중환자실과 응급실 간호사의 표준주의 수행에 영향을 미치는 요인 (Factors Influencing Compliance with Standard Precautions in Intensive Care Unit and Emergency Room Nurses)

  • Purpose: The purpose of this study was to investigate the level of knowledge, attitude and compliance with standard precautions and to identify predicting factors of compliance with standard precautions in ICU and ER nurses. Methods: The participants were 228 ICU and ER nurses working in one of three university hospitals and one general hospital. Collected data included general characteristics, features related to infection risk, and knowledge, attitude and compliance with standard precautions. For the final analysis, 218 questionnaires were used. Results: Within the previous one year, 103 (47.3%) nurses experienced pricking injury from syringe needles or other sharp materials, 111 (50.9%) nurses reported exposure to patients' blood and body fluid. In general, the scores for knowledge, attitude and compliance were all high. Compliance scores for nurses in intensive care units were significantly higher than those of nurses in emergency. There were significant correlations of knowledge, attitude, and compliance with standard precautions. Attitude and work place were significant factors predicting compliance with standard precautions. Conclusion: The results indicate that to increase nurse's compliance with standard precautions, continuous efforts to enhance positive attitudes and at the same time, improve work environments and use individualized approaches based on the work units are needed.

병원간호사의 감염예방 표준주의지침 지식, 인식, 안전환경 및 수행도에 관한 연구 (Knowledge, Perception, Safety Climate, and Compliance with Hospital Infection Standard Precautions among Hospital Nurses)

  • Purpose: The purpose of this study was to examine knowledge, perception, safety climate and compliance with hospital infection standard precautions and to identify the factors influencing nurse's compliance with standard precautions. Methods: Using the structured survey, data were collected from 292 nurses working at 4 general hospitals in the metropolitan city Seoul in October, 2008. Data were entered and analyzed with SPSS 12.0. Results: The majority of nurses were female with a mean age of 27.8. The mean score for knowledge of standard precautions was 21.2 out of 25. The nurses lacked knowledge on reusable gloves or gowns. Nurses perceived use of protective devices may not only increase time strain but also hinder development of therapeutic relationships with patients. Of safety climate factors, lack of time was the most frequently reported barrier to compliance with standard precautions. Knowledge, perception, and safety climate explained 16.1% variance of compliance with standard precautions. Conclusion: To improve nurses' compliance with standard precautions, provision of education and support for safety climate are necessary.

  • Purpose: The purpose of this study was to identify nursing students' knowledge, self-confidence in compliance, and compliance with standard precautions, and to identify predicting factors of compliance with standard precautions. Method: From May 18 to June 15, 2012, data were collected via questionnaire surveys from 222 nursing students of 2 colleges in G-province. Descriptive statistics, t-tests, Pearson correlation coefficients, and multiple regression analyses were used with SPSS 22.0 to analyze the data. Results: The average knowledge score was $0.89{\pm}0.08/1.0$, $3.59{\pm}0.57/4.0$ for self-confidence in compliance, and $0.91{\pm}0.11/1.0$ for compliance. Significant positive correlations were observed between knowledge and self-confidence in compliance(r=.14, p=.040), and between self-confidence in compliance and compliance(r=.25, p=.008). Self-confidence in compliance and blood and body fluid exposure were significant factors of compliance(F=5.46, p<.001). Conclusion: To increase nursing students' compliance with standard precautions, continuous efforts must be made to enhance self-confidence in compliance and decrease blood and body fluid exposure.

간호대학생의 표준주의 수행 구조모형 (A Structural Equation Model on Standard Precautions Compliance of Nursing Students)

  • Purpose: This study is to establish a structural model for standard precautions compliance of nursing students. This conceptual model was based on the IMB (Information-Motivation-Behavioral skills) model of Fisher and Fisher. Methods: Data were collected from October 12 to December 1, 2020, and the subjects were nursing students from G metropolitan city and J province, and the data of a total of 334 subjects were analyzed. For data analysis, this study used the SPSS 24.0 and AMOS 24.0 programs. Results: The hypothetical model showed a good fit to the data: 𝑥2=106.46 (p<.001), 𝑥2/df=2.54, RMSEA=.07, SRMR=.04, CFI=.96, TLI=.94. It was confirmed that the variables that have a statistically significant influence on the standard precautions compliance in nursing students were in the order of self-efficacy, social support, personal attitude, and standard precautions knowledge. The model explained 48.3% of the variance in standard precautions compliance of nursing students. Conclusion: It is necessary to develop and apply various specialized extracurricular programs that can induce an individual attitude toward observing standard precautions compliance in a positive direction in relationships with teachers and peers and gain their support.

