it is moving along the ditch for excavation. However, Lowland paddy field in the Monsoon Asia are made particulary in rain season, therefore, heavy machinery is not easy excavation for ditch. It is very important to know exact ground support power of the working site and select machines with corresponding ground pressure. Ground support power is variable subject to quality and water content of soil and therefore selection of machines should be made duly considering ground condition of the site at the time of construction works. Farm ditches dug and compacted by mannual labar are of poor quality and subject to destruction after one or two years of operation. On the other hand, excavation and compaction by bulldozer is not practical for ditches. Backboe is suitable for slope land, but this is required cycle time of bucket excavation and dumped out. If a small-scale farm ditch trencher adaptable to lowland paddy field is invented, such a machine could greatly accelerate the massive construction work envisaged in many countries and thus significantly speed up the most difficult part of irrigation development and management in Monsoon Asia.

Trends and Implication of International Discussion on Forest Sector: Focusing on United Nations Forum on Forests (산림부문 관련 국제적 논의 동향과 시사점: 유엔산림포럼을 중심으로)

  • In accordance with the appearance of global issue called Sustainable Development Goals (SDGs), United Nations Forum on Forests (UNFF) has shown a significant progress in such as emphasizing the potential contribution of forest sector and establishing long-term strategies. In relation to the SDGs which would be discussed mainly in different international agreements and conferences related to forest sector until 2030, this study reviewed the discussion trend by establishing integrated databases and focusing on UNFF which serves as an integrated coordinator. It also aims to identify the meaning of United Nations Strategic Plan for Forest (UNSPF) established by UNFF. Furthermore, the following proposals on policies are presented in this study aiming for active discussion on forest sector not only in international but also in national context, and to implement UNSPF and GFGs effectively in a long-term manner: (1) Cooperation among sectors, organizations, and countries is more important than anything to make an active discussion on forest sector; (2) Recognizing and understanding the role and contributions of forests should be encouraged in actual field, and it should serve as the basis when establishing national policies and goals; (3) By fully utilizing UNFF report which is for the review of UNSPF implementation, a rational reporting system should be provided, and simultaneously, the efficiency on reporting items in the process of implementation should be improved.

Optimal Phase of Dynamic Computed Tomography for Reliable Size Measurement of Metastatic Neuroendocrine Tumors of the Liver: Comparison between Pre- and Post-Contrast Phases

  • Objective: The reliability of size measurements of liver metastases from neuroendocrine tumors (NETs) on contrast-enhanced computed tomography (CT) phases made by different readers may be hampered due to transient, variable rim enhancement in arterial phase (AP) or portal venous phase (PVP) images. We aimed to assess the reliability of tumor size measurements in pre- and post-contrast scans. Materials and Methods: The study coordinator selected target lesions according to Response Evaluation Criteria in Solid Tumors 1.1 guidelines in 44 consecutive patients with pathologically confirmed NET liver metastases. Two blinded readers measured the longest diameters of target lesions on pre-contrast, AP, and PVP images twice with a 4-week interval. Inter- and intra-observer agreements were evaluated using Bland-Altman plots and 95% limit of agreement (LOA) calculations. Results: Of the 79 target lesions (approximate mean size of 3 cm), 45 showed rim enhancement. Inter-observer agreement assessed based on LOA was highest in pre-contrast CT images (-6.1-5.7 mm), followed by PVP (-7.9-7.1 mm) and AP (-8.5-7.4 mm) images. Intra-observer agreement showed the same trend: -2.8-2.9 mm and -2.9-2.9 mm for readers 1 and 2, respectively, on pre-contrast CT, -2.8-2.9 mm and -3.0-3.2 mm, respectively, on PVP, and -3.2-4.2 mm and -3.4-3.2 mm, respectively, on AP images. Mean tumor diameters differed significantly among the phases in the following increasing order: pre-contrast CT, PVP, and AP images. Conclusion: There was better inter- and intra-observer agreement in size measurements of NET liver metastases on pre-contrast scans than on AP and PVP scans. Pre-contrast CT may be the optimal for measuring NET liver metastases if its accuracy is proven.
