  • ${\ulcorner}$Hamsansachon Dongyi Suse Bowon Gabogubon${\lrcorner}$ , discovered in 2000, can give very precious information in order to study the formation and development process of ${\ulcorner}$Dongyi Suse Bowon${\lrcorner}$ ${\ulcorner}$The Dircourse on the Constitutional Symptoms and Disease${\lrcorner}$ . I examined, by comparison, changes in understanding pathology explained in ${\ulcorner}$Dongyi Suse Bowon${\lrcorner}$ ${\ulcorner}$The Discourse on the Constitutional Symptoms and Disease${\lrcorner}$ of Gabobon and Sinchukbon, and consequently tried to define the concept of Dispositional Symptom(Dispositional disease) as below, in a point of view that ‘Dispositional Symptom(Dispositional disease)’ should be the key word in explaining the changes in understanding of pathology. Dispositional Symptom(dispositional disease) is a new concept that was first troduced in the Kyongjabon, not found in the Gabobon, and that played a key role in editing ${\ulcorner}$Dongyi Suse Bowon${\lrcorner}$ ${\ulcorner}$The Discourse on the Constitutional Symptom and Disease${\lrcorner}$ . Dispositional Symptom(dispositional disease) means an innate temperament or a pathological tendency, which is already constructed in the system of an individual, prior to expression of specific diseases and symptoms, and can be a primary basis to tell the susceptibility and developing pattern of a certain disease, to decide how to treat and forecast the prognosis. Sinchukbon inductively categorized symptoms of the dispositional symptom (dispositional disease) into the concept of ‘Eight principles’, or eight standards of diagnosis, such as superficies-interior, cold-heat, and weakness-strength.

  • The Sasang Constitutional Medicine seeks the psychosomatic balance, and the Basic principle of treatment according to the symptom(病證藥理) in Sasang Constitutional Medicine could be summarize as follows. 1. The pursuit of Shape-Image Medicine (形象醫學) and the Symptoms of shape - Symptoms of disease (形置病證). It established emotion, symptoms of constitution, and symptoms of constitutional disease on the base of Shape-Image Medicine (形象醫學) which means qi (氣) is inside and shape is outside (氣裡形表). 2. The inductive medicine and positive medicine. It systematized the Symptoms of shape - Symptoms of disease (形證病證) on the base of the medical practice of the ancients and Lee Jae-Ma (李濟馬)'s own experience inductively, and it present show to control the psychosomatic balance practically. 3. The pathology centering on human. Oriental medicine is based on Yin-Yang (陰陽) and Five elements (五行) and the harmony of nature and human (天人相應) of Taoism, but Sasang Constitutional Medicine is based on emotional and ethical pathology of Confucianism (儒學). 4. The treatment according to the symptom (病證藥理) in Sasang Constitutional Medicine centering on clear qi (正氣). The promotion and demotion (補瀉) which is based on clear qi (正氣) and evil qi (邪氣) is a basic principle in Oriental medicine, but In Sasang Constitutional Medicine it is attached importance to control of clear qi and classify obedience symptom and disobedience symptom (順 逆症) as the type to oppose disease. 5. The treatment according to the psychomatic symptoms. It takes importances to control of one-sided emotion as well as the treatment of psychical symptoms, and also to remedy disease as keeping under control one's mind. 6. The preventive medicine and the medicine to develop one's health. It is in pursuit of social health to develop personality through clear qi (正氣) in real life, to prevent disease, and to spread the sickness control of individual person. Sasang Constitutional Medicine pursuits the psychosomatic balance through 'Balance', 'Control', 'Self regulating Control' of the 'Golden Mean (中庸)' with the treatment according to the symptom (病證藥理) in Sasang Constitutional Medicine.