아동병동 간호사의 병원감염 표준주의에 대한 인지도와 이행도 (Study on Pediatric Nurses' Attitudes and Compliance with Hospital Infection Standard Precautions)

  • Purpose: The purpose of this study was to identify pediatric nurses' attitudes to, and compliance with, standard precautions in the prevention of hospital infections by enhancing their practice of standard precautions. Methods: The participants were 206 nurses who worked in pediatric nursing departments of a general medical institution in Seoul. The questionnaire was a modification of Kim (2008) and consisted of 18 questions about hand washing, personal protective equipment, sharps, linen and patient care equipment. Collected data were processed using SPSS 15.0 WIN. Results: Mean scores for attitudes to standard precautions and for compliance with standard precautions were 4.43 (${\pm}0.83$) and 4.22 (${\pm}0.44$) respectively. This difference was statistically significant (t=3.368, p=.001). The nurses' compliance with standard precautions differed significantly according to the general characteristics of age (F=8.705, p<.001), total clinical experience (F=9.426, p<.001), current department experience (F=6.555, p<.001), and education experience (t=0.616, p<.043). There was a positive correlation between attitudes to, and compliance with, standard precautions (r=.156, p=.025). Conclusion: The results of this study indicate that educational programs and policy on infection control and standard precautions for pediatric nurses are needed. Also these results should contribute to baseline data for establishing appropriate clinical policy on infection control.

응급실 간호사의 감염관리 조직문화와 자기효능감이 표준주의 지침 수행에 미치는 영향 (The Effects of Organizational Culture for Infection Control and Self-Efficacy on Compliance with Standard Precautions of Emergency Room Nurses)

  • Purpose: This study was conducted to identify the effects of organizational culture for the infection control and self-efficacy on compliance with standard precautions of emergency room nurses. Methods: This cross-sectional descriptive study was conducted from 1st July to 3rd August, 2017. The participants were 200 nurses working in emergency rooms. The data was collected using structured self-report questionnaires. Results: The average scores for the organizational culture for infection control, self-efficacy and compliance with standard precautions were 5.54, 3.65, and 4.31, respectively. The organizational culture for infection control (r = .59, p< .001) and self-efficacy (r= .28, p< .001) were found to have a positive, meaningful correlation with compliance with the standard precautions. Multiple regression analysis showed that compliance with standard precautions was significantly affected by the organizational culture for infection control (${\beta}=0.55$, p< .001) and self-efficacy (${\beta}=0.13$, p= .033). Conclusion: The results for this study indicate that the organizational culture for infection control and self-efficacy were highly influential factors affecting compliance with standard precautions of emergency room nurses. Therefore, in order to improve the nurses' use of the standard precautions, it was necessary to develop and evaluate a program that considers the environmental factors and self-efficacy.

  • Purpose: This study was done to identify the intensive care unit nurses' knowledge of and compliance with the standard precautions (universal precaution) as stated in infection control guidelines. Method: From September 14 to September 28, 2006, data were collected via a questionnaire survey from 189 Intensive Care Unit nurses working at three university branch hospitals and one general hospital in Gyeonggi province. Results: The mean knowledge score was 18.8/20.0 (93.9%). The mean compliance score was 3.4/4.0 (85.8%). Two factors influencing compliance were perception of the standard precautions and experience of needle stick injuries over the past year (p<.05). Two factors influencing knowledge were support of co-workers in the use of protective devices and the availability of hand-washing device or waterless alcohol gel (p<.05). Conclusion: In order to improve knowledge and compliance with standard precautions, all factors of importance for knowledge and compliance must be taken into consideration in the clinical work place and in education.

