Optical Spectroscopic Analysis of Muscle Spasticity for Low-Level Laser Therapy (LLLT)

  • Current therapeutic methods for suppressing muscle spasticity are intensive functional training, surgery, or pharmacological interventions. However, these methods have not been fully supported by confirmed efficacy due to the aggravation of the muscle spasticity in some patients. In this study, a combined system was developed to treat with a low-level laser and to monitor the region of the treatment using an optical spectroscopic probe that measures oxygen saturation and deoxygenation during low-level laser therapy (LLLT). The evaluation of the wavelength dependence for LLLT was performed using a Monte Carlo simulation and the results showed that the greatest amount of heat generation was seen in the deep tissue at ${\lambda}$ = 830 nm. In the oxy- and deoxygenation measurements during and after the treatment, oxygen-Hb concentration was significantly increased in the laser-irradiated group when compared to the control group. These findings suggest that LLLT using ${\lambda}$ = 830 nm may be of benefit in accelerating recovery of muscle spasticity. The combined system that we have developed can monitor the physiological condition of muscle spasticity during the laser treatment in real time and may also be applied to various myotonia conditions such as muscle fatigue, back-pain treatment/monitoring, and ulcer due to paralysis.

CNN을 사용한 차선검출 시스템 (Lane Detection System using CNN)

  • Lane detection is a widely researched topic. Although simple road detection is easily achieved by previous methods, lane detection becomes very difficult in several complex cases involving noisy edges. To address this, we use a Convolution neural network (CNN) for image enhancement. CNN is a deep learning method that has been very successfully applied in object detection and recognition. In this paper, we introduce a robust lane detection method based on a CNN combined with random sample consensus (RANSAC) algorithm. Initially, we calculate edges in an image using a hat shaped kernel, then we detect lanes using the CNN combined with the RANSAC. In the training process of the CNN, input data consists of edge images and target data is images that have real white color lanes on an otherwise black background. The CNN structure consists of 8 layers with 3 convolutional layers, 2 subsampling layers and multi-layer perceptron (MLP) of 3 fully-connected layers. Convolutional and subsampling layers are hierarchically arranged to form a deep structure. Our proposed lane detection algorithm successfully eliminates noise lines and was found to perform better than other formal line detection algorithms such as RANSAC

Effect of Home Exercise with Taping on the Pain and Functional Disability in Patients with Carpal Tunnel Syndrome

  • Objective: Carpal tunnel syndrome is one of the most common upper extremity compressive nerve disorders in modern people. Various conservative treatments such as taping, exercise, and electrotherapy have been applied to carpal tunnel syndrome and their effectiveness has been proven. This study aims to determine whether home exercise combined with taping is effective for pain and functional disability in patients with carpal tunnel syndrome. Design: Randomized controlled study Methods: 21 patients were recruited and were randomly divided into HEx+T (Home exercise+taping) group (n=10) and HEx (Home exercise) group (n=11). All participants performed Home exercise(tendon and neural gliding exercise) 5 times a week for 6 weeks. Additionally, subject in the HEx group, taping was applied over the wrist and forearm during the training period. Wrist pain was measured by VAS (Visual Analogue Scale). Functional disability was measured using a BCTSQ(The Boston Carpal Tunnel Syndrome Questionnaire). Results: Both the HEx+T group and the HEx group showed a significant improvement in wrist pain(p<0.05), and the HEx+T group showed a more significant decrease in pain than the HEx group(p<0.05). There was a significant improvement in both groups in functional disability (p<0.05), and the HEX+T group showed more improvement than the HEX group(p<0.05). Conclusions: This study suggests that home exercise combined with taping is effective for pain and functional disability in patients with carpal tunnel syndrome.