A Development of Sasang Constitution Diagnosis Program Based on Characteristics of Body Shape Face Physiological Symptom and Personality (체형 안면 소증 및 성정 특성 기반 사상체질 진단 프로그램 개발)

  • 1. Objectives : In this study we derived constitutional characteristics both by using questionnaire on character and symptom and by collecting objective measurement data through face, body shape. Furthermore, by developing a program, we intend to help one's diagnosis of Sasang constitution. 2. Methods : Through November 2007 to July 2009, we obtained questionnaire, face, and body shape data of 958 constitution confirmed subjects within the age range of 10-80 from 19 oriental medical facilities in the country. According to sex, we divided the subjects into two groups as real constitution and non-constitution group respectively. We analyzed the questionnaire through chi-square test (p<.01) and facial and body shape data through unpaired T-test (p<.01). By using weight law for questionnaire and euclidean distance for body shape and face data, we expressed constitution possibility to develop a supplementary program. 3. Results : The concordance rates of constitutional diagnosis by using the program for Taeeumin, Soeumin, Soyangin are, 76.7%, 61.5%, 67.4% resulting in 69.6% in male and 64.1%, 64.7%, 65.1%, resulting in 64.7% In female respectively. New cases tested on the program showed concordance rate of 65% in male and 62% in female with real constitution. 4. Conclusion : The constitutional diagnostic program based on the face, body shape and questionnaire, may have significant meaning as a supplementary tool in the constitutional diagnosis for clinical expert.

The Analysis on Clinical Characteristics of Out-Patients with Gastrointestinal Symptoms in a Korean Medicine Hospital (일개 한방병원에 위장관 증상을 주소로 내원한 외래환자의 사상의학적 특성에 관한 고찰)

  • Objectives The aim of this study was to make an analysis of clinical characteristics and status of Sasang constitutional medicine of patients with gastrointestinal symptoms who visited a Korean medicine hospital. Methods We studied 287 patients with gastrointestinal symptoms who visited a Korean medicine hospital from March 1, 2010 to February 30, 2012. We retrospectively collected medical history and characteristics of subjects using data of EMR (electronic medical record). Results The proportion of female was higher than male. Most patients was suffered from chronic gastrointestinal disorder. And they were not under western medicine treatment. Among the 287 patients, 139 persons (48.43%) were Soyangin, 88 persons (30.66%) Taeeumin and 60 persons Soeumin (20.91%). The proportion of Soyangin was higher than other Sasang types. In the distribution of prescription according Sasang Constitutional Medicine symptomatology, the proportion of exterior disease's prescription was higher than interior disease in Soyangin (93.53%). And the proportion of interior disease's prescription was higher than exterior disease in Taeeumin (75.86%) and Soeumin (95.00%). The majority of disease were Gyolhyung-symptom (51.54%) and Mangeum-syndrome (46.15%) of exterior disease in Soyangin, Dry-febrile symptom (73.56%) of interior disease in Taeeumin, and Taeeum-syndrome (95.00%) of interior disease in Soeumin. Conclusions By analyzing on clinical characteristics and distribution of Sasang Constitutional Medicine symptomatology could contribute to the development of guideline for treatment of gastrointestinal disease of Sasang Constitutional Medicine.

The Effects of GalGunSeunggi-tang (GST) on Mice Induced Allergic Contact Dermatitis (ACD) by DNCB (DNCB로 유발된 생쥐의 알레르기접촉피부염에 갈근승기탕(葛根承氣湯)이 미치는 영향)

  • Objectives The aim of this study was to investigate the effects of GalGunSeungGi-Tang (GST) on allergic contact dermatitis (ACD) induced by 2,4-dintrochlorobenzone (DNCB) Methods In this study, The changes of body weight, ear weight, ear thickness, spleen weight, dorsum skin thickness, symptom score by eyesight, histological finding, proliferation rates of splenocyte in vitro and in vivo are investigated to check effects of GST. The mice are divided into four group; Normal (naive mice), Control (DW administered), GST-L (GST 500mg/Kg/day administered), GST-H (GST 1,000mg/Kg/day administered). Results GST inhibited weight of ear significantly (P < 0.05) and also thickness (P < 0.01). In addition, There are significant decrease in thickness of dorsum skin and proliferation rates of splenocyte in vivo in GST administered group. Finally, GST reduced symptom score and hyperkeratosis, hyperpigmentation, increase of granulocyte and parakeratosis in histological finding. Conclusions These results suggest that GST can decrease symptoms of ACD.