의료종사자의 주사침 자상 관련 특성과 지식 및 표준주의 이행도 (Characteristics and Knowledge of Needlestick Injuries and Compliance with Standard Precautions in Healthcare Workers)

  • Purpose: This study was conducted to examine the characteristics and knowledge of needlestick injuries, and compliance with standard precautions, in healthcare workers. Method: The participants were 185 healthcare workers working at university hospitals. Data were collected in May 2016 and analyzed using descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients using SPSS 18.0 program. Results: It was found that 45.4% of the subjects had experienced needlestick injuries. The most common rate of getting injured was 1 or 2 times, and the most common reason for the occurrence of needlestick injuires was carelessness(66.6%). The mean scores for knowledge of needlestick injuries and compliance with standard precautions were above average. Significant correlations were found between knowledge of needlestick injuries and compliance with standard precautions in health workers, and Characteristics were related to the significant differences seen in both. Conclusion: These findings indicate that standard precautions and guidelines for the use of sharp instruments should be emphasized in order to prevent needlestick injuries in healthcare workers.

  • This study was conducted to investigate the degree of nurses' knowledge, attitudes, and compliance with hospital infection standard precautions. The study subjects were nurses in a university hospital in Daegu, South Korea, and a self-administered survey was carried out using a standardized questionnaire from 1st to 15th August, 2012. A total of 187 questionnaires were used for analysis. Those who were injured by a syringe needle for the last year accounted for 29.96%, and those exposed to patients' blood or body fluid on their damaged skin/mucous membranes accounted for 26.2%. There were statistically significant differences in attitude and compliance according to safety environment for hospital infection standard precautions. The knowledge score of the subjects about standard precautions was $15.30{\pm}1.51$ on a 19 point scale and had significant difference according to the subjects' education level. The attitude score was $2.86{\pm}0.49$ on a 4 point scale and had significant difference depending on how experienced they were on the career. The compliance score was $3.41{\pm}0.38$ on a 4 point scale and had significant difference depending on their age and their experience on the career. In conclusion, it can be said that systematic and continuous practical training is required to improve compliance with standard precautions. Particularly, it is needed to provide less experienced nurses with educational opportunities along with their supervisors' ongoing attention and position personnel appropriately.

간호사의 도덕적 민감성과 안전 환경이 감염 표준주의 이행도에 미치는 영향 (Influences of Moral Sensitivity and Safety Environment on Compliance with Standard Precautions among Nurses)

  • 본 연구는 간호사의 도덕적 민감성과 병원 안전 환경이 간호사의 감염 표준주의 이행도에 미치는 영향을 파악하기 위해 시행되었다. 연구대상자는 일 개 종합병원 간호사 214명이었으며 2017년 8월 1일부터 15일까지 도덕적 민감성, 표준주의 안전 환경, 감염 표준주의 이행도에 관한 자가 보고식 설문지로 자료 수집하였다. 수집한 자료는 평균과 표준편차, 빈도 등 서술통계, 독립표본 차이 검정, 피어슨 상관관계, 분산분석, 다중회귀분석 방법으로 분석하였다. 대상자의 도덕적 민감성 평균점수는 7점 만점에 5.05점이었으며, 표준주의에 대한 안전 환경은 7점 만점에 5.76점, 감염 표준주의 이행도는 5점 만점에 4.50점이었다. 감염 표준주의 이행도는 도덕적 민감성 및 표준주의 안전 환경과 유의한 양의 상관관계를 보였다. 감염 표준주의 이행도에 영향을 미치는 요인은 근무 부서(응급실, 중환자실), 나이, 표준주의 안전 환경이었다. 이들 변수의 설명력은 25.3%였으며, 도덕적 민감성은 영향 요인으로 확인되지 않았다. 따라서 감염 표준주의 이행도를 높이기 위해서는 신규간호사들을 대상으로 한 지속적인 교육프로그램, 부서별 특성에 따른 맞춤 증진활동이 필요하며 표준주의 안전 환경 확립을 위한 기관의 인적, 행정적 지원이 요구된다.