클라이밍 운동프로그램이 대학생의 자기효능감에 미치는 영향 (The Effect of Climbing Exercise Program on Self-efficacy for the College Students)

  • 본 연구는 6주간 20대의 일반 대학생을 대상으로 척추 정렬과 균형능력에 대한 클라이밍 운동 프로그램이 자기효능감에 미치는 영향에 대해 알아보았으며, 실험대상자는 총 30명이었으며, 6주간 주 3회 씩 상지와 하지에 대한 클라이밍 운동프로그램을 실시하였고, 결과는 다음과 같았다. 자기효능감의 군내 비교에서는 훈련군에서 유의한 차이가 있었다. 따라서, 클라이밍 운동이 자기효능감에 정상성인의 동기부여를 향상하는데 영향을 줄 수 있다는 것을 알 수 있었다. 결론적으로 추후 다양한 실험자들에게 클라이밍 운동을 실시할 시 참고 자료로 사용될 수 있을 것이라 사료된다.

Effectiveness of Heel-Raise-Lower Exercise Combined with Taping in Patients with Stroke

  • Objective: This study was conducted to investigate the effect of heel-raise-lower exercise on spasticity, strength, and gait speed after the application of tapingin patients with stroke. Design: Randomized controlled study Methods: The participants were randomly divided into the heel raise-lower exercise+taping (HREx+T) group and the heel raise-lower exercise (HREx) group, with 20 participants assigned to each group. Both groups performed heel lifting exercise 100 times a day 5 times a week for 6 weeks. HREx+T group additionally applied taping to the plantar flexor muscles. The spasticity of the ankle plantar flexors was measured using the composite spasticity score. A handheld dynamometer and a 10-m walk test were used to measure plantar flexor strength and gait speed, respectively. Results: Spasticity was significantly more improved in the HREx+T group than in the HREx group (p<0.05). Similarly, plantar flexor strength was significantly more improved in the HREx+T group than in the HREx group (p<0.05). Moreover, participants assigned to the HREx+T group showed significantly greater improvement in gait speed than those in the HREx group(p<0.05). Conclusions: Thus, heel-raise-lower training combined with taping may be used to improve the spasticity, muscle strength and gait speed in stroke patients.

융복합 의사소통 훈련 프로그램이 간호대학생의 의사소통 자기효능감, 의사소통 능력 및 대인관계에 미치는 효과 (Effect of the Convergence Communication Training Program for Communication self efficacy, Communicative competence, and Interpersonal relations of Nursing students)

  • 본 연구는 융복합 의사소통 훈련 프로그램을 적용한 인간관계와 의사소통 교과목을 운영하여 간호대학생의 의사소통 자기효능감, 의사소통 능력 및 대인관계에 미치는 효과를 알아보고자 시도하였다. 연구방법은 비동등성 대조군 전후 설계의 유사실험 연구이며, 자료수집 기간은 2016년 9월 5일에서 12월 13일까지이다. 연구대상자는 D시 소재의 1개 대학교 1학년 중 실험군 41명, 대조군 42명으로 총 83명이다. 자료분석 방법은 SPSS/WIN 21.0 program을 이용하여 기술통계, ${\chi}^2$ test 및 t-test,로 분석하였다. 연구결과는 융복합 의사소통 훈련 프로그램을 적용한 인간관계와 의사소통 교과목 운영 실시 전후 두 집단 간 차이를 분석한 결과, 의사소통 자기효능감(t=15.23, p<.001), 의사소통 능력(t=11.42, p<.001) 및 대인관계(t=19.24, p<.001)는 통계적으로 유의미한 차이가 있었다. 이러한 결과를 바탕으로 인간관계와 의사소통 교과목 운영에 융복합 의사소통 훈련 프로그램을 적용하여 간호대학생의 의사소통 자기효능감, 의사소통 능력 및 대인관계 향상에 도움이 되기를 기대한다.