A Case Report of San-syndrome (疝症) Patient Diagnosed as Soeumin Greater Yang Disease Reverting Yin Pattern (소음인 태양병 궐음증으로 진단한 산증(疝症) 환자 치험 1례)

  • Objectives The purpose of this case study was to report that San-syndrome patient was treated by diagnosing as Greater Yang Disease Reverting Yin Pattern. Methods The patient was administered with Insamosuyu-tang. Numeral Rating Scale (NRS) and National Institutes of Health Chronic Prostatitis Symptom Index (NIH-CPSI) was used to assess the improvements of symptoms. Results and Conclusions The perineal region discomfort was reduced from NRS 4 to NRS 1. NIH-CPSI score was reduced from 23 to 11, which means symptom relief. Nocturia and sleep disorder were resolved, and general weakness was relieved accordingly.

  • Objectives This study was aimed to report significant improvement of gastroesophageal reflux disease (GERD) treated by Cheongsimyeonja-tang and acupuncture treatment in Taeeumin. Methods Patient who has GERD was treated with herbal medicine and acupunture from June 15 to July 6. Gastrointestinal Symptom Rating Scale (GSRS) was used to observe changes of digestive symptoms. Results After treatment, the GSRS of the patient improved from 4.8 to 2.6. Also, the patient's symptoms of sore throat, cough, phlegm and bowel sound were reported to be improved after treatment. Conclusions This study shows that Cheongsimyeonja-tang might be effective in Taeeumin type patient's GERD.

  • 1. Objective: This study is about a Soyangin patient with vertigo after subarachnoid hemorrhage. In this study, we report the effects of Sasang constitutional treatment to this patient. 2. Methods: This patient was treated by Soyangin's constitutional medications and acupunture according to the result of Sasang constitutional diagnosis. 3. Results and Conclusions: This patient's chief complaints were vertigo and gait disturbance. By using Yangkyuksanhwa-tang mainly, she showed positive response about her symptom. This study shows that Yangkyuksanhwa-tang has effect to vertigo in Soyangin.

  • 1. Objective This study is about a Soeumin patient who had Yul Gyuk similarity with anorexia, nausea, sialism. In this study, we report the effects of Sasang constitutional treatment to this patient. 2. Methods This patient was treated by Soeumin's constitutional medications and acupunture according to the result of Sasang constitutional diagnosis. 3. Results and Conclusions This patient's chief complaints were anorexia, nausea, and sialism. By using Gyejibanhasaenggang-tang mainly, he showed positive response about his symptom. This study shows that Gyejibanhasaenggang-tang has effect to Yul Gyuk similarity in Soeumin.

A Case Study of Taeeumin Patient Who Experienced Refusing Food Intake after Stroke (뇌경색 발병 후 오식증(惡食症)을 보인 태음인(太陰人) 환자 1례)

  • Objectives The purpose of this case thesis paper is to report the improvement of symptoms for a 58-year-old male patient who experienced refusing food intake 1 month after onset of stroke by treating the patient with Chungpyesagan-tang. Methods We have determined the patient to have the Taeeumin type physique with Dry Heat Syndrome and provided Chungpyesagan-tang to alleviate the patient's symptoms. The level of improvement was measured based on changes of food intake amount. Results The patient was hospitalized to receive treatment for roughly 2 weeks. The symptom of refusing food intake as well as other symptoms that simultaneously appeared including insomnia, headache, dizziness, and general weakness have improved. Conclusions In this case, the symptom of refusing food intake appeared one month after experiencing stroke, and the patient refused not only meals, but the intake of all other forms of food including water and medicine. The cause of such symptoms were deemed to be emotional rather than the types of food which generally provide the causes for Oh Shik Jeung. As the patient was deemed to have the Taeeumin type physique with Dry Heat Syndrome. Chungpyesagan-tang was provided for treatment. Acupuncture and oriental cupping therapy were also provided concurrently, and the main symptom and other accompanying conditions have largely improved after 2 weeks of hospitalized treatment.