Effect of a Combined Functional Electrical Stimulation with Action Observation Training on the Upper Limb Global Synkinesis and Function of Patients with Stroke

  • Background: Multifaceted approaches will be needed, such as global synkinesis (GS) achieve functional improvements in the arms of stroke patients from involuntary movements during exercise. Objective: To identify changes in arm GS and muscle activity, functional evaluation and the correlation with variables through action observation training, combined with functional electrical stimulation (FES), thereby verifying the effect on stroke patients. Design: A quasi-experimental study. Methods: The subjects of this study were 20 stroke patients who were divided into two groups: Control group (n=10) and experimental group (n=10). Before the intervention, arm GS and muscle activity were measured using surface electromyography (EMG), and arm function was evaluated using the Fugl-Meyer Assessment (FMA) scale. At the end of the intervention, which lasted 4-wk, arm GS and muscle activity were measured again using the same scale. Results: There was a decrease statistically significant difference in GS during the bending action in experimental group (P<.01). Both groups showed a significant difference increased only in the activity of the anterior deltoid (AD) and biceps brachii (BB) (P<.05). The results of the arm functional assessment revealed a significant difference increase in both groups (P<.05). In the between-group comparison, there was a significant difference decrease in GS during the bending action (P<.05). Only the muscle activity of the AD and BB were significantly increase different (P<.05). There was a significant between-group difference increase in the arm functional assessment (P<.05). There was a positive correlation between GS and muscle activity on the FMA in the control group (r=.678, P<.05). In experimental group, GS during the bending arm action exhibited a negative correlation (r=-.749, P<.05), and the muscle activity of the AD and BB showed a positive correlation (r=.701, P<.05). Furthermore, in experimental group, the activity of the extensor carpi radialis increased, and the activity of the flexor carpi radialis decreased, which exhibited a negative correlation (r=-.708, P<.05). Conclusion: These results suggest that brain plasticity could be more efficiently stimulated by combining surface stimulation in the affected arm of stroke patients.

Exosomal miR-181b-5p Downregulation in Ascites Serves as a Potential Diagnostic Biomarker for Gastric Cancer-associated Malignant Ascites

  • Purpose: Peritoneal carcinomatosis in gastric cancer (GC) patients results in extremely poor prognosis. Malignant ascites samples are the most appropriate biological material to use to evaluate biomarkers for peritoneal carcinomatosis. This study identified exosomal MicroRNAs (miRNAs) differently expressed between benign liver cirrhosis-associated ascites (LC-ascites) and malignant gastric cancer-associated ascites (GC-ascites), and validated their role as diagnostic biomarkers for GC-ascites. Materials and Methods: Total RNA was extracted from exosomes isolated from 165 ascites samples (73 LC-ascites and 92 GC-ascites). Initially, microarrays were used to screen the expression levels of 2,006 miRNAs in the discovery cohort (n=22). Subsequently, quantitative reverse transcriptase-polymerase chain reaction (qRT-PCR) analyses were performed to validate the expression levels of selected exosomal miRNAs in the training (n=70) and validation (n=73) cohorts. Furthermore, carcinoembryonic antigen (CEA) levels were determined in ascites samples. Results: The miR-574-3p, miR-181b-5p, miR-4481, and miR-181d were significantly downregulated in the GC-ascites samples compared to the LC-ascites samples, and miR-181b-5p showed the best diagnostic performance for GC-ascites (area under the curve [AUC]=0.798 and 0.846 for the training and validation cohorts, respectively). The diagnostic performance of CEA for GC-ascites was improved by the combined analysis of miR-181b-5p and CEA (AUC=0.981 and 0.946 for the training and validation cohorts, respectively). Conclusions: We identified exosomal miRNAs capable of distinguishing between non-malignant and GC-ascites, showing that the combined use of miR-181b-5p and CEA could improve diagnosis.

작업치료사를 위한 운동성 평가도구의 교육효과 (The Effect of Education on Motor Skill Assessment Tool for Occupational Therapists)

  • 목적 : 본 연구는 운동발달을 평가하고, 발달성협응장애를 판별할 수 있는 운동성 평가도구인 Movement Assessment Battery for Children-2(MABC-2)을 활용하기 위해 6시간의 교육과정을 통하여 작업치료사의 평가역량이 향상될 수 있는지를 규명하고자 하였다. 연구방법 : 본 연구에 MABC-2를 임상에서 사용하길 희망하는 작업치료사 33인이 참여하였다. MABC-2 전문가 2인이 교육 참여자들에게 6시간의 교육을 실시하였다. 이론 강의(2시간)는 MABC-2 소개와 운동평가도구의 연구동향, 실습강의(4시간)는 MABC-2 평가항목 소개, 녹화된 평가 장면 관찰, 도구를 사용하여 직접 평가실습을 경험한 후 준비된 사례영상을 보고 채점과 결과분석을 실시하였다. 교육의 효과를 검증하기 위해 모든 교육과정을 마친 후 MABC-2의 전문가 1인과 교육에 참여한 작업치료사 33명이 동시에 평가동영상 샘플을 보면서 평가지에 원점수를 기록하였다. 이중 운동성 평가도구의 경험이 없는 23인의 자료를 활용해 평가자간 신뢰도를 산출하여 교육의 효과성을 검증하였다. 결과 : 본 연구에 참여한 작업치료사는 평균 5년 3개월의 경력을 지닌 30세 이상의 여성으로 약 70%의 교육생은 MABC-2 외에 국내에서 사용 중인 운동성 평가도구를 임상에 적용해 본 경험이 없었다. MABC-2 전문가가 제공한 교육을 받은 작업치료사들과 전문가 간의 ICC 결과는 .95이상으로 높은 평가자간 신뢰도가 확보되었다(p<.001). 결론 : 이론과 실습으로 구성된 6시간의 교육과정에 참여한 작업치료사의 평가자간 신뢰도가 .95이상으로 나타났으며, 운동기술 평가도구를 적절하게 활용할 수 있는 평가역량이 향상되었다. 이를 통하여 MABC-2에 관한 특정한 교육을 제공할 경우 작업치료사가 새로운 평가도구를 적절하게 활용할 수 있으며, 이를 통해 평가와 관련한 직무능력을 향상시킬 수 있을 것으로 판단된다.

16주 복합운동프로그램을 통한 저체력군 고등학생의 대사증후군 지표와 자율신경계의 변화 (The effects of the 16-weeks' combined exercise program on metabolic syndrome and autonomic nerve system of low-level physical strength group)

  • 본 연구의 목적은 PAPS를 실시하여 4, 5급에 해당하는 저체력군 학생을 대상으로 16주 복합운동프로그램을 통해 대사증후군과 자율신경계에 어떠한 변화가 나타나는가를 규명하는 것이다. 16주 복합운동프로그램은 주 5회에 걸쳐 준비운동, 본운동, 정리운동으로 구성하고 1회 50분씩 실시하였다. 자료처리는 SPSS 19.0 통계프로그램을 이용하여 모든 종속변인들의 평균과 표준편차를 산출하였다. 샤피로-윌크 검정값을 이용하여 정규성 검정을 실시하였고, 복합운동집단과 통제집단의 모평균 차이를 위하여 독립표본 t-검정을 실시하였으며, 16주 복합운동프로그램 전 후 각 집단 내변인들의 변화를 알아보기 위해 대응표본 t-검정을 실시하였다. 모든 통계치의 유의수준은 ${\alpha}$=.05로 처리하여 다음과 같은 결과를 얻었다. 본 연구 결과 대사증후군 지표에서는 허리둘레, 중성지방, 공복혈당, HDL-C에서 유의한 차이를 나타냈고, 자율신경계 요인에서는 통계적으로 모두 유의한 차이가 나타나 저체력군 고등학생에 긍정적인 영향을 미친 것으로 사료되어 진다.
